Dialysis in neonates with inborn errors of metabolism.

BACKGROUND: Certain inborn errors of metabolism become manifest during the neonatal period by acute accumulation of neurotoxic metabolites leading to coma and death or irreversible neurological damage. Outcome critically depends on the immediate elimination of the accumulated neurotoxins. Recent technological progress provides improved tools to optimize the efficacy of neonatal dialysis. METHODS: We report our experience with continuous venovenous haemodialysis (CVVHD) in six neonates with hyperammonaemic coma due to urea-cycle disorders or propionic acidaemia and in one child with leucine ac

Modified use of the arrow-trerotola percutaneous thrombolytic device for the treatment of thrombosed hemodialysis access grafts.

PURPOSE: To assess the safety and efficacy of using the Arrow-Trerotola percutaneous thrombolytic device (PTD) as the sole means of mechanical thrombolysis in hemodialysis access grafts, including in situ treatment of the arterial plug. PATIENTS AND METHODS: Fifty consecutive patients (22 women, 28 men; mean age, 58 years; mean graft age, 29 months), in whom mechanical thrombolysis of a thrombosed hemodialysis access graft using the PTD was planned, were included in the study. In all patients, the PTD was used to treat the arterial plug in situ at the arterial anastomosis, instead of using a

Peritoneal dialysis for acute renal failure in infants: a comparison of three types of peritoneal access.

Peritoneal access for peritoneal dialysis (PD) poses a significant problem in infants due to their small size and can result in considerable morbidity and occasional mortality. This study was carried out to compare the complications associated with three different types of PH catheters for intermittent PD. A total of 79 session of PD were given to 51 infants with acute renal failure. Twenty-nine infants received 1, 18 received 2 and 2 infants received 3 and 4 sessions of PD, respectively. For PD access an intravenous cannula was used in 36, stylet catheter in 18, and guide wire inserted femora

Indwelling silicone femoral catheters: experience of three haemodialysis centres.

The aim of this study is to describe the experience of three haemodialysis centres using indwelling femoral silicone catheter (model SSL 1220M, Medcomp, USA) in 55 patients, three with acute renal failure, one requiring plasmapheresis, and 51 with chronic renal failure but no other available vascular access. Sixty-four catheters were in place for a mean duration of 41.5 +/- 30 days. The rate of catheter-related complications, including mechanical problems, thromboses, and infections was low and they were never life-threatening. The results of the study suggest that femoral cannulation with mod

[Femoral indwelling silicone catheter: experience of three hemodialysis centers].

The use of femoral vein for temporary access in hemodialysis patients is still considered as a slightly desirable route. However recent technical improvements have made this approach more reliable because the new femoral catheters can be left in place for a long time and used for ambulatory treatment. We describe the experience of three hemodialysis centres with temporary indwelling femoral catheter made of silicone (SSL 1220 M, Medcomp) in 55 patients: 3 patients with acute renal failure, 1 requiring plasmapheresis and 51 with chronic renal failure but no other available vascular access. [Mechanical thrombolysis with the Amplatz thrombectomy catheter].

PURPOSE: To determine the efficiency of the Amplatz thrombectomy device (ATD) in thromboembolic occlusions of lower limb arteries and haemodialysis shunts. PATIENTS AND METHODS: 9 patients (mean age 61 years) with either occluded native leg arteries (n = 7) or haemodialysis graft access (n = 2) were treated with the ATD. Mean length of occlusion was 10 +/- 4.47 cm. RESULTS: The occlusions were primarily successfully recanalized by ATD within 44 +/- 17 seconds. Remaining stenoses were treated with percutaneous transluminal angioplasty (PTA, n = 6) and an additional implantation of a vascular

Occluded hemodialysis shunts: Dutch multicenter experience with the hydrolyser catheter.

PURPOSE: To evaluate mechanical thrombectomy of occluded hemodialysis access shunts with a recently developed hydrodynamic device. MATERIALS AND METHODS: Sixty-five thrombosed hemodialysis access shunts were treated in 49 patients. The shunts were of three types: Brescia-Cimino fistulas (24 procedures), polytetrafluoroethylene (PTFE; Gore-Tex) loop grafts (18 procedures), and manufactured homologous vein loop grafts (23 procedures). Clots were removed by means of aspiration caused by the hydrodynamic effect of a high-velocity flow of saline through the catheter (Venturi effect). RESULTS: Suc

Hemodialysis vascular access: emerging concepts.

Vascular access complications remain a leading cause of morbidity and hospitalization in patients on chronic maintenance hemodialysis. Although arteriovenous fistulas remain the dialysis access of choice, an increasing percentage of patients are dialyzed using synthetic grafts or permanent catheters. The most frequent vascular access complications are the development of stenosis and of thrombosis. In recent years, increasing attention is being paid to screening techniques designed to detect vascular access stenosis prior to thrombosis. Aggressive screening techniques coupled with interventiona

Pulse-spray pharmacomechanical thrombolysis of thrombosed hemodialysis access grafts: long-term experience and comparison of original and current techniques.

