Keratoprosthesis is the last solution for corneally blind patients who cannot benefit from corneal grafts. The indications are limited to blind patients with monophthalmia and bilateral problems that are otherwise untreatable. Continuous (central, circular) curvilinear capsulorhexis (CCC), first introduced by Gimbel and Neuhann, has become increasingly popular, especially for endolenticular phacoemulsification with intracapsular intraocular lens implantation. The principles of simple mechanics are used to facilitate teaching the procedure and to reduce the incidence of complications. A safe and simple method of performing CCC based on these principles is described.

Eight cases of atonic pupil, an unusual complication following cataract surgery, are presented. Six of the eyes involved had intracapsular cataract extraction and anterior chamber intraocular lens implantation; one case underwent extracapsular cataract extraction with a posterior chamber intraocular lens implantation; the other had an extracapsular procedure with no implant. A retrospective analysis was undertaken of the clinical diagnoses of 1594 eyes that underwent penetrating keratoplasty performed in a private-referral corneal practice over a 9-year period, 1980-1988. Progressive endothelial cell loss and endothelial cell loss induced at the time of surgery occurs in all eyes with rigid anterior chamber intraocular lenses (IOLs). A series of 61 consecutive procedures of chest wall resection and reconstruction in 58 patients during the period between August, 1986 and December, 1990 is reported. The ages ranged between 6-77 years. The chest wall resection was indicated for malignant affections in 54 cases. We reviewed about 120 p-HEMA intraocular lenses (IOL), which had been implanted during the last 2 years. Neither posterior synechia of the iris with the IOL occurred, nor could severe anterior chamber reactions be encountered. The clinical findings indicated excellent biocompatibility of the p-HEMA material. In some cases we found a moderate diffuse clouding of the IOL, which did not impair the visual acuity. In 21 eyes the IOL was decentered. The low mechanical stability as well as the few clinical experiences prevent the general use of the p-HEMA lens as a routine IOL in our clinic.

A retrospective analysis of 40 cases of sulcus-fixated one-piece poly(methyl methacrylate) posterior chamber lenses revealed ten cases (25%) that demonstrated oval deformation of the pupil, similar to the condition associated with anterior chamber lenses. This complication was not noted until several weeks after surgery, appears to be progressive, and might be the cause of glare symptoms. Corneal surgeons were surveyed with regard to their technique of suture fixation of posterior chamber intraocular lenses in the absence of posterior capsular support. Fifty-nine percent of the 260 respondents stated they perform the procedure almost exclusively during penetrating keratoplasty. When large portions of the internal orbit are destroyed (two to four walls), standard bone-grafting techniques for immediate and late orbital reconstruction may not yield predictable eye position. Critical bone support is most often deficient inferomedially. Forty-three displaced tibial plateau fractures were treated with open reduction and internal fixation. Five explanted posterior chamber lenses were examined under the scanning electron microscope to determine the changes. The intraocular time of the lenses was 1 week to 3 years. The polypropylene loops showed superficial cracks in all cases, but only in the curve and the insertion areas. The findings therefore indicated that the morphological changes are more likely caused by mechanical stress than by biological degradation alone.

Mechanical retinal fixation is a useful tool in the armamentarium of vitreoretinal surgeons.
