Safety and Outcomes of Percutaneous Vegetation Aspiration During Transvenous Lead Extraction: A Multicenter Experience.

BACKGROUND: Transvenous lead extraction (TLE) for cardiac implantable electronic device infection has high procedural success, but with a 1-year mortality approaching 25%. Potential mechanisms include septic embolization and persistent infected lead casts or "ghosts." Percutaneous mechanical aspiration of lead-related vegetations during TLE is a potential strategy to reduce vegetation burden, but safety and outcomes data are limited. OBJECTIVES: To describe a multicenter experience utilizing percutaneous vegetation aspiration during TLE, including vegetation characteristics, procedural techni

The evolving role of cardiac CT: Emerging technologies and clinical impact.

Cardiac computed tomography (CT) has progressed from a non-invasive test primarily used to exclude obstructive coronary artery disease into a comprehensive cardiovascular imaging platform for coronary artery disease, myocardial disease, valvular heart disease, and structural heart disease. Coronary CT angiography (CCTA) is now endorsed as a first-line test in appropriately selected patients with stable chest pain or suspected chronic coronary syndromes because it can exclude obstructive disease, depict coronary atherosclerotic burden, support preventive treatment, and inform referral for invas

Advances in Biodegradable Supercapacitors for Implantable Bioelectronics: Materials, Energy Storage, Degradation, and Clinical Translation.

The emergence of transient bioelectronics has accelerated the development of biodegradable supercapacitors as promising energy-storage systems for temporary medical implants, including neural stimulators, cardiac pacemakers, controlled drug delivery devices, and biosensors. Biodegradable supercapacitors can safely resorb after completing their function, eliminating secondary surgical removal and reducing long-term foreign-body complications, unlike conventional batteries. This review comprehensively discusses recent advances in degradable supercapacitors, focusing on biocompatible polymers, bi

Unlocking the biology of left ventricular unloading: From mechanisms to clinical translation.

The concept of left ventricular unloading is deeply embedded in the core principles of cardiac physiology, with ventricular mechanics and energetics serving as its foundation. Modern mechanical circulatory support devices vary in their loading effects. Some actively unload the LV, whereas others primarily support systemic perfusion. Notably, extracorporeal life support systems can paradoxically elevate left ventricular afterload, thereby necessitating supplementary unloading strategies. The recent DanGer-Shock trial revealed a mortality benefit associated with microaxial flow pump support in s

Congestion as an electrophysiological state in heart failure: from mechano-electric coupling to dynamic arrhythmic risk.

Heart failure (HF) is traditionally characterized by hemodynamic impairment and neurohormonal activation, with congestion representing a central clinical feature. However, the electrophysiological consequences of congestion remain underexplored. Emerging evidence suggests that volume overload and elevated filling pressures actively modulate cardiac electrophysiology through mechano-electric coupling, myocardial stretch, and interstitial edema. Mechanical stretch activates stretch-sensitive ion channels, alters calcium handling, and increases cellular automaticity, thereby promoting triggered a

Perioperative use of temporary diaphragm pacing in surgical patients with unilateral diaphragm dysfunction to decrease mechanical ventilation.

BACKGROUND: Unilateral diaphragmatic dysfunction, most commonly from phrenic nerve injury, is associated with difficult ventilator weaning and increased postoperative pulmonary complications. Diaphragm pacing prevents ventilator-induced diaphragm dysfunction and supports ventilation during periods of perioperative vulnerability. We report perioperative utilization of temporary diaphragm pacing in patients with pre-existing or anticipated postoperative unilateral diaphragmatic dysfunction. METHODS: This cohort analysis was derived from a prospective, institutional review board-approved databas

Combined Percutaneous Mechanical Aspiration and Transvenous Lead Extraction in CIED-Related Endocarditis: A Case Report.

INTRODUCTION: Current guidelines recommend complete removal of a cardiac implantable electronic device (CIED) in infective endocarditis (IE), with surgical extraction indicated for large vegetations (> 20-25 mm). CASE: We report a 68-year-old patient with CIED-related IE and a 3 × 3.5 cm lead vegetation successfully treated by percutaneous mechanical aspiration (PMA) using the FlowTriever system, followed by transvenous lead extraction (TLE). The combined approach achieved complete removal without complications or recurrence. CONCLUSION: This case highlights PMA as a feasible, less invasive

Comparison of clinical outcomes of acute myocardial infarction patients complicated with cardiogenic shock treated with intra-aortic balloon pump versus microaxial flow pump.

