U.G.H. syndrome is a known complication of IOL implantation associated with the use of anterior and rarely, posterior chamber intraocular lenses. It is due to mechanical excoriation of the angle or iris by the haptics or optic of an IOL and consists of uveitis, glaucoma and hyphema (U.G.H.). The advised therapeutic approach is explantation of the IOL. BACKGROUND: In the last years heparin surface-modified intraocular lenses (HSM IOLs) were used for special indications like uveitis, synechia, congenital cataract or pseudoexfoliation syndrome. A monolayer of heparin molecules on the surface of PMMA-IOLs causes a reduced postoperative inflammatory reaction and produces a higher long-term biocompatibility than PMMA-IOLs without surface-modification. We studied 573 patients with the Marfan syndrome for the presence of ocular misalignment, refractive errors, and amblyopia. A total of 110 patients (19.2%) had strabismus. Of 573 patients, exotropia occurred in 67 (11.7%), esotropia in 12 (2.1%), vertical deviations in eight (1.4%), and primary inferior oblique muscle overaction in three (0.5%). Of 67 patients with exotropia, 18 had anisometropia and 39 had amblyopia. Of the 12 patients with esotropia, four had amblyopia and none had anisometropia. I modified the corneoscleral incision for cataract surgery so the incision is located between the 9 o'clock and 12 o'clock positions and termed it the BENT incision (abbreviated from "between nine and twelve o'clock").
