Candida mediastinitis and septic shock following occult esophageal perforation in a patient with posttraumatic quadriplegia.

Unexplained septic shock was ultimately shown to be caused by Candida mediastinitis after perforation of the cervical esophagus by a dislodged cervical methylmethacrylate construct in a 25-year-old patient with traumatic quadriplegia. Communication between the prevertebral abscess and pleural space further led to the formation of a esophageal-pleural-cervical fistula. Despite antibiotics, surgical removal of the construct, and drainage of the esophagus and mediastinum, the patient died from refractory shock and respiratory failure.

[Experience with peflacine in the treatment of osteomyelitis].

Authors report on the treatment of 20 chronic osteomyelitis patients in whom direct antibiotic therapy was performed using the preparation Péflacine. The infected area was exposed in every case, the bony focus was excised and the wound was drained. During the operation, as an induction 800 mg Péflacine in slow drop infusion was given. Oral preparation (2 x 400 mg/die) followed, and was continued for 30 days. The secretion of the wounds ceased in every case, after 3 weeks the latest. In 2 cases mild gastrointestinal symptoms as side effects were observed. Relapse the first time occurred 5-7 mon

Flexible external fixation for craniodorsal coxofemoral luxations in dogs.

An external fixator consisting of two Ellis pins connected by a flexible band was developed and evaluated as a treatment for craniodorsal coxofemoral luxations in dogs. The technique for closed application of the fixator without injury to the coxofemoral joint or sciatic nerve was developed in six dog cadavers. The coxofemoral joints were then surgically destabilized and the limbs were manipulated through a full range of motion to assess the efficacy of the fixator in maintaining joint reduction. The fixator maintained joint reduction and stability after a surgically created craniodorsal luxat

The effect of infection and lag screw fixation on the union of membranous bone grafts in a rabbit model.

Infection complicating craniofacial procedures contributes significantly to patient morbidity and health care costs. The role of fixation materials in this setting remains unclear. As foreign material, does fixation hardware increase patients' susceptibility to developing postoperative infection? Furthermore, once infection is established, should fixation hardware be removed? To answer these questions, we performed an onlay membranous bone grafting procedure to the mandible in 94 New Zealand White rabbits, applied lag-screw fixation in half the animals, and inoculated the wounds with different

Pin tract infections: silver vs uncoated pins.

To test the hypothesis that coating external fixation pins with a silver-containing compound (Spi-Argent, Spire Corp, Bedford, Mass) will reduce bacterial colonization and/or pin tract infection, 36 silver-coated and 12 conventional stainless steel pins were placed in the iliac crest of six sheep and inoculated with Staphylococcus aureus. After 2 1/2 weeks the pin sites were examined for motion and inflammation, and the pin tips were quantitatively cultured and examined with scanning electron microscopy (SEM). We found that 84% of the uncoated pins were infected, while 62% of the silver-coated

Pathological fractures of the mandible: a diagnostic and treatment dilemma.

Pathological fractures of the facial bones are relatively rare. Four pathological mandibular fractures are presented and the difficulty in diagnosing the underlying cause and the dilemma of treatment choice are discussed with a review of the literature. The authors found the incidence of pathological fractures of the facial bones in their series was 0.47%. Biopsy followed by rigid internal fixation, preferably with an AO reconstruction plate bridging the area of pathology after its removal, permits normal function and allows good and undisturbed healing which will inevitably be prolonged in su

Septicemia in a child undergoing callotasis limb lengthening.

We report a patient undergoing femoral callotasis lengthening using the dynamic axial fixator to correct a post-infective leg length discrepancy of 7.8 cm. Seventeen days after the operation, the patient developed a pin site infection, which was successfully treated by oral antibiotics. On the 34th post-operative day, the infection reoccurred, and was accompanied by generalized malaise, vomiting and pyrexia. Serology identified Staphylococcus aureus enterotoxin. Following removal of the fixator, the child recovered, but only four cm of lengthening was achieved. The pins probably acted as a per

[Resorbable rods and screws of polyglycolide in stabilizing malleolar fractures. A clinical study of 600 patients].

Absorbable internal fracture fixation implants have been in large-scale clinical use since 1984. The main advantage of these devices is that no subsequent operation for implant removal is required. In this study the results obtained in a series of 600 patients with displaced malleolar fractures who were operated on over a 6-year period (1985-1990) using rods or screws made of polyglycolide were analysed. Rod fixation was used in 63% and screw fixation in 37% of the patients. Recovery was uneventful in 91.5%. Bacterial wound infection occurred in 1.2% and failure of fixation necessitating reope

Maxillofacial and mandibular fractures.

Any traumatic event that produces maxillofacial and/or mandibular fractures generally results in gross and usually severe patient disfigurement and often results in the patient's inability to eat and drink. These fractures are exceptionally rewarding cases as simple techniques may be performed resulting in a successful functional outcome (ability to eat and drink) within a very short period of time (24 hours) after fracture stabilization. A markedly improved cosmetic appearance follows shortly thereafter once inflammation and edema resolve. The primary principle of fracture treatment, ie, prov

Rigid internal fixation for the treatment of infected mandibular fractures.

