Hematologic complications and design challenges in cardiovascular devices.

In this review, we provide an engineering assessment of prevalent blood-wetted cardiovascular devices, their hematologic complications, and their interconnections to identify key areas for future research. In fact, the incidence of cardiovascular diseases has promoted the development of various devices to treat these conditions, including mechanical circulatory supports (mechanical pumps that assist or replace failing hearts), artificial valves to replace faulty heart valves, stents to relieve obstructions in blood vessels, vascular grafts to repair/reconstruct damaged vessels, patches to rein

Assessment of Vertical/Horizontal Ridge Augmentation in Atrophic Alveolar Ridge Using Autogenous Onlay Versus Inlay Grafting Techniques: A Systematic Review.

Alveolar ridge atrophy presents a major challenge in implant dentistry, often necessitating surgical augmentation to restore adequate bone volume for predictable implant placement. Among the various autogenous grafting approaches, the inlay and onlay techniques are widely utilized, yet their comparative clinical efficacy remains unclear. This systematic review aimed to evaluate the outcomes of vertical and horizontal ridge augmentation using inlay versus onlay grafting techniques in atrophic alveolar ridges. An extensive electronic and manual search of major databases was conducted in accordan

Mechanical thrombectomy for acute ischemic stroke after cardiac surgery or intervention: a retrospective cohort analysis.

BACKGROUND: Acute stroke due to large vessel occlusion (LVO) is a serious complication of cardiac surgery or other cardiac interventions. Little is known about the epidemiological characteristics of affected patients, the temporal relationship between stroke detection and surgery/intervention, the efficacy of mechanical thrombectomy, or the associated clinical outcomes. METHODS: We retrospectively analyzed the demographic and thrombectomy characteristics and neurological outcomes of patients who underwent mechanical thrombectomy for acute ischemic stroke due to LVO after cardiac surgery/inter

Non-Surgical Pneumoperitoneum After Fenestrated Endovascular Aortic Repair- Case Report.

ObjectivePost-operative pneumoperitoneum is mainly related to gastrointestinal perforation, although non-surgical pneumoperitoneum may also be present, with mechanical ventilation being the leading cause. Herein, we report a case of non-surgical pneumoperitoneum after percutaneous fenestrated endovascular aortic repair (fEVAR).Case reportA 79-year-old female presented with a 58 mm asymptomatic juxtarenal abdominal aortic aneurysm. The preoperative computed tomography angiography (CTA) detected also a diaphragmatic hernia. According to the latest guidelines, an indication for fEVAR was set. The

Heart transplantation following a successful bridge-to-bridge strategy in acute type A aortic dissection with left main coronary malperfusion.

A 55-year-old woman developed Stanford type A acute aortic dissection complicated by occlusion of the left main coronary artery. The patient underwent an emergency ascending aortic replacement with concomitant coronary artery bypass grafting to the left anterior descending artery. Owing to extensive myocardial ischemia and the resultant severe cardiopulmonary impairment, the patient could not be weaned from the cardiopulmonary bypass. Therefore, central veno-arterial extracorporeal membrane oxygenation was initiated, with aortic perfusion via a branch of the ascending aortic graft and right at

Reinforced biotubes as readily available and regenerative vascular grafts.

The increasing prevalence of cardiovascular diseases and haemodialysis access drive high clinical demand for vascular grafts. Bioinert synthetic and decellularized or cryopreserved artery grafts exhibit poor regeneration and associated complications. Here, we present an approach that combines in vivo bioengineering and decellularization to prepare polycaprolactone skeleton-reinforced biotubes (dPB), which demonstrate appropriate mechanical strength, anti-kinking properties, resistance to repeat puncture, and are compatible with long-term storage at 4 °C. The loose and porous microstructure and

Anisotropy, heterogeneity, and limited sex differences in aortic suture retention.

BACKGROUND: Suture-line complications following aortic graft repair can have life-threatening consequences. To mitigate the clinical risk of anastomotic dehiscence, vascular prostheses are evaluated using the suture retention test defined by ISO 7198:2016. While appropriate for synthetic grafts under controlled loading conditions, this standard does not account for the nonlinear, anisotropic, and heterogeneous mechanical behavior of native elastic arteries, their composite microstructure, or variability among animal models used in preclinical testing. METHODS: We applied ISO suture pull-to-fa

Innominate Artery Compression Syndrome Following Endovascular Stenting of Tracheoinnominate Artery Fistula.

Tracheoinnominate artery fistula (TIF) is a life-threatening complication associated with tracheostomy placement. It is characterized by the development of a connection from the tracheal lumen to the innominate artery. We describe the multidisciplinary management of a patient who developed innominate artery compression syndrome as a sequela of endovascular stenting of a TIF, and the subsequent surgical repair that ultimately led to relief of airway obstruction, allowing for tracheostomy decannulation. A 17-year-old male underwent percutaneous tracheostomy after suffering a gunshot wound to the

Chronic Radiation Dermatitis After Breast Cancer Radiotherapy: Mechanistic Insights, Therapeutic Challenges, and Emerging Roles of Piezo1-Mediated Mechanotransduction.

