Predicting kidney graft survival with a machine learning model based on for-cause biopsy transcriptomics.

Kidney transplantation significantly improves outcomes in end-stage renal disease, however, long-term graft survival remains suboptimal. Traditional prognostic models often yield limited predictive power. Given the routine use of for-cause biopsies in clinical practice, we aimed to develop and compare ML models to predict graft survival using gene expression profiles from indication biopsies. Data from for-cause renal biopsies were collected from six cohorts in the Gene Expression Omnibus: GSE21374 (n = 282), GSE48581 (n = 306), GSE36059 (n = 411), GSE50058 (n = 101), GSE72925 (n = 168) and GS

Tubule-derived growth differentiation factor 15 limits renal transplant injury by reprogramming macrophage responses.

Kidney transplantation remains the gold standard for end-stage renal disease, but ischemia-reperfusion injury and delayed graft function continue to hinder outcomes. Growth differentiation factor 15 (GDF15), a stress-responsive cytokine from the transforming growth factor-β superfamily, is upregulated in response to cellular injury and hypoxia. Although GDF15 has been studied in acute kidney injury and sepsis, its role in kidney transplantation remains unclear. In this study, we combined transcriptomic analysis of human kidney allografts with murine models to investigate GDF15's role in transp

Risk factors for cancer and the importance of screening in adult recipients of living-donor kidney transplant.

INTRODUCTION: The incidence of cancer after kidney transplantation (KT) increases owing to the use of immunosuppressive (IS) medications, which may lead to death with a functioning graft (DWFG) and reduced long-term graft survival. However, the effect of IS medications, including their blood levels, on cancer development before graft failure remains underexplored. METHODS: This single-center retrospective cohort study involved 1274 living-donor KTs performed between January 2008 and December 2021. Cancer was diagnosed in 141 recipients before graft failure. DWFG and mortality-free survival we

Clinical Characteristics, Long-Term Pharmacokinetics, and Outcomes in Kidney Transplant Recipients from an African Tertiary Centre: A 10-Year Single-Centre Retrospective Review.

Background: Kidney transplantation outcomes in resource-limited settings remain underreported. This 10-year retrospective review examined the clinical characteristics, long-term pharmacokinetics, and outcomes of kidney transplant recipients at a South African public hospital. Methods: Data from kidney transplant recipients between January 2012 and December 2022 were analysed. Graft and patient survival were assessed using Kaplan-Meier analysis. Cox proportional hazards models were used to evaluate the associations between clinical and pharmacokinetic variables and outcomes. Results: The one- a

Injured epithelial cell states impact kidney allograft survival after T-cell-mediated rejection.

T-cell-mediated rejection (TCMR) remains a major cause of kidney transplant failure, despite being considered treatable. Its impact reflects a limited understanding of the underlying molecular mechanisms and their clinical consequences. To address this, we induced acute TCMR in mouse kidney transplants and profiled molecular changes using single-nucleus RNA sequencing (snRNA-seq), spatial transcriptomics and immunofluorescence. Results were compared with human snRNA-seq data from TCMR and stable allografts, as well as single-cell deconvolution analysis of bulk transcriptomic data from kidney t

Update on diagnostic and therapeutic strategies for antibody-mediated rejection in kidney transplantation.

Antibody-mediated rejection (AMR) remains a leading cause of kidney allograft failure, posing significant clinical and economic challenges. Donor-specific antibodies against human leukocyte antigens or non-human leukocyte antigens are critical risk factors for AMR and graft loss. The diagnostic criteria and classification of AMR have evolved considerably over the past three decades, driven largely by the Banff classification. The latest Banff 2022 classification introduced two additional subcategories of "microvascular inflammation, donor-specific antibody-negative, C4d-negative" and "probable

Preformed vs de novo anti-human leukocyte antigens-DQ antibodies in kidney transplantation: A retrospective study.

BACKGROUND: Donor-specific antibodies (DSAs) against human leukocyte antigen (HLA)-DQ are increasingly recognized as major contributors to antibody-mediated rejection (AMR) and graft failure in kidney transplantation. However, their clinical impact remains understudied in Morocco. AIM: To evaluate the presence and implications of anti-HLA-DQ DSAs in Moroccan kidney transplant recipients. METHODS: We retrospectively analyzed the immunological profiles and clinical outcomes of kidney transplant recipients screened for anti-HLA antibodies between 2015 and 2020, who developed anti-HLA-DQ DSAs ei

Global Diabetes: Rapidly Changing Landscape and a Rising Tide of Opportunities: The 2025 Distinguished International Service in the Cause of Diabetes Award Lecture.

Diabetes affects an estimated 828 million people worldwide; prevalence is growing rapidly in low- and middle-income countries (LMICs), with major health and development consequences. Diabetes-related complications and mortality rates have declined across several high-income countries (HICs), largely through structured chronic disease care, sustained implementation of quality improvement strategies, and active risk factor control. However, rates of diabetes complications and mortality have been rising in LMICs. Along with evidence there are tools (e.g., teams, task shifting, data systems, decis

Untargeted Plasma Proteomic Signatures and Late Graft Failure in Kidney Transplant Recipients.

BACKGROUND: Long-term kidney graft monitoring still relies on delayed clinical indicators such as eGFR and proteinuria, which change only after substantial injury. To investigate biological processes not captured by standard metrics, we hypothesized that plasma proteomics could identify biomarkers and reveal the pathways underlying the pathological process, thereby providing independent prognostic value for late graft failure. METHODS: In this retrospective matched case-control study, we analyzed pre-event plasma samples from 29 kidney transplant recipients who later experienced late, death-c

An integrative algorithm combining HLA epitope registry, PIRCHE-T2, and PIRCHE-B outcomes to improve immunological risk stratification in kidney transplantation.