OBJECTIVE: Pulse-spray pharmacomechanical thrombolysis is an evolving method for the treatment of vascular occlusions in which a highly concentrated fibrinolytic agent is injected as a high-pressure spray directly into thrombus. The purpose of this retrospective study was to analyze our long-term experience with this technique for the treatment of clotted hemodialysis grafts and to compare the efficacy and safety of the original and current methods. SUBJECTS AND METHODS: Over 6 years, 284 cases of dialysis graft thrombosis were considered suitable for treatment with pulse-spray thrombolysis.

Thrombosed hemodialysis grafts: percutaneous mechanical balloon declotting versus thrombolysis.

PURPOSE: To compare a technique of mechanical balloon declotting of thrombosed hemodialysis grafts with conventional pulsed-spray thrombolysis. MATERIALS AND METHODS: Forty patients had 53 episodes of graft thrombosis over a 19-month period. Twenty-nine grafts were randomly treated with thrombolysis with urokinase and 24 grafts with mechanical declotting by placement of crossed balloon catheters within the graft. Patency was determined by retrospective review of hemodialysis records. RESULTS: Successful hemodialysis for 1 week after the procedure was achieved in 21 (88%) of the 24 grafts tre

[Positioning of hemodialysis catheters after locating the internal jugular vein by echo-Doppler].

Despite significant progress, vascular access remains a major concern for nephrologists. For the last 6 years we have used as temporary vascular access silastic twin catheters implanted into the internal jugular vein based on Canaud's method. We have developed in 15 patients an ultrasound-guided method using an ultrasonic Doppler to improve access to the internal jugular vein, prior to its catheterization. Ultrasound assistance was provided by a 5MHz mechanical sectorscanning transducer (Toshiba 270A). The internal jugular vein and carotid artery were visualized. The skin overlying the interna

[Temporary vascular access: from peripheral to central, from temporary to permanent].

Temporary vascular access (TVA) is a basic requirement in clinical nephrology. TVA permits immediate and repeated hemodialysis for all type of ESRD patient. Going from peripheral to central and from temporary to permanent angioaccess are the two main trends observed in TVA in contemporary dialysis. Our experience over the last decade illustrates this TVA changes. On the one hand peripheral arterio-venous shunt has been forsaken in favour of percutaneous implanted central venous catheters. On the other hand, two types of central venous catheters have been used covering up our preferences: cathe

Mechanical versus pharmacomechanical thrombolysis for the treatment of thrombosed dialysis access grafts.

Pharmacomechanical thrombolysis offers the first practical approach for non-surgical therapy of thrombosed dialysis access grafts. This technique involves both lysis using a fibrinolytic enzyme and mechanical maceration of the clot. The technique can be accomplished in a short period of time, has a high degree of success and has a low level of complications. To evaluate the effectiveness of the mechanical aspects of this technique used alone without the lytic enzyme, a study was designed in which 103 cases of thrombosed PTFE grafts were randomly assigned to either a mechanical (M) group consis

Thrombosed dialysis access grafts: percutaneous mechanical declotting without urokinase.

PURPOSE: To evaluate percutaneous declotting of dialysis access grafts with available catheters without urokinase. MATERIALS AND METHODS: Thirty-four clotted grafts were treated in 24 patients. Clot was macerated and pushed into the central circulation with balloon catheters. RESULTS: Successful mechanical declotting was performed in all but two patients (94%). The procedure was abandoned after successful declotting in four patients with poor venous outflow, resulting in a 24-hour success rate of 82%. Mean total procedure time was 116 minutes. Eight grafts clotted within 1 week. Using succes

Permanent vascular access: a nephrologist's view.

Vascular access complications are the greatest cause of morbidity in hemodialysis patients in the United States. Although arteriovenous fistulas have been recommended as the preferred mode of vascular access, recent data indicate that the majority of patients on hemodialysis in the United States have prosthetic graft fistulas. The most frequent complications of prosthetic graft fistulas are thrombosis and stenosis. Hospitalization rates for fistula complications are higher in patients with diabetes mellitus and of black race. Pathogenesis of intimal hyperplasia may include elaboration of plate

Medical complications of spinal cord disease.

Spinal cord injury increases the risk of many life-threatening medical problems, including respiratory failure, pulmonary embolism, and renal failure. Respiratory failure results from paralysis of muscles of inspiration (which impairs oxygen transport to alveoli) and of expiration (which impairs cough and predisposes to pneumonia and atelectasis). Respiratory failure in patients with spinal cord injury can be prevented by proper positioning of the patient, training of ventilatory muscles, pulmonary toilet, and aggressive use of antibiotics and bronchodilators. When respiratory failure occurs,

Central venous catheter stiffness and its relation to vascular perforation.

Delayed central venous perforation is an uncommon but serious complication of central venous catheter insertion. An increase in catheter stiffness may have been responsible for our association of venous perforation with use of a guidewire insertion technique. A bench model was used to investigate the stiffness characteristics of thirty-four different types of catheters. The initial stiffness is poorly described by material or catheter gauge. A large range of values is seen between apparently similar catheters--the 16 gauge polyethylene catheter associated with two perforations at our instituti