BACKGROUND: Chronic radiation dermatitis (CRD) is a common late complication of postoperative radiotherapy for breast cancer. Its pathological process involves multiple interrelated factors, including persistent inflammation, microvascular injury, fibrosis, and tissue remodeling, and may substantially impair patients' quality of life. Current clinical management remains largely symptomatic, including topical therapies, hyperbaric oxygen therapy, laser treatment, and fat grafting; however, overall therapeutic efficacy remains limited, suggesting that the underlying mechanisms of CRD have not ye

A Novel Decellularized Fibrocartilage Graft Promotes Tympanic Membrane Repair.

Tympanoplasty repairs tympanic membrane (TM) perforations using grafts. In pediatric patients, cartilage grafts are preferred over fascia due to superior mechanical properties that prevent complications from negative middle ear pressure, including graft failure, retraction, and cholesteatoma formation. However, autologous grafts cause donor-site morbidity and increased surgical time. To overcome these limitations, we engineered an allogeneic porcine meniscus decellularized (MEND) fibrocartilage graft with a unique microchannel structure created by selective elastin and vascular digestion to pr

A rare complication of vascular surgery - subclinical esophageal perforation secondary to supra-aortic debranching and TEVAR: a case report.

INTRODUCTION AND IMPORTANCE: Combined supra-aortic debranching and thoracic endovascular aortic repair (TEVAR) are well-established therapeutic options for complex aortic arch pathologies. Although rare, late complications such as esophageal injury can be life-threatening and require prompt multidisciplinary management. Several cases of delayed esophageal perforation complicated by mediastinitis have been reported in the literature. However, perforation presenting solely as a graft infection causing microembolizations remains exceedingly rare. CASE PRESENTATION: A 56-year-old male patient wit

Impella versus Non-Impella for Nonemergent High-Risk Percutaneous Coronary Intervention.

Источник пометил публикацию как отозванную

The benefit of mechanical circulatory support (MCS) with Impella (Abiomed, Inc, Danvers, MA) for patients undergoing nonemergent, high-risk percutaneous coronary intervention (HR-PCI) is unclear and currently the subject of a large randomized clinical trial (RCT), PROTECT IV. While contemporary registry data from PROTECT III demonstrated improvement of outcomes with Impella when compared with historical data (PROTECT II), there is lack of direct comparison to the HR-PCI cohort that did not receive Impella support. We retrospectively identified patients from our institution meeting PROTECT III

Concomitant Surgical Procedures and Aspirin Avoidance With Left Ventricular Assist Device Therapy.

BACKGROUND: ARIES-HM3 (Antiplatelet Removal and Hemocompatibility Events With the HeartMate 3 Pump) demonstrated that aspirin avoidance with a fully magnetically levitated HeartMate 3 (HM3) left ventricular assist device (LVAD) reduces bleeding complications and does not increase thromboembolism. Whether a concomitant surgical procedure modifies the observed safety and benefits remains uncertain. OBJECTIVES: This prespecified analysis of ARIES-HM3 studied clinical outcomes when concomitant surgical procedures are performed during LVAD implantation with excluding aspirin but maintaining a vita

Impact of planned concomitant coronary artery bypass grafting on risk of major adverse cardiovascular events in elective aortic hemiarch surgery.

BACKGROUND: Hemiarch replacement of the ascending aorta has become routine in many aortic centers. While the addition of coronary bypass does not add a lot of time to the procedure, it carries with more significant comorbidities. We hypothesize that the addition of CABG carries a higher risk of complication than hemiarch alone. METHODS: This is a single-center, retrospective cohort study of 419 patients undergoing elective hemiarch surgery between February 2010 and May 2023. Patients were categorized into concomitant CABG (n = 42) and non-CABG (n = 379) groups. Perioperative variables and out

High-dose versus low-dose sufentanil anesthesia for patients undergoing off-pump coronary artery bypass grafting: A retrospective study from single cardiovascular center.

High-dose opioid anesthesia has been the mainstay of cardiac anesthesia for decades. Yet emerging evidence has shown that low-dose opioid anesthesia may be of equal safety and effectiveness in cardiac surgery. We compared the outcomes of 2 different sufentanil dosing regimens (<4 μg/kg vs > 4 μg/kg) in off-pump coronary artery bypass grafting (OPCAB) surgery. The study identified all patients undergoing elective OPCAB at a single large-volume cardiovascular center between January 2020 and December 2020. Based on total intraoperative sufentanil dosing, patients were divided into 2 groups: low-d

The Impact of Age on Aortic Root Replacement.

OBJECTIVES: Aortic root replacement is increasingly being performed in the treatment of aortic root and valve pathology. While many of these procedures are performed during the fifth or sixth decade of life, differences in outcomes based on age remain largely unknown. Therefore, we examined short- and long-term outcomes of root replacement stratified by age. METHODS: From 2004 to 2021, all adult patients undergoing aortic root replacement in a retrospective database were included. RESULTS: A total of 2748 patients were divided into 6 age groups: 18-29 (n = 163.6%), 30-39 (n = 286.10%), 40-49

Bacterial adhesion and erythrocyte integrity on polycaprolactone nanowire surfaces.

Blood-contacting devices (BCDs), such as blood collection bags, blood tubes, artificial heart valves, vascular grafts and catheters, are subject to complications such as thrombosis, restenosis, hemolysis etc. These complications result in poor hemocompatibility and bacterial infections on the surfaces of BCDs that ultimately affect the patient's health. Natural and synthetic biocompatible polymers are used as potential solutions for these issues due to their superior biodegradability. Recent advancements in nanoscale fabrication and modification of these surfaces have shown improved results wi