BACKGROUND: The Danish-German (DanGer) Shock trial demonstrated that the use of a microaxial flow pump (Impella) reduced mortality compared to medical management in patients with acute ST elevation myocardial infarction complicated with cardiogenic shock (STEMI-CS). However, patients in the control arm of the DanGer Shock trial were not treated with an intra-aortic balloon pump (IABP), and data directly comparing Impella to IABP in this population is limited. METHODS: We identified STEMI-CS patients treated with percutaneous coronary intervention (PCI) and IABP at our institution between Janu

Anticoagulation for Pregnant Individuals With Mechanical Heart Valves: A Methodologic Review of Systematic Reviews.

In this study we aimed to describe methodologic variations in systematic reviews (SRs) that estimated clinical outcomes with different anticoagulant strategies for pregnant individuals with mechanical heart valves (MHVs), and present the most reliable risk estimates. We identified eligible SRs through a search involving 8 databases and critically appraised: 1) study quality using A MeaSurement Tool to Assess systematic Reviews (AMSTAR 2); 2) search strategies using Peer Review of Electronic Search Strategies (PRESS) guidelines and operationalized criteria of SR searches; and 3) meta-analytic a

How Would We Treat Our Own Weaning From Microaxial Flow Pumps for Left Ventricular Unloading?

A personalized rather than "one-size-fits-all" approach is essential when weaning patients from microaxial flow pumps (Impella). We detail, for the first time, a structured, stepwise, multimodal, bedside strategy to guide safe and effective discontinuation from a microaxial flow pump, based on echocardiography, reconditioning, and use of inotropes, Impella support, stepwise reduction titrated according to hemodynamics, and lung ultrasound with VExUS when appropriate ("E.R.I.L." protocol). Integrating these 4 pillars into daily clinical practice, E.R.I.L. aims to identify the optimal timing for

Intravascular Lithotripsy to Facilitate Extraction of Very Old Cardiac Implantable Electronic Devices Leads.

BACKGROUND: One of the challenges encountered when extracting transvenous leads with long dwell times is the presence of dense calcifications encasing the leads. This is the most likely reason for failure to advance a laser extraction sheath. The Shockwave intravascular lithotripsy (IVL) device is an angioplasty balloon that delivers intravascular lithotripsy and fractures calcified lesions in the vasculature, approved for use in coronary and arterial angioplasty. It has also been reported as an adjunctive tool in transvenous lead extraction. OBJECTIVE: To report the Vanderbilt University Med

Adverse events during catheter ablation in patients with cardiac implantable electronic devices.

BACKGROUND: Reviews of adverse events (AEs) in patients with cardiac implantable electronic devices (CIEDs) during catheter ablation predate newer devices, procedures, and ablation technologies (eg, pulsed field [PF], lattice-tip radiofrequency [RF]). OBJECTIVE: This study aimed to characterize AEs in contemporary patients with CIED undergoing ablation and identify implications for safer practice. METHODS: We analyzed 433 AEs from the US Food and Drug Administration's Manufacturer and User Facility Device Experience database and peer-reviewed literature (2020-2025), focusing on newer technol

Improved outcomes with a stepwise ventricular assist device strategy in Glenn physiology: A single-center experience.

OBJECTIVE: Patients with failing single-ventricle physiology at the bidirectional Glenn stage remain among the most challenging candidates for mechanical circulatory support. Historically, outcomes after ventricular assist device (VAD) implantation were poor with early reports describing survival as low as 30%; unique aspects of Glenn circulation may limit effective ventricular decompression and exacerbate cyanosis. More contemporary multicenter experience suggests improving survival. We evaluated the outcomes of a stepwise VAD strategy in patients with failing Glenn circulation, consisting of

Novel Approach Using Aspiration Thrombectomy for Aortic Root Thrombus After LVAD Implantation.

BACKGROUND: Left ventricular assist devices (LVADs) have become a cornerstone in the management of advanced heart failure. However, they are also associated with a range of complications, among which the development of an aortic root thrombus is particularly serious. CASE SUMMARY: A 72-year-old man developed refractory ventricular arrhythmias shortly after implantation of a HeartMate 3 LVAD, and he was found to have an aortic root thrombus extending into the left main artery. Given his high surgical risk, we performed to our knowledge the first documented percutaneous aspiration thrombectomy

Impact of underlying kidney disease on the outcomes of cardiac implantable electronic devices in patients with heart failure.

BACKGROUND: Heart failure (HF) and kidney disease commonly coexist and are associated with high mortality, as kidney disease is known to increase the risk of arrhythmias and sudden cardiac death in HF patients. The use of cardiac implantable electronic devices (CIEDs) has proven to be an effective therapy for HF patients. However, the literature on the outcomes of these devices in HF patients with underlying kidney disease is limited, given their exclusion from major trials. METHODS: Utilizing the National Inpatient Sample (NIS) database, hospitalizations with a discharge diagnosis of HF and C
