[The long-term results of primary total knee arthroplasty using bone grafts].

In the presented paper the authors evaluated the results of treatment in patients following primary total knee arthroplasty using bone grafts due to sever valgus or varus deformity. The clinical material from the years 2000-2004 consists of 18 patients aged from 64 to 77; namely, 14 women (77.8%) and 4 men (22.2%). The longest time of follow-up was 3 years and the shortestwas 17 months. The authors evaluated achieved results in the period between 17th and 24th month since total knee arthroplasty in 18 patients on the basis of the time criteria referring to the time of healing the bone implants

Dislocation of the elbow with fractures of the coronoid process and radial head.

INTRODUCTION: The aim of the study was to evaluate the factors relevant to prognosis after operative treatment of an elbow dislocation/fracture involving the coronoid process and the radial head. In 30-50% of cases, elbow dislocations are accompanied by concomitant bony injuries. Here, the ulnar coronoid process and the radial head are particularly crucial to the stability of the elbow joint. MATERIALS AND METHODS: In a retrospective study, 27 out of 37 patients who were treated surgically in our clinic between 1990 and 1999 for elbow dislocation with involvement of the coronoid process and t

Implant-related infection model in rat spine.

OBJECTIVE: The rate of postoperative infections is approximately 1% in spine surgery. However, when metal implants are used, postoperative infection rates significantly increase and were reported between 2.1 and 8.5%. This study aim to set up an infection model in the rat spine with a metal implant. MATERIALS AND METHODS: Forty white male Sprague Dawley rats were randomly divided in four groups. In all rats, under operation microscope, a 3 mm titanium microscrew was implanted in the thoracolumbar area (T10-L1) after laminar decortication. In Group I (control group), sterile isotonic solution

One-stage combined surgery with mesh cages for treatment of septic spondylitis.

UNLABELLED: There is a controversy regarding most effective operative method for treatment of pyogenic spondylitis and whether to use metallic implants on the site of infection. This retrospective study reports on the outcome of 17 patients with persistent cervical and lumbar pyogenic spondylitis who had one-stage combined surgery and fusion with use of a titanium mesh cage for intractable pain, kyphosis, and neurologic impairment. All patients tolerated the combined operation and were followed up on for 45 months. Incomplete neurologic lesions improved postoperatively an average 1.4 Frankel g

Fusion of the septic knee with external hybrid fixator.

Arthrodesis is a well-recognized salvage procedure in patients with septic destruction of knee joints. It offers the opportunity for restoring weight bearing capability and significant pain relief as well as eradication of infection, but at the expense of joint motion. However, arthrodesis in order to joint sepsis may be difficult to achieve because of poor bone stock, persistent infection and soft tissue compromise. From 2000 to June 2004, in 70 consecutive patients arthrodesis of the knee was indicated. Nineteen patients were considered to have external hybrid fixator (EHF) and were therefor

[Vacuum assisted closure for the treatment of sternal wound infections -- rapid infection control and bridging to reconstructive surgery].

Vacuum Assisted Closure (V.A.C.) is a sound strategy to control severe post-sternotomy infection. Secondary re-wiring of the sternal bone or reconstructive surgery may be required later to achieve complete wound closure. Ten patients with severe sternal bone infection underwent initial V.A.C. therapy and delayed surgery for wound closure. Complete wound healing was achieved in all cases. As a case report we describe a patient with right-sided mastectomy and irradiation for breast cancer, who developed severe post-sternotomy infection and sternal bone necrosis following CABG. A combination of V

[Pelvic stabilization in cases of septic instability. Triangular osteosynthesis in case of infection related vertical pelvic ring instability].

The combination of transiliac screws and lumbopelvic distraction osteosynthesis is usually an appropriate procedure to treat vertical pelvic ring instabilities under the condition of full weight bearing. In this case, due to the extent of septic destruction of the dorsal portion of the iliac bone, the common triangular fixation method using conventional pedicle screws was not possible. Using the transiliac dorsoventral screw position with special long screws, we achieved high mechanical triangular stability sufficient for pelvic ring fusion despite the large bony defect.

