Pathomic analysis of 5-year surveillance biopsies as predictors of kidney allograft loss.

This study aimed to assess whether various pathomic features in automatically segmented glomeruli and tubules were associated with death-censored graft loss in kidney transplant recipients. We studied 906 patients who were transplanted between 2000 and 2013, returned to Mayo Clinic for a 5-year surveillance biopsy, and had available subsequent follow-up. Banff scores were obtained from the pathology reports and clinical characteristics from the patients' charts. A multicompartment segmentation model segmented nonsclerotic glomeruli and tubules, and within these compartments, morphometric and t

Live donation of a deceased donor's kidney.

Kidney retransplantation-the reuse of a previously transplanted allograft-is rare, although limited reports suggest clinical feasibility and an underrecognized opportunity within the existing donor pool. We present a novel case involving a living individual donating a previously transplanted deceased donor kidney after early graft failure from recurrent focal segmental glomerulosclerosis. Five days later, that kidney was transplanted into a second recipient, where it has continued to function for over 8 years. This case report also exposed structural barriers that limit broader adoption. The c

Sequential Liver, Kidney, and Heart Transplantation in a Single Recipient: Long-Term Survival Complicated by Chronic Renal Allograft Failure, Lung Cancer, and Recurrent Infection-A Case Report.

BACKGROUND: Multi-organ transplant recipients face a substantial long-term risk of infection, malignancy, and chronic rejection secondary to prolonged immunosuppression. Sequential liver, kidney, and heart transplantations remain rare, resulting in limited evidence to guide perioperative management and long-term follow-up. CASE SUMMARY: A 61-year-old man underwent sequential liver, kidney, and heart transplantations in 2012, 2018, and 2022, respectively. Initial graft function was favorable under an immunosuppressive regimen comprising tacrolimus, mycophenolate mofetil, and corticosteroids. F

Precision Immunosuppression in Heart Transplantation: Biomarker-Guided Therapy Comes of Age.

PURPOSE OF REVIEW: Heart transplantation is the definitive treatment for end-stage heart failure. While the number of heart transplant performed yearly in the United States has nearly doubled in the last decade, median survival has stagnated, largely due to the complications posed by lifelong immunosuppression. A shift towards precision medicine is needed to achieve the optimal balance between risk of allograft rejection and drug-related toxicities to improve post-transplant outcomes. RECENT FINDINGS: Novel molecular based diagnostic tools and biomarkers are being investigated, and hold promi

Raman Urine Fingerprinting to Predict DGF, Assess Renal Allograft Quality, and Monitor Its State of Health.

OBJECTIVES: To determine whether Raman spectroscopy-based urine molecular fingerprinting (UMF) may predict delayed graft function (DGF) and assess renal allograft quality prior to transplant and monitor its state of health after transplantation. BACKGROUND: Current methods for assessing the quality of the renal allograft at baseline and its state of health post-transplantation are inadequate. METHODS: We conducted a pilot study in 26 patients receiving DCD allografts. UMF was performed on urine samples collected from study participants immediately before transplantation (baseline), on postop

Triglyceride- and Body Weight-Based Nomogram for Early Postoperative Mycophenolate Mofetil Dose Selection Following Kidney Transplantation.

BACKGROUND: Early mycophenolic acid (MPA) exposure varies widely after kidney transplantation, and fixed initial mycophenolate mofetil (MMF) dosing may cause overexposure before therapeutic drug monitoring (TDM) becomes available. We explored routinely available pretransplant clinical variables and developed a simple pre-TDM nomogram using triglycerides (TG) and body weight to support early postoperative MMF dose selection in tacrolimus-treated recipients. METHODS: In this single-center retrospective derivation study, 117 consecutive adult kidney transplant recipients were screened, and 116 w

Heart Transplant for Advanced Heart Failure: A Review.

IMPORTANCE: Heart failure affects more than 6 million US adults; approximately 4% to 5% progress to advanced disease despite optimal therapy. Heart transplant is a class I guideline-recommended therapy for select individuals with advanced heart failure. OBSERVATIONS: Early referral for heart transplant is guided by the "I NEED HELP" mnemonic, a tool that identifies patients with advanced heart failure based on criteria such as use of inotropes, persistent symptoms (New York Heart Association class III or IV), end-organ dysfunction, left ventricular ejection fraction, defibrillator shocks, hos

Impact of MELD 3.0 Score on Outcomes After Combined Heart-Liver Transplantation: A UNOS Database Analysis.

INTRODUCTION: Combined heart-liver transplantation (CHLT) is performed for select patients with end-stage heart and liver disease, though the prognostic value of the Model for End-Stage Liver Disease (MELD) score in this population remains unclear. METHODS: We evaluated adult CHLT recipients between March 2005 and May 2025 from the United Network for Organ Sharing database. Groups were stratified by MELD 3.0 score at transplant: 1-14 (Low), 15-24 (Medium), ≥25 (High). Kaplan-Meier analyses assessed overall survival and freedom from liver graft failure, and Cox proportional hazards analysis ev

Development in Left Ventricular Mass After Early and Late Conversion From Calcineurin Inhibitors to Everolimus in Heart Transplant Recipients-Results From Two Randomized Controlled Trials.

Left ventricular hypertrophy in heart transplant recipients has been attributed to the use of calcineurin inhibitors. We assessed left ventricular mass across treatment arms in 83 heart transplant recipients in the Scandinavian heart transplant everolimus de novo study with early calcineurin inhibitors avoidance (SCHEDULE), and in 75 maintenance recipients in the Nordic Certican Trial in Heart and Lung Transplantation (NOCTET). In SCHEDULE, we randomized participants to treatment with calcineurin inhibitors or replacement with everolimus within 7-11 weeks after transplant. In NOCTET, participa

Proton pump inhibitor use after kidney transplantation and graft outcomes: A matched national cohort study.

