[Hydrogel posterior chamber lenses--an improvement?].

Although the design of PMMA intraocular lenses has reached a stage of sophistication at which further modifications are unlikely to result in any major improvements in implant surgery, new materials with possibly improved mechanical and biologic properties should be evaluated carefully. The hydrogel lens (containing 38% poly-HEMA) is a soft, flexible, hydrophilic "single-piece" posterior chamber lens which could help to minimize intraoperative and postoperative IOL trauma. Fifty hydrogel intraocular lenses were implanted during a ten-month period using a standard extracapsular cataract extract

Cystoid macular edema: the apparent role of mechanical factors.

CME is the final common pathway of many intraocular and systemic diseases. It involves the retinal vasculature and often has a choroidal and vitreal component. Obviously, at some level of this complex pathophysiological process, mediators must be involved. The question is whether the characteristic distribution of vascular leakage and retinal edema is best explained by the diffusion of mediators released by from a remote site, as proposed by Miyake (see Miyake et al., 1989), or whether it reflects the distribution of pre-existing anatomical structures causing the local release of mediators by

Intraocular pressure following ECCE, phacoemulsification, and PC-IOL implantation.

Abnormal intraocular pressure (IOP), either transient or permanent, may follow extracapsular cataract extraction (ECCE) with phacoemulsification (PE) and posterior chamber intraocular lens (PC-IOL) implantation. We retrospectively studied IOP measurements at different intervals post ECCE and PE in 242 eyes of 211 patients: 105 males, 106 females, 198 Caucasians and 13 blacks. Elevated IOP (greater than 23 mm Hg) was observed in 20 eyes (8.2%). Only two patients (0.8%) had persistent (greater than 3 months) IOP elevation and needed antiglaucoma therapy. Six more eyes (2.5%), however, developed

Air, methylcellulose, sodium hyaluronate and the corneal endothelium. Endothelial protective agents.

In a randomised trial the endothelial protective agent used during extracapsular cataract extraction and intraocular lens insertion was air in 19 eyes (group 1), methylcellulose in 25 eyes (group 2) and sodium hyaluronate in 22 (group 3). The cell population densities of each eye were estimated immediately before and three months after the operations to determine the degree of cell loss. Eyes showing mechanical (touch) damage on the second postoperative day were eliminated. The numbers of eyes in each group which showed a statistically significant cell loss were compared, and the mean cell los

Management of pseudophakic corneal edema with reconstruction of the anterior ocular segment.

The following surgical techniques were used to treat pseudophakic corneal edema in 13 consecutive eyes: penetrating keratoplasty (13 eyes), mechanical anterior vitrectomy (ten eyes), removal of intraocular lens (13 eyes), gonioplasty with opening of peripheral anterior synechiae using a dental mirror (seven eyes), iridoplasty using a round needle (six eyes), and insertion of a replacement intraocular lens (12 eyes). The most common intraoperative complication was hemorrhage in the angle. Although the angle was open 360 degrees to the ciliary body in 12 of the 13 eyes at the end of surgery, a f

Glaucoma following retinal detachment operations.

After retinal detachment operations 1-5% of the patients develop glaucoma. If this occurs directly after the operation it is the result of mechanical narrowing of the chamber angle. This can be the result of indentation due to an exoplant or encircling band causing forward displacement of the lens/iris diaphragm. Other causes are torsion of the ciliary body or a ciliary block due to serous detachment of the choroid caused by venous compression or diathermy. Ischaemia of the anterior segment sometimes leads to glaucoma in the long run; this is then the result of rubeosis iridis. Conducive facto

Tissue expansion in reconstruction.

Tissue expansion offers a new alternative in plastic surgery. Its applications are numerous for reconstruction of the scalp, head and neck, trunk, breast, and extremity. Over the past year I have used tissue expansion for reconstruction in 13 patients, with multiple expanders in six patients. Complications included two exposed expanders requiring early removal, and two mechanical malfunctions requiring replacement. Expansion of previously irradiated tissue has been unsatisfactory.

Medical aspects of contraception.

It is evident that choice of contraceptive has important medical implications, whether a mechanical or a hormonal method is elected. Each technology has unique benefits and risks, constraints, applicability, and inconvenience. Wise advice in a medical context must be based on thorough familiarity with the options available to each woman.

[Artificial lenses at the Institute of Tropical Ophthalmology of Africa].

The authors present a report on the first set of twenty-three intra-ocular lenses fitting on subjects from Mali, performed at the "Tropical African Ophthalmology Institute" (I.O.T.A.) from december 1983 up to december 1984. This report deals mainly about the implants of the inner aqueous chamber of the eye after the intracapsular extraction. The mechanical complications and inflammatory corneal and retinal consequences which occurred after the surgical operation are being tested. None of these complications has questioned the visual and functional prognosis. So, the results seem quite good. Ne

Pathogenesis of pupillary capture after posterior chamber intraocular lens implantation.

Pupillary capture is an unusual complication of posterior chamber intraocular lens implantation and may occur in the early or late postoperative period. We describe a series of patients with early or late onset pupillary capture and describe the pathogenesis. Early capture results from mechanical factors and is usually preventable. Late capture may be mechanical but is usually a sequel to the formation and subsequent contraction of iridocapsular synechiae. Synechiae result from lens epithelial or iris stromal hyperplasia and myofibroblastic metaplasia; they may be minimised by an atraumatic te

Coralline hydroxyapatite bone graft substitutes in a canine diaphyseal defect model: radiographic features of failed and successful union.

