Robot-Assisted Partial Nephrectomy in a High-Complexity Renal Tumor in a Horseshoe Kidney.

INTRODUCTION: Horseshoe kidney is an uncommon congenital fusion anomaly that can make renal tumor surgery especially challenging because of altered rotation, limited mobility, variable vascular supply, and an unpredictable collecting system (1-7). This video presents a robot-assisted partial nephrectomy for a high-complexity renal tumor in this setting. CASE PRESENTATION: A 33-year-old man, with ECOG 0 and no relevant comorbidities, was diagnosed with a 7.5-cm solid renal mass in the central posterior portion of the left moiety of a horseshoe kidney. The lesion had a RENAL score of 10p. Contr

Extraperitoneal Single-Port Robotic-Assisted Radical Cystectomy with Orthotopic Ne-obladder: Technique and Surgical Considerations.

INTRODUCTION: Radical cystectomy (RC) is the standard therapy for muscle-invasive bladder cancer (MIBC) and refractory high-risk non-muscle-invasive bladder cancer (1-3). Single-port robot-assisted radical cystectomy (RARC) offers notable minimally invasive advantages (4-6), whereas conventional transperitoneal approaches are associated with intestinal and gastrointestinal complications (7). This study evaluated an optimized extraperitoneal single-port RARC with orthotopic neobladder reconstruction for improved surgical safety and clinical outcomes. MATERIALS AND METHODS: A 57-year-old male p

A New Combined Technique for Neophallus Elongation and Girth Enhancement for Metoidioplasty (Barroso Procedure): Total Corpora Mobilization with Martius Flap.

PURPOSE: Metoidioplasty is used in transmasculine gender-affirming surgery; however, limited neophallus length and girth remain significant challenges (1-4). We describe a combined technique using Total Corpora Mobilization (TCM) for elongation and a Martius flap for girth enhancement (5-9). MATERIALS AND METHODS: Fifteen transgender men underwent first-stage metoidioplasty with TCM and Martius flap between 2023 and 2026. The vestibular plate and urethra were mobilized, followed by circumferential clitoral degloving and suspensory ligament division. A pubic periosteal incision was made, and s

Extracellular vesicle signature is associated with cardio-metabolic improvement after bariatric surgery.

AIMS: Bariatric surgery (BS) reduces obesity-associated systemic inflammation leading to multiple cardiovascular (CV) and metabolic benefits. Here, we tested whether measuring vaso-inflammatory cytokines, gut hormones, and circulating extracellular vesicles (EV) provide vaso-inflammatory-metabolic signatures that better correlate to CV-metabolic outcomes after BS, compared to a standard clinical assessment including body weight (BW) loss and traditional CV risk factors. METHODS AND RESULTS: In 111 patients with severe obesity, conventional clinical-biochemical parameters and non-conventional

Associations between Patterns of Neuroendocrine Liver Metastatic Burden and Outcomes after Liver-Directed Therapy, Systemic Chemotherapy, and Peptide Receptor Radionuclide Therapy.

INTRODUCTION: Treatment selection is a challenge for liver-dominant metastatic neuroendocrine tumors (NETs). Post hoc analysis of NETTER-1 suggested that tumor size but not tumor burden predicted progression-free survival (PFS), contrary to prior publications. Analysis of imaging datasets from capecitabine-temozolomide (CapTem), liver-directed therapy (LDT), and peptide receptor radionuclide therapy (PRRT) cohorts was performed to investigate whether imaging features of liver metastatic disease may guide treatment selection. METHODS: Patients with measurable liver-dominant metastatic NET were

Impact of Pulmonary Arterial Hypertension Therapies on Gas Exchange in Portopulmonary Hypertension.

BACKGROUND: Portopulmonary hypertension (PoPH) may coexist with hepatopulmonary syndrome (HPS), a condition characterized by intrapulmonary vascular dilatations (IPVDs) and hypoxemia. Although pulmonary arterial hypertension (PAH) therapies are commonly used to treat PoPH, their effects on gas exchange remain insufficiently characterized. RESEARCH QUESTION: What are the effects of PAH-specific therapies on gas exchange and the development of IPVDs in patients with PoPH? STUDY DESIGN AND METHODS: A retrospective cohort of patients with PoPH was used to assess changes in the alveolar-arterial

Safety and outcomes of upfront large-size rotational atherectomy burrs in left main coronary interventions.

