Factors determining a low dose of haemodialysis as measured by ionic dialysance in critical patients with acute kidney injury.

BACKGROUND: Estimating the dialysis dose is a requirement commonly used to assess the quality of renal replacement therapy (RRT) in patients with chronic kidney disease (CKD). In patients with acute kidney injury (AKI), this value is not always evaluated and it has been estimated that the prescribed dose is seldom obtained. Reports addressing this issue in AKI individuals are scarce and most have not included an adequate number of patients or treatments, nor were patients treated with extended therapies. Kt values obtained by the ionic dialysance method have been validated for the evaluation o

Treating thrombotic prosthetic arteriovenous access with cross-balloon occlusive thrombolysis and angioplasty.

OBJECTIVE: Clot embolism remains a concern due to flushing of clot and thrombolytic agent centrally in the process of percutaneous pharmacomechanical thrombolysis (PMT) for a thrombosed prosthetic arteriovenous access (PAVA), which might be reduced by a modified technique. METHODS: We retrospectively review this modified technique that uses two balloon-catheters in crisscross fashion and occludes both ends of PAVA during thrombolysis. Underlying stenotic lesions were dilated simultaneously with balloon angioplasty when needed. RESULTS: Among the 23 patients treated, 21 (91.3%), 10 (43.5%), a

Hemodialysis access usage patterns in the incident dialysis year and associated catheter-related complications.

BACKGROUND: Hemodialysis (HD) access is considered a critical and actionable determinant of morbidity, with a growing literature suggesting that initial HD access type is an important marker of long-term outcomes. Accordingly, we examined HD access during the incident dialysis period, focusing on infection risk and successful fistula creation during the first dialysis year. STUDY DESIGN: Longitudinal cohort. SETTING & PARTICIPANTS: All US adults admitted to Fresenius Medical Care North America facilities within 15 days of first maintenance dialysis session between January 1 and December 31,

How to deal with dialysis catheters in the ICU setting.

Acute kidney insufficiency (AKI) occurs frequently in intensive care units (ICU). In the management of vascular access for renal replacement therapy (RRT), several factors need to be taken into consideration to achieve an optimal RRT dose and to limit complications. In the medium and long term, some individuals may become chronic dialysis patients and so preserving the vascular network is of major importance. Few studies have focused on the use of dialysis catheters (DC) in ICUs, and clinical practice is driven by the knowledge and management of long-term dialysis catheter in chronic dialysis

Case study: first implantation of a frozen, devitalized tissue-engineered vascular graft for urgent hemodialysis access.

Previously we reported on the mid- to long-term follow-up in the first clinical trial to use a completely autologous tissue-engineered graft in the high pressure circulation. In these early studies, living grafts were built from autologous fibroblasts and endothelial cells obtained from small skin and vein biopsies. The graft was assembled using a technique called tissue-engineering by self-assembly (TESA), where robust conduits were grown without support from exogenous biomaterials or synthetic scaffolding. One limitation with this earlier work was the long lead times required to build the co

[Complications after placement of peritoneal catheter].

INTRODUCTION: Peritoneal dialysis is one of the modalities used for treatment of end-stage chronic kidney failure. Nowadays, this method is complementary to haemodialysis and renal transplantation. Owing to the rich vascularization of the peritoneum, it is used in the processes of osmosis and diffusion, enabling the removal of uremic material from the body. The procedure includes introduction of peritoneal fluid via the peritoneal catheter. COMPLICATIONS: The catheter is placed through the anterior abdominal wall with its tip positioned in the small pelvis. There are several techniques for ca

Mechanical lock of the lumen: researching towards new approaches for permanent vascular access.

PURPOSE: Theoretical proposals for two new vascular access devices (a central venous catheter (CVC) and a peripheral vascular access system, PVAS) are presented in this article. METHODS: The new CVC concept is based on a mechanical obturator used for CVC locking. Compared to conventional locks, it should, theoretically, allow the reduction of bacterial contamination, biofilm and thrombotic formation. A new tunneling technique, based on a "rigid tunnel sheath" providing a more stable connection, as compared to a traditional CVC cuff, and an increasingly protected exit site, allows CVC changing

Subclavian vein dialysis access catheter-complications are low.

Internal jugular vein has been recommended as the vascular access for haemodialysis because it is associated with less thrombotic/stenotic complication. Despite this recommendation, subclavian vein access is still being frequently used. This paper aims at reviewing the rate of complications associated with temporary double lumen subclavian vein dialysis access catheter in a personal series. It is a prospective observational study. Patients undergoing haemodialysis through a temporary double lumen subclavian catheter, all inserted by author, in the haemodialysis unit of B and B Hospital, have b

A novel fluoroscopic-assisted balloon thrombectomy technique for thrombosed hemodialysis prosthetic grafts [corrected].

