Sternal wound reconstruction in geriatric patients: A comparison of patient characteristics and outcomes over thirty years.

Sternal wound (SW) complications after median sternotomy remain a major source of morbidity and mortality. Large-scale, single-surgeon outcome data remain limited, especially in older patient populations. Given their comorbidity burden, geriatric patients may be at elevated risk. We analyzed an older cohort of patients who underwent sternal reconstruction to evaluate the safety and reliability of bilateral pectoralis major flap reconstruction across age groups. A retrospective review of 640 patients who underwent SW reconstruction at our institution by the senior author (J.A.A.) from 1995 to 2

Initial wound healing following interrupted closing sutures with versus without tension-relieving mattress suture in guided bone regeneration: a preliminary randomized clinical trial.

PURPOSE: This prospective clinical study aimed to evaluate whether adding a tension-relieving horizontal mattress suture to simple interrupted sutures influences the occurrence and severity of postoperative wound dehiscence in horizontal guided bone regeneration (GBR) procedures. METHODS: Forty patients requiring horizontal GBR, either prior to or simultaneous with implant placement, were randomly allocated to 2 groups. Group "S" received simple interrupted sutures only, whereas group "S+H" received additional horizontal mattress sutures placed prior to interrupted sutures to relieve flap ten

Preoperative Hypoalbuminemia Predicts Infection, Fracture, and Repeat Revision After Revision Total Hip Arthroplasty; Prealbumin Stratification Does Not Refine Risk: A Retrospective Database Analysis.

Background/Objectives: Hypoalbuminemia is a marker of poor nutritional status and has been associated with increased postoperative complications following total joint arthroplasty. However, its long-term implications in the revision total hip arthroplasty (THA) population are poorly characterized, and the utility of prealbumin to further risk-stratify these patients remains unclear. We aimed to study the association between preoperative hypoalbuminemia and complications after rTHA. Methods: We identified patients who underwent rTHA with preoperative albumin levels obtained within one month of

Congenital Peritoneopericardial Diaphragmatic Hernia in a Dog: Ventral Advancement Diaphragmatic Technique for Repair.

Peritoneopericardial diaphragmatic hernia (PPDH) is a rare congenital anomaly that requires surgical treatment. Its clinical presentation varies and can affect one or more abdominal organs. This case report refers to a recently described surgical technique called ventral advancement of the diaphragm (VADT). Previously, a complete diagnostic approach was made in a 4-month-old, 6 kg (13.22 lbs.), uncastrated, mixed breed dog, with a history of cough from the time of adoption, with no known record of trauma. Physical examination revealed tachycardia, tachypnea, body condition score of 5/9, positi

GLP-1 Receptor Agonist Use and Wound Outcomes After Free Flap Breast Reconstruction.

BACKGROUND: Glucagon-like peptide-1 receptor agonists (GLP-1s) are increasingly used for glycemic control and weight loss, but their impact on surgical wound complications remains unclear. Some studies suggest GLP-1s reduce complications such as infection and dehiscence, while others report increased risk in certain procedures. This study evaluates whether preoperative GLP-1 use is associated with postoperative wound complications in free flap breast reconstruction. METHODS: We conducted a retrospective cohort study using the TriNetX Research Network, a global database of de-identified health

Association of Recent Antibiotic Therapy on Post-Thyroidectomy Wound Complications.

BACKGROUND: To assess the association between antibiotic use, for any indication, within 30 days prior to thyroidectomy and the incidence of surgical site infections (SSIs) using a large global electronic medical records database. METHODS: A retrospective cohort study was conducted using the TriNetX Global Collaborative Network. Adults who underwent thyroidectomy were stratified into two groups: those who received antibiotics within 30 days prior to the surgical procedure and those who did not. Propensity score matching (PSM) was employed to adjust for demographic and clinical variables. The

Treatment, survival and subsequent fertility following uterine torsion and dystocia in alpacas.

OBJECTIVE: Compared to domestic ruminants, dystocia is uncommon in alpacas. Uterine torsion is the most frequent obstetrical emergency and also a leading concern in late pregnancy. North American studies report favorable outcomes for dams and guarded outcomes for crias; comparable data for European populations are lacking. This study thus evaluated causes and outcomes of late-pregnant and obstetrical cases at a veterinary teaching hospital. MATERIALS AND METHODS: A retrospective cohort study included all alpacas presented between January 2009 and August 2025. Cases of antepartal uterine torsi

Role of prophylactic mesh in emergency midline laparotomy: a systematic review and meta-analysis.

PURPOSE: The role of prophylactic mesh reinforcement in emergency laparotomy closure remains controversial. While prophylactic mesh may reduce incisional hernia, its use in unstable and contaminated settings raises concerns regarding operative time, seroma development, and wound complications. This meta-analysis of randomized controlled trials (RCTs) evaluated the safety and efficacy of prophylactic mesh versus primary suture closure in emergency midline laparotomy. METHODS: A systematic search was performed for RCTs comparing prophylactic mesh with suture closure in adult patients undergoing

Identifying modifiable perioperative risk factors for wound healing complications after immediate autologous breast reconstruction.

BACKGROUND: Autologous breast reconstruction (ABR) offers superior aesthetics but carries significant risks of wound healing complications. We aimed to identify modifiable perioperative factors associated with these complications to optimize surgical outcomes. METHODS: We analyzed the ACS-NSQIP database (2011-2022) for patients undergoing immediate ABR. Multivariable regression identified independent associations with 30-day wound complications. RESULTS: Among 7489 patients, wound complications occurred 634 times: superficial surgical site infection (SSSI, n = 358), deep incisional infection

Endoscopic versus open surgery for soft tissue vascular anomalies and benign tumors: a two-center propensity score-matched study.

BACKGROUND: No large-scale study has compared the outcomes of soft-tissue endoscopic surgery (SOFTES) with conventional open surgery (OS) for vascular anomalies and benign soft tissue tumors. We aimed to compare the operative safety, efficacy, and outcomes of the two approaches for soft tissue lesions. METHODS: Patients (n = 414) undergoing SOFTES or OS in two centers were retrospectively reviewed. Propensity score matching was used to minimize selection bias and group differences. The treatment outcomes were compared between the two groups. RESULTS: After matching, 150 patients (SOFTES: 75;

Combined Therapy With Incisional Negative Pressure Wound Therapy and Penrose Drainage for Flap Surgery.

Pressure ulcers are well recognized, yet severe cases often require surgical closure. However, contaminated wounds and external forces from postoperative positioning can lead to high complication rates, prolonging treatment and increasing burdens on patients, providers, and healthcare costs. Negative pressure wound therapy has traditionally been used for chronic ulcers and traumatic wounds, and recent studies underscore the efficacy of incisional negative pressure wound therapy (INPWT) in reducing postoperative complications. Conventional closed suction drains, however, often lose suction pres

Small-Bite Versus Large-Bite Closure for the Prevention of Incisional Hernia: A Meta-Analysis.

Incisional hernia remains a common and clinically significant complication following midline laparotomy, contributing to patient morbidity, impaired quality of life, and increased healthcare costs. The fascial closure technique has been recognized as a potentially modifiable factor influencing hernia development. In recent years, the small-bite (short stitch) technique has been proposed as superior to the traditional large-bite (long stitch) method. This meta-analysis aimed to systematically evaluate and compare the effectiveness of small-bite versus large-bite fascial closure in preventing in
