An interferonopathy associated with a monoallelic variant in TRIM28.

BACKGROUND: In this work, we describe the clinical and functional changes in a family with a novel type I interferonopathy phenotype which we suggest could be associated with a TRIM28 variant, presenting with chilblains, skin ulceration, lipodystrophy and calcinosis. TRIM28 has multiple roles in regulating human health and disease, which include DNA repair and anti-viral response. The latter includes negative regulation of IRF7, one of the master regulators of the interferon response. METHODS: Whole exome sequencing and targeted panel sequencing was performed on DNA extracted from peripheral

Delayed Umbilical Extrusion of a Ventriculoperitoneal Shunt Following Robotic-Assisted Laparoscopic Hysterectomy: A Case Report.

Ventriculoperitoneal shunts (VPSs) are the standard treatment for hydrocephalus but are associated with significant long-term complications, including distal catheter migration. Umbilical extrusion is an exceptionally rare manifestation, particularly in adults, and has seldom been described following minimally invasive gynecologic surgery. We report the case of a 35-year-old female patient with a VPS placed in early childhood who presented with persistent serous drainage from a prior umbilical port site four months after a robotic-assisted laparoscopic hysterectomy. Initial imaging demonstrate

A Retrospective Cohort Study of Distal Biceps Brachii Tendon Rupture and Subsequent Repair Methods: A Comparison of Two Combined Treatment Pathways.

Background Distal biceps brachii tendon rupture is an uncommon injury that is predominantly caused by excessive eccentric force to a flexed elbow. Operative repair is generally preferred to restore function, with a variety of surgical fixation techniques and postoperative rehabilitation protocols currently employed but with no clear consensus regarding the optimal approach. Comparative evidence evaluating surgical techniques remains limited. Objective The objective of this study is to compare two combined treatment pathways for distal biceps tendon repair using postoperative complications such

Copenhagen longitudinal study on quality of life in a young adult population with gliomas.

BACKGROUND: Patients diagnosed with glioma in the brain are prone to lowered quality of life (QoL). The risk of neurological symptoms and emotional burden may restrict participation in everyday life. The glioma diagnosis is considered chronic with a limited overall survival rate, which is particularly concerning for adolescent and young adults (AYA). METHODS: In a longitudinal and prospective study with consecutive inclusion, we followed the QoL in glioma patients aged 18-35 (n = 46). Assessment was conducted online with the self-report measures Functional Assessment of Cancer Treatment-Brain

Health systems interventions for prevention of maternal peripartum infection in low and middle-income countries: a systematic review.

BACKGROUND: Globally, there is growing recognition that a functional health system is key to improving maternal health and reducing maternal mortality. Maternal peripartum infection remains a major cause of maternal morbidity and mortality, particularly in low- and middle-income countries. This infection is a multifaceted problem with many players. Adopting a health systems approach could help avert this preventable infection by addressing the gaps across all the components of care. OBJECTIVE: The aim of this systematic review was to identify and evaluate interventions to prevent maternal per

Central nervous system, cranial, and cerebrovascular conditions beyond the Kimonis criteria in patients with Gorlin syndrome: a retrospective cohort study.

BACKGROUND: Gorlin syndrome is a multisystem developmental and cancer-predisposing disorder commonly caused by germline PTCH1 variants. The Kimonis clinical criteria capture the canonical phenotype; however, central nervous system (CNS), cranial, and cerebrovascular conditions documented beyond these criteria remain poorly defined. METHODS: We conducted a retrospective descriptive cohort study of 32 patients who fulfilled the Kimonis clinical criteria and received care at either or both hospitals in Japan. Clinical information through March 31, 2026, including sufficiently documented care els

Dietary and Parenteral Nutritional Management of Short Bowel Syndrome with Intestinal Failure in Adults: A Narrative Review.

Short bowel syndrome (SBS) is the leading cause of chronic intestinal failure in adults, arising when the residual small bowel can no longer absorb the macronutrients, water, and electrolytes needed to sustain health without intravenous supplementation. Its management is, at its core, a nutritional problem. This narrative review summarises and critically appraises current evidence and guideline recommendations on the two pillars of that management: dietary (oral/enteral) therapy and parenteral nutritional support. We frame both around the two variables that dominate clinical behaviour, namely

Reevaluating the Role and Timing of Fever in Acute Cholecystitis.

INTRODUCTION: Acute cholecystitis is diagnosed based on clinical and diagnostic findings. Fever is commonly considered a sign of systemic inflammation in acute cholecystitis, although its diagnostic performance may vary depending on the timing of presentation. This study analyzed the prevalence of fever as a diagnostic parameter for acute cholecystitis, according to the symptom onset at the time of medical consultation (≤ 24 hours versus > 24 hours), and its potential association with both medical and surgical complications. METHODS: We conducted a retrospective cohort study in two urban, ter

Risk factor analysis and development of a prediction model for trismus after surgical treatment of buccal mucosa squamous cell carcinoma.

