Pathomic analysis of 5-year surveillance biopsies as predictors of kidney allograft loss.

This study aimed to assess whether various pathomic features in automatically segmented glomeruli and tubules were associated with death-censored graft loss in kidney transplant recipients. We studied 906 patients who were transplanted between 2000 and 2013, returned to Mayo Clinic for a 5-year surveillance biopsy, and had available subsequent follow-up. Banff scores were obtained from the pathology reports and clinical characteristics from the patients' charts. A multicompartment segmentation model segmented nonsclerotic glomeruli and tubules, and within these compartments, morphometric and t

Live donation of a deceased donor's kidney.

Kidney retransplantation-the reuse of a previously transplanted allograft-is rare, although limited reports suggest clinical feasibility and an underrecognized opportunity within the existing donor pool. We present a novel case involving a living individual donating a previously transplanted deceased donor kidney after early graft failure from recurrent focal segmental glomerulosclerosis. Five days later, that kidney was transplanted into a second recipient, where it has continued to function for over 8 years. This case report also exposed structural barriers that limit broader adoption. The c

Sequential Liver, Kidney, and Heart Transplantation in a Single Recipient: Long-Term Survival Complicated by Chronic Renal Allograft Failure, Lung Cancer, and Recurrent Infection-A Case Report.

BACKGROUND: Multi-organ transplant recipients face a substantial long-term risk of infection, malignancy, and chronic rejection secondary to prolonged immunosuppression. Sequential liver, kidney, and heart transplantations remain rare, resulting in limited evidence to guide perioperative management and long-term follow-up. CASE SUMMARY: A 61-year-old man underwent sequential liver, kidney, and heart transplantations in 2012, 2018, and 2022, respectively. Initial graft function was favorable under an immunosuppressive regimen comprising tacrolimus, mycophenolate mofetil, and corticosteroids. F

Raman Urine Fingerprinting to Predict DGF, Assess Renal Allograft Quality, and Monitor Its State of Health.

OBJECTIVES: To determine whether Raman spectroscopy-based urine molecular fingerprinting (UMF) may predict delayed graft function (DGF) and assess renal allograft quality prior to transplant and monitor its state of health after transplantation. BACKGROUND: Current methods for assessing the quality of the renal allograft at baseline and its state of health post-transplantation are inadequate. METHODS: We conducted a pilot study in 26 patients receiving DCD allografts. UMF was performed on urine samples collected from study participants immediately before transplantation (baseline), on postop

Triglyceride- and Body Weight-Based Nomogram for Early Postoperative Mycophenolate Mofetil Dose Selection Following Kidney Transplantation.

BACKGROUND: Early mycophenolic acid (MPA) exposure varies widely after kidney transplantation, and fixed initial mycophenolate mofetil (MMF) dosing may cause overexposure before therapeutic drug monitoring (TDM) becomes available. We explored routinely available pretransplant clinical variables and developed a simple pre-TDM nomogram using triglycerides (TG) and body weight to support early postoperative MMF dose selection in tacrolimus-treated recipients. METHODS: In this single-center retrospective derivation study, 117 consecutive adult kidney transplant recipients were screened, and 116 w

Proton pump inhibitor use after kidney transplantation and graft outcomes: A matched national cohort study.

This study evaluates the impact of proton pump inhibitor (PPI) use on graft-related outcomes following renal transplant. Proton pump inhibitors (PPIs) are frequently continued after kidney transplantation, often without a clear ongoing indication. Although their use has been associated with adverse renal outcomes in non-transplant populations, their clinical relevance in transplant recipients remains uncertain. We performed a retrospective cohort study using the TriNetX database of US health networks. We included 23,306 patients from 2000-2021 enrolled 4-6 weeks following their transplant. Pat

Surgical and Clinical Outcomes of Kidney Transplantation in Obese Recipients: A Single-Center Retrospective Study from Switzerland.

BACKGROUND: The impact of recipient obesity on kidney transplant outcomes remains controversial. This study aimed to assess the effect of recipient obesity on short- and long-term outcomes following kidney transplantation (KT). METHODS: This single-center retrospective cohort study included all consecutive adult patients who underwent KT between January 1999 and December 2023 at the Transplantation Center of Lausanne University Hospital (CHUV), Lausanne, Switzerland. Surgical complications, delayed graft function (DGF), and graft survival were compared between obese (BMI ≥ 30 kg/m2) and non-o

Early Onset Bacteremia in Kidney Transplant Recipients in the Current Era.

