Fixator-Assisted Plating and Nailing for Multiplanar Lower Limb Deformities in Adolescents: Indications, Technical Considerations, and Radiographic Outcomes.

BACKGROUND: Uncorrected lower limb deformities in adolescents, particularly those with severe multiplanar deviations, can lead to early joint degeneration and functional impairment. When growth modulation is no longer feasible, corrective osteotomy becomes necessary. This study aims to demonstrate the efficacy of fixator-assisted techniques-fixator-assisted locked plating (FALP) and fixator-assisted nailing (FAN)-in achieving accurate correction of complex femoral and tibial deformities across the frontal, sagittal, and torsional planes. METHODS: A prospective study was conducted involving 27

Clinical and radiological outcomes of using locked intramedullary nails in the treatment of severe frontal plane lower limb deformity in adolescents with hypophosphatemic rickets (mid-term results).

INTRODUCTION: Lower limb deformities in patients with hypophosphatemic rickets are multi-apical and require multiple osteotomies for correction. Intramedullary nails (IMNs) are used to fix multiple osteotomies. Authors of this studyaimed to assess the clinical and radiographic outcomes of using IMNs for correcting lower limb deformity in adolescents with hypophosphatemic rickets. METHODS: This prospective study included patients with hypophosphatemic rickets who underwent deformity correction between November 2020 and November 2023 using IMNs, with a minimum follow-up of 2 years. Clinical out

Integrated mechanical and metabolic limb salvage for advanced diabetic Charcot hindfoot collapse with non-traumatic calcaneal fracture: an 8-year follow-up case report.

BACKGROUND: Advanced Charcot neuro-osteoarthropathy involving the hindfoot is associated with severe deformity, instability, ulceration, infection, and major lower-limb amputation. Non-traumatic calcaneal fracture associated with diabetic Charcot hindfoot collapse is uncommon and difficult to treat because of neuropathy, impaired bone quality, tendon imbalance, soft tissue compromise, and a high risk of fixation failure. Reports describing long-term functional limb preservation in patients with advanced Charcot hindfoot collapse associated with severe metabolic bone impairment remain limited.

Angular position of proximal screw fixation during minimally invasive hallux valgus surgery in the sagittal plane.

Minimally invasive surgery for hallux valgus correction has evolved from early techniques, which used minimal or no hardware, to the current generation of more advanced, fixated osteotomies. These modern techniques often incorporate various forms of fixation, with recommendations on screw placement to optimize outcomes. Specifically, it is recommended that the proximal screw in the transverse plane should purchase two cortices of the first metatarsal to ensure maximum stability. While the ideal position of the screw in the anteroposterior view has been well documented in the literature, there

Comparative Outcomes of 8.5 mm Intramedullary Nails Versus Extramedullary Constructs for Femoral Lengthening in Pediatric Patients.

INTRODUCTION: Limb-length discrepancies (LLDs) have traditionally been treated with external fixators, but magnetically driven intramedullary nails (MILNs) are increasingly favored for improved comfort and reduced infection risk. This study compared intramedullary (IM) versus extramedullary (EM) lengthening nails in pediatric patients with narrow femoral canals to evaluate mechanical axis deviation, nail bending, tourniquet time, and complications. METHODS: A retrospective, single-center review was conducted of 75 pediatric patients who underwent femoral lengthening between 2015 and 2022, eac

Concurrent Salter-Harris type II distal tibia fracture with ipsilateral talus and calcaneus fractures in a paediatric patient .

Paediatric bones exhibit greater elasticity due to their distinct biomechanical properties, enabling them to absorb and dissipate mechanical forces more effectively than adult bones. This inherent flexibility provides increased resistance to fractures under comparable loading conditions. However, despite this resilience, high-energy trauma can still result in complex injury patterns. Fractures involving the physis are of particular concern, as they carry a significant risk of growth disturbances and long-term deformities. We present a rare case of a girl in early childhood (preschool) who susta

Computational modeling of bone allograft reconstruction following femoral shaft tumor resection: Investigating the impact of supplementary plate fixation.

BACKGROUND AND OBJECTIVE: The use of bone allograft reconstructions after tumor resection can introduce significant complications. Stable fixation is required to decrease the incidence of mechanical complications of segmental bone allografts. The purpose of the present study is to compare plating fixation methods of diaphyseal allografts after intercalary resection of the femur. METHODS: We created four defined fixation models using plates and/or intramedullary polymethylmethacrylate (PMMA) to simulate typical bone tumor resection with intercalary allograft reconstruction. One angularly stabl

Combined monolateral external fixation and intramedullary nailing for oligotrophic femoral nonunion with limb length discrepancy: a case series of technical strategy and surgical outcomes.

Bone lengthening using a monolateral external fixator is an effective technique for treating femoral nonunion associated with limb length discrepancy (LLD). This method involves gradual femoral lengthening to correct shortening and rotational deformities, while promoting bone healing through compression at the nonunion site. We present a case series of three patients demonstrating the successful integration of monolateral external fixation with intramedullary nailing for oligotrophic nonunions (4-8 cm LLD). Preoperative planning includes assessment of limb length discrepancy, evaluation the ca

Postoperative results after open-wedge high tibial osteotomy with lateral hinge fracture.

BackgroundLateral hinge fracture (LHF) is a known complication of open-wedge high tibial osteotomy (OWHTO), potentially resulting in postoperative instability, displacement, loss of correction, malunion, and non-union. This study tested the hypothesis that patients who develop LHF after OWHTO can achieve clinical outcomes comparable to those without LHF, without requiring additional surgical intervention.MethodsThis retrospective study included 96 patients who underwent OWHTO with a locking plate between 2019 and 2021. Postoperative radiographs and computed tomography (CT) scans identified LHF

One-stage surgical correction of Shepherd's crook deformity in fibrous dysplasia using fibular strut allograft augmentation: a retrospective case-series.

PURPOSE: Shepherd's crook deformity is a characteristic complication of fibrous dysplasia (FD) of the femur, leading to coxa vara and mechanical instability. Two-stage surgical approaches involve initial valgus osteotomy followed by delayed intramedullary femoral stabilization, prolonged treatment, and increasing morbidity. This study evaluates a one-stage surgical correction that employs fibular strut allograft augmentation for both the femoral neck and shaft. METHODS: A retrospective review was performed on 17 patients with Shepherd's crook deformity who underwent one-stage hip lag screw an

Does gait influence biomechanics in a distal femoral osteotomy? An early post operative fracture after DFO above a Tomofix® plate in a multiple sclerosis and low-density bone affected patient: choose a longer plate-a case report.

BACKGROUND: Distal femur osteotomies are a well known and valuable treatment option to manage valgus malalignment with unicompartmental arthritis. Early postoperative complications are well known, and risk factors, such as pulmonary diseases, smoke, high dependent functional status, and body mass index, have been studied, but no study is available about osteotomies when gait is abnormal because of neurodegenerative conditions or when mineral density is below the normal rate. CASE PRESENTATION: We report the case of a 44 year-old female Mediterranean patient who underwent a biplanar distal fem

Risk factors for malalignment after intramedullary nail treatment of distal tibia fractures with associated fibula fractures.

PURPOSE: Malalignment of distal tibia fractures can lead to malunion/nonunion or alter the limb mechanical axis which may cause arthritis. Proposed methods to decrease malalignment include fibular fixation or multiplanar interlocking screws, however these remain controversial. This study aimed to identify factors associated with malalignment in distal tibial fractures with associated fibular shaft fractures. METHODS: A retrospective review was performed of distal tibia fractures with associated fibular shaft fractures treated with intramedullary nailing at two level one trauma centers between
