Effects of Dementia on Adverse Outcomes in Geriatric Patients Undergoing Elective Total Hip Arthroplasty: Analysis of the US Nationwide Inpatient Sample.

BACKGROUND: Total hip arthroplasty (THA) significantly improves the quality of life of patients with end-stage osteoarthritis. Dementia, which is prevalent in geriatric THA recipients, is correlated with several adverse outcomes. This study further investigated the impact of dementia on perioperative outcomes (complications, mortality, skilled nursing facilities [SNFs] transfer, total cost, and length of hospital stay [LOS]) in geriatric patients undergoing elective THA. METHODS: This study retrospectively analysed data from patients aged 65 years and older who underwent elective THA in the N

Patello-femoral Tracking Optimization in Robotic-Assisted Total Knee Arthroplasty.

BACKGROUND: Robotic-assisted total knee arthroplasty (RA-TKA) allows for intraoperative component positioning to personalize alignment and gaps. However, traditional trochlear designs not optimized for patellar tracking in kinematic alignment result in femoral internal rotation (IR) relative to the surgical transepicondylar axis. We sought to determine the femoral component alignment's effect on patellar tracking in RA-TKA. METHODS: We retrospectively reviewed 932 RA-TKA cases performed from January 2023 to August 2024 using a computed tomography-based robotic platform with a single radius fe

Three-dimensional characterization of glenoid defects in failed shoulder arthroplasties.

BACKGROUND: Glenoid vault defects are often observed in revision shoulder arthroplasty, and improper management of glenoid bone loss during revision to reverse total shoulder arthroplasty (rTSA) increases the risk of complications. However, there is limited understanding of the types of glenoid vault defects that occur in patients. Therefore, the purpose of this study was to three-dimensionally characterize the underlying glenoid morphology of patients with failed shoulder arthroplasties subsequently undergoing revision arthroplasty to rTSA. METHODS: Computed tomography scans of 100 patients,

Cemented versus uncemented femoral stem fixation in hip arthroplasty for displaced femoral neck fractures in elderly patients: A systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: The optimal method of femoral stem fixation in hip arthroplasty for displaced femoral neck fractures in elderly patients remains controversial. While cemented stems provide immediate fixation, concerns persist regarding perioperative risks. Uncemented stems avoid cement-related complications but may be associated with mechanical instability in osteoporotic bone. METHODS: This systematic review and meta-analysis included randomized controlled trials comparing cemented and uncemented femoral stem fixation in elderly patients undergoing hemiarthroplasty or total hip arthroplasty for

Comparative performance of the risk analysis index versus traditional frailty measures in predicting outcomes following revision total hip arthroplasty for mechanical prosthetic complications.

BACKGROUND: Frailty assessment has become increasingly important in predicting surgical outcomes, particularly in complex revision procedures. Among patients undergoing revision total hip arthroplasty (rTHA) for mechanical prosthetic complications, optimal frailty stratification remains poorly defined. This study compared the Risk Analysis Index (RAI) and the modified 5-item frailty index (mFI-5) in predicting postoperative outcomes within this high-risk population. METHODS: We conducted a retrospective cohort study of 5631 adult patients undergoing rTHA for mechanical prosthetic complication

Predicting patellar kinematics and contact forces after TKA: a simulation study on quadriceps malalignment.

BACKGROUND: Patellofemoral complications remain a frequent cause of dissatisfaction following total knee arthroplasty (TKA), despite continuous advances in implant design and surgical techniques. Abnormal patellar tracking may lead to increased contact stresses, instability, and anterior knee pain, particularly in patients with preoperative quadriceps malalignment. Subject-specific assessment tools are therefore needed to improve surgical planning and postoperative outcomes. METHODS: A subject-specific multibody dynamics (MBD) framework was developed to simulate patellofemoral mechanics follo

Early complications after surgery for periprosthetic femoral fractures: incidence, risk factors, and clinical implications.

PURPOSE: Periprosthetic femoral fractures (PFF) are the second cause of revision surgery after total hip arthroplasty and occur predominantly in elderly, fragile patients. Despite high associated morbidity and mortality, data specifically focusing on early postoperative complications remain scarce. METHODS: A single-center retrospective study was conducted on 192 patients who underwent surgery for PFF between January 2010 and April 2023. Patients were treated by open reduction and internal fixation (ORIF, n = 75) or revision surgery (n = 117). All complications occurring within the first post

Internal Density Calibration in the Proximal Humerus to Estimate Bone Stiffness for Stemless Shoulder Arthroplasty.

End-stage osteoarthritis (OA) alters bone density in the humeral head, complicating implant fixation during stemless shoulder arthroplasty. Current preoperative assessments fail to consider the mechanical properties of bone directly supporting the humeral component. While in-scan phantom calibration is used to determine volumetric bone mineral density (vBMD), phantoms are rarely used clinically. An internal density calibration method has been developed, but not yet applied in the proximal humerus. This study examined correlations between phantom and internal density calibration, and between vB

From bench to bedside: a novel suture-augmented prosthesis for greater trochanteric fixation in osteoporotic hip arthroplasty.