BACKGROUND: Bloodstream infection (BSI) after kidney transplantation increases the risk of graft loss and mortality. We aimed to identify contemporary risk factors and outcomes of early BSI (eBSI) at our transplant center. METHODS: We conducted a retrospective chart review of adult kidney transplant recipients (KTRs) from 2019 to 2023. Those with the eBSI (within 90 days of KTR) were matched by age and gender to controls. Risk factors were identified using multivariable logistic regression. All KTRs surviving to 90 days, were included in graft and patient survival analysis. RESULTS: Among 14

DSA-negative, C4d-negative microvascular inflammation after kidney transplantation as an endothelial injury syndrome: A narrative review.

BACKGROUND: Microvascular inflammation (MVI), defined by glomerulitis and peritubular capillaritis, is a major predictor of kidney allograft failure. A clinically important subset develops MVI despite persistently negative donor-specific antibody (DSA) testing and the absence of C4d staining. Although recognized in the Banff 2022 classification, its mechanisms, prognosis, and management remain incompletely defined. OBJECTIVE: To synthesize the evidence on DSA-negative, C4d-negative MVI in kidney transplantation, focusing on prognosis, biological heterogeneity, early post-transplant C4d-negati

Refractory Invasive Pulmonary Aspergillosis Following Treatment of Acute Rejection in a Kidney Transplant Recipient: A Case Report.

Invasive pulmonary aspergillosis (IPA) is an uncommon but highly morbid complication after solid organ transplantation, particularly following treatment for acute rejection. We report a case of refractory IPA caused by Aspergillus terreus in a 70-year-old kidney transplant recipient, diagnosed 2 weeks after receiving thymoglobulin and pulse corticosteroids for acute T cell-mediated rejection. The patient presented with acute hypoxic respiratory failure requiring intubation, and a chest computed tomography scan demonstrated a large, necrotic, mass-like cavitary opacity in the left upper lobe an

Case Report: Kidney allograft recovery after conversion to belatacept-based immunosuppression in simultaneous heart-kidney transplantation.

Renal allograft dysfunction after simultaneous heart-kidney transplantation (SHKT) is often multifactorial, reflecting perioperative hemodynamic instability, ischemia-reperfusion injury, alloimmune activation, and calcineurin inhibitor exposure. The therapeutic role of belatacept in this dual-organ setting remains poorly defined. We report the case of a 47-year-old woman with systemic lupus erythematosus, antiphospholipid syndrome, previous kidney allograft loss and heart failure, who underwent SHKT. The post-transplant course was complicated by persistent anuria requiring prolonged dialysis d

Current and Future Applications of AI-Driven Predictive Modeling and a Proposed Framework for AI-Bioprognostics in Kidney Care.

Artificial intelligence (AI) is reshaping kidney care by integrating electronic health records (EHRs), imaging, digital pathology, genomics, wearable biosensors, and longitudinal physiologic data into dynamic risk-prediction systems. Traditional nephrology risk stratification has relied on static variables and regression-based models such as estimated glomerular filtration rate (eGFR), albuminuria, and composite clinical scores, which often inadequately capture nonlinear disease trajectories and phenotypic heterogeneity. Advances in machine learning, deep learning, and multimodal foundation mo

Five-Year Outcomes and Clinicopathological Correlations of Kidney Transplants From Deceased Donors With Diabetes.

BACKGROUND: Kidneys from deceased donors with diabetes mellitus are refused because diabetes is considered a marker of lower organ quality. We evaluated 5-year outcomes according to donor-recipient diabetes status and, secondarily, the prognostic value of pre-implantation histopathological findings among accepted diabetic donor kidneys. METHODS: This retrospective single-center cohort study included adult recipients of deceased-donor kidney transplants performed between 2013 and 2017. Five-year kidney function and death-censored graft survival were compared according to donor diabetes status

INHBA Promotes the Progression of Gastric Cancer by Activating MAPK Signaling Pathway via Targeting ITGA6.

