Complex surgical management of right-sided infective endocarditis following ventricular septal defect closure with residual shunt and ascending aortic pseudoaneurysm: a case report.

INTRODUCTION AND IMPORTANCE: Right-sided infective endocarditis is an uncommon condition, especially in patients with surgically repaired congenital heart disease. A residual shunt after ventricular septal defect closure can act as a substrate for late-onset infection, but such cases are rarely reported. High-velocity jet flow from a residual shunt may contribute to endothelial injury and subsequent infection. Furthermore, differentiating a late pseudoaneurysm at a prior cannulation site from an infectious aortic aneurysm is essential for guiding the appropriate treatment strategy. CASE PRESE

Infective endocarditis with aortic root abscess and septic coronary occlusion: Aortic allograft implantation and DOR ventriculoplasty in a redo-operation.

Our goal is to present how we handled a complex case of prosthetic aortic valve endocarditis that resulted in severe complications, including an aortic root abscess, complete heart block and coronary artery embolic occlusion leading to myocardial infarction and a left ventricular aneurysm. The patient needed multi-component surgery to treat the infection and address the ventricular issues. Our approach involved exposing the right axillary artery, performing a redo sternotomy, lysing pericardial adhesions and then establishing axillary and bicaval cannulation. Myocardial protection was achieved

Aortic root abscess in a high-risk case requiring modified hemi-UFO procedure with custom-made pericardial conduit - a case report.

BACKGROUND: The UFO procedure is an established surgical technique to treat extensive endocarditis of the aortic or mitral valve with involvement of the intervalvular fibrous body (IVFB). This technique can be used for radical resection of the whole infected tissue. Independently of the size of the infected region it is unavoidable to replace both valves. In this high-risk re-do case we performed a modified so-called hemi-UFO procedure with preservation of the mitral valve. We present a 71-year-old male patient initially diagnosed with severe stenosis of the aortic valve. An aortic valve repla

Lip piercing-related prosthetic aortic graft infection and mediastinitis in filaminopathy: A case report.

We report a rare case in which a lip piercing led to Staphylococcus aureus bacteremia and subsequent prosthetic aortic graft infection complicated by anterior mediastinitis in a patient with filaminopathy A. A 32-year-old woman, who had undergone valve-sparing aortic root and arch replacement with a hybrid thoracic stent graft 5 years earlier, presented 3 weeks after a lip piercing with fever, chest pain, and elevated inflammatory markers. Computed tomography revealed a periprosthetic fluid collection and mediastinal inflammation consistent with graft infection. Blood cultures grew methicillin

Fatal Aortic Regurgitation in Behçet's Disease: A Case Report Highlighting Pitfalls and Lessons in Preoperative Diagnosis.

INTRODUCTION: Behçet's disease (BD), a chronic multisystem inflammatory disorder, rarely involves the heart. Aortic regurgitation (AR) is the predominant valvular lesion. When AR precedes characteristic mucocutaneous symptoms, misdiagnosis and treatment delays often occur. CASE PRESENTATION: We, herein, report the case of a 37-year-old male presenting with isolated aortic regurgitation (AR) as the initial manifestation of Behçet's disease (BD). Initial echocardiography revealed severe eccentric AR with left coronary cusp prolapse and a vegetation-like lesion, raising suspicion of infective en

Re-operative hemi-commando operation without a homograft: an alternative approach for complex aortic root abscess.

The patient was in his late 70s with a prior history of aortic valve replacement, recent emergent percutaneous coronary intervention for the right coronary artery (RCA), and a chronic type A aortic dissection and a 6-cm ascending aortic aneurysm. He developed Staphylococcus epidermidis bacteraemia and was found to have an aortic root abscess. Echocardiography demonstrated dehiscence of the aortic bioprosthesis with an aortic root abscess and moderate-to-severe mitral regurgitation. Radical debridement was performed, including resection of the aorto-mitral curtain through an extended transsepta

Infective endocarditis treated surgically: correlation between imaging, and surgical findings.

