BACKGROUND: Elbow arthroplasty is an established treatment of distal humeral fractures not amenable to internal fixation. Total elbow arthroplasty (TEA) is the most common modality, but elbow hemiarthroplasty (EHA) is becoming more popular, and it is still unclear which option provides the best results. We hypothesized that EHA is associated with a lower revision rate than TEA, due to fewer mechanical complications.
METHODS: This study is a retrospective multicenter cohort study. We identified all patients with an acute distal humeral fracture treated with an elbow arthroplasty in Denmark in

PURPOSE: Monteggia-variant fractures involve a fracture of the proximal ulna combined with a radiocapitellar dislocation and/or additional injuries to the radial head. These injuries are associated with severe outcomes, high complication rates, and frequent need for revision surgery. This study aimed to investigate strategies for early revision following failed operative treatment of these injuries.
METHODS: We retrospectively included all patients over 18 years of age who underwent revision surgery within the first 3 months after failed initial treatment of a Monteggia-variant fracture due t

Several complications are associated with total shoulder arthroplasty (TSA), including glenoid loosening and joint instability. These complications are linked to imbalances in glenohumeral muscle forces, which cause eccentric loading that occurs on the outer edge of the glenoid component, rather than in the center of the glenoid, and can be damaging to the implant and destabilizing the joint. The purpose of this study was to develop musculoskeletal models of TSA patients using their skeletal geometry acquired from CT and their kinematics acquired with high-speed stereo radiography (HSSR) to pr

Chondrosarcomas of the pelvis represent a challenge in clinical management because surgical resection is the only effective therapeutic option, given the resistance to radiotherapy and chemotherapy of malignant chondroid tumors. Reconstruction after resection involves critical structures, such as the acetabulum or hip joint, and is a demanding surgical procedure with high morbidity. Complications are frequent due to the complex anatomy of the pelvis, which on the one hand makes it difficult to obtain adequate surgical margins, and on the other hand, the various implants designed for reconstruc

STATEMENT OF PROBLEM: Screw loosening is a common mechanical complication that can compromise the long-term success of implant-supported restorations. Implant-abutment connection design and cyclic loading influence load transfer, preload retention, and joint stability; however, their specific contributions to screw loosening under functional conditions remain unclear.
PURPOSE: The purpose of this systematic review and meta-analysis was to evaluate the effect of implant-abutment connection geometry on abutment screw stability under cyclic mechanical loading.

Minimally invasive surgery for hallux valgus correction has evolved from early techniques, which used minimal or no hardware, to the current generation of more advanced, fixated osteotomies. These modern techniques often incorporate various forms of fixation, with recommendations on screw placement to optimize outcomes. Specifically, it is recommended that the proximal screw in the transverse plane should purchase two cortices of the first metatarsal to ensure maximum stability. While the ideal position of the screw in the anteroposterior view has been well documented in the literature, there

INTRODUCTION: Limb-length discrepancies (LLDs) have traditionally been treated with external fixators, but magnetically driven intramedullary nails (MILNs) are increasingly favored for improved comfort and reduced infection risk. This study compared intramedullary (IM) versus extramedullary (EM) lengthening nails in pediatric patients with narrow femoral canals to evaluate mechanical axis deviation, nail bending, tourniquet time, and complications.
METHODS: A retrospective, single-center review was conducted of 75 pediatric patients who underwent femoral lengthening between 2015 and 2022, eac

Pretibial abscess is a rare but serious complication after anterior cruciate ligament (ACL) reconstruction. We report six cases of pretibial abscesses from 364 ACL reconstructions at our center between January 2022 and June 2023. Patients presented with poor wound healing, sinus tract formation, and magnetic resonance imaging showed fluid accumulation and bone marrow edema in the tibial tunnel but no deep infection. Bacterial cultures identified Pseudomonas aeruginosa and Burkholderia cepacia. Further investigation found contamination in the hexagonal reamers used for screw implantation, which

Trochanteric fractures in elderly patients are frequently treated with gamma intramedullary nails. Although commonly performed, this procedure can result in rare but severe complications such as visceral perforation. The case involves an elderly woman who suffered iatrogenic perforation of both the urinary bladder and small intestine during gamma nail insertion. The complication was not immediately recognized, and the patient presented several weeks later with severe sepsis and pelvic peritonitis. Imaging and cystoscopy confirmed the diagnosis. Emergency surgery allowed for small bowel resecti

Paediatric bones exhibit greater elasticity due to their distinct biomechanical properties, enabling them to absorb and dissipate mechanical forces more effectively than adult bones. This inherent flexibility provides increased resistance to fractures under comparable loading conditions. However, despite this resilience, high-energy trauma can still result in complex injury patterns. Fractures involving the physis are of particular concern, as they carry a significant risk of growth disturbances and long-term deformities. We present a rare case of a girl in early childhood (preschool) who susta

OBJECTIVES: Condylar head fractures are common in facial trauma and can result in significantly impaired mandibular function. Open Reduction and internal fixation overall produce better functional outcomes than closed treatment. Multiple methods of fixation have been proposed, including the use of biodegradable magnesium-based cannulated bone screws. This study aims to investigate the long-term efficacy of this fixation method using three-dimensional radiographic imaging and clinical outcomes.

