Precision Immunosuppression in Heart Transplantation: Biomarker-Guided Therapy Comes of Age.

PURPOSE OF REVIEW: Heart transplantation is the definitive treatment for end-stage heart failure. While the number of heart transplant performed yearly in the United States has nearly doubled in the last decade, median survival has stagnated, largely due to the complications posed by lifelong immunosuppression. A shift towards precision medicine is needed to achieve the optimal balance between risk of allograft rejection and drug-related toxicities to improve post-transplant outcomes. RECENT FINDINGS: Novel molecular based diagnostic tools and biomarkers are being investigated, and hold promi

Heart Transplant for Advanced Heart Failure: A Review.

IMPORTANCE: Heart failure affects more than 6 million US adults; approximately 4% to 5% progress to advanced disease despite optimal therapy. Heart transplant is a class I guideline-recommended therapy for select individuals with advanced heart failure. OBSERVATIONS: Early referral for heart transplant is guided by the "I NEED HELP" mnemonic, a tool that identifies patients with advanced heart failure based on criteria such as use of inotropes, persistent symptoms (New York Heart Association class III or IV), end-organ dysfunction, left ventricular ejection fraction, defibrillator shocks, hos

Impact of MELD 3.0 Score on Outcomes After Combined Heart-Liver Transplantation: A UNOS Database Analysis.

INTRODUCTION: Combined heart-liver transplantation (CHLT) is performed for select patients with end-stage heart and liver disease, though the prognostic value of the Model for End-Stage Liver Disease (MELD) score in this population remains unclear. METHODS: We evaluated adult CHLT recipients between March 2005 and May 2025 from the United Network for Organ Sharing database. Groups were stratified by MELD 3.0 score at transplant: 1-14 (Low), 15-24 (Medium), ≥25 (High). Kaplan-Meier analyses assessed overall survival and freedom from liver graft failure, and Cox proportional hazards analysis ev

Development in Left Ventricular Mass After Early and Late Conversion From Calcineurin Inhibitors to Everolimus in Heart Transplant Recipients-Results From Two Randomized Controlled Trials.

Left ventricular hypertrophy in heart transplant recipients has been attributed to the use of calcineurin inhibitors. We assessed left ventricular mass across treatment arms in 83 heart transplant recipients in the Scandinavian heart transplant everolimus de novo study with early calcineurin inhibitors avoidance (SCHEDULE), and in 75 maintenance recipients in the Nordic Certican Trial in Heart and Lung Transplantation (NOCTET). In SCHEDULE, we randomized participants to treatment with calcineurin inhibitors or replacement with everolimus within 7-11 weeks after transplant. In NOCTET, participa

From rejection to vasculopathy: a review of non-invasive biomarkers in alloimmune responses in heart transplantation.

BACKGROUND: Heart transplantation is the gold standard treatment for end-stage heart failure. Despite immunosuppression as the cornerstone therapy for post-transplant management, the presence of immunological complications including acute cellular rejection (ACR), antibody-mediated rejection (AMR), and cardiac allograft vasculopathy (CAV)-remain major barriers to long-term graft survival. Graft monitoring largely remains dependent on protocolized endomyocardial biopsy (EMBx), an invasive, risky procedure. SCOPE: This narrative review focuses on aggregating the published evidence, summarizing

A new Dawn: Advances and innovation in heart transplantation part I - cardiac magnetic resonance imaging in heart transplantation.

Heart transplantation (HTx) remains the treatment of choice for patients with advanced heart failure. HTx recipients require intensive surveillance over time. Major complications that should be monitored, prevented, and treated in a timely manner include graft dysfunction secondary to cellular rejection or antibody-mediated rejection (AMR) and coronary allograft vasculopathy (CAV). While echocardiographic surveillance plays a key role in detecting major changes in graft function and endomyocardial biopsy remains the most widely used invasive technique for the surveillance of graft rejection, t

Impact Of Ventricular Assist Device Use On Pediatric Heart Transplant Outcomes In A Linked Cohort Of The Advanced Cardiac Therapies Improving Outcomes Network (ACTION) And Pediatric Heart Transplant Society (PHTS) Databases.