OBJECTIVES: Gastric cancer (GC) is among the most prevalent malignancies worldwide, ranking as the fifth most common cancer and the fifth leading cause of cancer-related mortality. This study intends to investigate how Inhibin subunit beta A (INHBA) promotes the progression of GC by activating the mitogen-activated protein kinase (MAPK) signaling pathway via targeting Integrin alpha-6 (ITGA6). METHODS: Quantitative reverse transcription-Polymerase Chain Reaction (qRT-PCR) and Immunohistochemistry (IHC) were utilised to validate the expression levels of INHBA in GC, which were subsequently cor

Single-center, open-label, randomized controlled trial comparing the tolerability of maribavir 400 mg twice daily versus standard-dose valganciclovir for cytomegalovirus prophylaxis in high-risk kidney transplant recipients.

Cytomegalovirus (CMV) remains a major contributor to morbidity after a kidney transplant. Although valganciclovir (VGC) prophylaxis is effective, its use is limited by leukopenia. Maribavir's (MAR) role in primary CMV prophylaxis is not established. We conducted a 12-month, single-center, open-label, randomized controlled trial including 70 adult recipients at high CMV risk randomized 1:1 to MAR 400 mg twice daily or VGC 900 mg daily. The incidence of clinically significant leukopenia during prophylaxis did not differ between arms (22.9% vs 5.7%, P = .084). For the secondary endpoint, VGC reci

Neutrophil infiltration of kidney grafts: incidence, associated factors and graft survival.

BACKGROUND: Neutrophil tubulointerstitial infiltration (NTI) of kidney grafts is commonly attributed to urinary tract infection ('ascending nephritis'). Without clinical evidence of urinary tract infection, it is an unexplained finding and was therefore explored in this retrospective study. METHODS: NTI after kidney transplantation was examined in 1753 recipients with 6749 biopsies, including protocol biopsies (59.1%) to determine the unbiased natural prevalence of NTI. We determined associated factors for NTI by logistic regression and graft outcomes with NTI by Kaplan-Meier survival analyse

Evolving Therapeutic Approaches for Treatment of Antibody-Mediated Rejection in Renal Allograft Recipients.

Antibody-mediated rejection (AMR) is now recognized as the primary cause of the decline of functional integrity and ultimately, graft failure in kidney transplants. Evolving understanding of effectors responsible for B-cell activation and plasma cell biology has led to the informed design of new therapeutics. Currently, several novel agents with varying mechanisms of action have emerged. These include imlifidase, FcRn inhibitors, anti-cytokine (IL-6/IL-6R) therapies, and plasma cell-directed therapies, especially anti-CD38s. As our understanding of critical effector pathways responsible for AR

HLA-DR Matching in Kidney Transplantation: Ethnic Disparities in Clinical Benefit and Policy Implications From a UK Registry Analysis.

BACKGROUND: The UK Kidney Allocation Scheme (KAS) prioritizes organ allocation based on HLA mismatches, assigning the greatest weight to HLA-DR compatibility. However, the clinical relevance of this approach across different ethnicities in the era of modern immunosuppression remains uncertain. METHODS: We conducted a retrospective cohort study of 25 094 adult deceased donor kidney transplants in the United Kingdom between 2008 and 2020. Using competing risk Cox regression, we evaluated the impact of individual HLA locus mismatches and grouped mismatch levels (as defined by UK-KAS) on graft su

Development of a conceptual model of BKV impacts on health-related quality of life in kidney transplant recipients: a qualitative study.

BACKGROUND: BK virus (BKV) is a common latent virus that can reactivate in kidney transplant recipients due to immunosuppressive therapy, potentially leading to graft dysfunction or loss. While clinical management of BKV is well studied, little is known about its broader impact on patients’ daily lives and well-being. No conceptual model currently exists to describe the health-related quality of life (HRQoL) impacts of BKV from the patient perspective. METHODS: We conducted a qualitative study using semi-structured concept elicitation interviews with 12 adult kidney transplant recipients who h

Dual targeting of B cell compartments using belatacept and daratumumab in highly sensitized kidney transplant candidates: A mechanistic proof-of-concept desensitization trial.

Highly sensitized kidney transplant candidates face limited access to compatible organs due to persistent anti-human leukocyte antigen (HLA) antibodies (Abs) and memory B cell (mBc) responses, which are generally not concomitantly targeted by current desensitization therapies. In this single-arm phase 1b/2 trial (NCT05145296), 5 highly sensitized kidney transplant candidates (calculated panel-reactive Ab >99%) were treated with belatacept and daratumumab to evaluate safety and mechanistic efficacy.
