Fourteen years of hemodialysis with a central venous catheter: mechanical long-term complications.

The ideal dialysis access ensures adequate blood flow for dialysis, has a long life, and is associated with a low complication rate. Although no current type of access fulfills all these criteria, the native arteriovenous fistula (AVF) is close to doing so. Unfortunately, various kinds of vascular access (VA) are becoming more and more necessary to enable hemodialysis (HD). The central venous catheter (CVC), which is associated with higher morbidity and mortality, could be the only viable option to maintain permanent VA. We report an unusual complication in a patient, a 74-year-old female, who

[Are permanent catheters a safe vascular access in chronically hemodialysed children?].

UNLABELLED: The paper's aim is an analysis of the complications related to permanent catheters used as a vascular access in chronically hemodialysed (HD) children. From 1998 to 2005. 34 surgically implanted permanent catheters were used as a vascular access for HD in 21 patients at average age of 13,6 (range 5-26) yrs. The catheters were placed mainly into right internal jugular vein or less often into left i.j.v., left subclavian v. and femoral veins. In 12 patients only one catheter was used for HD, in the others--the catheters had to be replaced. The average patient's observation period was

A new mechanical device for declotting of hemodialysis access grafts: first clinical experience.

PURPOSE: To review the early clinical experience with a new mechanical thrombectomy device for declotting polytetrafluoroethylene (PTFE) hemodialysis access (HDA) grafts. METHODS: The XTD is a 6-F introducer-compatible device that pulverizes clot with a rotating curved tip driven by a flexible spiral shaft. Fragments are aspirated into a collection container. Fifty patients (32 women; mean age 66 years) with clotted 6-mm polytetrafluoroethylene HDA grafts were treated at 2 medical centers in a study spanning 22 months. In all, 59 procedures were performed on an outpatient basis under monitore

Mechanical thrombectomy of occluded hemodialysis native fistulas and grafts using a hydrodynamic thrombectomy catheter: preliminary experience.

The purpose of this study was to evaluate the efficacy and safety of a new hydrodynamic percutaneous thrombectomy catheter in the treatment of thrombosed hemodialysis fistulas and grafts. Twenty-two patients (median age: 47 years; range: 31-79 years) underwent mechanical thrombectomy for thrombosed hemodialysis fistulas or polytetrafluoroethylene (PTFE) grafts. In all cases, an Oasis hydrodynamic catheter was used. Five patients had native fistulas and 17 had PTFE grafts. Six patients required repeat procedures. All patients with native fistulas and 15 of the 17 with PTFE grafts also underwent

Efficacy and safety of femoral vascular access for peripheral blood stem cell (PBSC) collection.

Central venous catheters are frequently used in leukapheresis to provide high flow rates. The most common locations are the subclavian or jugular vein, but insertion-related complications and inadequate flow are frequent problems. Experience using femoral venous access is limited, because this has been discouraged due to the high incidence of infectious or thromboembolic complications. We evaluated the safety and efficacy of 108 short-term femoral venous dialysis catheters used for the collection of peripheral blood stem cells (PBSCs). All catheters were placed by a member of the dialysis unit

Use of mechanical thrombectomy for declotting hemodialysis grafts.

Preservation of hemodialysis graft function is a high priority. Graft thrombosis is a common problem that can be treated percutaneously using a variety of methods, including pharmacological thrombolysis, balloon thrombectomy, and mechanical thrombectomy. These treatments provide similar clinical efficacy and primary patency rates, but differ in their risk for potential complications such as bleeding and pulmonary microembolization. The rheolytic thrombectomy catheter is discussed as an option for hemodialysis graft declotting. Rheolytic thrombectomy was found to offer rapid and effective treat

[Development of peritoneal dialysis at Rijeka Clinical Hospital Center].

The dialysis population is steadily rising as a consequence of the growing incidence of terminal renal failure patients and lack of organs for transplantation. Peritoneal dialysis (PD) has become an established form of renal replacement therapy. The development of new methods, techniques, PD fluids and catheters has significantly lowered the incidence of complications and increased the use of PD throughout the world. The development of PD at Rijeka University Hospital Center, the incidence of PD patients, their underlying renal disease leading to terminal renal failure, demographic characteris

Transrenal arteriovenous dialysis graft creation: survival and patency in a swine model.

