BACKGROUND: Tunnelled central venous catheters (t-CVCs) remain essential for hemodialysis when arteriovenous access is not feasible, but catheter-related bloodstream infection (CRBSI) is a major risk. In temporary CVCs, needle-free connectors have been associated with fewer infections; however, data on high-flow hemodialysis catheters are limited. We evaluated the short-term safety and feasibility of high-flow split-septum needle-free connectors on hemodialysis temporary central venous catheters (t-CVCs).

BACKGROUND: Disruption of an anastomotic pseudoaneurysm in an arteriovenous graft (AVG) is a rare but life-threatening complication in hemodialysis patients, distinct from the more common graft-body pseudoaneurysms caused by repeated needle puncture. A systematic management framework for this specific entity is currently lacking.
OBJECTIVE: To propose a preliminary etiology-based diagnostic and therapeutic conceptual framework for AVG anastomotic rupture through the analysis of three cases with different underlying mechanisms, and to explore its clinical applicability.
METHODS: We retrospect

INTRODUCTION: Central venous cannulation is essential for life-saving interventions including resuscitation of critically ill patients, hemodynamic monitoring, endovascular intervention, pressor therapy, dialysis and apheresis, and extracorporeal life support. Cannulation of major arteries is required for cardiac catheterization, endovascular interventions such as arterial stenting or aneurysm repair, as well as emergency interventions such as intra-aortic balloon pumps and extracorporeal membrane oxygenation (ECMO). Both venous and arterial cannulation are commonly performed by specialized pr

Catheter-related right atrial thrombosis is a rare, potentially fatal complication of central venous catheterization. While most reported cases involve thrombi attached to the catheter tip or mobile intracardiac masses, systemic sclerosis (SSc) is associated with endothelial injury and impaired vascular repair, presenting a unique risk profile characterized by increased susceptibility to thrombus formation. A 56-year-old woman on maintenance hemodialysis for 20 years for IgA nephropathy has had SSc for 10 years after developing digital ulcers. One year before presentation, she developed steal

RATIONALE: Infective endocarditis (i.e.) results from the combined effects of multiple factors, such as frequent vascular access procedures and impaired immune system function, and is relatively common among patients with uremia undergoing long-term hemodialysis. Myocardial infarction with nonobstructive coronary arteries (MINOCA) may present as an atypical clinical presentation, especially in patients with multiple comorbidities.
PATIENT CONCERNS: We report a rare case of a 52-year-old male who underwent regular hemodialysis using a tunneled-cuffed catheter and presented with persistent acut

BACKGROUND: Cholesterol crystal embolism syndrome (CCES) represents a catastrophic complication of endovascular procedures. While mechanical trauma during left atrial catheter ablation acts as a known trigger, the specific embolic risks associated with Vein of Marshall ethanol infusion (VOM-EI) remain unexplored.
CASE PRESENTATION: A 66-year-old male with severe atherosclerosis and chronic kidney disease underwent persistent atrial fibrillation ablation involving VOM-EI (8 mL of ≥ 99.7% dehydrated ethanol). Six hours post-procedure, he developed extensive, non-pruritic livedo reticularis. He

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Hemodialysis for chronic kidney disease (CKD) relies on vascular access (VA) devices, such as arteriovenous fistulas (AVF), grafts (AVG), or catheters, to maintain blood flow. Nonetheless, unpredictable progressive vascular stenosis due to neointimal formation or complete occlusion from acute thrombosis remains the primary cause of mature VA failure. Despite emergent surgical intervention efforts, the lack of a reliable early detection tool significantly reduces patient outcomes and survival rates. This study introduces a soft, wearable device that continuously monitors blood flow for early de

Tunneled dialysis catheters (TDCs) are typically the vascular access of choice for critically ill patients in need of urgent dialysis for prolonged periods of time. The standard of practice is to insert these catheters under fluoroscopic guidance to ensure proper catheter placement in the cavo-atrial junction or right atrium. However, there is increasing interest in bedside placement of TDCs without fluoroscopic guidance in patients deemed too critically ill to be transported to the fluoroscopy suite or operating room. Our aim was to systematically review the literature on bedside placement of

