We studied the frequency of venereal disease in 329 prostitutes working at massage parlours in Santiago. The mean age was 24 years and half of them had high school or university studies. In spite of frequent sexual contacts averaging 2.8 per day, the prevalence of significant venereal disease was not high in this group. Notably, AIDS infection was not detected. Vaginitis due to Trichomonas or Candida infection was the commonest problem and this was half as frequent among gum preservative users. We reviewed a 20-year experience with squamous carcinoma of the gums in 347 patients who received definitive therapy. More than three-quarters of the lesions involved the lower gum and all but 37 patients were previously untreated. The proportion of patients with localized tumors (N0) remained the same (64 percent). Surgery continued to be the treatment of choice in 97 percent of patients, but proportionately more patients had a more conservative procedure which preserved lower jaw continuity. Sixty-eight patients with adenoid cystic carcinoma (ACC) of the head and neck treated by radiation with or without surgery were reviewed. 78 patients with squamous cell carcinoma of the gum were treated in our hospital from 1964 to 1982. 54 were male and 24 female, the sex ratio was 2.3:1. The peak age ranged from 50 to 59 years. 19 (24%) patients had lesions of stages I-II and 56 (71.7%) of stage III. All the patients were treated with surgery only or radiotherapy plus surgery. One hundred six patients with carcinoma of the lower gum were treated with radiation (30 Gy/three weeks) and bleomycin (90 mg/three weeks), and the results were analyzed. Of these, 66 (62%) patients experienced a complete clinical response (CR), and 47 were observed with no further treatment. The local control rate of these patients was 31/47 (66%) at two years. Higher CR and local control rates of 43/61 (70%) and 24/33 (73%), respectively, were obtained in patients with small (T1, T2) tumors than in those with large (T3, T4) tumors [23/45 (51%) and 7/14 (50%), respectively].

In an effort to improve tumor control by radiation therapy, a treatment regimen consisting of concurrent combination of bleomycin (90 mg/3 weeks) and radiation (30 Gy/3 weeks) was applied. Between 1972 and 1981, 287 patients with squamous cell carcinoma in the oral cavity were subjected to this bleomycin-radiation combination regimen. All except 4 patients experienced marked response after treatment using the bleomycin-radiation combination alone. One hundred thirty-four patients (47%) obtained CR and 149 (53%) PR. Thirty-eight patients with hard palate carcinoma and 82 with upper gum carcinoma received primary therapy consisting of radiation therapy alone or in combination with surgery. Fifty-three per cent of the primary lesions were successfully controlled by initial radiation therapy. Radiation therapy did not seem to give inferior results compared with surgery in the treatment of the primary lesion. The most important determining factors in the prognosis were the presence of cervical metastases, and bone involvement.

A clinical study comparing the use of radiation to the combination of bleomycin and radiation was carried out in 61 patients with carcinoma of the lower gum. Myectomy and partial resection of the levator labii superioris promises a far better and direct approach to the problem of upper gum exposure during smiling. This procedure attempts to eliminate the cause of the deformity. The resected muscles are of the striated muscle group and are composed of single muscle fibers running the entire length of the trunk. Adequate resection of these muscles will thoroughly eliminate the regeneration of muscle, and thus a permanent correction is achieved. The resulting smile is very attractive and pleasing to both patient and surgeon.

Longitudinal records of 109 individuals born at full term and 30 pairs of twins have been used to study factors which determine the form of the upper gum pad at the age of six months and before the emergence of teeth. The contribution of heredity was found to be less than at birth, and that derived from the underlying teeth to have increased during the first six months of life. It is concluded that the shape of the alveolar process may be modified by physical forces such as sucking habits before teeth erupt into the mouth.

The records of 61 patients with a diagnosis of squamous carcinoma of the lower alveolar ridge were reviewed. The overall two-year survival was 67% with a local or regional failure of 5%. Three cases of congenital strand-like adhesions between the upper and lower gum pads associated with cleft lip and palate and lower lip fistulae are reported. A consideration of the literature would suggest that this may represent an autosomal dominant trait with variable penetrance and expressivity though a positive family history was found in only one case. The adhesions may have arisen by persistence of the oropharyngeal membrane or anomalous fusion of adjacent epithelial surfaces, the latter seeming rather more likely in the present series. The clinical implications are discussed.

With reference to 5 personal cases involving the mandible 3 times and the lower gum twice, the authors recall the principal characteristics of calcified odontogenic cysts: very little clinical manifestation frequently limited to simple tumefaction, well bounded radiological image with low density, cyst with an epithelial border comprized of calcification and ghost cells. Calcified odontogenic cysts which have a definite morphological likeness to Malherbe's epithelioma, also have the same benigh course as the latter.
