Preoperative IV iron and postoperative blood transfusion reduction in non-anaemic patients undergoing cardiac surgery: a systematic review and meta-analysis of RCTs.

BACKGROUND: Preoperative intravenous iron has become increasingly popular as a strategy to optimize hemoglobin before major surgery. However, its potential benefit in non-anaemic patients undergoing cardiac surgery remains unclear. To address this uncertainty, we conducted a systematic review and meta-analysis to investigate whether preoperative IV iron reduces red blood cell transfusion requirements and improves hematologic and clinical outcomes in adults with normal baseline hemoglobin undergoing cardiac surgery. METHODS: We conducted a systematic review and meta-analysis following PRISMA 2

Remimazolam-Induced Anaphylaxis After Spinal Anesthesia: A Case Report and Literature Review.

Perioperative anaphylaxis, though rare, is a potentially life-threatening complication. While antibiotics and neuromuscular blocking agents are common triggers, benzodiazepine-induced reactions have been considered uncommon. Remimazolam, a novel benzodiazepine sedative, has gained widespread use in Korea due to its rapid onset, short recovery, hemodynamic stability, and availability of flumazenil. However, increasing utilization has coincided with rising reports of hypersensitivity. We report the case of a 62-year-old female undergoing contralateral total knee replacement under spinal anesthes

Food-induced anaphylaxis following skin prick testing: a case report and literature review.

A case of legume-induced anaphylaxis following prick-to-prick (PtP) testing in a 2.5-year-old girl is described. Very few cases of food-induced anaphylaxis following skin prick testing are reported in the literature. Fish and nuts are the most commonly implicated allergens; however, milk, egg, legumes, and vegetables may also be involved. Some potential risk factors have been suggested, including young age, concomitant atopic dermatitis (AD) or asthma, large diameter of the skin test reaction, a history of previous anaphylaxis, and the use of fresh food allergens. The most convincing risk fact

Anaphylaxis during hemodialysis associated with a polysulfone membrane: evidence from basophil activation testing.

Dialyzer-associated anaphylaxis is an uncommon but serious complication of hemodialysis. Although severe anaphylaxis is rare, milder hypersensitivity reactions are likely underrecognized and frequently misattributed to cardiopulmonary or volume-related causes. In clinical practice, the diagnosis is often inferred indirectly from symptom resolution after dialyzer substitution rather than confirmed by immunologic testing. Serum tryptase is widely used as a marker of mast cell activation in anaphylaxis; however, its diagnostic utility is limited in patients with chronic kidney disease (CKD), in w

Association between results of component-resolved diagnostics and basophil activation in Hymenoptera venom allergy: A registry-based cross-sectional study in adults.

BACKGROUND: Allergy to Hymenoptera venom is one of the most frequent causes of anaphylaxis in adults. Conventional diagnostic approaches, including skin testing and measurement of allergen-specific IgE (sIgE) to venom extracts, do not always allow for precise identification of the culprit venom. This remains particularly challenging in patients with double-positive or inconclusive conventional test results, which may complicate the decision-making process regarding qualification for appropriate treatment. Component-resolved diagnostics (CRD) and the basophil activation test (BAT) may provide c

Electroacupuncture at LI11 and SP10 is associated with alleviation of acute urticaria-like reactions in passive cutaneous anaphylaxis: an exploratory analysis of complement-related proteins and multiscale omics.

BACKGROUND: The lifetime prevalence of acute urticaria is approximately 20%. Although electroacupuncture (EA) has been investigated as an adjunctive treatment for urticaria, the molecular mechanisms underlying these effects remain elusive. Passive cutaneous anaphylaxis (PCA) serves as an experimental model of localized acute IgE-mediated hypersensitivity in the skin, and it recapitulates the key phenotypic characteristics of acute urticaria. In this study, we leverage an ovalbumin (OVA)-induced PCA model to characterize molecular alterations in the skin and serum following EA intervention at L

Alpha-gal Immunoglobulin E Seroprevalence Among Blood Donors - 10 States, 2024-2025.

Alpha-gal syndrome (AGS) is an emerging, noninfectious tickborne disease characterized by an allergic reaction to galactose-α-1,3-galactose (alpha-gal), an oligosaccharide found in red (mammalian) meat and other mammalian products such as dairy and gelatin. As of 2022, AGS was estimated to affect up to 450,000 persons in the United States (1). Anaphylactic AGS reactions can be fatal, and AGS allergic reactions encompass a range of symptoms including urticaria, angioedema, wheezing, and gastrointestinal distress. AGS is primarily managed through an avoidance diet. The U.S. geographic distributi

Lactiplantibacillus plantarum LSC3 attenuates anaphylactic response in a murine model of food allergy.

