The use of bioabsorbable implants in orthopaedic surgery has become a common practice. Despite their many theoretical advantages, recent reports have proposed certain concerns and complications associated with their use. We report the case of a bioabsorbable screw that extruded from the tibial insertion site 15 months after anterior cruciate ligament reconstruction.

OBJECTIVE: To assess the long-term clinical and radiologic findings after insertion of a bioresorbable polylactide plates P(L/DL)LA 70/30 implant (PolyMax) in the repair of orbital floor and wall defects, with special focus on stability and clinical signs of foreign-body reaction.
METHODS: Forty-six patients who had orbital blowout fractures with at least 1.5-cm(2) bone defects in 1 or 2 walls were included in this retrospective study. Each defect was reconstructed within 2 weeks of injury using a triangle form plate of polylactide. Computed tomography (CT) was performed before the operation

A 48-year-old man who had received a bioresorbable plate fixation for a zygomatic bone fracture 13 months earlier visited our clinic complaining of sudden facial swelling. The facial computed tomographic scan showed the soft tissue swelling without any bony abnormality, and the symptoms did not improve after 1 week of antibacterial therapy. The patient had a diagnosis of a late infection caused by unresorbed plates, and exploratory surgery was performed. Partially resorbed plates and screws were seen, and we removed the remnants of such completely. The symptoms were relieved after the operatio

Biodegradable implants lead to problems such as cyst formation, soft-tissue inflammation, loose implant fragments or local osteolysis. This report represents the first published case of an intraosseous foreign body granuloma in the humeral head after arthroscopic rotator cuff tear fixation with a poly-L: -lactide (PLLA) suture anchor. A 48-year-old female patient presented with pain in her right shoulder. A refixation of her right supraspinatus tendon with a biodegradable suture anchor was performed 11 months ago at an external hospital. Laboratory tests showed normal values for C-reactive pro

PURPOSE: Tubercular infection has not been described, to our knowledge, in the literature after anterior cruciate ligament (ACL) reconstruction, and, hence, the purpose of our case series was to describe our experience, evaluate the clinical and laboratory findings, and assess the treatment outcome.
METHODS: We performed a retrospective analysis of 1,152 cases of arthroscopic ACL reconstruction with autografts performed at our institution between January 1998 and May 2007. Tubercular infection was considered to be present in the setting of recurrent negative bacterial cultures but a positive

External fixation of mandible fractures is a useful technique when an open treatment is contraindicated because of extensive comminution, bone or soft tissue loss, and infection. This technique can also be used temporarily until definitive treatment is delivered. A uniphasic or biphasic system can be placed to reduce and stabilize mandibular fractures. These systems use surgically placed threaded pins and different types of connectors that can be manipulated to optimize the reduction of fractures. External fixation remains a quick, safe, and simple method to treat mandible fractures in selecte

Computer-navigated joint arthroplasty surgery using optical tracking systems requires arrays fixated to bone via pins. Reports of fractures at pin sites have raised concern about safety. We reviewed the postoperative complications occurring in a single-surgeon series of 984 consecutive primary total knee arthroplasties. All pins were placed unicortically and connected by a dual pin array. Femoral pins were placed into the medial epicondyle, and tibial pins were placed in the shaft 10 cm inferior to the joint line. There were no fractures. Seventeen (1.7%) patients had minor pin-related complic

Intertrochanteric fractures of the femur were treated with an external fixator in 38 elderly high-risk patients between November 2005 and February 2007. The mean operation time was 25 min (range: 20 to 32 min), the mean fluoroscopy time was 15 sec (range: 10 to 18 sec), and the mean hospital stay was four days (range: 3 to 7 days). The average time to union was 9.5 weeks. No deep pin track infections occurred; superficial skin inflammation was seen in 30%. Implant failure or knee stiffness were noted in no instance. At the latest follow-up, the mean femoral shaft-neck angle was 130 degrees (ra

BACKGROUND: Oesophageal perforation related to anterior cervical surgery is an uncommon but well recognised and potentially life-threatening complication with an incidence of 0-3.4%. Our experience with this complication and a review of the literature are presented.
METHOD: We retrospectively reviewed our clinical experience over 10 years and found four patients in whom an oesophageal perforation was recognised after anterior surgery for cervical spine trauma. In three patients the perforation was noticed in the early post-operative period and the other had a delayed presentation. In all pati

A case report of a 41-year-old man who had a delayed pharyngo-esophageal perforation without instrumentation failure 7 years after anterior cervical spine plating is presented and the literature on this issue is reviewed. This injury resulted from repetitive friction/traction between the retropharyngo-esophageal wall and the cervical plate construct leading to a pseudodiverticulum and perforation. Successful treatment of the perforation was obtained after surgical repair using a sternocleidomastoid muscle flap. This case stresses the necessity of careful long-term follow-up in patients with an

BACKGROUND: Port-access minimally invasive cardiac surgery has been developed with the aim of reducing surgical invasion. We conduct port-access cardiac surgery, generally through an intercostal space. To ensure sufficient visual field, either the upper or lower rib may be divided. In the process of restoration of divided ribs, we have developed a bone fixation technique using a new osteosynthesis device by way of prevention of pseudoarthrosis.
METHODS: Forty-five consecutive patients underwent right-sided anterior intercostal thoracotomy under the port-access method between July 2006 and Jul

Bone-anchored hearing aids are a standard option in rehabilitation of patients with conductive or mixed hearing loss, and also CROS fitting. However, the skin-penetrating bone anchor repeatedly gives reason for discussion about the risk of infection of surrounding tissues as a major cause of malfunction. In the present study, explanted bone anchors with surrounding bone and soft tissue were examined and compared with the morphology of lost implants. The anchors originated from five patients. Two needed explantation due to deafness with the need of cochlea implantation. A third patient underwen

We evaluated costs and outcomes of patients with S. aureus bacteremia and orthopedic device infections (ODI). Patients with ODI had higher relapse of S. aureus infection, compared to bacteremic patients without ODI. Costs and outcomes were similar among ODI patients undergoing device removal and those treated with debridement and retention.

Nowadays, the use of minimally invasive plate osteosynthesis (MIPO) in the management of fracture of the distal tibia is common. The various advantages of the MIPO technique, namely, preserving blood supply and better bone healing, have been described extensively in the literature. However, this technique is not without complication. Among all the complications, infection is one of the commonest. In the last 3 years, we have performed 48 cases of MIPO in treating distal tibia fractures. Our study was to evaluate the clinical outcome of these cases, with special attention to the infection rate

BACKGROUND CONTEXT: Perforation of the esophagus after anterior cervical spine surgery is a rare, but well-recognized complication. The management of esophageal perforation is controversial, and either nonoperative or operative treatment can be selected.
PURPOSE: Several reports have described the use of a sternocleidomastoid muscle flap for esophageal repair. In this case report, we describe a longus colli muscle flap as a substitute for a sternocleidomastoid flap in a patient with an esophageal perforation.
STUDY DESIGN: Case report.
PATIENT SAMPLE: A 20-year-old man sustained cervical sp