PURPOSE: To investigate transrenal arteriovenous graft creation in an animal model and to report survival and patency. MATERIALS AND METHODS: Arteriovenous grafts were created from the renal artery to the renal vein in 10 swine. Renal access was accomplished with a combined percutaneous and transvascular approach. A reinforced 6-mm polytetrafluoroethylene conduit was tunneled between both flank access points, and stent-grafts were deployed from each of the renal origins into the conduit. Clopidogrel (10 mg per kilogram of body weight) was administered intravenously during the procedure, follo

Non-traumatic vascular emergencies: management of occluded hemodialysis shunts and venous access.

Shunt thrombosis is a relatively frequent complication of implant grafts but less common in native fistulas; both may undergo mechanical recanalization without thrombolysis. While in native fistulas, simple balloon angioplasty is a very useful tool for short-segment thrombosis, extensive thrombosis in native veins or grafts requires use of different mechanical devices or aspiration catheters. In venous access thrombosis, thrombolysis or fibrinous sheath stripping are percutaneous options.

[Dysfunctioning of central venous catheters: role of the radiologist].

Malfunctioning long-term central vein catheters are related to malposition, obstruction, mechanical problems and occlusion of the catheters. The diagnosis and the management of these disorders are based upon chest film examination, catheter or venous enhancement with contrast media. Vascular interventional radiology plays a major role in low invasive management of malfunctioning central vein catheters in hemodialysed patients: transfemoral malpositioned catheter manipulations, fibrin sleeve stripping, infusion of urokinase.

Percutaneous interventions for vascular dialysis access.

A prerequisite for the long-term patency of vascular access is early detection of a failing shunt, i.e., at the stage of stenosis. Fistulography performed to locate the lesion is followed by percutaneous transluminal angioplasty using a balloon catheter. The procedure takes 30-60 minutes to complete; the clinical success rate varies between 85% and 98%. A more serious condition of failled vascular access is its occlusion secondary to thrombosis. The high costs of hospitalisation and treatment of thrombosed shunts require effective yet less invasive outpatient procedures. Recent years have seen

Angio-access for haemodialysis--current perspective.

Vascular access for haemodialysis has seen many developments in recent times. Double lumen catheters introduced into wide bore veins have replaced the traditional Scribner shunt as temporary access thus reducing the complications and morbidity associated with it. Cuffed tunnelled internal jugular catheters and synthetic arteriovenous (AV) grafts usually made of polytetrafluoroethylene are the new additions to the vascular access armamentarium, but the AV fistula introduced in 1966 still remains the life-line for chronic haemodialysis patient. However, in elderly and diabetic patients, syntheti

Contemporary treatment of thrombosed hemodialysis grafts.

Maintaining hemodialysis grafts remains a difficult problem. Before the early 1990s, graft declotting was usually performed in the surgical suite. Percutaneous declotting has been evolving since the mid-1980s. Initially, a low-dose thrombolytic infusion of streptokinase through a single catheter was used. Crossing catheters with a higher-dose infusion of urokinase was then introduced. This technique was modified with the adjunctive use of pharmacomechanical techniques with the use of compliant balloons and the adjunctive use of heparin. The advent of the "lyse-and-wait" technique provided a si

Permanent tunneled silicone central venous catheters for autologous PBPC harvest in children and young adults.

Children with high risk malignancies are usually given permanent (Hickman-type) tunneled silicone rubber central venous catheters (silicone CVCs) for the administration of chemotherapy. In the past, these children received an additional short-term polyurethane dialysis CVC for stem cell apheresis. To avoid placement of an additional short-term CVC, we started in 1995 to use pre-existing silicone CVCs for PBPC harvests. From May 1996 to February 1999 we evaluated 165 harvests in 37 children and 14 young adults (16-28 years) treated with high-dose chemotherapy and stem cell support, comparing CD