This study evaluates the impact of proton pump inhibitor (PPI) use on graft-related outcomes following renal transplant. Proton pump inhibitors (PPIs) are frequently continued after kidney transplantation, often without a clear ongoing indication. Although their use has been associated with adverse renal outcomes in non-transplant populations, their clinical relevance in transplant recipients remains uncertain. We performed a retrospective cohort study using the TriNetX database of US health networks. We included 23,306 patients from 2000-2021 enrolled 4-6 weeks following their transplant. Pat

From rejection to vasculopathy: a review of non-invasive biomarkers in alloimmune responses in heart transplantation.

BACKGROUND: Heart transplantation is the gold standard treatment for end-stage heart failure. Despite immunosuppression as the cornerstone therapy for post-transplant management, the presence of immunological complications including acute cellular rejection (ACR), antibody-mediated rejection (AMR), and cardiac allograft vasculopathy (CAV)-remain major barriers to long-term graft survival. Graft monitoring largely remains dependent on protocolized endomyocardial biopsy (EMBx), an invasive, risky procedure. SCOPE: This narrative review focuses on aggregating the published evidence, summarizing

Early Onset Bacteremia in Kidney Transplant Recipients in the Current Era.

BACKGROUND: Bloodstream infection (BSI) after kidney transplantation increases the risk of graft loss and mortality. We aimed to identify contemporary risk factors and outcomes of early BSI (eBSI) at our transplant center. METHODS: We conducted a retrospective chart review of adult kidney transplant recipients (KTRs) from 2019 to 2023. Those with the eBSI (within 90 days of KTR) were matched by age and gender to controls. Risk factors were identified using multivariable logistic regression. All KTRs surviving to 90 days, were included in graft and patient survival analysis. RESULTS: Among 14

DSA-negative, C4d-negative microvascular inflammation after kidney transplantation as an endothelial injury syndrome: A narrative review.

BACKGROUND: Microvascular inflammation (MVI), defined by glomerulitis and peritubular capillaritis, is a major predictor of kidney allograft failure. A clinically important subset develops MVI despite persistently negative donor-specific antibody (DSA) testing and the absence of C4d staining. Although recognized in the Banff 2022 classification, its mechanisms, prognosis, and management remain incompletely defined. OBJECTIVE: To synthesize the evidence on DSA-negative, C4d-negative MVI in kidney transplantation, focusing on prognosis, biological heterogeneity, early post-transplant C4d-negati

Refractory Invasive Pulmonary Aspergillosis Following Treatment of Acute Rejection in a Kidney Transplant Recipient: A Case Report.

Invasive pulmonary aspergillosis (IPA) is an uncommon but highly morbid complication after solid organ transplantation, particularly following treatment for acute rejection. We report a case of refractory IPA caused by Aspergillus terreus in a 70-year-old kidney transplant recipient, diagnosed 2 weeks after receiving thymoglobulin and pulse corticosteroids for acute T cell-mediated rejection. The patient presented with acute hypoxic respiratory failure requiring intubation, and a chest computed tomography scan demonstrated a large, necrotic, mass-like cavitary opacity in the left upper lobe an

A new Dawn: Advances and innovation in heart transplantation part I - cardiac magnetic resonance imaging in heart transplantation.

Heart transplantation (HTx) remains the treatment of choice for patients with advanced heart failure. HTx recipients require intensive surveillance over time. Major complications that should be monitored, prevented, and treated in a timely manner include graft dysfunction secondary to cellular rejection or antibody-mediated rejection (AMR) and coronary allograft vasculopathy (CAV). While echocardiographic surveillance plays a key role in detecting major changes in graft function and endomyocardial biopsy remains the most widely used invasive technique for the surveillance of graft rejection, t

Case Report: Kidney allograft recovery after conversion to belatacept-based immunosuppression in simultaneous heart-kidney transplantation.

Renal allograft dysfunction after simultaneous heart-kidney transplantation (SHKT) is often multifactorial, reflecting perioperative hemodynamic instability, ischemia-reperfusion injury, alloimmune activation, and calcineurin inhibitor exposure. The therapeutic role of belatacept in this dual-organ setting remains poorly defined. We report the case of a 47-year-old woman with systemic lupus erythematosus, antiphospholipid syndrome, previous kidney allograft loss and heart failure, who underwent SHKT. The post-transplant course was complicated by persistent anuria requiring prolonged dialysis d

Current and Future Applications of AI-Driven Predictive Modeling and a Proposed Framework for AI-Bioprognostics in Kidney Care.

Artificial intelligence (AI) is reshaping kidney care by integrating electronic health records (EHRs), imaging, digital pathology, genomics, wearable biosensors, and longitudinal physiologic data into dynamic risk-prediction systems. Traditional nephrology risk stratification has relied on static variables and regression-based models such as estimated glomerular filtration rate (eGFR), albuminuria, and composite clinical scores, which often inadequately capture nonlinear disease trajectories and phenotypic heterogeneity. Impact of Operative Time on Pediatric Heart Transplant Outcomes: An Analysis of the United Network for Organ Sharing Registry.

BACKGROUND: Heart transplantation is definitive therapy for children with end-stage heart failure. Because donor availability is unpredictable, transplantation occurs overnight or on weekends. Although studies report mixed findings on the association between operative timing and outcomes, pediatric data remain limited. We evaluated the impact of operative timing on pediatric heart transplant outcomes. METHODS: We studied pediatric heart transplant recipients (< 18 years) in the United Network for Organ Sharing registry from 2000 to 2018. Transplants were classified by time of day (day: 9 AM-8
