Reinforced biotubes as readily available and regenerative vascular grafts.

The increasing prevalence of cardiovascular diseases and haemodialysis access drive high clinical demand for vascular grafts. Bioinert synthetic and decellularized or cryopreserved artery grafts exhibit poor regeneration and associated complications. Here, we present an approach that combines in vivo bioengineering and decellularization to prepare polycaprolactone skeleton-reinforced biotubes (dPB), which demonstrate appropriate mechanical strength, anti-kinking properties, resistance to repeat puncture, and are compatible with long-term storage at 4 °C. The loose and porous microstructure and

Beyond Continuous-Flow: The Next Wave of Fully Implantable LVADs and Total Artificial Hearts.

PURPOSE OF REVIEW: Ventricular assist devices (VADs) have evolved from a bridge-to-transplantation strategy into a cornerstone of long-term advanced heart failure management. While continuous-flow technology has dramatically improved survival and device reliability, the paradigm shift toward extended mechanical circulatory support has exposed critical unresolved challenges. This review examines current limitations and four transformative technological directions shaping the next generation of VAD therapy. RECENT FINDINGS: Four key areas are driving innovation in the field. First, growing evid

Needle-free connectors in tunneled central venous catheters for hemodialysis: A prospective single-centre safety and feasibility study.

BACKGROUND: Tunnelled central venous catheters (t-CVCs) remain essential for hemodialysis when arteriovenous access is not feasible, but catheter-related bloodstream infection (CRBSI) is a major risk. In temporary CVCs, needle-free connectors have been associated with fewer infections; however, data on high-flow hemodialysis catheters are limited. We evaluated the short-term safety and feasibility of high-flow split-septum needle-free connectors on hemodialysis temporary central venous catheters (t-CVCs). METHODS: In a prospective single-centre study (Trbovlje General Hospital, Slovenia; June

Local Anesthesia and Analgosedation for Surgical Micro-Axial Flow Pump Implantation.

Temporary mechanical circulatory support is recommended for cardiogenic shock management. Although surgical device implantation usually requires general anesthesia and orotracheal intubation with inherent hemodynamic risk, local anesthesia with analgosedation in spontaneously breathing patients offers a promising alternative. This retrospective, single-center study analyzed 374 patients receiving surgical implantation of a micro-axial flow pump (mAFP) between January 2023 and December 2024. Fifty (13.4%) of them were performed under local anesthesia. Of the 49 investigated patients, 79.6% were

Anisotropy, heterogeneity, and limited sex differences in aortic suture retention.

BACKGROUND: Suture-line complications following aortic graft repair can have life-threatening consequences. To mitigate the clinical risk of anastomotic dehiscence, vascular prostheses are evaluated using the suture retention test defined by ISO 7198:2016. While appropriate for synthetic grafts under controlled loading conditions, this standard does not account for the nonlinear, anisotropic, and heterogeneous mechanical behavior of native elastic arteries, their composite microstructure, or variability among animal models used in preclinical testing. METHODS: We applied ISO suture pull-to-fa

Disrupted anastomotic pseudoaneurysm in arteriovenous grafts: an etiology-driven case series with a proposed preliminary management framework.

BACKGROUND: Disruption of an anastomotic pseudoaneurysm in an arteriovenous graft (AVG) is a rare but life-threatening complication in hemodialysis patients, distinct from the more common graft-body pseudoaneurysms caused by repeated needle puncture. A systematic management framework for this specific entity is currently lacking. OBJECTIVE: To propose a preliminary etiology-based diagnostic and therapeutic conceptual framework for AVG anastomotic rupture through the analysis of three cases with different underlying mechanisms, and to explore its clinical applicability. METHODS: We retrospect

Vasoplegia as a distinct hemodynamic phenotype after heart transplantation: Prevalence, determinants, and clinical consequences despite preserved allograft function.

BACKGROUND: Vasoplegia is a recognized complication of cardiopulmonary bypass, yet its prevalence and clinical impact after heart transplantation (HT) remain incompletely defined, particularly when primary graft dysfunction is excluded. This study aims to characterize posttransplant vasoplegia using a consensus-based framework, identify perioperative risk factors, and quantify its impact on posttransplant outcomes. METHODS: We conducted a single-center retrospective cohort study of 488 adults undergoing isolated HT between 2020 and 2025. Patients with moderate/severe primary graft dysfunction

Superior venous use of bridge balloon for percutaneous lead extraction in a patient with an interrupted inferior vena cava: a case report.

BACKGROUND: Transvenous lead extraction (TLE) is the gold-standard treatment for cardiac implantable electronic device removal but carries a small risk of major complications, including superior vena cava (SVC) injury. The prompt use of endovascular occlusion devices can provide temporary haemostasis and haemodynamic stability in the rare event of SVC laceration. The presence of congenital heart disease and venous anomalies introduces additional technical complexity and necessitates careful procedural planning. Temporal Trends in VA-ECMO and Concurrent Mechanical Circulatory Support for Cardiogenic Shock.

BACKGROUND: Venoarterial extracorporeal membrane oxygenation (VA-ECMO) is often combined with other temporary mechanical circulatory support (tMCS) in cardiogenic shock, yet combination-specific outcomes remain poorly defined. We compared temporal trends, mortality, complications, and device sequencing across VA-ECMO strategies. MATERIALS AND METHODS: VA-ECMO patients in the Cardiogenic Shock Working Group registry were analyzed. Patients were grouped as VA-ECMO alone or by first overlapping device: Impella CP, Impella 5.5, or intra-aortic balloon pump (IABP). The primary outcome was in-hospi

Left Ventricular Unloading in Anterior ST-Segment Elevation Myocardial Infarction Without Shock: The ST-Segment Elevation Myocardial Infarction Door to Unload Randomized Controlled Trial.

BACKGROUND: Despite rapid percutaneous coronary intervention (PCI) for ST-segment elevation myocardial infarction (STEMI), large infarcts contribute to heart failure and mortality. Left ventricular (LV) wall tension and load are major determinants of infarct size. Preclinical studies identified that delaying reperfusion to permit LV unloading with a transvalvular microaxial flow pump (TV-mAFP) reduces infarct size. We tested whether this combination reduces infarct size compared with reperfusion alone in patients with anterior STEMI without cardiogenic shock. OBJECTIVES: The STEMI-Door to Unl
