BACKGROUND: Kidney transplantation is the definitive treatment for end-stage renal disease. Late allograft dysfunction has complex and heterogeneous etiologies, and concurrent overlapping pathologies pose substantial clinical challenges that require integrated multidisciplinary management.
CASE PRESENTATION: A 23-year-old female with previously stable allograft function on maintenance immunosuppression presented with progressive clinical deterioration 6 years after deceased-donor kidney transplantation. Initial respiratory symptoms progressed to tonic-clonic seizure, neuropsychiatric disturba

BACKGROUND: Abdominal normothermic regional perfusion (aNRP) may mitigate ischemic injury in donation after circulatory death (DCD). Specific aNRP-DCD kidney transplant (KTx) outcomes are inadequately defined. Kidney Donor Profile Index (KDPI) assigns equivalent DCD risk components to all DCD kidneys and may not capture procurement-related differences.
METHODS: We retrospectively analyzed KTx from donors in a single donation service area procured between January 2023-December 2024. DCD-restricted multivariable logistic and Cox models compared aNRP-DCD with super-rapid recovery (SRR)-DCD and e

INTRODUCTION: C4d deposition is common after ABO-incompatible (ABOi) kidney transplantation and, unlike ABO-compatible grafts, is not necessarily indicative of antibody-mediated injury.
METHODS: Between 2015 and 2022, 254 ABOi kidney transplantations were performed at our center, and 69 recipients with paired 3-month and 1-year protocol biopsies were classified as persistently C4d-positive (n = 54) or C4d-negative/nonpersistent (n = 15). Pre-transplant background, cellular infiltration, chronic Banff lesions according to the Banff 2019 classification, graft function, and graft survival were c

Background: Cardiovascular disease remains a major determinant of long-term outcomes after kidney transplantation. Haptoglobin (HP) is a genetically polymorphic hemoglobin-binding protein involved in hemoglobin scavenging, oxidative stress, and inflammation. Although the HP 2-2 genotype has been associated with cardiometabolic risk in non-transplant populations, its clinical relevance after kidney transplantation remains uncertain. Methods: We conducted a single-center retrospective cohort study of 229 adult kidney transplant recipients who underwent recipient HP genotyping by polymerase chain

Heart transplantation (HTx) remains the gold standard treatment for selected patients with advanced heart failure and survival after transplantation has substantially improved over recent decades. As a consequence, the number of long-term HTx recipients continues to increase, and these patients are now frequently encountered in routine practice outside specialized transplant centers. Their management remains complex due to the use of chronic immunosuppression, transplant-specific complications, and a growing burden of comorbidities in the perspective of increasing longevity. Non-transplant car

Advanced heart failure affects over 6 million adults in the United States alone. Orthotopic heart transplantation remains the definitive treatment, with over 90% one-year survival and meaningful gains in functional status and quality of life. The therapy is, however, constrained by an intractable supply problem. For decades, xenotransplantation (XTx), the transplantation of organs across species, has been proposed as a means of closing this gap, though for most of that time it remained a theoretical aspiration. With the discovery of CRISPR-Cas9 gene editing, improved immunosuppressive strategi

BACKGROUND: Appropriate matching between donor nephron mass and recipient metabolic demand is increasingly recognized as an important determinant of kidney graft outcomes. However, practical and widely applicable methods for estimating graft-recipient size matching remain limited. We investigated whether renal parenchymal volume measured by routine ultrasonography and normalized to recipient body mass index (BMI) is associated with kidney graft function one year after transplantation.
METHODS: In this prospective single-center study, adult kidney transplant recipients underwent routine ultras

Kidney transplantation (KT) remains the optimal therapy for end-stage kidney disease (ESKD), yet long-term graft survival is limited by unremitting allograft dysfunction and non-immune treatment toxicities. Sterile inflammation primarily drives graft failure. This inflammatory process releases damage-associated molecular patterns (DAMPs). DAMPs activate pattern recognition receptors (PRRs), such as Toll-like receptors (TLRs) 2 and 4, from proximal tubule epithelial cells, endothelial compartments, and infiltrating myeloid lineages. PRR signaling and the release of pro-inflammatory cytokines (I

Renal artery intimal injury during kidney transplant is infrequent but potentially devastating, especially when situated near the hilar trifurcation, where standard reconstruction techniques are frequently unfeasible. Prompt identification is difficult because the arterial pulse may persist despite significant luminal obstruction. We report a case involving a 24-year-old female living donor whose left kidney was transplanted into a 54-year-old male recipient with end-stage renal disease. Although initial reperfusion appeared satisfactory, the graft gradually became cyanotic and soft during the