Complications of transpedicular stabilizations of thoraco-lumbar burst fractures are presented on the basis of literature review and own experiences. Unstable thoraco-lumbar burst fractures create the most difficult biomechanical conditions for a stabilizer. A literature review was done to estimate the effectiveness of vertebral body height restoration and its maintenance, the effectiveness of transpedicular grafting, the fusion rate and the implant-related complications rate. Transpedicular stabilization systems demonstrate a marked stiffness in all directions which is greater than in the cas

BACKGROUND AND OBJECTIVE: The use of bone allograft reconstructions after tumor resection can introduce significant complications. Stable fixation is required to decrease the incidence of mechanical complications of segmental bone allografts. The purpose of the present study is to compare plating fixation methods of diaphyseal allografts after intercalary resection of the femur.
METHODS: We created four defined fixation models using plates and/or intramedullary polymethylmethacrylate (PMMA) to simulate typical bone tumor resection with intercalary allograft reconstruction. One angularly stabl

PURPOSE: In mechanically cardiopulmonary resuscitated (CPR) patients, chest compressions at the level of the 3rd to 5th rib on the sternum result in reproducibly similar injury patterns: parasternal osteochondral dissociation (OCS) on both sides in combination with a sternal fracture with or without an additional serial rib fracture in the anterolateral column (ALS). This injury biomechanically impairs physiological breathing, resulting in an inverse breathing pattern. Trauma patients, on the other hand, often show a mixed pattern depending on the location of the main energy. The aim of the st

Background and Objectives: Despite the well-established position of orthognathic surgery as a field of surgical treatment of deformities within the facial skeleton, it has not been possible to develop unanimous recommendations on how to approach fixation hardware after the healing period. In the absence of clear guidelines from opinion leaders and scientific societies on how to approach osteosynthesis after surgery, the decision to leave or remove fixation hardware is made individually by treatment centers, mostly based on their own experience. It is also important whether or not surgical proc

Cephalomedullary nails are widely used to fix unstable femoral trochanteric fractures nowadays. However, for reverse obliquity trochanteric (ROT) fractures, the fixation failure rate of existing cephalomedullary nails is high, resulting in many complications. Our team aimed to propose the modified intramedullary nail-II (MIN-II) to improve the fixation effects of ROT fractures and make biomechanical comparisons between MIN-II and three other cephalomedullary nails by finite element method. AO/OTA 31-A3.1 and 31-A3.3 ROT fracture models were established via a series of femoral CT data. Four cep

BACKGROUND: Periprosthetic fractures around the knee (TKAPF) often occur in old patients with concomitant diseases. Like any other fragility fracture, the aim of TKAPF management should be a patient-specific strategy considering clinical factors and associated comorbidities. The aim of the present study was to analyse if, in selected groups of unfit patients, osteosynthesis alone without revision even in cases of implant loosening (SOFCOT type 2 and 3) could be a good compromise to avoid invasive surgery and to reduce the mortality rate with reasonable results. The hypothesis was that for TKAP

Metal plates are commonly used for the fixation of metacarpal fractures, but are associated with complications such as tendon adhesions that impair finger mobility. AdhFix, a novel osteosynthesis material, may reduce these complications, but its biomechanical performance in the metacarpal region remains untested. This study aimed to evaluate internal bending moments during postoperative rehabilitation and assess the biomechanical potential of AdhFix in treating comminuted metacarpal shaft fractures using a human cadaveric model. In five cadaveric hands, unstable shaft fractures of the second,

INTRODUCTION: Total hip arthroplasty (THA) is a major success in the management of osteoarthritis and is increasingly performed worldwide. As a result, complications related to hip prostheses-particularly periprosthetic femoral fractures (PPF)-are becoming more frequent, occurring in 4-5% of THA cases. These fractures often require surgical intervention. However, the optimal treatment strategy remains controversial, especially for Vancouver B1 and C fractures with stable implants. The literature lacks sufficient comparative studies evaluating the benefit of cortical strut allografts in this co

Bone lengthening using a monolateral external fixator is an effective technique for treating femoral nonunion associated with limb length discrepancy (LLD). This method involves gradual femoral lengthening to correct shortening and rotational deformities, while promoting bone healing through compression at the nonunion site. We present a case series of three patients demonstrating the successful integration of monolateral external fixation with intramedullary nailing for oligotrophic nonunions (4-8 cm LLD). Preoperative planning includes assessment of limb length discrepancy, evaluation the ca