BACKGROUND: Buccal mucosa squamous cell carcinoma (BMSCC) arises in a functionally complex anatomical region adjacent to the buccinator muscle, the anterior border of the masseter muscle, the pterygomandibular raphe, the retromolar region, and the pterygomandibular space. Tumor infiltration, extended resection, post-reconstruction scarring, and postoperative radiotherapy may affect the masticatory muscle complex and mandibular movement. Compared with other oral subsites, such as tongue and floor-of-mouth cancers, postoperative trismus after BMSCC has more pronounced subsite-specific characteri

Effect of Different Cortical Targets of Transcranial Magnetic Stimulation on Freezing of Gait in Parkinsonism: A Systematic Review.

BACKGROUND AND PURPOSE: Freezing of gait (FOG) is a disabling and episodic gait disturbance in patients with Parkinson's, with varying responses to noninvasive brain stimulation. Transcranial magnetic stimulation (TMS) has been explored as a therapeutic option; however, the optimal cortical target for reducing FOG remains unclear. This systematic review synthesizes and critically evaluates the available evidence regarding the effects of TMS applied to different cortical targets on FOG-related outcomes, with emphasis on the indirect nature of comparisons between targets. METHODS: A systematic

Preoperative antibiotics for mild or moderate acute cholecystitis (POMMAC): study protocol for a non-inferiority, open-label, randomized controlled trial.

BACKGROUND: Evidence is lacking on the effect of preoperative antibiotic therapy on postoperative infections in acute cholecystitis. The POMMAC trial aims to investigate whether a single dose of antibiotic prophylaxis is non-inferior to continuous preoperative antibiotic therapy in terms of postoperative infections. METHODS: This is a non-inferior, open-label randomized controlled trial aiming to include 456 participants. Eligible participants with mild or moderate acute cholecystitis will be randomized at a 1:1 ratio to receive either continuous or a single dose of preoperative antibiotic pr

Beyond Fever-The Changing Face of Acute Leptospirosis: A Retrospective Observational Study from Western India.

BACKGROUND: Leptospirosis is a zoonotic infection caused by pathogenic Leptospira species and remains a significant cause of acute febrile illness in tropical regions. Its clinical spectrum ranges from mild, self-limiting illness to severe multi-organ dysfunction. The disease is broadly classified into anicteric and icteric (Weil's disease) forms, the latter associated with jaundice and increased morbidity. In India, diagnosis is often challenging due to overlapping features with other endemic infections such as dengue and scrub typhus. Coinfections and changing epidemiological patterns, inclu

Nonoperative Management of an Acute Complete Achilles Tendon Rupture With Mesenchymal Stem Cells and Extracellular Vesicles: A Case Report.

An acute complete rupture of the Achilles tendon with a large intertendinous gap is conventionally regarded as a surgical indication, yet operative repair carries a significant burden on wound-healing and potential for neurovascular complications, whereas nonoperative care may be limited by re-rupture and incomplete functional recovery. Human umbilical cord-derived mesenchymal stem/stromal cells (hUC-MSCs) and their secreted extracellular vesicles (EVs) exert immunomodulatory, pro-angiogenic and pro-tenogenic paracrine effects that could support tendon healing. A healthy 45-year-old recreation

Corneal Variables in Children: A Literature Review.

BACKGROUND: Corneal variables such as thickness, curvature and diameter are important for ocular assessments in children. Despite this, there is limited normative data relevant to children for these corneal variables. Developmental changes, together with potential demographic differences, complicate the direct extrapolation of adult reference values to paediatric populations. PURPOSE: To map literature related to corneal thickness, curvature, and diameter in healthy children. METHODS: A comprehensive literature search was conducted across different electronic databases to identify articles p

Evolution of daytime PaCO2 in patients with neuromuscular disorders and OSA using CPAP: a retrospective study.

BACKGROUND: Neuromuscular disorders (NMD) are frequently complicated by obstructive sleep apnea (OSA), which may add an obstructive ventilatory load and contribute to CO2 retention. While non-invasive ventilation (NIV) is recommended for established hypoventilation, the impact of continuous positive airway pressure (CPAP) on daytime PaCO2 in NMD remains poorly documented. METHODS: We conducted a retrospective single-center cohort study in a tertiary neuromuscular referral center. Consecutive adults with NMD undergoing respiratory assessment were included if they had (i) OSA treated with CPAP