Advanced heart failure afflicts over 6 million adults in the United States alone, yet fewer than 4,000 donor hearts become available for transplantation each year. Mechanical circulatory support devices extend survival for many of these patients, but device-related complications, anatomic contraindications, and limited applicability to certain populations leave a substantial unmet need. For decades, xenotransplantation (XTx)-the transplantation of organs across species-has been proposed as a means of closing this gap. This study aimed to assess whether various pathomic features in automatically segmented glomeruli and tubules were associated with death-censored graft loss in kidney transplant recipients. We studied 906 patients who were transplanted between 2000 and 2013, returned to Mayo Clinic for a 5-year surveillance biopsy, and had available subsequent follow-up. Banff scores were obtained from the pathology reports and clinical characteristics from the patients' charts. A multicompartment segmentation model segmented nonsclerotic glomeruli and tubules, and within these compartments, morphometric and t

Kidney retransplantation-the reuse of a previously transplanted allograft-is rare, although limited reports suggest clinical feasibility and an underrecognized opportunity within the existing donor pool. We present a novel case involving a living individual donating a previously transplanted deceased donor kidney after early graft failure from recurrent focal segmental glomerulosclerosis. Five days later, that kidney was transplanted into a second recipient, where it has continued to function for over 8 years. This case report also exposed structural barriers that limit broader adoption. The c

BACKGROUND: Multi-organ transplant recipients face a substantial long-term risk of infection, malignancy, and chronic rejection secondary to prolonged immunosuppression. Sequential liver, kidney, and heart transplantations remain rare, resulting in limited evidence to guide perioperative management and long-term follow-up.
CASE SUMMARY: A 61-year-old man underwent sequential liver, kidney, and heart transplantations in 2012, 2018, and 2022, respectively. Initial graft function was favorable under an immunosuppressive regimen comprising tacrolimus, mycophenolate mofetil, and corticosteroids. F

PURPOSE OF REVIEW: Heart transplantation is the definitive treatment for end-stage heart failure. While the number of heart transplant performed yearly in the United States has nearly doubled in the last decade, median survival has stagnated, largely due to the complications posed by lifelong immunosuppression. A shift towards precision medicine is needed to achieve the optimal balance between risk of allograft rejection and drug-related toxicities to improve post-transplant outcomes.
RECENT FINDINGS: Novel molecular based diagnostic tools and biomarkers are being investigated, and hold promi

OBJECTIVES: To determine whether Raman spectroscopy-based urine molecular fingerprinting (UMF) may predict delayed graft function (DGF) and assess renal allograft quality prior to transplant and monitor its state of health after transplantation.
BACKGROUND: Current methods for assessing the quality of the renal allograft at baseline and its state of health post-transplantation are inadequate.
METHODS: We conducted a pilot study in 26 patients receiving DCD allografts. UMF was performed on urine samples collected from study participants immediately before transplantation (baseline), on postop

BACKGROUND: Early mycophenolic acid (MPA) exposure varies widely after kidney transplantation, and fixed initial mycophenolate mofetil (MMF) dosing may cause overexposure before therapeutic drug monitoring (TDM) becomes available. We explored routinely available pretransplant clinical variables and developed a simple pre-TDM nomogram using triglycerides (TG) and body weight to support early postoperative MMF dose selection in tacrolimus-treated recipients.
METHODS: In this single-center retrospective derivation study, 117 consecutive adult kidney transplant recipients were screened, and 116 w

IMPORTANCE: Heart failure affects more than 6 million US adults; approximately 4% to 5% progress to advanced disease despite optimal therapy. Heart transplant is a class I guideline-recommended therapy for select individuals with advanced heart failure.
OBSERVATIONS: Early referral for heart transplant is guided by the "I NEED HELP" mnemonic, a tool that identifies patients with advanced heart failure based on criteria such as use of inotropes, persistent symptoms (New York Heart Association class III or IV), end-organ dysfunction, left ventricular ejection fraction, defibrillator shocks, hos

INTRODUCTION: Combined heart-liver transplantation (CHLT) is performed for select patients with end-stage heart and liver disease, though the prognostic value of the Model for End-Stage Liver Disease (MELD) score in this population remains unclear.
METHODS: We evaluated adult CHLT recipients between March 2005 and May 2025 from the United Network for Organ Sharing database. Groups were stratified by MELD 3.0 score at transplant: 1-14 (Low), 15-24 (Medium), ≥25 (High). Kaplan-Meier analyses assessed overall survival and freedom from liver graft failure, and Cox proportional hazards analysis ev

Left ventricular hypertrophy in heart transplant recipients has been attributed to the use of calcineurin inhibitors. We assessed left ventricular mass across treatment arms in 83 heart transplant recipients in the Scandinavian heart transplant everolimus de novo study with early calcineurin inhibitors avoidance (SCHEDULE), and in 75 maintenance recipients in the Nordic Certican Trial in Heart and Lung Transplantation (NOCTET). In SCHEDULE, we randomized participants to treatment with calcineurin inhibitors or replacement with everolimus within 7-11 weeks after transplant. In NOCTET, participa
