Hematologic complications and design challenges in cardiovascular devices.

In this review, we provide an engineering assessment of prevalent blood-wetted cardiovascular devices, their hematologic complications, and their interconnections to identify key areas for future research. In fact, the incidence of cardiovascular diseases has promoted the development of various devices to treat these conditions, including mechanical circulatory supports (mechanical pumps that assist or replace failing hearts), artificial valves to replace faulty heart valves, stents to relieve obstructions in blood vessels, vascular grafts to repair/reconstruct damaged vessels, patches to rein

Escalation of mechanical circulatory support in patients with STEMI-related cardiogenic shock: a post hoc analysis of the DanGer Shock trial.

AIMS: In STEMI-related cardiogenic shock, left ventricular unloading with a micro-axial flow pump (mAFP) improved survival in the Danish German (DanGer) Shock trial, though some patients in both groups required escalation to higher volume temporary mechanical circulatory support systems (tMCS). This study aims to investigate tMCS escalation strategies and outcome in non-comatose patients with STEMI-related cardiogenic shock. METHODS AND RESULTS: In this post hoc analysis of the DanGer Shock trial, patients in both study groups were stratified as escalated vs. not escalated to treatments other

Advanced Cardiac Imaging for Risk Prediction of Pacing-Induced Cardiomyopathy: A Narrative Literature Review.

Background/Objective: Pacing-induced cardiomyopathy (PICM) is a common complication of right ventricular (RV) pacing, affecting 6-25% of patients with frequent RV pacing, due to electrical and mechanical dyssynchrony. Certain clinical and electrocardiographic risk factors have been identified, including high RV-pacing burden and longer paced QRS, but their ability to predict the development PICM remains limited. Additionally, other forms of PICM have been described, including heart failure with preserved ejection fraction and RV failure. The goal of this narrative review is to summarize the cu

Echocardiographic Assessment Before, During, and After Impella Positioning: State of the Art.

Echocardiographic assessment is essential for evaluating patients with cardiogenic shock (CS) and determining their potential need for mechanical circulatory support (MCS) implantation. The use of Impella devices has increased significantly in recent years, paralleling the growing recognition of their hemodynamic benefits in selected patient populations. As the clinical experience with these devices has expanded, the need for a more standardized imaging approach has emerged. Both transthoracic echocardiography (TTE) and transesophageal echocardiography (TEE) play complementary roles in guiding

Bleeding and thrombotic events during micro-axial flow pump support: A European multicenter cohort study.

BACKGROUND: Bleeding and thrombosis are major complications of micro-axial flow pump (mAFP) support in cardiogenic shock (CS), yet contemporary data on their incidence, temporal trends, and prognostic impact remain unclear. METHODS: We retrospectively analyzed patients supported with mAFP between 2010 and 2023 across eleven high-volume European centers. Bleeding and thrombotic events during support were adjudicated and normalized to annual implant volumes. Kaplan–Meier analysis assessed 180-day survival, and multivariable logistic regression identified independent predictors of bleeding and th

Percutaneous Intervention in External Outflow Graft Obstruction of Magnetically Levitated Left Ventricular Assist Device: Long-Term Follow-Up and Quality of Life.

OBJECTIVES: External compression of the outflow graft causing obstruction (eOGO) is a potentially lethal complication in patients on long-term mechanical circulatory support with the HeartMate 3 (HM3, Abbott) left ventricular assist device (LVAD). This complication results from the build-up of gelatinous substance between the bend relief and outflow graft and can be resolved by percutaneous intervention, surgery, or transplantation. This single-centre follow-up study evaluated the suitability of percutaneous intervention as a treatment strategy and long-term outcomes of eOGO patients in terms

Beyond Continuous-Flow: The Next Wave of Fully Implantable LVADs and Total Artificial Hearts.

PURPOSE OF REVIEW: Ventricular assist devices (VADs) have evolved from a bridge-to-transplantation strategy into a cornerstone of long-term advanced heart failure management. While continuous-flow technology has dramatically improved survival and device reliability, the paradigm shift toward extended mechanical circulatory support has exposed critical unresolved challenges. This review examines current limitations and four transformative technological directions shaping the next generation of VAD therapy. RECENT FINDINGS: Four key areas are driving innovation in the field. First, growing evid

Local Anesthesia and Analgosedation for Surgical Micro-Axial Flow Pump Implantation.