A review of the literature is presented that shows that treatment of infected fractures by rigid internal fixation is biologically sound. A protocol for managing infected mandibular fractures with plate and screw fixation was developed and used on 11 patients. All patients had osseous union of the fracture and none required removal of the bone plate. This protocol is recommended as a viable option in the treatment of infected mandibular fractures.

Synovitis of the knee after intraarticular fracture fixation with Biofix. Report of two cases.

We report two cases of severe aseptic synovitis of the knee 8 and 13 weeks after biodegradable internal fixation (Biofix rods) of a fracture of the intercondylar eminence. Both knees were treated by surgical revision and synovectomy. Histologic examination revealed a severe foreign-body type of reactive synovitis in the absence of infection. It is not advisable to use Biofix intraarticularly.

[Recent fractures of the humeral shaft in adults. Role of surgical treatment using screwed plates. Apropos of 35 cases surgically treated].

Chronic pain disorders are often associated with negative emotions, including anxiety and depression. The central nucleus of the amygdala (CeA) has emerged as an integrative hub for nociceptive and affective components during central pain development. Prior adverse injuries are precipitating factors thought to transform nociceptors into a primed state for chronic pain. However, the cellular basis underlying the primed state and the subsequent development of chronic pain remains unknown. Here, we investigated the cellular and synaptic alterations of the CeA in a mouse model of chronic muscle pa

Toxic shock syndrome in patients with external fixators.

Toxic shock syndrome (TSS), first described by Todd et al. in 1978, can be a life-threatening entity. Familiarity with the pathogenesis and clinical presentation of TSS may help achieve early diagnosis and prompt appropriate intervention. TSS is not a septicemia, but a toxemia. The most extensively described pathogenesis involves a focus of specific Staphylococcus aureus strains capable of producing an exotoxin (TSST-1). We report two patients who developed TSS while in external fixators and describe their initial symptoms, management, and subsequent problems. This report will serve to alert p

Hip surgery skin cellulitis.

Of 583 erysipelas/cellulitis seen in our Department between 1981 and 1991, six (1.04%) were localized to the buttock and hip. Five patients had had a dynamic hip screw implanted for coxarthritis on the side where the skin infection developed. This does not appear to be secondary to superinfection of the surgical wound as the erysipelas/cellulitis occurred weeks (7 to 520) after surgery. The possible surgical compromise of the venous/lymphatic circulation appears to be the causative factor. In all patients, the course was satisfactory upon systemic antibiotic therapy and no recurrence was seen

[New principles of using biodegradable implants in the treatment of fractures].

So far the operative treatment of bone fractures has resolved itself to immobilisation and bone fragment joining with the help of metal implants such as nails, screws, plates and wires. Once the bone had healed, they had to be removed. This action bears some risk, e.g. extremity disfunction, wound infection. This requires re-hospitalization and increases social cost of the treatment. These reasons gave rise to research on new, biodegradable materials. One of them is "Biofix" manufactures as threads or nails, employed when immobilising bone fragments. Application of "Biofix" allows early rehabi

The integrated approach to suppurative mediastinitis: rewiring the sternum over transposed omentum.

Acute mediastinitis after cardiopulmonary procedures remains a devastating complication and a challenge to the reconstructive surgeon. A review of the literature and our own experience confirm the need for early aggressive drainage followed by timely reconstruction. In carefully selected patients, the sternum may be reclosed, provided that omental tissue has been transposed into the defect between the myocardium and the posterior cortex of the sternum. The technique is outlined and the results are analyzed and compared with three additional patient subgroups: (1) sternal wounds rewired over dr

Early temporary porosis of bone induced by internal fixation implants. A reaction to necrosis, not to stress protection?

Stabilization of the fracture using implants requires contact surfaces between implant and bone. Such contact has been observed to induce bone porosis first seen at one month after surgery. Bone loss in the vicinity of implants has hitherto been explained as being induced by mechanical unloading of the bone (stress protection). Experiments in sheep, dogs, and rabbits combining intravital staining of blood circulation and polychrome fluorescent labeling of bone remodeling leads to the conclusion that early bone porosis in the vicinity of the implants is the result of internal remodeling of cort

The effect of interdental continuous loop wire splinting and intermaxillary fixation on the marginal gingiva.

To study the influence of interdental loop-wire splinting and intermaxillary fixation on the marginal gingiva, 30 patients were evaluated clinically using different periodontal parameters, at 5 examination times. It was shown that despite a standardized oral hygiene regime including the use of a mouthrinse, gingival inflammation occurred for the duration of the splinting period. Factors other than the presence of limited plaque, such as gingival trauma due to splint application and subsequent mechanical irritation should be considered as possible aetiological factors. All investigated marginal
