Clinical Impact of Preoperative Obesity on Living-Donor Kidney Transplant Recipients in Japan: A Multicenter Experience.

Background: Obesity is increasingly prevalent among kidney transplant candidates; however, its impact on graft outcomes in Asian populations is not well characterized. We evaluated the association between preoperative obesity and living-donor kidney transplantation outcomes in Japan. Methods: We analyzed 623 living-donor kidney transplants performed from 1998 to 2021 at six centers in northern Japan. Recipients were categorized by body mass index (BMI) at transplant, and multivariable Cox regression was employed for assessing graft outcomes. Results: Obesity (BMI, ≥30 kg/m2; n = 27 [4.3%]) was

Kidney transplantation after chronic active Epstein-Barr virus infection remission.

Chronic active Epstein-Barr virus is a rare but severe condition characterized by persistent Epstein-Barr virus activation, leading to chronic lymphoproliferation, significant morbidity, and mortality. We report the case of a 34-year-old male with autosomal dominant polycystic kidney disease who achieved chronic active Epstein-Barr virus remission through hematopoietic stem cell transplantation in childhood. Despite long-term remission, the patient developed end-stage renal disease and underwent ABO-incompatible living-donor kidney transplantation. Post-transplant monitoring over 7 months reve

Changes by Era in Risk Factors and Outcomes Among Deceased Donor Kidney Transplant Recipients With Delayed Graft Function.

INTRODUCTION: There are no effective therapeutic agents for preventing or treating delayed graft function (DGF) among deceased donor kidney transplant recipients (DDKTRs). Donor and recipient factors are important to predicting DGF and associated outcomes, which we hypothesize differed over time. METHODS: DDKTRs were stratified by transplant year into four eras-E1 (2000-2005), E2 (2006-2011), E3 (2012-2017), and E4 (2018-2021). We analyzed risk factors for DGF, along with one-year uncensored graft failure (UCGF), death-censored graft failure (DCGF), death with a functioning graft (DWFG), and

The importance of staying within range: associations between tacrolimus intrapatient variability and kidney transplant outcomes.

BACKGROUND: Kidney transplantation remains the treatment of choice for end-stage kidney disease. Maintaining immunosuppression within the appropriate therapeutic range is essential to prevent rejection and ensure long-term graft survival. This study evaluated the clinical relevance of different tacrolimus exposure metrics and their association with post-transplant outcomes in a real-world kidney transplant cohort. METHODS: Of 881 adult deceased-donor kidney transplants performed between 2011 and 2020 at Heidelberg University Hospital, 372 recipients with a functioning graft at day 180 met the

Mismatched Blood Types between Donors and Recipients Increase the Risk of BK Viremia One Year after Kidney Transplantation.

BACKGROUND: Kidney transplant recipients are at increased risk for infections such as BK virus, with few treatment options. This study assessed how nonidentical but compatible ABO blood types influence BKPyV-DNAemia risk. METHODS: We conducted a retrospective single-center study from 1/1/2011 to 1/12/2021, focusing on BK viremia (>10 000 copies/mL in two consecutive measurements one week apart) within the first posttransplant year. We used stepwise forward regression for multivariate analysis. RESULTS: Of the 1244 transplants, 1084 involved identical blood types. The percentage of living don

Exploring the impact of sleep apnea on renal transplant outcomes.

BACKGROUND: Sleep apnea (SA), a common diagnosis, has been shown to exhibit increased prevalence in patients with end-stage renal disease (ESRD). There is no clear consensus on the impact of SA on renal transplant outcomes. METHODS: We performed a retrospective analysis of the United States Renal Data System (USRDS) to evaluate the impact of SA on renal transplant outcomes, including transplant failure or rejection, post-transplantation new onset chronic kidney disease (CKD) levels 3, 4, or 5, and mortality. Demographic variables of interest included age at transplant, race, sex, ethnicity, a

Pregnancy Outcomes Among Women With Kidney Allografts in Brazil: An Integrative Review.

OBJECTIVES: Chronic kidney disease can reduce fertility, but fertility may improve after transplant. Posttransplant pregnancy carries risks like preeclampsia, miscarriage, and prematurity. Although pregnancy is an immune-tolerant state, changes in immunosuppressive drugs during pregnancy can increase graft rejection risk. This study evaluated maternal and fetal outcomes in Brazilian kidney transplant recipients with post-transplant pregnancies. MATERIALS AND METHODS: We conducted an integrative review of articles from PubMed/Medline databases, analyzing study periods, study design, immunosup-

PANoptosis-Based Diagnostic Model Using Single-Cell and Transcriptome RNA Sequencing to Predict Rejection in Kidney Transplant Patients.

OBJECTIVES: Kidney transplantation is the most effective treatment for end-stage renal failure, but transplant rejection remains a major challenge. The role of PANoptosis in rejection is not fully understood. MATERIALS AND METHODS: We performed single-cell analysis of PANoptosis-related differentially expressed genes in kidney transplant rejection using data from the GEO database. We identified 7 core PANoptosis genes associated with rejection from 2 machine learning algorithms. We constructed a clinical predictive model, which we evaluated for efficacy and calibration. We predicted potential

Simultaneous islet-kidney transplantation as an alternative for patients ineligible for pancreas transplant: Initial experience at a North American transplant center.

Simultaneous pancreas-kidney (SPK) transplantation is the gold standard for patients with diabetes mellitus and end-stage renal disease, but many are ineligible due to surgical risk. Simultaneous islet-kidney (SIK) transplantation offers a less invasive alternative by combining kidney transplantation with islet infusion. We report our initial North American experience with SIK. Between July 2022 and April 2025, 13 patients with type 1 diabetes mellitus and end-stage renal disease underwent SIK after being declined for SPK, primarily due to peripheral vascular disease. Follow-up ranged from 4 t

Investigational New Drug Enabling Nonclinical Study of Xenogeneic Life-Supporting Porcine Kidneys With 10 Gene Edits (10 GE) in a Nonhuman Primate Test System.