Echocardiography is crucial in the diagnosis of infectious endocarditis (IE). We assessed the correlation between transesophageal echocardiography (TEE) and surgical findings. A retrospective study of IE patients who underwent surgery. Discrepancies between TEE and surgical findings were defined as ≥ 1 infected valves, vegetations and abscess, and vegetations size ≥5 mm. Descriptive statistics and regression analysis were performed. Eighty patients were included. Mean age was 55.03 years, 26.3% had prosthetic valves. S. aureus and S. viridans spp were identified in 22.5% and 20% of cases, resp

Complex infective endocarditis with perivalvular abscess and fistulation: two high-risk cases requiring extensive reconstruction and valve replacement.

BACKGROUND: Infective endocarditis (IE) complicated by aortic root abscess, fistulation and multivalve involvement is rare but carries high mortality. Fistulation from the left ventricular outflow tract (LVOT) to the right atrium (RA) represents an exceptionally uncommon sequela due to extensive perivalvular tissue destruction. Early surgical intervention remains crucial, but postoperative outcomes vary widely depending on host factors, organism control and the feasibility of durable reconstruction. CASE PRESENTATION: We report two cases of severe aortic valve endocarditis with aortic root ab

Bacteraemia and infective endocarditis following left-sided heart valve surgery.

BACKGROUND AND AIMS: In patients undergoing heart valve surgery, subsequent bacteraemia and infective endocarditis (IE) are feared events. Data on the incidence and bacterial microbiological etiology following left-sided heart valve surgery are sparse. METHODS AND RESULTS: Between 2010 and 2021, all patients undergoing left-sided valve surgery were identified using Danish nationwide registries. Incidence and type bacteraemia within 1-year post-surgery was analysed. Secondary outcome of interest was IE. Cumulative incidence curves were stratified for bacterial species and for subgroups of inte

Comparison between Duke, European Society of Cardiology 2015, International Society for Cardiovascular Infectious Diseases 2023, and European Society of Cardiology 2023 criteria for the diagnosis of transcatheter aortic valve replacement-related infective endocarditis.

AIMS: Transcatheter aortic valve replacement-related infective endocarditis (TAVR-IE) is associated with a poor prognosis. TAVR-IE diagnosis is challenging, and benefits of the most recent classifications [European Society of Cardiology (ESC)-2015, International Society for Cardiovascular Infectious Diseases (ISCVID)-2023, and ESC-2023] have not been compared with the conventional Duke criteria on this population. The primary objective was to compare the diagnostic value of the Duke, ESC-2015, ISCVID-2023, and ESC-2023 criteria for the diagnosis of TAVR-IE. The secondary objectives were to det

Pathology of Self-Expanding Transcatheter Aortic Bioprostheses and Hypoattenuated Leaflet Thickening.

BACKGROUND: Hypoattenuated leaflet thickening (HALT) is believed to reflect leaflet thrombosis; however, no systematic histological examination of HALT has ever been performed. The aim of this study was to evaluate histological findings of explanted self-expanding transcatheter aortic bioprosthetic valves from clinical trials and to compare microCT findings of suspected HALT with histology findings of valve thrombosis and its characterization over time. METHODS: A total of 123 self-expanding transcatheter aortic valves were collected through autopsy (n=89) or surgical explant (n=34) from 11 C

Impact of Chronic Inflammatory Diseases on Clinical Outcomes in Patients undergoing Aortic Valve Replacement: A Systematic Review and Meta-analysis.

Patients with autoimmune chronic inflammatory disease (CID) are at an increased risk of valvular heart disease, including aortic valve stenosis, insufficiency, or both. The impact of CID on the prognosis of surgical or transcatheter aortic valve replacement (AVR) remains unclear. This meta-analysis aims to evaluate the impact of CID on major clinical outcomes in this population. A comprehensive literature search of PubMed, Cochrane, and Embase databases was conducted to identify relevant studies. The primary endpoint was 30-day all-cause mortality between patients with CID and controls. Second

Dacron® Graft Kinking Following Ascending Aorta Replacement Is Not Only a Cosmetic Issue.

