Deviations from the standard of care in the treatment of pregnant people with opioid use disorder in jails; a mixed methods study.

BACKGROUND: Medications for opioid use disorder (MOUD) with methadone or buprenorphine are the standard of care for opioid use disorder (OUD) during pregnancy and postpartum. MOUD is not routinely available in US jails. Little is known about OUD management practices and patient experiences when MOUD is unavailable in jails. This study aims to assess deviations from standard pregnancy OUD care in jail and demonstrate patients' experiences of such deviations. METHODS: Mixed-methods, secondary analysis from two prior studies: (1) a survey of US jails (n = 836) regarding availability MOUD in preg

Fetal and neonatal outcomes of pregnancies complicated by maternal sleep-disordered breathing: a systematic review.

INTRODUCTION: Maternal obstructive sleep apnea (OSA) and the broader spectrum of sleep-disordered breathing (SDB) during pregnancy may be associated with fetal and neonatal outcomes, but the literature is heterogeneous and uses clinically distinct exposure definitions. This systematic review aimed to evaluate associations between maternal SDB and fetal/neonatal outcomes while preserving differences in exposure ascertainment. METHODS: Following PRISMA 2020, the review was re-audited using updated PubMed and Web of Science searches, backward and forward citation searching, and ClinicalTrials.go

Association of continuous glucose monitoring metrics with cardiovascular and kidney outcomes in type 2 diabetes: Data from the GRADE-CGM substudy.

AIMS: We compared associations of CGM-derived time in range (TIR) and glucose management indicator (GMI) vs. HbA1c with CVD and eGFR decline in type 2 diabetes. METHODS: We studied 1363 participants with ≥10-day blinded CGM in the GRADE-CGM Substudy. Exposures were TIR and GMI, with HbA1c as comparator. Cox models estimated hazard ratios (HR) for the composite of any CVD or all-cause death. Linear mixed-effects models estimated eGFR slope. Models were adjusted for age, sex, race/ethnicity, treatment group, diabetes duration, and baseline eGFR, then mutually adjusted each CGM metric with HbA1c

Headache impact and quality of life following endometriosis surgery in women: a single-center observational study.

BACKGROUND: Endometriosis and migraine are common chronic conditions in women of reproductive age that impair quality of life and frequently co-occur as comorbid conditions. Evidence regarding the effects of treatments targeting one of these conditions on the course of the other remains limited. In particular, the association between endometriosis surgery and headache burden and quality of life has not been sufficiently investigated. The aim of this study was to examine changes in headache burden and quality of life following endometriosis surgery in women with and without migraine. METHODS:

Retrospective Comparison of the Pericapsular Nerve Group Block Versus the Fascia Iliaca Block for Hip Fractures in the Emergency Department.

INTRODUCTION: Hip fractures are a common injury among the elderly and a substantial cause of morbidity and mortality worldwide. Pain management is an essential component of care for patients with hip fractures, particularly for improving patient outcomes. Ultrasound-guided nerve blocks such as the fascia iliaca block (FIB) are evidence-based means of providing perioperative analgesia and reducing opioid use. Newer nerve blocks, such as the pericapsular nerve group (PENG) block, may serve as alternatives to the FIB. The objective of this study was to retrospectively compare pain reduction follo

Negative pressure wound therapy for the prevention of surgical site complications in emergency laparotomies: an updated systematic review and meta-analysis.

INTRODUCTION: Surgical site infections (SSIs) are common after emergency laparotomy and are associated with substantial morbidity, prolonged recovery, and increased health care costs. Negative pressure wound therapy (NPWT) has been increasingly used prophylactically on closed incisions in high-risk surgical patients; however, evidence supporting its effectiveness after emergency laparotomy remains heterogeneous. This updated systematic review and meta-analysis aimed to reassess the effectiveness of NPWT compared with standard postoperative dressings for the prevention of SSI and other wound-re

Overcoming inertia in anticoagulant reversal: the current evidence gap and need for critical assessment of factor replacement strategies.

INTRODUCTION: Direct oral anticoagulants (DOACs) include the factor Xa inhibitors (FXaI) apixaban and rivaroxaban and were approved in the United States and European Union in approximately 2010. Three- and four-factor prothrombin complex concentrates (3F-PCC and 4F-PCC) have been available in the US and EU since 1986 and 2005 for repletion of factors in patients anticoagulated with vitamin K antagonists. Despite their long-standing availability, no large, well-designed prospective clinical trials have been conducted to assess efficacy and safety of PCCs in bleeding patients anticoagulated with

Avoidance of vascular injury during trans-psoas approaches; awareness of venous anatomy on MRI.

PURPOSE: Iliolumbar venous anatomy shows many variations between individuals, which can potentially be hazardous during lumbar spine surgery. The iliolumbar veins and any large variants can be observed on routine pre-operative lumbar spine MRI. Knowledge of the venous anatomy is important during lumbar spinal surgery, to avoid any potential haemorrhagic complications. This is particularly relevant for anterior lumbar surgery and lateral trans-psoas surgery. METHODS: In this study, we assessed the venous anatomy on the pre-operative MRI of 144 patients who underwent trans-psoas lateral access
