Association of body mass index with complications in cardiac implantation electronic devices: A retrospective single-center cohort study.

Cardiac implantable electronic devices (CIEDs) are increasingly used for managing arrhythmias and advanced heart failure, but implantation procedures carry risks of bleeding, infection, and mechanical complications. Patient-related factors such as obesity and comorbidities, alongside perioperative anticoagulation, may influence outcomes, yet evidence from South Asian populations is limited. We conducted a single-center observational study of 1925 patients who underwent CIED procedures between January 2010 and October 2025 at a tertiary heart center in Pakistan. Patients were categorized by bod

Antithrombotic therapy in patients with atrial fibrillation: unresolved issues and future directions.

Antithrombotic therapy is highly effective for stroke prevention in patients with atrial fibrillation (AF) but increases bleeding. Direct oral anticoagulants are preferred over vitamin K antagonists for many patients with AF because they provide similar stroke protection with less intracranial bleeding, and greater convenience, yet important knowledge gaps remain. In this review, we examine the limitations of current AF risk scores for predicting stroke, bleeding, and net benefit; identify AF populations in whom vitamin K antagonists remain preferred over direct oral anticoagulants, including

Prosthetic Valve Thrombosis: Current Management, Surgical Treatment, and the Evolution of Thrombolysis.

Prosthetic valve thrombosis (PVT) remains one of the most serious and potentially life-threatening complications of mechanical heart valves and continues to pose a major clinical challenge worldwide. Its incidence varies widely depending on anticoagulation quality, valve type, and access to specialized care, with a higher burden in regions where rheumatic heart disease remains prevalent. Historically, urgent surgery was considered the standard therapy because it provides immediate restoration of valve function; however, operative mortality is significant, particularly in critically ill patient

Long-Term Outcomes of Open Mitral Commissurotomy vs Mechanical Mitral Valve Replacement in Rheumatic Mitral Stenosis.

BACKGROUND: Mechanical mitral valve replacement (MVR) is widely performed for severe rheumatic mitral stenosis (MS) but requires lifelong anticoagulation and carries prosthesis-related risks. Open mitral commissurotomy offers a valve-preserving alternative, yet contemporary long-term data are scarce. We compared long-term outcomes of open mitral commissurotomy and MVR in severe MS. METHODS: We retrospectively analyzed 249 patients undergoing mitral valve surgery for severe rheumatic MS (1995-2021): open mitral commissurotomy (n = 58) and MVR (n = 191). The primary outcome was a composite of a

Semaglutide-associated Twiddler syndrome.

52-year-old woman with cardiac sarcoidosis and a dual-chamber implantable cardioverter defibrillator (ICD) was treated with semaglutide for refractory obesity (body mass index (BMI) of 40.1 (kg/m2)). After rapid weight loss of 25 kg, she developed painful device mobility and right ventricular lead dysfunction. Chest x-ray revealed lead entanglement consistent with Twiddler syndrome, which is a mechanical complication in which a pacemaker or ICD rotates within its pocket, causing the leads to twist or dislodge and resulting in device malfunction. Lead extraction was complicated by cardiac tampo

Transvenous Lead Extraction Using Mechanical Rotational Dilator Sheaths: A 19-Year Single-Center Experience from a Pediatric Cardiology Center.

The increasing use of cardiac implantable electronic devices (CIEDs) in pediatric and adolescent populations has led to a growing need for transvenous lead extraction (TLE). However, data on long-term outcomes remain limited. This study aimed to evaluate the efficacy and safety of TLE using mechanical rotational dilator sheaths in a pediatric cohort. This retrospective single-center study included 35 patients who underwent TLE between 2007 and 2025. Outcomes were compared between Evolution® (Cook Medical, Bloomington, IN, USA) and TightRail™ (Spectranetics/Philips, Colorado Springs, CO, USA) s

Cerebral-Protected Percutaneous Thrombectomy for Near-Occlusive LVAD Outflow Graft Thrombosis.

BACKGROUND: Late low-flow alarms in left ventricular assist devices (LVADs) most commonly result from volume depletion or right-sided failure, but mechanical outflow graft obstruction is an infrequent yet life-threatening cause. CASE SUMMARY: A 70-year-old man with a HeartMate 3 LVAD presented with persistent low-flow (0 L/min) alarms after defecation but felt entirely well. Laboratory testing showed no overt hemolysis, and transthoracic echocardiography excluded right-sided failure. Computed tomographic angiography revealed >90% thrombotic narrowing of the distal outflow graft. The patient u

Intra-aortic balloon pump versus medical management for acute myocardial infarction complicated by cardiogenic shock: a retrospective cohort analysis.

BACKGROUND: Acute myocardial infarction complicated by cardiogenic shock (AMI-CS) carries the highest in-hospital mortality among acute coronary syndromes, with medical management and various adjunctive therapies employed to care for these critically-ill patients. OBJECTIVE: Intra-aortic balloon pumps (IABP) are a widely-used therapy, thus we aim to evaluate differences in mortality, clinical outcomes, and hospital courses of patients with AMI-CS treated with IABP versus non-IABP medical management. METHODS: We conducted a retrospective cohort study of the National Inpatient Sample database

Lip piercing-related prosthetic aortic graft infection and mediastinitis in filaminopathy: A case report.