A 58-year-old man, who has undergone ascending aorta replacement, started to complain of pain in the lower limbs, shortness of breath, and progressive fatigue a few months after surgery. Transthoracic and transesophageal Doppler echocardiographies revealed a diseased bicuspid aortic valve and a subocclusive mass in the ascending aorta. Thoracic computed tomography angiography confirmed the presence of a subocclusive mass, pseudoaneurysm formation, and a distorted shape of the Dacron® graft. The patient underwent urgent surgery to remove the mass, which appeared to be a thrombus, and aortic val

Ruptured Root Abscess with Aortopulmonary Fistula Caused by Vancomycin-Resistant-Enterococcus Prosthetic Valve Endocarditis.

Vancomycin-Resistant-Enterococcus (VRE) prosthetic valve endocarditis leading to a ruptured aortic root abscess and an aortopulmonary fistula is rare. A 77-year-old woman with recent VRE prosthetic valve (TAVR) endocarditis being treated with ampicillin and daptomycin for two weeks presented with bradycardia and hypotension in the setting of marked hypokalemia. Her hospital course was complicated by acute kidney injury. On day two of her hospitalization, she developed expressive aphasia, and an MRI was performed, which showed a corona radiata stroke. A transesophageal echocardiogram was pursue

Cell-free mitochondrial DNA may predict the risk of post-operative complications and outcomes in surgical aortic valve replacement patients.

Surgical aortic valve replacement (AVR) is the standard of care for severe aortic valve disease; however, post-operative complications continue to pose challenges. Inflammation caused by increased release of cell-free mitochondrial DNA (cf-mtDNA) during surgery may be a contributing factor. The aim of this pilot study was to investigate the relationship between cf-mtDNA release and the risk of inflammatory-mediated post-operative complications in adult AVR patients. Plasma was collected from patients undergoing an AVR with cardiopulmonary bypass (CPB) at baseline, intra-operatively and post IC

Infective endocarditis with perivalvular abscess following sutureless valve implantation, successfully treated with aortic root reconstruction and biological Bentall procedure.

BACKGROUND: Aortic valve prosthetic infections can lead to the spread of infection, causing annular abscesses and annular destruction, which may require annular reconstruction and aortic root replacement. Reports on the infection and reoperation of sutureless valves are rare. Here, we present a case of infection involving a Perceval sutureless valve, which led to an annular abscess. CASE PRESENTATION: We report the case of a 73-year-old woman who had previously undergone sutureless valve implantation for severe stenosis of a bicuspid aortic valve. Approximately 23 months after surgery, the pa

Untargeted metabolomics reveals Syzygium cumini (L.) alleviates ER stress and redox damage via activation of ATF-6/CHOP/NF-κB signaling axis in diabetic rats.

UNLABELLED: This study reports the therapeutic potential of aqueous seed extract Syzygium cumini (AESC) in alleviating type 1 diabetes mellitus (T1DM) associated endoplasmic reticulum (ER) and oxidative stress in a streptozotocin (STZ)-induced rat model. The LC-MS/MS analysis of AESC revealed gallic acid, ellagic acid, quercetin, malic acid, and citric acid as major antidiabetic phytoconstituents. Administration of AESC (250 mg/kg) normalized fasting blood glucose levels to those of healthy control rats. Moreover, AESC increases antioxidant levels and downregulates ER and inflammatory markers.

Aortic Valve Infective Endocarditis with Root Abscess: Root Repair Versus Root Replacement.

BACKGROUND: Aortic valve infective endocarditis (IE) complicated by an aortic root abscess is a challenging problem that leads to increased morbidity and mortality. Aortic root repair or replacement are two potential treatment options. We aimed to compare patients undergoing aortic root repair or replacement with short- and mid-term outcomes. METHODS: Consecutive patients with active aortic valve IE complicated by aortic root abscess undergoing cardiac surgery from January 2012 to January 2022 were included. Patients receiving aortic root repair were compared to patients undergoing aortic roo

A rare case of splenic hematoma after percutaneous drainage of splenic abscess with infective endocarditis.

Infective endocarditis with splenic abscesses remains a complex and unusual clinical scenario. We present a rare case of splenic hematoma caused by percutaneous drainage of splenic abscess with infective endocarditis. After antibiotic therapy, mitral valve replacement combined with splenectomy was performed simultaneously. When infective endocarditis is complicated with splenic abscess, splenectomy rather than percutaneous drainage of the splenic abscess is preferred to lower the risk of splenic haemorrhage and eradicate the source of infection. The order of surgery is controversial and should