False magnetic resonance imaging persistence of a biodegradable anterior cruciate ligament interference screw with chronic inflammation after 4 years in vivo.

Surgeons should be aware that the degradation kinetics of biodegradable implants likely influence the rate of osseous integration, and depend on numerous factors such as polymer weight, crystallinity, and stereocopolymer ratio. We present a case of inflammatory reaction at a polylactide tibial interference screw site more than 4 years after primary anterior cruciate ligament (ACL) reconstruction. This inflammation was presumptively treated as an infection with surgical irrigation and debridement. Preoperatively, the screw was clearly delineated by magnetic resonance imaging (MRI), yet was gros

Clinical and histologic evaluation of a mineralized block allograft: results from the developmental period (2001-2004).

This article reports on a multicenter evaluation of a novel, sterile, antigen-inactivated, mineralized block allograft in humans during a 3-year developmental period. Consecutive partially edentulous patients (n = 73) with severe localized ridge defects (n = 82) were treated with the material. After 4 to 6 months of healing, tapered screw implants were placed in the grafted bone and subsequently restored. Patients were monitored 25 to 36 months after prosthetic restoration. A biopsy was taken of one patient and submitted for histomorphometric analysis. Block allograft survival was 93% at 12 mo

Tissue reactions to bioabsorbable ciprofloxacin-releasing polylactide-polyglycolide 80/20 screws in rabbits' cranial bone.

The aim of this study was to assess tissue reactions to bioabsorbable self-reinforced ciprofloxacin-releasing polylactide/polyglycolide (SR-PLGA) 80/20 screws in rabbits' cranial bone. Two screws were implanted in each rabbit, one screw on either side of the sagittal suture (n = 28 rabbits). Animals were sacrificed after 2, 4, 8, 16, 24, 54 and 72 weeks, four animals per group. On histological examination the number of macrophages, giant cells, active osteoblasts and fibrous tissue layers were assessed and degradation of the screws was evaluated. At 2 weeks, the highest number of macrophages a

[Implant-associated post-traumatic osteomyelitis. Bacterial biofilms and the immune defence as protagonists of the local inflammatory process].

BACKGROUND: Formation of bacterial biofilms on implants is a severe complication following orthopaedic surgery. In the present study we addressed the role of the immune response, particularly with regard to the pathogenesis of the disease. METHODS: In a prospective study comprising 74 patients with implant-associated post-traumatic osteomyelitis, peripheral blood cells as well as cells recovered from the infected site during surgery were characterised phenotypically and functionally. RESULTS: We found massive infiltration of polymorphonuclear neutrophils (PMN), which were highly activated, p

Sterile pretibial abscess after anterior cruciate reconstruction from bioabsorbable interference screws: a report of 2 cases.

We report 2 cases of extra-articular pretibial sterile abscesses with minimal osteolysis after anterior cruciate ligament reconstruction with poly-L-lactic acid bioabsorbable interference screw fixation. No joint involvement was evident in either case. Both patients underwent surgical exploration for incision, drainage, and excision with debridement of the biodegradable screw debris from the tibial bone tunnel. The cystic area in case 1 contained 8 mL of chalky white fluid with fragments of particulate matter. Microscopic evaluation revealed birefringent foreign material in a benign histiocyte

The care of pin sites with external fixation.

Two protocols for the operative technique and care of the pin-site with external fixation were compared prospectively. There was a total of 120 patients with 46 in group A and 74 in group B. Infection was defined as an episode of pain or inflammation at a pin site, accompanied by a discharge which was either positive on bacterial culture or responded to a course of antibiotics. Patients in group B had a lower proportion of infected pin sites (p = 0.003) and the time to the first episode of infection was longer (p < 0.001). The risk of pin-site infection is lower if attention is paid to avoidin