BACKGROUND: Stable fixation of the greater trochanter during hip arthroplasty for unstable osteoporotic intertrochanteric fractures remains a significant challenge. Conventional techniques depend on securing the bone-implant interface, which is particularly compromised in osteoporotic bone. Here, we propose a novel conceptual approach that establishes a direct mechanical bridge from the abductor tendon to the prosthesis, thereby reducing reliance on the fragile bone-implant interface. METHODS: In this two-stage translational study, we progressed from biomechanical validation to clinical appli

Clinical Outcomes and Complications of Superior Capsular Reconstruction Versus Reverse Shoulder Arthroplasty for Massive Rotator Cuff Tears: A Meta-Analysis.

BACKGROUND: Irreparable rotator cuff tears without advanced arthritis are difficult to manage. Superior capsular reconstruction (SCR) preserves the native joint, while reverse total shoulder arthroplasty (rTSA) offers reliable function but with higher implant-related risks, especially in younger patients. This study compared outcomes and complications between SCR and rTSA. METHODS: This study was conducted following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 guidelines. PubMed, Scopus, and Ovid Medline were searched from inception to May 2, 2025, for stud

Open reduction and internal fixation vs acute total hip arthroplasty for geriatric acetabular fractures: A multicenter matched cohort study.

BACKGROUND: Optimal management of acetabular fractures remains controversial. Open reduction and internal fixation (ORIF) may be followed by post-traumatic degeneration and late conversion arthroplasty, whereas acute total hip arthroplasty (THA) may introduce implant-related risks. We compared short- and long-term outcomes after ORIF versus acute THA in a large, multicenter electronic health record cohort. METHODS: We performed a retrospective cohort study using the TriNetX Network. Adults with isolated, closed, acute acetabular fractures treated with either ORIF or acute primary THA were ide

Ablation, Osteoplasty, Reinforcement, and Internal Fixation Allows for Subsequent Hip Arthroplasty Reconstruction.

CASE: A 34-year-old woman with stage IV breast cancer presented with bilateral acetabular metastases, a right pathological femoral fracture, and a left subtrochanteric impending fracture. Imaging confirmed extensive metastatic disease of bone. A staged approach included prophylactic femoral nailing of the left femur, ablation, osteoplasty, reinforcement, and internal fixation (AORIF) of both acetabula, followed with a right hybrid arthroplasty and prophylactic fixation of the right femur. She regained independent ambulation, remained pain-free, and continued systemic therapy after 1 year. CON

How does a prior olecranon osteotomy impact the outcome of subsequent total elbow arthroplasty?

BACKGROUND: Olecranon osteotomy is a common surgical exposure technique used for internal fixation of distal humerus fractures. Unfortunately, retained ulnar hardware and potential osteotomy complications (malunion, nonunion, or extensor mechanism insufficiency) may negatively impact subsequent total elbow arthroplasty (TEA) if needed. Although TEA is a reasonable option for management of failed distal humerus internal fixation or posttraumatic arthritis, little is known about how prior olecranon osteotomy influences TEA outcomes. This study aimed to identify operative considerations, evaluate

Atraumatic anterior hip dislocation after cephalomedullary nailing for an intertrochanteric femoral fracture: a case report highlighting the importance of initial reduction and prevention of excessive telescoping.

BACKGROUND: Anterior hip dislocation after internal fixation of a proximal femoral fracture is extremely rare. Previously reported cases have mainly involved trochanteric fractures treated with a dynamic hip screw or sliding hip screw [1-3]. To our knowledge, atraumatic anterior hip dislocation after cephalomedullary nailing for an intertrochanteric femoral fracture has not been previously reported. CASE PRESENTATION: An 87-year-old woman sustained a left intertrochanteric femoral fracture (AO/OTA 31-A3) after a fall in her residential facility. Before injury, she ambulated with a walker. Her

Total Knee Arthroplasty in Postpolio Residual Paralysis: A Systematic Review and Pooled Analysis of Its Outcomes and Complications.

BACKGROUND: Total knee arthroplasty (TKA) in patients who have postpolio residual paralysis (PPRP) is technically challenging due to bone deformities, ligamentous laxity, and variable quadriceps muscle strength. Despite these challenges, TKA is increasingly being performed in this population, yet outcomes remain variably reported. This review aimed to systematically assess postoperative complications and revision rates following TKA in patients who had PPRP through a pooled analysis of available studies. METHODS: A systematic review was conducted in accordance with the Preferred Reporting Ite

Comparative Outcomes of Early Versus Late Conversion to Total Hip Arthroplasty Following Hip Fracture Fixation.

BACKGROUND: Hip fractures are prevalent orthopaedic injuries with substantial morbidity and mortality. Failed primary treatments of these fractures often necessitate conversion to total hip arthroplasty (THA), a complex procedure requiring extensive exposure and hardware removal. It poses major challenges and is associated with high rates of complications such as infection, dislocation, and periprosthetic fractures. Despite the documented risks, there are limited data comparing complication rates between early (within 1 year) and late (after 1 year) conversion THA (CTHA). This study aimed to c