We report a rare case in which a lip piercing led to Staphylococcus aureus bacteremia and subsequent prosthetic aortic graft infection complicated by anterior mediastinitis in a patient with filaminopathy A. A 32-year-old woman, who had undergone valve-sparing aortic root and arch replacement with a hybrid thoracic stent graft 5 years earlier, presented 3 weeks after a lip piercing with fever, chest pain, and elevated inflammatory markers. Computed tomography revealed a periprosthetic fluid collection and mediastinal inflammation consistent with graft infection. Blood cultures grew methicillin

Autonomous Vascular Access Devices: Current Solutions and Future Challenges.

INTRODUCTION: Central venous cannulation is essential for life-saving interventions including resuscitation of critically ill patients, hemodynamic monitoring, endovascular intervention, pressor therapy, dialysis and apheresis, and extracorporeal life support. Cannulation of major arteries is required for cardiac catheterization, endovascular interventions such as arterial stenting or aneurysm repair, as well as emergency interventions such as intra-aortic balloon pumps and extracorporeal membrane oxygenation (ECMO). Both venous and arterial cannulation are commonly performed by specialized pr

Chronic Radiation Dermatitis After Breast Cancer Radiotherapy: Mechanistic Insights, Therapeutic Challenges, and Emerging Roles of Piezo1-Mediated Mechanotransduction.

BACKGROUND: Chronic radiation dermatitis (CRD) is a common late complication of postoperative radiotherapy for breast cancer. Its pathological process involves multiple interrelated factors, including persistent inflammation, microvascular injury, fibrosis, and tissue remodeling, and may substantially impair patients' quality of life. Current clinical management remains largely symptomatic, including topical therapies, hyperbaric oxygen therapy, laser treatment, and fat grafting; however, overall therapeutic efficacy remains limited, suggesting that the underlying mechanisms of CRD have not ye

Fatal Aortic Regurgitation in Behçet's Disease: A Case Report Highlighting Pitfalls and Lessons in Preoperative Diagnosis.

INTRODUCTION: Behçet's disease (BD), a chronic multisystem inflammatory disorder, rarely involves the heart. Aortic regurgitation (AR) is the predominant valvular lesion. When AR precedes characteristic mucocutaneous symptoms, misdiagnosis and treatment delays often occur. CASE PRESENTATION: We, herein, report the case of a 37-year-old male presenting with isolated aortic regurgitation (AR) as the initial manifestation of Behçet's disease (BD). Initial echocardiography revealed severe eccentric AR with left coronary cusp prolapse and a vegetation-like lesion, raising suspicion of infective en

Acute mitral regurgitation following giant coronary artery aneurysm resection: a case report.

BACKGROUND: Giant coronary artery aneurysms carry a high risk of rupture, thrombosis, and distal embolization. When percutaneous coronary intervention is not feasible in symptomatic cases, surgical resection combined with coronary artery bypass grafting is recommended. However, surgical ligation can interrupt flow to downstream branch arteries and precipitate regional myocardial ischemia. CASE PRESENTATION: In a 70-year-old woman with coronary artery disease, coronary angiography showed a 20-mm aneurysm with 90% stenosis of the proximal left anterior descending (LAD) artery, 90% stenosis of t

First-in-Human Mechanical Leaflet Modification of Native Aortic Stenosis During TAVR.

BACKGROUND: Coronary obstruction is a life-threatening complication of transcatheter aortic valve replacement (TAVR). Mechanical leaflet modification (LM) has been developed to mitigate this risk in patients with degenerated bioprostheses. However, their use in native valves has not previously been reported. CASE SUMMARY: We report 2 cases of mechanical LM performed in patients with severe native aortic stenosis at high risk for coronary obstruction during TAVR. In both cases, successful LM was achieved using a dedicated device (ShortCut, Pi-Cardia), followed by TAVR. Imaging validated leafle

Conservative surgical treatment: The present and the future.

Conservative management of aortic insufficiency relies on three physiological strategies preserving native valve function and root dynamics: aortic valve repair; the Ross procedure; and personalized external aortic root support (PEARS). Aortic valve repair is the cornerstone of conservative surgery for dystrophic bicuspid and tricuspid aortic insufficiency, with or without aneurysm, as it restores normal life expectancy while reducing lifelong prosthetic valve-related complications. Standardized techniques combining calibrated annuloplasty with systematic cusp effective height assessment have

Non-catheter-adherent right atrial mural thrombus in a patient with systemic sclerosis undergoing hemodialysis: a case report.

Catheter-related right atrial thrombosis is a rare, potentially fatal complication of central venous catheterization. While most reported cases involve thrombi attached to the catheter tip or mobile intracardiac masses, systemic sclerosis (SSc) is associated with endothelial injury and impaired vascular repair, presenting a unique risk profile characterized by increased susceptibility to thrombus formation. A 56-year-old woman on maintenance hemodialysis for 20 years for IgA nephropathy has had SSc for 10 years after developing digital ulcers. One year before presentation, she developed steal

Escalation to transplantation in refractory stone heart syndrome: a case report of advanced mechanical circulatory support failure.

BACKGROUND: Stone heart syndrome was first described by Denton Cooley in 1972 and represents an extremely rare but severe complication following cardiac surgery. The condition is characterized by an irreversible, concentric, spastic contracture of the myocardium occurring during reperfusion which does not respond to either pharmacological or mechanical interventions. The underlying pathophysiology remains poorly understood, but an association with suboptimal cardioprotection and prolonged ischemia time has been suggested. CASE PRESENTATION: A 22-year-old otherwise healthy patient was admitted