OBJECTIVES: Data on the safety and efficacy of percutaneous coronary intervention (PCI) with adjunctive rotational atherectomy (RA) in left main (LM) disease are limited. Current guidelines recommend using a small-size RA burr initially, with upsizing if required. The safety of upfront use of a large-size RA burr (≥1.75 mm) is unknown. The authors assessed the safety of an upfront large rotablation burr in the treatment of LM calcification and the procedural and long-term outcome of this approach in comparison to a small-size burr. METHODS: Retrospective analysis of all patients undergoing PC

Carotid plaque mechanical heterogeneity and stroke risk: a comprehensive study integrating multi-site viscoelastic analysis and RADS classification.

OBJECTIVE: Carotid atherosclerosis is one of the important etiologies of ischemic stroke, and the mechanical properties of plaques play a key role in the mechanisms of stroke occurrence. This study aims to systematically evaluate the viscoelastic characteristics of carotid plaques at different locations (proximal shoulder, base, distal shoulder, overall) and with different echogenicities (hypoechoic, hyperechoic), and to explore their relationship with Plaque-RADS scores. METHODS: This study consecutively enrolled 38 patients with carotid atherosclerosis who visited our hospital from June to

A Machine Learning Model to Predict Complications Following Head and Neck Microvascular Surgery.

OBJECTIVES: Microvascular free tissue transfer is the primary reconstructive method in patients with complex defects of the head and neck. Defect site complications vary; incidence ranges from 15% to 70%. The use of machine learning via mutual information analysis has recently been described in the context of head and neck cancer, but this does not distinguish between causal relationships. The current study explores the first causal machine learning model to predict surgical outcomes in patients undergoing head and neck microvascular reconstruction. METHODS: A retrospective chart review was c

Combined endovascular and surgical management of scalp cirsoid aneurysms: A case series from Northeast India.

OBJECTIVE: Cirsoid aneurysms are rare, high-flow extracranial arteriovenous malformations of the scalp characterized by complex vascular anatomy and significant arteriovenous shunting. Owing to their rarity, optimal management strategies remain poorly defined. This study aims to describe the clinical presentation and evaluate outcomes following a combined endovascular and surgical approach. METHODS: This retrospective consecutive case series included all patients treated for scalp cirsoid aneurysms between October 2022 and December 2023 at a tertiary neurosurgical center. All patients underwe

Short-frame, self-expanding, fully recapturable transcatheter aortic valve replacement device in pure aortic regurgitation: Procedural performance and 30-day outcomes.

BACKGROUND: Achieving stable anchoring and effective sealing remains the principal limitation of transcatheter aortic valve replacement (TAVR) in patients with pure native aortic regurgitation (PNAR). A short-frame, fully recapturable self-expanding valve may expand the procedural margin by enabling iterative repositioning and implantation depth optimisation in non-calcified anatomy. METHODS: We analysed 13 consecutive patients with symptomatic moderate-to-severe or severe PNAR who underwent transfemoral TAVR with the ScienCrown® (Lepu Medical, Beijing, China) system. The primary endpoint was

Outcomes of Open vs Endovascular Repair of Abdominal Aortic Aneurysm Involving the Iliac Bifurcation.

BACKGROUND: Aneurysmal involvement of the iliac bifurcation increases the complexity of both open and endovascular repair of abdominal aortic aneurysm (AAA), potentially raising the risk of postoperative complications. Randomized trials comparing endovascular aneurysm repair (EVAR) and open aortic repair (OAR) for AAA showed that EVAR is associated with a lower postoperative mortality rate. However, it remains unclear whether these results apply to AAA involving the iliac bifurcation (AAAIB). Several small single-institutional studies have shown conflicting results. This study aims to compare
