BACKGROUND: Allergen immunotherapy aims to redirect pathogenic IgE responses toward protective IgG antibodies. IL-1 regulates humoral immunity, yet its role in shaping allergen-specific IgG responses remains unclear.
OBJECTIVE: We investigated whether IL-1 promotes allergen-specific IgG and protection against allergic anaphylaxis in murine models.
METHODS: Allergic sensitization and challenge were performed in wild-type and IL-1 receptor antagonist-deficient (IL-1Ra-/-) mice. IL-1β was blocked or coadministered with allergen during immunization. Anaphylaxis severity, allergen-specific antibo

OBJECTIVE: This study investigated the effects of electroacupuncture (EA) on mast cell activation and transient receptor potential vanilloid 1 (TRPV1)-related signaling in skin tissue using a rat model of passive cutaneous anaphylaxis (PCA)-like urticaria.
METHODS: An immunoglobulin E (IgE)/high-affinity Fc receptor for IgE (FcεRI)-mediated PCA-like urticaria model was established in rats by anti-ovalbumin serum injection followed by ovalbumin challenge. Rats were randomly divided into negative control, model, EA, Cap (capsaicin + model), and Cap + EA groups. Capsaicin was administered subcut

BACKGROUND: Chronic spontaneous urticaria (CSU) is a common immune-related skin disorder with an unclear pathogenesis and no universally accepted animal model. The passive cutaneous anaphylaxis (PCA) model is the commonly used model in CSU research.
METHODS: To propose a modification to the PCA model to improve its alignment with CSU, we established a recurrent PCA (Re-PCA) model via 6 repeated injections of 2,4-dinitrophenyl-human serum albumin (DNP-HSA)-specific IgE and DNP-HSA and compared it to the PCA model. We assessed vascular permeability, scratch frequency scoring, histologic stainin

BACKGROUND: Measurement of IgE levels, skin prick tests (SPT), and basophil activation tests (BAT) are the gold standards for diagnosing Hymenoptera allergy. Currently, BAT most accurately resembles patient reactivity. However, data examining the influence of serum IgE and basophil FcεRI expression on BAT performance and their clinical relevance are limited.
METHODS: Total and specific IgE (sIgE) levels and their ratios, SPT results, high-affinity IgE receptor (FcεRI) expression on basophils, and BAT results from 31 patients with Hymenoptera-induced anaphylaxis were analyzed and correlated wi

Anaphylaxis is a severe, potentially life-threatening hypersensitivity reaction. Although anaphylaxis rarely occurs during dental anesthesia, preparation for a prompt and appropriate response is crucial. Severe anaphylaxis may occur in the absence of typical skin manifestations. We encountered anaphylaxis approximately 15 min after induction of general anesthesia, presenting with a gradual decrease in SpO2, acute hypotension, and tachycardia without skin manifestations. After administration of epinephrine, the patient's hemodynamic status improved rapidly. Elevated serum tryptase and plasma hi

This is one of the first reported cases of storage mite anaphylaxis in Australia, occurring in an infant who had two episodes of severe anaphylaxis, triggered by previously tolerated foods. Both episodes occurred following extended periods of food storage, and in both instances, the child required two doses of intramuscular adrenaline. Skin prick testing was negative to implicated foods, but demonstrated sensitisation to house dust mite, and serum-specific IgE confirmed storage mite sensitisation. Clinician recognition of this condition is essential for timely diagnosis and implementation of p

Intravenous paracetamol-induced anaphylaxis is uncommon but can be life-threatening during general anesthesia. The principal lesson of this case, however, was not the allergic reaction itself but the failure of multiple perioperative safety barriers that allowed the administration of a suspected allergen. We report the case of a 50-year-old woman who developed circulatory collapse, with systolic blood pressure decreasing from approximately 100 mmHg to 60 mmHg before becoming unmeasurable, approximately five minutes after inadvertent intravenous paracetamol administration during breast surgery

PURPOSE OF REVIEW: Anaphylaxis is a severe type 1 hypersensitivity reaction which is the result of mast cell degranulation causing multiorgan dysfunction. Currently, the most accepted biomarker measurement for mast cell mediated anaphylaxis is serum tryptase measurement. Serum tryptase measurement has limitations including an ideal window of collection during anaphylaxis and must be obtained by venipuncture collection. Urinary mast cell mediators are emerging mast cell biomarkers that can be used for evaluation of mast cell mediated anaphylaxis and mast cell activation. This review will highli

Sunflower seed allergy is uncommon, and reports of severe systemic reactions, such as anaphylaxis, are limited, particularly in Japan where sunflower seeds are not routinely consumed. A 38-year-old woman developed throat discomfort, dyspnea, and generalized urticaria within minutes of ingesting cookies containing sunflower seeds and was diagnosed with anaphylaxis. Her medical history included bronchial asthma and pollinosis caused by Japanese cedar and cypress. Skin prick testing (SPT) showed a strong positive (4+) reaction to sunflower seeds, while the results were negative for mugwort, cashe

INTRODUCTION: Adrenaline is the first-line treatment for anaphylactic shock. However, adrenaline administration may exacerbate myocardial ischemia and induce fatal arrhythmias. It can also cause hypokalemia, thereby increasing the risk of life-threatening cardiac events.
CASE PRESENTATION: A 58-year-old man developed refractory anaphylactic shock accompanied by electrocardiographic evidence of myocardial ischemia during spinal fusion surgery. A total of 3.6 mg of intravenous adrenaline was required. At 135 minutes after onset, the serum potassium level decreased to a nadir of 2.1 mmol/L, resu