Radiographic and densitometric evaluation of a new type of bone graft substitute derived from reef-building sea coral via a hydrothermal chemical exchange process was undertaken in a canine diaphyseal defect model. Comparably sized blocks of this material and autogenous iliac cortical-cancellous graft were implanted into the respective radial diaphyses of seven dogs. Qualitative and quantitative radiographic assessment was performed during the immediate postoperative period and at 3 months following surgery. Significant complications were observed radiographically in over half of the coralline

Post-mastectomy breast reconstruction using the inflatable tissue expander.

Breast reconstruction following mastectomy has previously relied on the insertion of a silicone gel implant or the use of a myocutaneous flap. We report the use of an innovation, the inflatable tissue expander, for both immediate and delayed breast reconstruction in 26 patients where soft tissue cover was inadequate to permit the use of the silicone implant. By serial inflation of the tissue expander with saline, sufficient tissue cover was achieved for a second operative placement of a silicone prosthesis of appropriate volume to match the normal breast. To date, 10 patients undergoing delaye

Lumbo-peritoneal shunt in non-hydrocephalic patients. A review of 41 cases.

A population of 41 non-hydrocephalic patients in whom a lumbo-peritoneal shunt (LPS) was inserted for various conditions is reviewed. 19 had persistent cerebro-spinal fluid rhinorrhoea following cranial injury, basal skull surgery or of unknown origin, 3 had recalcitrant benign intra-cranial hypertension, 14 had a persistent bulging craniotomy site after operations for intra-cranial tumours or head trauma, 4 had syringomyelia and 1 had a postoperative cervical meningocele. There was no shunt-related mortality. LPS was effective in treating the initial symptomatology in 31 patients. Further rev

Manipulation of the iris in extracapsular lens extraction.

Successful extracapsular cataract surgery depends on adequate mydriasis, which will facilitate the anterior capsulotomy and provide a full field of view throughout the operation. The surgeon, especially the neophyte, must recognise which irises may present a difficulty in establishing, maintaining, and reversing mydriasis, with or without the introduction of an intraocular lens. The author describes the preliminary assessment of the iris for disease, previous iris surgery, ease of dilatation, and the place of primary iridectomy. Topical and subconjunctival preoperative mydriatics are assessed,

An analysis of semiflexible, closed-loop anterior chamber intraocular lenses.

We present the pathologic and scanning electron microscopic findings from 44 semiflexible, all polymethylmethacrylate, anterior chamber intraocular lenses (IOLs). These IOLs, which have round, small diameter, closed loops were removed following a variety of complications, including uveitis and/or the uveitis-glaucoma-hyphema syndrome, secondary glaucoma, pseudophakic bullous keratopathy, cystoid macular edema, erosion into uveal tissue (with and without oval pupil), and iris neovascularization. Over 200,000 of these lens styles have been implanted. Even though the number of these IOLs accessio

Current concepts in posterior chamber lens technology.

Although extracapsular cataract extraction and posterior chamber lens technology is of unrivaled safety and efficacy, the following aspects command considerable attention: chronic postoperative inflammation, inhibition of posterior capsular opacification, facilitation of YAG-laser posterior capsulectomy, prevention of ultraviolet radiation exposure, and sulcus versus capsular-bag fixation of posterior chamber lenses. The differential diagnosis of lens-induced and intraocular-lens-induced inflammation is presented, along with recommendations for the treatment and management of the second eye. U

Current and future concepts of the design of the intraocular lens.

Anterior chamber lenses are currently much less popular than posterior chamber lenses. They are mainly used in conjunction with intracapsular cataract extraction, as a secondary lens implant, and as a back up lens in case of intraoperative complications of extracapsular cataract extraction. A major disadvantage of anterior chamber lenses is improper sizing. Rigid lenses cause the greatest ocular tenderness and present serious problems with errors in sizing but they are the easiest to explant. Semiflexible and flexible lenses show a greater tendency to develop peripheral anterior synechiae. Thi

[Treatment of retinal detachment by vitrectomy and injection of silicone oil. Long-term results and complications in 105 cases].

The present study is a two year-follow up of 105 eyes (including 27 aphakic eyes), operated on retinal detachment by silicone oil injection after pars plana vitrectomy. This procedure was chosen either as an initial treatment (37 eyes) or after a classical treatment by external indentation had failed (68 eyes). All cases of retinal detachment were of bad prognosis: macular hole, massive periretinal proliferation, isolated or associated with a tear. Cases of vitrectomy with silicone injection for proliferative retinopathy due to diabetes or hemoglobinopathy were excluded. Operations were perfor

Giant papillary conjunctivitis in connection with corneoscleral supramid (nylon) suture knots.

A material consisting of 27 patients had corneoscleral wound closure by 8-0 polyamide (supramid, nylon) sutures after intracapsular cataract extraction and intraocular lens implantation. A few months after surgery 16 patients had developed giant papillary conjunctivitis of the upper tarsus of the operated eye, associated with itching and blurring. The earlier stages of the condition, as described by other authors, could be defined in the remaining 11 patients. Cytologic tests - pipette samples of tear fluid and tarsal conjunctival scrapings - demonstrated eosinophil leucocytes and neutrophilia

[Pseudophakia and the pupil].

Postoperative disturbances of the pupil are discussed on the basis of experience with more than 2500 intraocular lenses implanted after extracapsular cataract extraction (planned extracapsular or phacoemulsification). Apart from the changes to the pupil caused by an iris-fixed intraocular lens itself, the pupil may also be affected by blood, fibrinous inflammation, or remnants of cortex material or the anterior capsule after posterior chamber lens implantation. These conditions are frequently followed by synechiae between the pseudophakia and the iris. Mechanical damage to the iris or the sphi
