We describe a case of Salmonella enteritidis infection of a prosthetic knee joint that was cured with ceftriaxone therapy for 6 weeks and replacement of the tibial component of the prosthesis. Eleven other cases of salmonella prosthetic joint infection have been reported in the English-language literature. Five infections occurred within 20 days of prosthesis placement, and seven occurred several months to years later; ten of 12 infections involved hip prostheses. Streptococcus agalactiae is a rare cause of vertebral osteomyelitis. We present four cases of spondylitis caused by this micro-organism and a review of 20 cases previously described in the literature. Only seven patients (29%) were under 50 years of age. Diabetes mellitus and neoplasms were the most frequent underlying conditions, although 37.5% of the patients did not have any predisposition. Neck or back pain was the most common symptom. The group G streptococci are beta-haemolytic streptococci which are characterised by the presence of the Lancefield group G antigen, and have been implicated in a variety of infections. 70 patients with confirmed (23 patients) or suspected (47 patients) bacterial infection in the skeleton following diseases, injuries or operations in orthopedic surgery were investigated using Tc-99m-HMPAO leucocyte scintigraphy in order to evaluate the accuracy of this procedure. In 11 subluxations and four dislocations of the hip, following the internal fixation of intracapsular or intertrochanteric fractures, sepsis was proved in 12. Subluxations were diagnosed radiographically between 2 and 26 months following injury. Dislocations occurred following unstable intertrochanteric fractures, stabilized by valgus reduction, medial displacement, and osteotomy of the greater trochanter, and were diagnosed between 10 days and 9 weeks post-fracture. Treatment alternatives included implant removal, resection arthroplasty, and replacement arthroplasty in selected cases.

This paper reports a retrospective study of acute femoral neck fractures in 123 patients treated by primary prosthetic replacement. The average age in our patient population was 80-5 years. A Christiansen trunnion-bearing hip prosthesis (Christiansen, 1969 and 1974) was used in all patients except one. The prosthesis was cemented to the femoral shaft. Total body scans under identical conditions and using 18F and 99mTc-EHDP were carried out in patients with known, or subsequently proven, metastases. There was no statistical evidence for any difference in the number of scintigraphically demonstrable lesions. Patients with internal prostheses who developed infections of the prosthesis show no significant difference in the uptake of either nucleid. Soft tissue lesions showed a significantly higher uptake of 99mTc-EHDP than of 18F. 18F can be replaced for nearly all problems by 99mTc-EHDP without loss of diagnostic information.
