Romosozumab mitigates progression from radiological to symptomatic adjacent-level fractures compared to teriparatide.

UNLABELLED: Romosozumab and teriparatide were compared for preventing symptomatic subsequent vertebral compression fractures (SVCF) after cement augmentation. Across multicenter and hospital-based cohorts, Romosozumab significantly reduced the 2-year incidence and adjusted risk of symptomatic SVCF, particularly adjacent-level fracture, mainly by lowering conversion rate from radiological to symptomatic fractures requiring surgical intervention. INTRODUCTION: Subsequent vertebral compression fracture (SVCF) was a frequent and severe complication after vertebral cement augmentation, particularl

Burden of accumulating fragility fractures on health service usage and costs among post-menopausal women.

UNLABELLED: Cumulative fragility fractures markedly raise mortality, care needs, and healthcare costs among post-menopausal women. Women with ≥3 fractures show 2.2 times higher mortality rate and 1.4 times higher costs. These findings suggest that effective fracture prevention strategies may help reduce healthcare costs and alleviate some of the rising societal burden as populations age. PURPOSE: This study aimed to evaluate the cumulative burden of fragility fractures on health service usage and costs among post-menopausal women over a long follow-up period. METHODS: Pooled data from the Ku

Surgical versus conservative treatment of odontoid fractures in the elderly: A randomized controlled clinical study (SCORE).

BACKGROUND: Odontoid fractures of the second cervical vertebra commonly affect elderly patients due to osteoporosis and low-energy trauma. Treatment is controversial, with prolonged cervical collar immobilization risking non-union and complications, and surgical C1-C2 stabilization involving higher upfront surgical risks. High-level evidence from randomized controlled trials to guide optimal treatment decisions is lacking. The SCORE study aims to determine whether surgical stabilization is non-inferior to conservative collar management in maintaining functional independence for elderly patient

Surgical versus non‑surgical treatment of proximal humerus fracture in patients aged 50-65 years: young shoulder CARE (displaCed proximAl humeRus fracturE) trial-a pragmatic randomized controlled trial study protocol.

BACKGROUND: Proximal humerus fractures (PHF) are common in adults above 50 years, often following low-energy trauma and underlying osteoporosis. Randomized trials in patients 60 years and older have found no clinically important difference in 1- and 2-year outcomes between surgical and non-surgical treatments. There is limited evidence for the 50-65 age group, who may have different functional demands and even a different overall health status. There is limited knowledge to guide treatment decisions for this age group. METHOD: A multicenter, parallel-group, randomized, superiority trial compa

A systematic review comparing the outcomes of utilizing a tricortical screw, quadricortical screw, or suture button for syndesmotic fixation.

INTRODUCTION: Optimal fixation for distal tibiofibular syndesmotic injuries remains debated, with numerous studies comparing syndesmotic screws (SS) to suture-button (SB) devices. However, limited attention has been given to differentiating tricortical screw (TS) from quadricortical screw (QS) fixation. PURPOSE: To systematically review outcomes of SB, TS, and QS fixation to determine which technique provides superior clinical, radiographic, and patient-reported outcome measures (PROMs), as well as to determine which topics regarding syndesmotic fixation warrant further discussion. METHODS:

Outcomes of Kyphoplasty Versus Pedicle Screw Fixation for Osteoporotic Vertebral Compression Fractures: A Comparative Analysis.

BACKGROUND: The incidence of osteoporotic vertebral compression fractures (OVCFs) has steadily risen in recent years due to an aging general population. Symptomatic OVCFs can be treated via different methods, including kyphoplasty (KP) and pedicle screw fixation (PSF). Few studies have compared short- and long-term outcomes in patients undergoing either KP or PSF. METHODS: A systematic search of PubMed, SCOPUS, and Cochrane was conducted to identify meta-analyses, randomized controlled trials, observational studies, and cohort studies comparing KP and PSF for the management of OVCFs according

The Independent Effect of Nerve Injury and Tendon Injury on Rotator Cuff Fatty Infiltration.

