[Oesophageal perforation of osteosynthesis material after anterior cervical surgery].

The rare, potentially life-threatening complication to anterior cervical surgery, oesophageal perforation, occurs after surgical trauma or due to erosion by migrating hardware. Symptoms are hoarseness, dysphagia, neck/throat pain, subcutaneous emphysema and fever. Imaging and endoscopic diagnosis can give false negative results. We present a case of a 74-year-old male, who was readmitted with sepsis and abscess in the operation area three weeks after anterior cervical surgery. Veillonella parvula was found in the abscess material and computed tomography confirmed the diagnosis of oesophageal p

Unusual complication of repair of fractured mandibular angle.

Internal fixation with miniplates remains an accepted and reliable approach to the management of mandibular fractures, and erosion through the oral mucosa is a well-recognised long-term complication. We report a case of erosion of a miniplate through the skin 5 years after internal fixation of a fracture of the left mandibular angle.

Combined in vivo optical and µCT imaging to monitor infection, inflammation, and bone anatomy in an orthopaedic implant infection in mice.

Multimodality imaging has emerged as a common technological approach used in both preclinical and clinical research. Advanced techniques that combine in vivo optical and μCT imaging allow the visualization of biological phenomena in an anatomical context. These imaging modalities may be especially useful to study conditions that impact bone. In particular, orthopaedic implant infections are an important problem in clinical orthopaedic surgery. These infections are difficult to treat because bacterial biofilms form on the foreign surgically implanted materials, leading to persistent inflammatio

Complications of hinged external fixation compared with cross-pinning of the elbow for acute and subacute instability.

BACKGROUND: Elbows that are unstable after injury or reconstructive surgery often are stabilized using external fixation or cross-pinning of the joint supplemented by cast immobilization. The superiority of one approach or the other remains a matter of debate. QUESTIONS/PURPOSES: We compared patients treated with external fixation or cross-pinning in terms of (1) adverse events, (2) Broberg and Morrey scores, and (3) ROM. METHODS: Between 1998 and 2010, 19 patients (19 elbows) had hinged external fixation and 10 patients (11 elbows) cross-pinning and casting for subacute or acute posttraumat

(18)F-FDG microPET imaging differentiates between septic and aseptic wound healing after orthopedic implant placement: a longitudinal study of an implant osteomyelitis in the rabbit tibia.

BACKGROUND AND PURPOSE: (18)F-FDG PET is a widely used tool for molecular imaging of oncological, cardiovascular, and neurological disorders. We evaluated (18)F-FDG microPET as an implant osteomyelitis imaging tool using a Staphylococcus aureus-induced peroperative implant infection in rabbits. METHODS: Intramedullary titanium nails were implanted in contaminated and uncontaminated (control) proximal right tibiae of rabbits. Tibiae were quantitatively assessed with microPET for (18)F-FDG uptake before and sequentially at 1, 3, and 6 weeks after surgery. Tracer uptake was assessed in soft tiss

Silver oxide-containing hydroxyapatite coating has in vivo antibacterial activity in the rat tibia.

Bacterial infection is a serious postoperative complication of joint replacement. To prevent infections related to implantation, we have developed a novel antibacterial coating with Ag-containing hydroxyapatite (Ag-HA). In the present study, we examined the antibacterial activity of Ag-HA implant coatings in the medullary cavity of rat tibiae. Forty 10-week-old rats received implantation of Ag-HA- or HA-coated titanium rods, then were inoculated with ∼1.0 × 10(2) colony-forming units of methicillin-resistant Staphylococcus aureus. Bacterial counts were calculated for rats euthanized at 24, 48,

Local gentamicin application does not interfere with bone healing in a rat model.

For the prophylaxis and treatment of bony infections antibiotics are locally used. Since several decades antibiotics mixed with bone cement (methylmethacrylate) are successfully used in prosthetic surgery and a gentamicin coated tibial nail is approved in Europe for fracture stabilization. The goal of the present study was to investigate if gentamicin, locally applied from a polymeric coating of intramedullary nails, might interfere with the bone healing process. Female Sprague Dawley rats (n = 72) were used and the tibiae were intramedullary stabilized with Kirschner-wires (k-wires) after ost

The value of ultrasound-assisted pinned resorbable osteosynthesis for cranial vault remodelling in craniosynostosis.