Temporary mechanical circulatory support is recommended for cardiogenic shock management. Although surgical device implantation usually requires general anesthesia and orotracheal intubation with inherent hemodynamic risk, local anesthesia with analgosedation in spontaneously breathing patients offers a promising alternative. This retrospective, single-center study analyzed 374 patients receiving surgical implantation of a micro-axial flow pump (mAFP) between January 2023 and December 2024. Fifty (13.4%) of them were performed under local anesthesia. Of the 49 investigated patients, 79.6% were

Incidence, Predictors and Outcome of Vasoplegia After Left Ventricular Assist Device Implantation.

BACKGROUND: Vasoplegia after heart failure surgery has been associated with increased morbidity and mortality. The aim of this study was to evaluate the incidence and impact of postoperative vasoplegia in patients undergoing durable continuous flow left ventricular assist device implantation (cfLVAD). METHODS: All patients who underwent cfLVAD implantation were included in this analysis. Vasoplegia was defined as the need for norepinephrine ≥0.2 μg/kgBW/min and/or any vasopressin dose for ≥12 h starting within 72 h postoperatively without bleeding/pericardial tamponade requiring surgical revi

Escalation to transplantation in refractory stone heart syndrome: a case report of advanced mechanical circulatory support failure.

BACKGROUND: Stone heart syndrome was first described by Denton Cooley in 1972 and represents an extremely rare but severe complication following cardiac surgery. The condition is characterized by an irreversible, concentric, spastic contracture of the myocardium occurring during reperfusion which does not respond to either pharmacological or mechanical interventions. The underlying pathophysiology remains poorly understood, but an association with suboptimal cardioprotection and prolonged ischemia time has been suggested. CASE PRESENTATION: A 22-year-old otherwise healthy patient was admitted

Clinical outcomes in acute myocardial infarction cardiogenic shock patients supported with a high-flow, surgically implanted micro-axial flow pump.

OBJECTIVE: Impella micro-axial pumps are increasingly used in cardiogenic shock. Recent randomized controlled data showed a survival benefit with use of the Impella CP in ST-elevation myocardial infarction-related cardiogenic shock. Use of the Impella 5.5, a high-flow surgically implanted micro-axial pump, in patients with acute myocardial infarction cardiogenic shock remains largely unexplored. We analyzed data from the Surgical Unloading Renal Protection and Sustainable Support study, a prospective, multicenter, observational registry evaluating real-world outcomes in patients with acute myo

Emergency Transcatheter Edge-to-Edge Mitral Valve Repair as a Bail-Out Strategy After Failed NeoChord Surgical Repair in Acute Degenerative Mitral Regurgitation.

Relevant mitral regurgitation (MR) is associated with significant morbidity and mortality. Surgical mitral valve (MV) repair represents the treatment of choice for symptomatic severe degenerative mitral regurgitation (DMR) in patients with acceptable surgical risk. However, surgical failure and high perioperative risk may limit reintervention options, particularly in elderly patients with multiple comorbidities. In this context, transcatheter edge-to-edge mitral valve repair (M-TEER) may represent a viable alternative strategy, particularly in patient who underwent previous surgery. We report

Perioperative Mechanical Circulatory Support for Cardiac Assistance in Thoracic Surgery: A Scoping Review.

BACKGROUND: Patients with pre-existing severe cardiovascular comorbidities are often deemed ineligible for potentially life-saving thoracic surgeries and are referred to other conservative therapies. However, this patient population may theoretically benefit from the timely perioperative implantation of temporary mechanical circulation support (tMCS) to both mitigate the surgical stress and stabilize hemodynamics. We performed a scoping review to summarize the evidence regarding the use of tMCS in thoracic surgery. METHODS: We conducted a systematic search across PubMed/MEDLINE, EMBASE, Cochr

The Past, Present, and Promising Future of Direct Cardiac Compression Devices.

Direct cardiac compression (DCC) devices, under development as a new modality for mechanical cardiac support (MCS), offer several advantages over presently available forms of MCS. DCC devices avoid the blood contact obligatory with other implantable MCS devices, the complications associated with blood contact and hematologic incompatibility, such as thrombosis, stroke, and the need for anticoagulation are avoided, and DCC does not require vascular access eliminating challenges such as bleeding and extremity ischemia. Arterial pressure pulsatility is also maintained with DCC. Significant and un