Although use of the autogenous arteriovenous fistula is the first choice for vascular access for hemodialysis, the tunneled cuffed catheter (TCC) remains an important alternative method that is applied widely in maintenance hemodialysis for patients with end-stage renal disease. In addition to common complications, such as thrombosis, infection, formation of a fibrin sheath, or central vein stenosis, TCC dysfunction, such as kinks, cuff detachment, or mechanical destruction, can be easily overlooked. The reasons for these clinical problems are multifactorial and include the insertion handling,

INTRODUCTION: Non-tunneled short-term hemodialysis (STHD) catheters are essential for managing acute kidney injury (AKI) and other conditions requiring temporary vascular access. Despite their widespread use, these catheters carry inherent risks, including infection and mechanical complications. This study evaluates the impact of catheter design features, such as lumen number, material, and configuration, on infection-related events in STHD catheters.
METHODS: A retrospective analysis was conducted using de-identified medical records from the Truveta database, comprising 3546 catheter inserti

Vascular access is required to draw the patient's blood into the dialysis machine and return the filtered blood to the patient during hemodialysis to treat end-stage renal disease. The most reliable vascular access is the arteriovenous fistula (AVF), which unfortunately may develop significant stenosis or obstruction as a major complication. To evaluate the AVF geometry for potential pathological features, this study aims to develop and validate a free-hand 3D ultrasound imaging system using conventional 2D ultrasound scanning with scanner motion data from an electromagnetic (EMT) sensor to sp

BACKGROUND: Current practice for managing acute myocardial infarction complicated by cardiogenic shock (AMI-CS) involves culprit-only revascularization with percutaneous coronary intervention (PCI). However, 50% of these patients present with multi-vessel disease (MVD). In the current era with the availability of more temporary mechanical circulatory support options, the decision to limit PCI to culprit only vessel remains controversial.
METHODS: Data from Nationwide Readmission Database between 2016 and 2021 was analyzed for patients over 18 years diagnosed with AMI and CS who underwent PCIs

PURPOSE: Facial nerve paralysis (FNP) may result in a range of ocular sequelae. Lower eyelid retraction and paralytic ectropion due to FNP contributes to lagophthalmos and sight-threatening exposure keratopathy. The pathophysiology of lower eyelid retraction secondary to FNP involves lateral canthal tendon laxity, middle lamella defect, and malar descent. The aim of this study is to describe a series of patients with FNP whose lid retraction was treated with a combined dermis spacer graft, lateral canthopexy and subperiosteal midface lifting.
METHODS: The study design was a retrospective non-

Vascular injury such as central venous stenosis (CVS) is a common complication in hemodialysis patients with central venous catheters (CVCs), yet the impact of the microstructure and partial physic characteristics of catheter surface on the chronic injury of central vein has not been elucidated. In this study, the microscopic morphology of tips and bodies of six different brands of polyurethane CVCs was observed and their roughness was assessed. Subsequently, an in vitro model was established to measure the coefficients of friction (COF) between CVCs (tips and bodies) and the vena cava intima

The dialysis catheter indwelling in human bodies has a high risk of inducing thrombus and stenosis. Biomechanical research showed that such physiological complications are triggered by the wall shear stress of the vascular vessel. This study aimed to assess the impact of CVC implantation on central venous haemodynamics and the potential alterations in the haemodynamic environment related to thrombus development. The SVC structure was built from the images from computed tomography. The blood flow was calculated using the Carreau model, and the fluid domain was determined by CFD. The vascular wa

Arteriovenous grafts (AVGs) provide vascular access for hemodialysis in patients with end-stage renal disease (ESRD). However, vascular access thrombosis often occurs, requiring frequent reinterventions to maintain access patency. This report describes the successful use of the InThrill Thrombectomy System (Irvine, CA: Inari Medical) for macerating and removing thrombus from an occluded and heavily thrombosed AVG. A 47-year-old male was sent to our institute for a thrombosed right upper extremity arteriovenous access with a HeRO graft (South Jordan, UT: Merit Medical). The patient underwent in

KEY POINTS: Replacement strategies for hemodialysis catheters with mechanical failure differed widely between services, which suggests clinical equipoise. For mechanical hemodialysis catheter failure, exchange did not result in more dysfunction or infection than removal and separate replacement. In Australia, infected catheters were almost universally removed and then replaced through a separate tunnel tract.
BACKGROUND: Tunneled hemodialysis catheters often have infectious or mechanical complications that require unplanned removal and replacement, but the optimal replacement strategy is unkn
