Mortality in ankylosing spondylitis: results from a nationwide population-based study.

OBJECTIVES: Information on mortality in ankylosing spondylitis (AS) is scarce. Our study therefore aimed to assess: (1) mortality in AS versus the general population, and (2) predictors of death in the AS population. METHODS: Nationwide cohorts of patients with AS diagnosed at rheumatology or internal medicine outpatient clinics (n=8600) and age-matched, sex-matched and county-matched general population comparators (n=40 460) were identified from the National Patient Register and the census register, respectively. The follow-up period began on 1 January 2006 or at the first date of registered

Are There Modifiable Risk Factors for Hospital Readmission After Total Hip Arthroplasty in a US Healthcare System?

BACKGROUND: Although total hip arthroplasty (THA) is a successful procedure, 4% to 11% of patients who undergo THA are readmitted to the hospital. Prior studies have reported rates and risk factors of THA readmission but have been limited to single-center samples, administrative claims data, or Medicare patients. As a result, hospital readmission risk factors for a large proportion of patients undergoing THA are not fully understood. QUESTIONS/PURPOSES: (1) What is the incidence of hospital readmissions after primary THA and the reasons for readmission? (2) What are the risk factors for hospi

Risk factors for metastatic osteoarticular infections after a long follow-up of patients with Staphylococcus aureus bacteraemia.

We aimed to identify risk factors associated with the development of haematogenous metastatic osteoarticular infection (MOI) after an episode of Staphylococcus aureus bacteraemia (SAB). We followed 198 patients with SAB during a median of 68.9 months. Nine (4.54%) developed an MOI (median: 6.77 months) after SAB. Factors associated with MOI were the presence of joint prosthesis (hazard ratio 17.56; 95% CI 4.48-68.85) and osteoporosis (hazard ratio 8.46; 95% CI 1.9-37.57). MOI is a common complication after SAB and is related to high morbidity and mortality. Patients with previous osteoarticula

Clinical Presentation, Risk Factors, and Outcomes of Hematogenous Prosthetic Joint Infection in Patients with Staphylococcus aureus Bacteremia.

BACKGROUND: Staphylococcus aureus bacteremia is a life-threatening condition that may lead to metastatic infection, including prosthetic joint infection. METHODS: To assess clinical factors associated with hematogenous prosthetic joint infection, we retrospectively reviewed all patients with a joint arthroplasty in place at the time of a first episode of S. aureus bacteremia over a 5-year period at our institution. Patients with postsurgical prosthetic joint infection without hematogenous prosthetic joint infection were excluded. RESULTS: There were 85 patients (143 arthroplasties) with eith

Two cases of cardiac device-related endocarditis due to Streptococcus dysgalactiae subsp. equisimilis (group C or G streptococci).

BACKGROUND: Cardiac device-related endocarditis is a very rare clinical manifestation of S. dysgalactiae subsp. equisimilis disease. This pathogen is a common cause of cellulitis. We here report two cases of cardiac device-related endocarditis due to Streptococcus dysgalactiae subsp. equisimilis. Blood cultures yielded this pathogen and both patients had recurrent bacteremia. Transthoracic and transesophageal echocardiography revealed lead vegetations. This is a new description of this pathogen to cause cardiac device-related endocarditis. CASE PRESENTATION: The first case is a 79-year-old fi

Serum C-reactive protein and procalcitonin kinetics in patients undergoing elective total hip arthroplasty.

BACKGROUND: The sensitivity and the specificity of different methods to detect periprosthetic infection have been questioned. The current study aimed to investigate the kinetics of C-reactive protein (CRP) and procalcitonin (PCT) in patients undergoing uncomplicated elective total hip arthroplasty (THA), to provide a better interpretation of their levels in noninfectious inflammatory reaction. METHODS: A total of 51 patients were included. Serum CRP and PCT concentrations were obtained before surgery, on the 1st, 3rd, and 7th postoperative days and after discharge on the 14th and 30th days an

Early infection of hip joint prosthesis by Clostridium difficile in an HIV-1 infected patient.

Anaerobes are less frequently described as causative pathogen of prosthetic joint infection (PJI). We report the first case of early PJI after hip arthroplasty due to Clostridium difficile in a diabetic and HIV-1 infected patient with bacteremia. Our patient was successfully treated through surgical debridement and prosthesis retention combined with targeted antibiotic therapy.

The future of biologic coatings for orthopaedic implants.

Implants are widely used for orthopaedic applications such as fixing fractures, repairing non-unions, obtaining a joint arthrodesis, total joint arthroplasty, spinal reconstruction, and soft tissue anchorage. Previously, orthopaedic implants were designed simply as mechanical devices; the biological aspects of the implant were a byproduct of stable internal/external fixation of the device to the surrounding bone or soft tissue. More recently, biologic coatings have been incorporated into orthopaedic implants in order to modulate the surrounding biological environment. This opinion article revi

Distal femoral replacement in periprosthetic fracture around total knee arthroplasty.