Introduction: Peanut oral immunotherapy (P-OIT) is an emerging treatment strategy for peanut allergy (PA). Although a standardized pharmaceutical product, Peanut (Arachis hypogaea) Allergen Powder-dnfp, has been approved in several countries, it is not universally available. In such contexts, real-world protocols using readily utilizable peanut products may represent an alternative approach. This study aimed to describe the feasibility, safety, and clinical outcomes of P-OIT using toasted peanuts in a real-world effort in a pediatric population. Methods: This single-center retrospective cohort

BACKGROUND: Lesigercept (also known as YH35324) has shown favorable safety, dose-proportional pharmacokinetics (PK), and sustained suppression of serum free immunoglobulin E (IgE) after single dosing in participants with atopy. We examined these outcomes following repeated dosing in participants with atopy or allergic diseases.
METHODS: In this phase 1b study, healthy adults who had atopy, with or without mild allergic diseases, and had serum total IgE level ≥30 IU/mL (Cohorts 1-4), and adults with moderate-to-severe atopic dermatitis (Cohort 5), were enrolled and randomized. Cohorts 1, 2, an

BACKGROUND: Banana allergy varies geographically, but molecular sensitization patterns and fruit co-reactivity remain incompletely defined.
METHODS: We analyzed adults with IgE-mediated banana allergy from Thailand (n = 169) and Austria (n = 70) using banana extract-specific IgE (ImmunoCAP), ALEX (a multiplex allergen-specific IgE macroarray), and complementary line blotting. Country-stratified multivariable models assessed associations of sensitization patterns with systemic reactions and reaction severity.
RESULTS: Systemic reactions predominated in Thailand (156/169, 92.3%), whereas isola

Paediatric allergic disease continues to increase globally, placing substantial pressure on healthcare systems and specialist allergy services. Current diagnostic approaches rely heavily on clinical history, skin prick testing, serum-specific immunoglobulin E (IgE), and oral food challenges, all of which have recognised limitations. Artificial intelligence (AI) has emerged as a promising tool capable of integrating complex data from structured electronic health records and unstructured notes to improve diagnostic accuracy, risk prediction, and healthcare delivery. This narrative review examine

BACKGROUND: Selective allergy to eggs from specific avian species, while maintaining tolerance to hen's eggs, is rare and may present diagnostic challenges.
CASE REPORT: We report the case of a 75-year-old woman with no history of atopic disease who experienced episodes of anaphylaxis after consuming duck and goose eggs, while tolerating hen's eggs. Skin prick tests with commercial hen's egg extracts were negative. Serum testing revealed very low total IgE levels (5 kU/L) and low specific IgE to ovomucoid (Gal d 1), with negative results for other egg proteins. Prick-to-prick testing showed p

Food-induced anaphylaxis (FIA) is a life-threatening allergic reaction, while wheat-dependent exercise-induced anaphylaxis (WDEIA) is triggered by wheat ingestion plus cofactors. To elucidate their differences, we profiled serum extracellular vesicle (EV) proteomes from 240 participants, including WDEIA, FIA, oral allergy syndrome (OAS), and healthy controls. All blood samples were obtained at least one month after the most recent acute allergic reaction, using TMT-based LC-MS/MS with ELISA validation. A total of 583 EV proteins were confidently identified, revealing distinct immune features.

This study describes a potent hypoallergenic casein-polyphenol formula based on preclinical assays of serum samples from patients allergic to cow's milk proteins. Despite some variability among patients, results showed a significant IgE-binding reduction. This proof of concept indicates the reliability of altering the native structure and potential epitopes of allergens by natural polyphenols, consistent with previous studies monitoring amide bonding by vibrational spectroscopy. Structural predictions, binding pocket scouting, and molecular docking were used to explore the nature of the bindin

BACKGROUND: Most peanut allergic patients have antibodies to Ara h 1. Many studies have focused on the C-terminal vicilin region, with less attention concentrated on the N-terminal α-hairpinin domain. This study evaluated human monoclonal antibodies targeted to both regions.
METHODS: Microarrays of Ara h 1 peptides were probed for serum IgE binding. Human monoclonal antibodies to Ara h 1 were discovered from single-cell sequencing and hybridoma technologies. Western blots evaluated the cleavage pattern of Ara h 1 in peanuts. A mouse model of passive systemic anaphylaxis (PSA) evaluated the fu

BACKGROUND: Hymenoptera venom allergy (HVA) is a leading cause of anaphylaxis in adults and requires precise diagnostic work-up to guide venom immunotherapy (VIT). However, the high prevalence of asymptomatic sensitization and frequent double sensitization complicate the identification of clinically relevant allergens.
MATERIALS AND METHODS: This narrative review summarizes current concepts and recent advances in in vitro diagnostics of HVA, including conventional IgE testing, component-resolved diagnostics (CRD), IgE ratio analysis, and cellular assays and biomarkers for risk stratification.

BACKGROUND: Platelet-activating factor (PAF), a key mediator of anaphylaxis, is inactivated by PAF-acetylhydrolase (PAF-AH). PAF-AH circulates complexed to low-density lipoprotein (LDL). Lipid-lowering medications interfere with PAF inactivation by lowering the concentration of LDL-PAF-AH complex, but their effect on anaphylaxis severity is unknown.
OBJECTIVE: To determine whether lipid-lowering medication use alters anaphylaxis severity. We hypothesized that lipid-lowering medication use would increase the risk of severe anaphylaxis.
METHODS: Retrospective chart review of patients from Cana