AIM: Kidney transplantation remains the most effective treatment for end-stage kidney disease. Still, the development of de novo donor-specific antibodies (dnDSA) increases the risk of rejection and allograft failure. While molecular matching algorithms assess B-cell and T-cell epitope mismatches, no single method fully captures rejection risk across immune pathways. This study combines the HLA Epitope Registry (Epregistry), PIRCHE-T2, and PIRCHE-B scores to enhance risk stratification, allowing for early intervention in high-risk recipients and improving long-term outcomes. METHODS: A retros

"Rethinking High-KDPI Kidneys: A Multidomain Approach to Predicting Success".

BACKGROUND: The allocation and acceptance of deceased-donor kidneys in the United States is influenced by theKidney Donor Profile Index (KDPI). We conducted a national analysis of high-KDPI kidney transplants performed from 2014 to 2021 to identify key predictors of post-transplant outcomes beyond those incorporated in KDPI. METHODS: This retrospective cohort study used data extracted from the Scientific Registry of Transplant Recipients (SRTR). Adult, first-time recipients of kidney-only deceased-donor transplants with KDPI greater than 85% were included. Regression models were used to ident

Collapsing FSGS in a kidney transplant with a single APOL1 risk variant: a case report.

BACKGROUND: Apolipoprotein L1 (APOL1) genetic risk variants substantially increase the risk of chronic kidney disease and kidney failure among individuals of recent African ancestry. While APOL1-associated nephropathy was long considered to follow a recessive inheritance pattern, recent studies have demonstrated an additive effect, indicating that even a single risk variant may be clinically significant. A severe manifestation of APOL1-mediated kidney disease, collapsing focal segmental glomerular sclerosis, has been reported mainly in allografts from donors with two risk variants. CASE PRESEN

HLA Experts' Perspectives on Precision Medicine and Molecular Matching in Kidney Transplantation: A Qualitative Study.

BACKGROUND: Kidney transplantation is the best treatment for chronic kidney failure, but antibody-mediated rejection (AMR) is a major cause of kidney transplant loss. Human leukocyte antigen (HLA) molecular-compatibility-based organ allocation aims to reduce the risk of donor-specific antibody formation and thereby lower the risk of AMR. However, integrating molecular compatibility in organ allocation could introduce barriers to access, consequently raising ethical concerns. OBJECTIVE: The objective of this study was to gather perspectives of HLA professionals on molecular matching in kidney

Exfoliated kidney cells from urine for non-invasive kidney transplant monitoring: A potential opportunity?

Kidney transplantation is usually the optimal treatment option for patients living with kidney failure given its associations with improved survival, quality of life outcomes and a reduction in the personal, economic, and societal burden of long-term dialysis. While advantages of kidney transplantation are recognized, post-transplant complications, such as graft rejection, ischemia-reperfusion injury, surgical-related complications, and long-term consequences of immunosuppressive therapies, are commonly observed. There has been increased research on developing non-invasive biomarkers for the m

Medication Level Variability During First Year After Pediatric Kidney Transplantation.

BACKGROUND: Variability in tacrolimus levels, measured with the Medication Level Variability Index (MLVI), and intrapatient variability (IPV) may be associated with graft outcomes in solid organ transplant recipients. However, the optimal method for measuring tacrolimus variability, the optimal timing for measurement, and intervention thresholds for pediatric kidney transplant recipients remain uncertain. METHODS: Our retrospective study included 149 pediatric kidney transplant recipients who received tacrolimus for maintenance immunosuppression. MLVI and IPV were assessed across 3 post-trans

Immunometabolic transcriptomic signatures of diabetes-related genes predict kidney transplant rejection.

OBJECTIVES: Kidney transplantation remains the gold standard for end-stage renal disease, yet long-term graft survival is limited by acute and chronic rejection. Emerging evidence highlights the critical role of immunometabolism, the bidirectional relationship between alloimmunity and metabolic dysregulation, specifically in the context of post-transplant diabetes mellitus (PTDM). This study investigates whether the expression of genes traditionally associated with diabetes susceptibility can predict kidney transplant rejection. METHODS: We performed a transcriptomic reanalysis of a publicly

Dual Kidney Transplantation Offers Prolonged Graft Survival.

INTRODUCTION: Dual kidney transplantation (DKT), an uncommonly performed procedure, provides a unique opportunity to transplant nonstandard kidneys that might otherwise not be utilized. We compared perioperative and five-year posttransplant outcomes between DKT, and single kidney transplants (SKT) performed at our institution. METHODS: We analyzed all adult deceased donor kidney-alone transplant recipients at our center between 2001 and 2020. Recipients of pediatric en bloc kidney transplants were excluded. Perioperative outcomes of interest included delayed graft function (DGF), posttranspla

The burden of critical illness in kidney transplant recipients-beyond 1 year post-transplant.

Patients with chronic kidney disease who undergo kidney transplantation are at risk of acute illness, acute graft failure, and intensive care admission in the years following transplantation. To identify individual risk factors, assess the effect of immunosuppression, and evaluate outcomes including loss of graft function in a European setting, we analyzed 266 intensive care unit (ICU) admissions of kidney transplant recipients (KTRs) with severe illness between 2005 and 2019. Admission to the ICU occurred predominantly 12 months or later after transplantation, with a median time of 52.7 month
