Closed-Incision Prophylactic Negative Pressure Wound Therapy in Emergency Laparotomy: A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

Surgical site infection (SSI) is common after emergency laparotomy, and closed-incision prophylactic negative pressure wound therapy (ciNPWT) applied over the closed incision has been proposed to reduce it, though trials disagree. We searched MEDLINE, Embase, CENTRAL, Scopus, and two trial registries to 21 June 2026, with no date or language restriction, for randomized controlled trials (RCTs) comparing ciNPWT with standard dressings in adults undergoing emergency laparotomy. The primary outcome was any incisional SSI within 30 days. We used random-effects meta-analysis with restricted maximum

Clinical characteristics and surgical outcomes of split cord malformations: A retrospective analysis.

BACKGROUND: Split cord malformation (SCM) is a rare congenital anomaly characterized by a longitudinal division of the spinal cord into two hemicords. Clinical manifestations are diverse, often including neurological, orthopedic, and cutaneous abnormalities. Surgical management remains the mainstay of treatment, though outcomes and complication profiles require further study. This study aimed to analyze the clinical spectrum, surgical management, and outcomes of pediatric and young adult patients with SCM. METHODS: A retrospective review was conducted of patients with SCM who underwent surger

Comparative analysis of clinical outcomes and healthcare costs following endoscopic versus open lumbar discectomy: a multidatabase study utilizing TriNetX, NIS, and NASS.

OBJECTIVE: Endoscopic lumbar discectomy (ELD) has emerged as a minimally invasive alternative to open lumbar discectomy (OLD), with advantages including reduced tissue disruption and amenability to outpatient settings. Comparative data on clinical outcomes and costs across care settings remain limited. This study leveraged three national databases, TriNetX, National Inpatient Sample (NIS), and Nationwide Ambulatory Surgery Sample (NASS), to analyze short- and long-term outcomes and inflation-adjusted healthcare costs between ELD and OLD. METHODS: A retrospective cohort study was performed usi

A Rare Case Report: Mycobacterium abscessus in Central Nervous System Infections in Astrocytoma Patient.

Mycobacterium abscessus (MAB), a rapid growing nontuberculous mycobacterium (NTM), is an emerging pathogen mostly associated with pulmonary disease, while central nervous system (CNS) involvement remains rare and has a poor prognosis. We present a case of CNS infection caused by MAB associated with a ventriculoperitoneal (VP) shunt in an astrocytoma patient. A 21-year-old male patient, pilocytic astrocytoma with noncommunicating hydrocephalus, presented with an exposed VP shunt due to wound dehiscence. Microbiological examination of cerebrospinal fluid and shunt components, confirmed by matrix

Continuous Mass Versus Layered Fascial Closure in Contaminated Emergency Midline Laparotomies: A Prospective Observational Cohort Study.

BACKGROUND: Emergency midline laparotomy in a contaminated field carries a high risk of acute fascial dehiscence (burst abdomen) and of late incisional hernia. Recommendations for abdominal wall closure derive largely from elective surgery, and the optimal anatomical closure technique in the emergency contaminated setting remains undefined. This study compared continuous mass closure with continuous layered closure in this population. MATERIALS AND METHODS: We conducted a hospital-based prospective observational cohort study of 340 consecutive adults undergoing emergency midline laparotomy fo

Paternal metabolic dysfunction and pregnancy loss following assisted reproduction: A cross-sectional study.

AIMS: Diabetes mellitus often causes anastomotic leakage leading to severe complications after surgery. The purpose of this study was to determine whether collagen synthesis is correlated with the anastomotic strength in diabetic animals in special reference to collagen type differences. METHODS: Thirty Lewis rats weighing approximately 250 g were made diabetic by tail vein injection of streptozotocin (STZ). Laparotomy was performed through a midline incision under ether anesthesia. A colonic segment 5 cm distal to the ileocecal junction was transected then anastomosed with absorbable suture.

Component separation is a technique used to provide adequate coverage for midline abdominal wall defects such as a large ventral hernia. This surgical technique is based on subcutaneous lateral dissection, fasciotomy lateral to the rectus abdominis muscle, and dissection on the plane between external and internal oblique muscles with medial advancement of the block that includes the rectus muscle and its fascia. This release allows for medial advancement of the fascia and closure of up to 20-cm wide defects in the midline area. Since its original description, components separation technique un

Outcomes of intraperitoneal compared to retromuscular mesh placement in ventral incisional hernia repair: a systematic review and meta-analysis.

INTRODUCTION: The ideal position of mesh in ventral hernia repair remains controversial. Intraperitoneal onlay mesh (IPOM) and retromuscular mesh (RM) placements are two widely used techniques, each with distinct anatomical and technical considerations. While it is well established that mesh reinforcement reduces hernia recurrence compared to suture repair, outcomes may vary according to the mesh location. This systematic review aimed to assess the recurrence and postoperative complications between IPOM and RM placement in ventral incisional hernia repair (VIHR). METHODS AND PROCEDURES: This

Can the Use of Closed Incision Negative Pressure Wound Therapy in Immediate Prepectoral Breast Reconstruction With Polyurethane-Coated Implants Reduce the Rate of Early Complications?: A Comparative Study.

BACKGROUND: Immediate prepectoral breast reconstruction with polyurethane-coated implants provides esthetic and functional benefits compared with subpectoral techniques, but early postoperative complications remain a concern. Closed incision negative pressure wound therapy (ciNPWT) has demonstrated benefits in other surgical fields, though evidence in breast reconstruction is limited. OBJECTIVES: To evaluate whether ciNPWT reduces early postoperative complications after immediate prepectoral reconstruction with polyurethane-coated implants, particularly in high-risk patients. METHODS: We ret

The association between skin-to-vessel distance and surgical complications in renal transplantation.

INTRODUCTION: Obesity is common among renal transplant recipients and increases the risk of perioperative complications. This study evaluated whether the distance from the skin to the external iliac vessels (SVD) is associated with adverse surgical outcomes in renal transplant recipients. METHODS: A retrospective cohort study of 167 consecutive patients with preoperative cross-sectional imaging who underwent renal transplantation was conducted at a single center. SVD was measured as the distance from the skin to the anterior edge of the external iliac vein at its bifurcation through the muscu

Combined prophylactic negative-pressure wound therapy and preoperative oral antibiotics reduce perineal wound complications after abdominoperineal resection.

PURPOSE: Perineal wound complications after abdominoperineal resection (APR) remain a major concern, often leading to poor patient outcomes. Prophylactic negative-pressure wound therapy (pNPWT) and oral antibiotics (OA) administration have individually shown promise in reducing surgical site infections (SSIs). The present study investigated their combined effect for the prevention of perineal wound complications after APR. METHODS: This retrospective study included patients who underwent APR for lower rectal cancer between April 2016 and March 2025. Patients were divided into two groups: conve

Imaging donor site complications after autologous breast reconstruction flaps: a pictorial review.

Autologous breast reconstruction using donor-site flaps such as the deep inferior epigastric perforator (DIEP) flap, transverse rectus abdominis myocutaneous (TRAM) flap, latissimus dorsi flap, profunda artery perforator (PAP) flap, and autologous fat grafting is a cornerstone of post-mastectomy reconstruction, offering durable results and significant psychosocial benefits. Despite these advantages, donor site complications are common and can pose diagnostic challenges for radiologists. This pictorial review illustrates the spectrum of these complications, including seroma, hematoma, infection