BACKGROUND: Muscular fatty infiltration (FI), atrophy, and fibrosis commonly occur after chronic rotator cuff tear and are barriers to successful rotator cuff repair. It is unclear if these phenomena result from tendon injury alone, neurological injury, or a combination. PURPOSE: To determine the role of suprascapular nerve mediated FI with and without chronic rotator cuff tear in a rabbit model. STUDY DESIGN: Controlled laboratory study. METHODS: A total of 22 rabbits underwent bilateral shoulder procedures. Extremities were randomized to surgical procedures: suprascapular neurotomy, supra

[Development and Testing of a Miniaturised Arthroscope for Use in Outpatient Wrist Arthroscopy].

Wrist arthroscopy is an established procedure for the diagnosis and treatment of wrist pathologies. It is routinely performed in the operating room under wrist distraction. An arthroscope with the smallest possible diameter could enable diagnostic "in-office" arthroscopy in outpatient clinics without a need for distraction on a horizontally positioned hand. Our goal was to develop such an arthroscope in collaboration with Polydiagnost GmbH (P) and to test and compare it with existing arthroscopy systems from Karl Storz SE & Co. KG (KS) and Arthrex GmbH (A).The prototype of a newly developed mi

Factors influencing the need for open reduction in ulnar-sided carpometacarpal joint fracture-dislocation: A retrospective cohort study.

BACKGROUND: Ulnar-side carpometacarpal joint (CMCJ) fracture-dislocation is a rare hand injury, accounting for <1% of all hand and wrist injuries, and is often overlooked on plain radiographs. Surgical treatment is usually required, but the optimal approach depends on the fracture pattern and timing of diagnosis. The aim of this study is to explore the factors that make open reduction necessary for proper fracture alignment and to evaluate patient recovery after such procedures. METHODS: This study retrospectively reviewed patients who underwent surgical treatment for ulnar-side CMCJ fracture

Factors Associated With Mid-term PROMIS Upper Extremity Scores in Conservatively and Operatively Treated Distal Radius Fractures.

BACKGROUND: While radiographic outcomes after distal radius fractures (DRFs) are emphasized in clinical guidelines, several studies demonstrated that radiographic measures do not correlate with patient-reported outcomes (PROs), especially in patients aged ≥65 years. This study aims to determine whether fracture severity and radiographic parameters following fracture healing are prognostic of PROs, hypothesizing no strong association exists between these factors. METHODS: This study retrospectively reviewed prospectively collected Patient-Reported Outcomes Measurement Information System Upper

Automated AI fracture detection in initial presentation pediatric wrist X-rays: effects and benefits of adding follow-up examinations.

BACKGROUND: Artificial Intelligence (AI) in radiology has shown promise in detecting fractures on initial X-rays. However, the role of follow-up examinations in enhancing AI performance remains unexplored. This study evaluates the impact of including follow-up X-rays on the performance of neural networks in detecting pediatric wrist fractures. METHODS: Using the publicly available GRAZPEDWRI-DX dataset of 20,327 pediatric wrist X-rays, we created four training datasets: initial X-rays alone and combinations with follow-up X-rays (with and without casts). Two neural networks, EfficientNet (ima

Quantitative analysis of forearm instability in an Essex-Lopresti injury model: effects of radial head replacement and interosseous membrane reconstruction.

BACKGROUND: Essex-Lopresti injuries, defined by a combination of radial head fracture, interosseous membrane (IOM) rupture, and distal radioulnar joint (DRUJ) disruption, result in forearm longitudinal instability, ulnar-positive variance, pain, and loss of rotation. Although radial head replacement (RHR) is commonly performed, the biomechanical contribution of IOM reconstruction remains controversial. This study quantified the relationship between sequential disruption of forearm stabilizers and resultant longitudinal and rotational instability, and evaluated the extent to which RHR with IOM

A Survey-Based Assessment of Treatment Preferences for Distal Radius Fractures in Participants Aged 65 Years and Older.