INTRODUCTION: Total knee arthroplasty is a common orthopaedic procedure in the UK; consequently, revision surgery and periprosthetic fractures are increasing in incidence. Strategies for management of these cases include non-operative strategies, internal plate fixation and revision of the distal femoral component. One under-reported practice is to perform distal femoral replacement in cases with poor distal femoral bone stock. MATERIALS AND METHODS: The department's electronic database was searched for all patients undergoing revision of total knee arthroplasty. From these, all patients havi

The role of infection-associated risk factors in prosthetic surgery.

The number of obese people with diabetes mellitus (DM) is increasing. DM is in itself a risk factor for all types of infections, including infections after total hip and knee prosthetic surgery. In particular, the prevalence of obesity is high among orthopaedic patients. Obesity is also a risk factor for post-operative infections in general. After total hip arthroplasty obese patients have a threefold increased risk for post-operative infections compared to a patient with normal weight. There is an association between DM and obesity, and this association is explained by the presence of metabol

Molecular characterization of an early invasive Staphylococcus epidermidis prosthetic joint infection.

Historically regarded as a skin commensal, Staphylococcus epidermidis has been increasingly implicated in invasive foreign body infections such as catheter-related bloodstream infections, indwelling device infections, and prosthetic joint infections. We report a case of an aggressive, difficult-to-eradicate, invasive prosthetic hip infection occurring early after hardware implant and associated with a high-grade bacteremia and assess its salient molecular characteristics. The clinical and molecular characteristics of this isolate mirror the pathogenesis and persistence commonly seen with invas

The estimated magnitude and direct hospital costs of prosthetic joint infections in the United States, 1997 to 2004.

To estimate the number and cost of prosthetic joint infection hospitalizations in civilian US hospitals, we analyzed the 1997 to 2004 National Hospital Discharge Survey for the 996.66 International Classification of Diseases discharge code (infection or inflammatory reaction secondary to internal joint prosthesis). The annual number of such hospitalizations averaged 17 589 from 1997 to 2000 and 29 225 from 2001 to 2004. The annual adjusted diagnostic-related group cost for such infection increased from $195 million to $283 million (1997-2004). The mean diagnostic-related group reimbursement ($

Imaging of infectious diseases using [18F] fluorodeoxyglucose PET.

The role of fluorodeoxyglucose positron emission tomography (FDG PET) in the diagnostic localization of infectious diseases has expanded rapidly in years. In general, sensitivity of FDG PET in depicting infections compares favorably to other diagnostic modalities . It is shown to be useful in patients with suspected osteomyelitis, especially in chronic low grade infections and in vertebral osteomyelitis. although the sensitivity of FDG PET in prosthetic joint infections is very high, reported specificity varies considerably. In experienced centers, FDG uptake localized along the interface betw

Causes of failure of the Kinemax Plus total knee replacement in the first five years.

In order to investigate whether any single batch of polyethylene inserts was responsible for premature failure of total knee replacements (TKR), we performed a retrospective database review of 1133 consecutive primary Kinemax Plus TKR performed by 2 surgical teams between January 1992 and January 2001 at our institution. Cases re-operated in any way within 5 years of the index arthroplasty were identified, case notes studied and radiographs reviewed. The causes contributing to the premature failure of the implants were identified where possible and the polyethylene lot numbers were recorded. N

Dislocation of the elbow with fractures of the coronoid process and radial head.

INTRODUCTION: The aim of the study was to evaluate the factors relevant to prognosis after operative treatment of an elbow dislocation/fracture involving the coronoid process and the radial head. In 30-50% of cases, elbow dislocations are accompanied by concomitant bony injuries. Here, the ulnar coronoid process and the radial head are particularly crucial to the stability of the elbow joint. MATERIALS AND METHODS: In a retrospective study, 27 out of 37 patients who were treated surgically in our clinic between 1990 and 1999 for elbow dislocation with involvement of the coronoid process and t

Lack of association of Staphylococcus aureus type A beta-lactamase with cefazolin combined with antimicrobial spacer placement prosthetic joint infection treatment failure.

Association of cefazolin treatment failure with type A beta-lactamase-producing Staphylococcus aureus has been suggested. The prevalence of beta-lactamase gene types among 23 methicillin-susceptible S. aureus (MSSA) isolates associated with prosthetic joint infection (PJI) treated with cefazolin was determined using polymerase chain reaction (PCR), and clinical and microbiologic outcomes were assessed. PCR revealed 4 isolates without blaZ, and 12 with type A, 2 with type B, and 5 with type C blaZ. Of 13 patients undergoing resection arthroplasty with subsequent reimplantation, all received ant
