BACKGROUND: Pelvic discontinuity is a distinct form of bone loss, occurring in association with total hip arthroplasty, in which the superior aspect of the pelvis is separated from the inferior aspect because of bone loss or a fracture through the acetabulum. An iatrogenic avulsion of the greater trochanter occurring during prosthetic hip replacement after a fracture of the neck of the femur is a rare complication. The long-stem, long-neck endoprosthesis is used in the treatment of pertrochanteric hip fractures in the elderly. Between 1978 and 1984, 157 patients 70 years of age and over were treated at our clinic for fresh unstable pertrochanteric hip fractures. Fifty-five patients had internal fixation. Because of the high incidence of mechanical complications, the long spherostem endoprosthesis became the treatment of choice between 1980 and 1984. In a follow-up study of 239 consecutive patients with fractures of the femoral neck treated by six surgeons in one hospital by Pugh nailing, Austin Moore prosthesis, or Knowles pinning, 96 per cent could be followed for a minimum of two years or until a definitve result had been reached.
