[Translated article] Accuracy of the Akmese sign as a diagnostic method for posterior root tears of the medial meniscus in Peruvian healthcare facilities.

Background Medial meniscus injuries are the most prevalent knee injuries, frequently causing pain, instability, and orthopaedic surgeries. Although several clinical tests are used for meniscal pathology, most were developed for general meniscal lesions rather than specifically for medial meniscus posterior root tears (MMPRT). Evidence on root-specific physical examination signs remains limited. Recently, the Akmese manoeuvre has been introduced as a non-invasive diagnostic tool, assessing pain or tenderness during physical examination. This study aimed to determine the accuracy of the Akmese s

Mechanically stable but biologically inactive Jones fracture nonunion managed without immediate revision surgery: a case report.

BACKGROUND: Jones fractures are prone to delayed union and nonunion because of the vascular characteristics and mechanical loading environment of the fifth metatarsal metaphyseal-diaphyseal junction. Although nonunion is often attributed to mechanical insufficiency, host-related biological and functional factors may also be clinically relevant in selected cases. We report an illustrative case of Jones fracture nonunion with maintained implant integrity and radiographic stability that united without immediate revision surgery after integrated clinical assessment and management. Preliminary Clinical and Radiographic Outcomes of the Prone-Angled Transpsoas Approach for Lateral Lumbar Interbody Fusion: A Consecutive Clinical Series.

BACKGROUND AND OBJECTIVES: The prone-angled transpsoas (PAT) approach is a novel lateral lumbar interbody fusion technique that combines the workflow and sagittal alignment advantages of prone positioning with a navigated angled anterior-to-psoas trajectory. Although technical feasibility has been described, clinical outcomes remain limited. This study reports the preliminary clinical, radiographic, and perioperative outcomes of PAT for degenerative lumbar pathology. METHODS: A retrospective consecutive cohort study included 35 patients who underwent PAT at a single institution. Clinical outc

Pre-hospital care delays in trauma-related mass casualty incidents: a comparative international survey of high-income and low- and middle-income country perspectives.

BACKGROUND: Mass casualty incidents (MCIs) place substantial demands on pre-hospital systems. Extended pre-hospital times are associated with adverse patient outcomes. The factors perceived as contributing to these delays are poorly characterized across different resource settings. This study aimed to identify and compare pre-hospital care providers' perceptions of factors associated with pre-hospital delays during trauma-related mass casualty incidents across high-income and low- and middle-income countries. METHODS: A cross-sectional online survey was conducted between May and August 2024.

Immobilisation versus early active motion following primary flexor pollicis longus tendon zones 1-2 repair: protocol for a registry-based multicentre randomised controlled trial in Sweden.

INTRODUCTION: Flexor pollicis longus (FPL) tendon injuries in the thumb are surgically challenging and require careful postoperative rehabilitation to avoid complications such as tendon rupture, stiffness and impaired hand function. Rupture rates after FPL repairs are substantially higher than those reported for finger flexor tendons. Although controlled early active motion (EAM) is widely used following FPL repairs, rehabilitation protocols developed for finger flexor tendons may not be directly transferable to the thumb because of differences in anatomy, biomechanics and functional demands.

Biodegradable temporizing matrix in the management of exposed cortical bone without periosteum: a flap-sparing and reproducible strategy.

BACKGROUND: Exposure of cortical bone without periosteum (CBWP) represent a complex wound with poor healing potential, regardless of its location. Coverage traditionally relies on flaps, a complex reconstruction procedure. The use of a synthetic biodegradable dermal matrix (Novosorb® BTM) may promote formation of a vascularized neodermis over cortical bone, allowing subsequent skin grafting or secondary healing. This study aims to evaluate the effectiveness and limitations of synthetic dermal matrix in covering CBWP, potentially redefining flap indications. METHODS: We conducted a retrospecti

Human-Derived Motor Neuron Progenitor Cell Implantation Rescues Motor Endplates Following Peripheral Nerve Injury.

PURPOSE: Degradation of motor endplates (MEPs) following peripheral nerve injury (PNI) is a major determinant of functional recovery, as prolonged denervation renders target muscles progressively unreceptive to reinnervation. While prior studies have demonstrated that stem cell-based therapies may provide neurotrophic support following PNI, their potential effects on MEP preservation have not been well studied. We hypothesized that transplantation of human-induced pluripotent stem cell (hiPSC)-derived motor neuron progenitors (MNPs) into chronically denervated muscle would be associated with p

Preoperative predictors of graft retear following arthroscopic superior capsule reconstruction with long head of biceps tendon autograft: a pilot retrospective case-control study.

BACKGROUND: Superior capsule reconstruction (SCR) using the long head of the biceps tendon (LHBT) is a joint-preserving option for large-to-massive rotator cuff tears, but predictors of graft retear remain incompletely characterized. We aimed to identify preoperative clinical and imaging predictors of retear after arthroscopic LHBT-SCR. METHODS: A retrospective pilot case-control study was conducted at two tertiary orthopedic centers. Cases had confirmed graft retear (Sugaya type IV-V on postoperative magnetic resonance imaging); controls had an intact graft and were sampled at an approximate

Single-Stage Reconstruction of a Complex Full-Thickness Auricular Defect Following Excision of Rapidly Progressive Squamous Cell Carcinoma in a 100-Year-Old Patient: A Case Report.

Auricular reconstruction following oncologic excision remains challenging because of the complex three-dimensional anatomy of the ear, limited local tissue reserve, and the need to preserve contour, projection, and external auditory canal patency. Large full-thickness defects involving multiple auricular subunits are particularly difficult to reconstruct without substantial alteration of auricular dimensions or the need for cartilage grafting or staged procedures. This case describes a 100-year-old woman with a rapidly progressive auricular lesion involving the right helix, antihelix, and a po

Transanal natural orifice specimen extraction for restorative minimally invasive total mesorectal excision, enabled by transanal transection and single-stapled anastomosis: a propensity score-matched study of wound complications and postoperative pain.

BACKGROUND AND AIMS: In restorative total mesorectal excision (TME), transanal rectal transection and single-stapled anastomosis (TTSS) provide an access route that enables transanal NOSE before anastomosis. We evaluated wound complications and postoperative pain associated with SS-NOSE strategy compared with conventional double-stapled anastomosis and transabdominal specimen extraction. METHODS: This single-center, retrospective, propensity score-matched (PSM) cohort study included patients undergoing TME for low rectal cancer, as defined by the Low Rectal Cancer Development Program (LOREC),