PURPOSE: The management of distal radius fractures (DRF) in patients aged ≥65 years is debated because there is evidence that operative and nonsurgical treatment yield similar outcomes 1 year after injury. When more than one reasonable treatment option exists, shared decision-making between the patient and physician should be used. This study elicited preferences for the treatment of DRF using a survey of participants aged ≥65 years. METHODS: A survey for participants ≥65 years was administered using Amazon Mechanical Turk. Participants were presented with a theoretical scenario in which they

Distal radius fractures: External fixation for 6-7 Weeks versus early mobilization following 3-4 Weeks of external fixation and light splintage.

BackgroundExternal fixation for geriatric C-type distal radius fractures (DRF) often restricts early wrist motion. This study compares a strategy of early mobilization after 3-4 weeks of external fixation with lightweight support against the conventional approach of 6-7 weeks of continuous external fixation.MethodsA total of 174 patients aged 60 or older with C-type DRF were included. They were assigned to either an early mobilization group (n = 86), where external fixation was replaced with a lightweight support at 3-4 weeks, or a control group (n = 88) with conventional 6-7 weeks of external

Sarcopenia is associated with early postoperative cognitive decline in older adults following hip fracture surgery: a prospective cohort study.

BACKGROUND: Early postoperative cognitive decline (POCD) frequently affects elderly individuals with hip fractures, and a significant number of these patients exhibit sarcopenia prior to surgery. Identifying patients with characteristics associated with an increased risk of early POCD at an early stage is essential for managing it effectively. Sarcopenia is a condition linked to aging, marked by a progressive reduction in muscle mass and function. Previous studies revealed that having lower muscle mass is linked to the development of perioperative neurocognitive disorders. This research aims t

Outcomes of Epiphysiodesis About the Distal Radius and Distal Ulna.

BACKGROUND: Wrist fractures comprise nearly 30% of all fractures in children. Fractures that damage the distal radius and/or distal ulnar growth plates may require epiphysiodesis to prevent skeletal deformity and/or length discrepancy. Epiphysiodesis is commonly used to correct leg-length discrepancies in children but has not been extensively studied for its use on the distal radius and ulna. The purpose of this study is to present the outcomes of epiphysiodesis procedures about the distal radius and/or distal ulna. METHODS: A retrospective review identified pediatric patients (<18 years) who

Comparative analysis of adult distal radius fracture reduction in the emergency room: fluoroscopy-guided vs. non-fluoroscopy-guided approaches.

INTRODUCTION: Distal radius fractures are a common injury, typically treated with closed reduction and plaster casting. Fluoroscopy is frequently used during the reduction process to ensure accurate alignment. However, the necessity of fluoroscopy in achieving optimal radiographic outcomes remains uncertain. While fluoroscopy is considered the gold standard in many settings, concerns about radiation exposure and the cost of its routine use, especially in resource-limited environments, have sparked interest in non-fluoroscopy-guided methods. RESEARCH OBJECTIVES: The primary objective of this s

Long Term Analysis of 11 Consecutive Contralateral C7 to Lower Trunk Transfer via a Pre-spinal Approach in Adult Pan Brachial Plexus Injury Patients.

Background: Contralateral C7 (CC7) is an important donor nerve for reconstruction in global brachial plexus palsy. In 2013, Shu Feng Wang introduced the direct repair of lower trunk (LT) to CC7. The outcomes of hand recovery reported in the study was significantly better than the traditional techniques. We did a retrospective analysis of 11 consecutive adult patients who underwent reconstruction for pan-brachial plexus injury by repair of the CC7 to LT along with other extraplexal nerve transfers. Methods: Patients were evaluated for motor recovery of flexion and extension at elbow, wrist and
