[Surgical therapy of urinary incontinence in the male].

From 1973 to 1990 a total of 340 male patients suffering from incontinence of various etiologies were treated by implantation of an artificial urinary sphincter. In 165 patients the sphincter was placed at the bladder neck, while 175 needed an implantation at the bulbous urethra. Because of the better results with regard to continence, we prefer the bladder neck implantation. The youngest patient treated was 12 years old and the oldest, 91. Most of the patients were between 40 and 80 years old. The highest continence rates were achieved in patients with postprostatectomy urinary incontinence.

Artificial urinary sphincters in children: radiographic evaluation.

Use of the artificial urinary sphincter to treat incontinence in children has increased over the past decade. The hydraulic fluid used in this device is radiopaque, and the radiologist is directly involved in its evaluation. Despite advances in design and surgical technique, mechanical failures and other complications occur. Our experience with 34 artificial sphincters implanted in 31 children during 1973-1983 is reviewed, giving a method of radiographic evaluation, diagnostic errors to avoid, examples of sphincter malfunction, and common complications.

Selection of patients for implantation of the Brantley Scott artificial urinary sphincter.

The Brantley Scott artificial urinary sphincter (AUS) is a safe, effective and relatively trouble-free device for the control of sphincter weakness incontinence. Suitable patients should have no other urodynamic dysfunction or have had such a dysfunction corrected or controlled. Patients with post-prostatectomy incontinence pose no problems in selection, although as a group they are the most prone to develop complications. Pure stress incontinence does not warrant implantation of an AUS except in very rare instances. In neuropathic dysfunction the AUS is one of several treatment options for th