AIMS: The present study aims to evaluate the impact of Lactiplantibacillus plantarum LSC3, previously isolated from the stool of an Algerian breastfed infant, on the development of allergic responses to milk proteins in BALB/c mice. METHODS AND RESULTS: BALB/c mice were given orally Lactiplantibacillus plantarum LSC3 and sensitized to β-lactoglobulin according to a food allergy protocol. The results show that oral administration of L. plantarum LSC3 induced a significant decrease in the serum levels of anti β-lactoglobulin IgG and IgE. In addition, oral administration of L. plantarum LSC3 als

Isolated angioedema in the emergency department: rarely an allergic reaction.

Angioedema is a common, potentially life-threatening emergency characterized by swelling of subcutaneous or submucosal tissues. While allergic reactions are common, a substantial proportion of ED angioedema presentations, particularly those without urticaria, involve non-allergic, bradykinin-mediated mechanisms. Differentiating between these pathways is critical, as standard anaphylaxis treatments like epinephrine are often ineffective for non-allergic variants. A key clinical indicator is the presence or absence of urticaria; isolated angioedema is rarely an allergy and suggests a bradykinin-

Hymenoptera allergy, difficulties in differential diagnosis: a case report.

BACKGROUND: The diagnosis of hymenoptera venom allergy can be particularly challenging, with the clinical history serving as a fundamental tool to guide the identification of the culprit insect. However, patient-reported visual recognition is often inaccurate and may lead to diagnostic errors unless supported by specific immunoallergological testing. CASE REPORT: A 37-year-old man who, during a stay in Taramundi (Asturias, Spain), sustained an insect sting that he visually identified as Nomada flava. Ten minutes later, he developed Müller grade III anaphylaxis, with loss of consciousness, ang

Acute Laryngeal Dystonia Mimicking Anaphylaxis Following Antipsychotic Initiation: A Case Report.

Acute laryngeal dystonia (ALD) is an uncommon but potentially life-threatening adverse effect of dopamine D2 receptor-blocking agents. Because airway symptoms predominate, it may be misdiagnosed as anaphylaxis. A 24-year-old woman developed recurrent episodes of throat tightness, dysphonia and stridor within days of antipsychotic initiation and subsequent dose escalation. Initial episodes were treated as anaphylaxis based on airway symptoms and mild tachycardia. Serum tryptase was mildly elevated, further contributing to diagnostic uncertainty. However, the absence of cutaneous findings or hyp

Diagnostic Accuracy of Tests for Hymenoptera Venom Hypersensitivity: A Systematic Review and Bayesian Meta-Analysis Protocol.

BACKGROUND AND AIMS: Hymenoptera venom hypersensitivity is a potentially life-threatening condition that may affect up to 7.5% of adults worldwide and can result in systemic allergic reactions ranging from generalized cutaneous symptoms to severe anaphylaxis. Accurate diagnosis following a systemic sting reaction is essential to guide initiation of venom immunotherapy, a highly effective but resource-intensive intervention. However, the diagnostic performance of commonly used tests, including venom skin testing, serum venom-specific IgE, and component-resolved diagnostics, varies across studie

Omalizumab: First Approved Anti-IgE Therapy for Multiple Food Allergies.

BACKGROUND: Approximately 10% of adults and 7.6% of children in the United States have food allergies. Allergic reactions to foods can be life threatening, which requires patients and caregivers to be constantly diligent in avoiding foods that may trigger an immune-mediated event. Until recently, the only FDA-approved treatment for food allergy was oral immunotherapy (OIT), which involves daily ingestion of small doses of antigen to gradually build up a patient's tolerance. This therapy can be burdensome, requiring multiple office visits, and it is not without serious risk, including anaphylax

Postmortem tryptase as a diagnostic marker in fatal anaphylaxis: A systematic review and meta-analysis.

Fatal anaphylaxis remains a diagnostically challenging condition in forensic practice, particularly in the postmortem setting where clinical information is often limited. Postmortem serum tryptase has been widely proposed as a marker of mast cell activation, but its quantitative significance and diagnostic performance have not been consistently defined. This systematic review and meta-analysis aimed to evaluate both the magnitude of tryptase elevation and its diagnostic accuracy in fatal anaphylaxis. Seven studies were included, comprising 105 anaphylactic deaths and 449 non-anaphylactic contr