BACKGROUND: Previous studies have shown that stenosis of the arterial anastomosis of thrombosed hemodialysis (HD) grafts, unmasked after conventional thrombectomy, very often necessitate subsequent arterial angioplasty. The aim of this study was to describe a novel fluoroscopic-assisted balloon thrombectomy technique which permits simultaneous arterial angioplasty (should this is required) for thrombosed HD grafts. METHODS: Thirty patients with 36 thrombotic episodes of their prosthetic HD grafts participated in this study. A balloon angioplasty catheter is placed beyond the arterial anastomo

Radial artery approach for endovascular salvage of occluded autogenous radial-cephalic fistulae.

BACKGROUND: The endovascular salvage of occluded autogenous radial-cephalic fistulae is a more challenging procedure than that for stenotic fistulae. To obtain an access to the fistula is one of the keys to success. Both retrograde venous approach and brachial artery approach have some disadvantages. The radial artery approach has been used in the endovascular therapy of fistula dysfunction, but few data focused on their feasibility and safety for the totally occluded fistulae. METHODS: We retrospectively reviewed the patients with occluded autogenous radial-cephalic fistulae receiving endova

Mechanical complications of long-term Tesio catheters.

Vascular access catheters such as Tesio-Caths are preferentially inserted in the internal jugular vein and serve as access for hemodialysis. Complications related to the removal of these types of lines are uncommon. We report four patients in whom the tip of the Tesio-Cath broke and was left stuck in the superior vena cava. Although there is no defined limit to the maximum length of stay of vascular access catheters for dialysis, the possibility of catheter entrapment should be considered. It remains to be determined whether removing Tesio-Caths every 16- 18 months is beneficial in avoiding th

Zymosan, but not lipopolysaccharide, triggers severe and progressive peritoneal injury accompanied by complement activation in a rat peritonitis model.

Fungal peritonitis is an important complication in peritoneal dialysis patients; either continuous or recurrent peritonitis may enhance peritoneal damage. Even when the peritoneal dialysis catheter is removed in patients with fungal peritonitis, peritoneal fibrosis can progress and evolve into encapsular peritoneal sclerosis. It is unclear why fungal infections are worse than bacterial in these respects. Zymosan is a cell wall component of yeast that strongly activates the complement system. In this study, we compared the effects of zymosan and bacterial LPS on peritoneal inflammation in a rat

A modified supraclavicular approach for central venous catheterization by manipulation of ventilation in ventilated patients.

BACKGROUND: Because of overuse and multiple implantations of hemodialysis catheters through internal jugular or subclavian vein (SCV) in patients with chronic hemodialysis, these veins often become stenotic or occlude, therefore necessitating alternative access. We introduce a new technique in ventilated patients for placement of tunneled cuffed chronic hemodialysis catheter: modified supraclavicular approach by cease of ventilation. METHODS: Patients who received implantation of the tunneled cuffed chronic hemodialysis catheters by supraclavicular approach were collected from February 2003 t

Fluid mechanics and clinical success of central venous catheters for dialysis--answers to simple but persisting problems.

Over 60% of patients initiating chronic hemodialysis in the United States have a chronic central venous catheter (CVC) as their first blood access device. Although it would be better if these patients started dialysis with fistulas, the CVC is used because it is a reliable and relatively safe method for obtaining blood access over a period of months. Drawing blood from a vein at 300-400 ml/minute is a relatively delicate and somewhat unpredictable process, and there is always a tendency for the vein wall to draw over the arterial tip and obstruct flow. Several methods have been employed to min

Hemodialysis in children weighing less than 15 kg: a single-center experience.

Despite significant technical improvements, hemodialysis in infants with end-stage renal disease (ESRD) is still associated with significant morbidity and mortality. The files of patients weighing less than 15 kg with ESRD who were treated with hemodialysis at our institute between 1995 and 2005 were reviewed for background and treatment characteristics, morbidity and outcome. The study group included 11 patients aged 7-75 months (mean 34.2 months) weighing 7.2-14.9 kg (mean 10.9 kg). Mean duration of dialysis was 11.3 months. Vascular access posed the major problem. Ten patients were dialyzed

Outcome and survival of temporary hemodialysis catheters: a prospective study from a single center in Iraq.

BACKGROUND: The use of temporary hemodialysis catheters (THC) has facilitated the delivery of hemodialysis to patients lacking functional vascular access. However, the use of these catheters is often associated with infections or mechanical complications. METHODS: A review of experiences at a tertiary referral center in Iraq with the use of 128 THC catheters in 103 patients was undertaken over one year, to identify indications for use and outcomes and issues limiting survival. RESULTS: The indications for insertion of THC included the following: as acute dialysis access for patients with chr