This Investigational New Drug (IND) enabling study evaluated life-supporting kidney xenotransplantation using porcine source animals with 10 gene edits (10 GE) in a nonhuman primate (NHP) test system. Twelve baboons received xenografts with either calcineurin inhibitor (CNI)-based or CD40/154 costimulation blockade immunosuppression. Source-specific screening prevented early xenograft antibody-mediated rejection in recipients, and clinically relevant preservation with hypothermic machine perfusion-maintained xenograft viability after off-site procurement at a high health herd facility. No Increased Risk of Infection Following a Protocol Change to Decrease Duration of Perioperative Antibiotic Prophylaxis in Liver Transplantation.

BACKGROUND: Liver transplant recipients are at high risk of bacterial surgical site infection (SSI) following transplantation. However, they also have increased risks of antibiotic complications such as C. difficile and resistant organism infections. The optimal duration of perioperative antibiotic prophylaxis for liver transplantation is uncertain. METHODS: We analyzed infection outcomes in a total of 192 patients before and after a protocol change that shortened antibiotic prophylaxis duration at our institution. Durations were risk-stratified with longer durations for patients hospitalized

Machine perfusion modulates cold-preservation injury in kidney transplantation: IDEAL stage 4 United States population cohort study.

Hypothermic machine perfusion (HMP) has surged in popularity for donor kidney preservation. Continuous HMP (cHMP) has shown clear benefits over static cold storage (SCS), whereas randomized trials on short-duration end-ischemic-HMP (eiHMP) have not. We assessed whether HMP modulates injury from increasing cold-preservation time (CPT) and analyzed the impact of HMP on short- and long-term outcomes, using Organ Procurement and Transplantation Network data (OPTN; 2010-2024) on single-kidney-transplant recipients (n = 137 835). Multivariable nonlinear (restricted cubic spline) regression with inte

Identification of lactylation and its hub genes in contributing immune activation and renal allograft fibrosis by integrative bioinformatics and machine learning.

BACKGROUND: Late graft loss due to chronic renal allograft fibrosis remains a major challenge after kidney transplantation. Excessive immune-cell activation is a key driver of allograft fibrosis; however, the underlying mechanisms remain incompletely understood. Recent studies have implicated lactylation, a post-translational protein modification derived from lactate, in immune activation. Nonetheless, the role of lactylation in renal allograft fibrosis has not been systematically explored. METHODS: Transcriptomic datasets from kidney transplant recipients with and without interstitial fibros

Characterizing the Impact of Induction Therapy on BK Polyoma Viral Reactivation: A Single-Center Analysis.

PURPOSE: BKPyV following kidney transplantation is well recognized to cause premature graft failure and ureteral complications. Cumulative immunosuppression, including induction therapy, has a sustained impact on immune responses to opportunistic infection. We hypothesized that induction with cell-depleting agents affects the 1-year risk of BKPyV reactivation. METHODS: Of 456 adult patients who underwent kidney transplants between January 2018 and February 2023 with at least 12 months of follow-up were screened for BK virus by blood PCR at 1.5, 3,6,9,12 months. RESULTS: Among the 456 patient

ABO Incompatible Kidney Transplantation in Pakistan: A Major Breakthrough.

Pakistan, a resource-limited country, faces a growing number of patients with end-stage kidney disease, many of whom could benefit from living-related kidney transplantation. To expand the living donor pool, we implemented the largest ABO-incompatible kidney transplantation program in the country. The program started in February 2023, and 21 transplants have been performed to date. Pretransplant desensitization consisted of rituximab (375 mg/m²) at day (D) -30, tacrolimus, mycophenolic acid, and steroids starting at D -15, along with apheresis sessions to achieve isoagglutinin titers below 1:4

Eculizumab Successfully Rescues Against de novo Atypical Hemolytic Uremic Syndrome Following Retransplantation.

Kidney transplantation has emerged as the optimal treatment for end-stage renal disease. However, the occurrence of atypical hemolytic uremic syndrome (aHUS) following renal transplantation is extremely uncommon and associated with adverse outcomes, often resulting in early graft loss, thus warranting heightened awareness. This report details a case of successful management of a patient who developed post-transplant aHUS following retransplantation. The patient had previously suffered graft failure shortly after the first kidney transplantation due to unexplained non-rejection mechanisms. Foll

Acute Kidney Injury in Pregnancy among Kidney Transplant Recipients.

Acute kidney injury (AKI) in pregnancy among women with kidney transplants is rarely addressed in the literature. There is no consensus definition of AKI among this population, and limited data exist on the different aetiologies and outcomes, thereby creating a significant knowledge gap. Pregnancy among patients with a kidney transplant is considered high risk due to the possibility of foetal and maternal risks, AKI and graft loss during pregnancy. This is a case series report of two post-kidney transplant patients who developed multiple episodes of AKI in the course of pregnancy. There was no

Risk stratification by donor-derived cell-free DNA: a three-year single center study of kidney transplant outcomes from 257 patients.

BACKGROUND: Donor-derived cell-free DNA is an established blood-based biomarker used to assess alloimmune activity after kidney transplantation. METHODS: We performed a retrospective study of 257 kidney transplant recipients that had at least one dd-cfDNA measurements during 3-year period. The primary aim was to assess the association between dd-cfDNA levels with graft function and survival. Secondary exploratory aims included examining the relationships between dd-cfDNA strata and biopsy-proven rejection, donor-specific antibodies, C4d deposition, and longitudinal eGFR trajectories.