Resorbable osteosynthesis is a widespread tool in craniofacial surgery, however only a limited number of studies have focused on ultrasound-assisted pinned resorbable systems in the treatment of craniosynostosis. Thirty-eight children with various types of craniosynostosis including scaphocephaly, trigonocephaly, anterior and posterior plagiocephaly were treated using the Sonic Welding resorbable osteosynthesis system. All patients were evaluated for operation time, stability of the surgical results, rate of local infections and visibility or palpability of the osteosynthesis material in the f

The future of biologic coatings for orthopaedic implants.

Implants are widely used for orthopaedic applications such as fixing fractures, repairing non-unions, obtaining a joint arthrodesis, total joint arthroplasty, spinal reconstruction, and soft tissue anchorage. Previously, orthopaedic implants were designed simply as mechanical devices; the biological aspects of the implant were a byproduct of stable internal/external fixation of the device to the surrounding bone or soft tissue. More recently, biologic coatings have been incorporated into orthopaedic implants in order to modulate the surrounding biological environment. This opinion article revi

Distal femoral replacement in periprosthetic fracture around total knee arthroplasty.

INTRODUCTION: Total knee arthroplasty is a common orthopaedic procedure in the UK; consequently, revision surgery and periprosthetic fractures are increasing in incidence. Strategies for management of these cases include non-operative strategies, internal plate fixation and revision of the distal femoral component. One under-reported practice is to perform distal femoral replacement in cases with poor distal femoral bone stock. MATERIALS AND METHODS: The department's electronic database was searched for all patients undergoing revision of total knee arthroplasty. From these, all patients havi

Knee arthrodesis using a customised modular intramedullary nail in failed infected total knee arthroplasty.

BACKGROUND: Knee arthrodesis is used to treat patients with failed infected total knee arthroplasty (TKA). Among fixation methods, intramedullary nailing increases the chances of bone union but may carry a risk of infection around the nail. This risk is not well understood, because available case-series studies were not confined to patients with knee infection. HYPOTHESIS: Infection recurrence rates after knee arthrodesis with intramedullary nailing used to treat failed infected TKA are similar to those seen with other fixation methods. METHODS: We retrospectively reviewed 31 cases of knee a

Pre-tibial reaction to biointerference screw in anterior cruciate ligament reconstruction.

BACKGROUND: We aim to report a series of cases that presented as pre-tibial swelling and pain following anterior cruciate ligament (ACL) reconstruction using bioabsorbable fixation devices. METHODS: All ACL reconstructions done between 2007 and 2010 were reviewed retrospectively to identify complications related to bioabsorbable fixation devices. We performed 273 ACL reconstructions over a period of three years from 2007 to 2010 using a bioabsorbable screw for tibial fixation of the quadrupled hamstring autograft. RESULTS: Fourteen patients (5%) at mean age of 30 (range 16-47) years, present

The pitfalls in surgical management of lumbar canal stenosis associated with rheumatoid arthritis.

There have been few clinical studies in the area of cervical spine that focused on surgery for treating degenerative lumbar disease in patients with rheumatoid arthritis (RA). High rates of wound complications and instrumentation failure have been reported more for RA than for non-RA patients, although clinical outcomes are similar between the two groups. Lumbar canal stenosis in RA is caused not only by degeneration but also by RA-related spondylitis, which includes facet arthritis and inflammation around the vertebral endplate. The pitfalls in surgical management of lumbar canal stenosis in

The correction of a 140 mm lower limb difference as a late complication of a childhood meningococcal sepsis with a fully implantable system.

Meningococci (Neisseria meningitidis) are the typical agents of a severe septic shock with fulminate diseases. Many children survive the initial phase of shock today; there are several late complications such as limb differences, large scar areas and impaired soft tissue. The current case report deals with the impaired bone growth and a resulting lower limb difference as a late complication of meningococcal sepsis. Usually, these cases are corrected by external fixator systems. In the present case, a lower leg-length difference of 140 mm was uneventfully corrected in two operative steps by ful

[Carbon/carbon implants in oral and maxillofacial surgery -- part 1].

UNLABELLED: Over the past 20-30 years various carbon implant materials have become more interesting, because they are well accepted by the biological environment. The traditional carbon-based polymers give rise to many complications. The polymer complication may be eliminated through carbon fibres bound by pyrocarbon (carbon/carbon). AIM: The aim of this study is to present the long-term results of carbon/carbon implants. METHODS: Mandibular reconstruction was performed with pure (99.99%) carbon implants in 16 patients. Predominantly tumorous cases were included (10 malignant tumors, 4 large