PURPOSE: Pediatric ventricular assist devices (VAD) improve waitlist mortality. We aimed to compare outcomes after heart transplant (HT) based on pre-HT support in patients <18 years at listing. METHODS: VAD and post-HT data were linked between ACTION and PHTS databases, respectively (4/1/2018-6/30/2022). Support groups were defined as medical (MG) and VAD (VG). Within VG, device groups were defined as paracorporeal pulsatile (PP), paracorporeal continuous (PC), and implantable continuous (IC). Kaplan-Meier analysis, the log rank test, and Cox proportional hazards modeling analyzed graft loss

Case Report: Kidney allograft recovery after conversion to belatacept-based immunosuppression in simultaneous heart-kidney transplantation.

Renal allograft dysfunction after simultaneous heart-kidney transplantation (SHKT) is often multifactorial, reflecting perioperative hemodynamic instability, ischemia-reperfusion injury, alloimmune activation, and calcineurin inhibitor exposure. The therapeutic role of belatacept in this dual-organ setting remains poorly defined. We report the case of a 47-year-old woman with systemic lupus erythematosus, antiphospholipid syndrome, previous kidney allograft loss and heart failure, who underwent SHKT. The post-transplant course was complicated by persistent anuria requiring prolonged dialysis d

Impact of Operative Time on Pediatric Heart Transplant Outcomes: An Analysis of the United Network for Organ Sharing Registry.

BACKGROUND: Heart transplantation is definitive therapy for children with end-stage heart failure. Because donor availability is unpredictable, transplantation occurs overnight or on weekends. Although studies report mixed findings on the association between operative timing and outcomes, pediatric data remain limited. We evaluated the impact of operative timing on pediatric heart transplant outcomes. METHODS: We studied pediatric heart transplant recipients (< 18 years) in the United Network for Organ Sharing registry from 2000 to 2018. Transplants were classified by time of day (day: 9 AM-8

Predicted Heart Mass and Severe Primary Graft Dysfunction in Donation After Circulatory Death Heart Transplant.

BACKGROUND: Donation after circulatory death (DCD) has expanded the US heart donor pool yet carries a higher risk of severe primary graft dysfunction (PGD) for recipients compared to donation after brain death (DBD). This study addresses an important knowledge gap of exploring the role of heart size mismatch in contributing to the risk of severe PGD in DCD-HT. METHODS: All heart-only, adult transplants from the united network for organ sharing (UNOS) database were collected from 09/2023-06/2025. Univariable (UV) and multivariable (MV) analyses were used to identify associations between variou

Global Diabetes: Rapidly Changing Landscape and a Rising Tide of Opportunities: The 2025 Distinguished International Service in the Cause of Diabetes Award Lecture.

Diabetes affects an estimated 828 million people worldwide; prevalence is growing rapidly in low- and middle-income countries (LMICs), with major health and development consequences. Diabetes-related complications and mortality rates have declined across several high-income countries (HICs), largely through structured chronic disease care, sustained implementation of quality improvement strategies, and active risk factor control. However, rates of diabetes complications and mortality have been rising in LMICs. Along with evidence there are tools (e.g., teams, task shifting, data systems, decis

Immune regulation following allogeneic iPSC-derived cardiomyocyte transplantation in non-human primates.

AIMS: This study explores the efficacy of immunosuppressive regimens commonly used in heart transplantation for promoting the survival of allogeneic induced pluripotent stem cell-derived cardiomyocyte (iPSC-CM) grafts in non-human primates, specifically cynomolgus monkeys. METHODS AND RESULTS: By combining methylprednisolone (MPL), calcineurin inhibitors (CNIs), and mycophenolate mofetil (MMF), we ensured adequate graft survival without acute rejection. Histological analysis showed iPSC-CM survival, vascularization, and minimal immune rejection, despite immaturity. Reducing the immunosuppress

Mechanisms of donor-specific transfusion synergy with anti-CD154 for transplant acceptance.

Organ transplantation is a life-saving treatment for patients with end-stage organ failure but requires lifelong immunosuppression that can result in significant complications. Achieving stable and durable donor-specific tolerance, whereby immunosuppression can be stopped without precipitating graft rejection, holds the promise to circumvent these problems. While transient inhibition of the CD40/CD154 costimulatory pathway delays transplant rejection in animal models, and antibodies blocking this interaction are currently in clinical trials, the efficacy of anti-CD154 (αCD154) in mouse models

[Long-term mechanical circulatory support in childhood in Hungary].

INTRODUCTION: Mechanical circulatory support devices play a key role in stabilizing patients with end-stage heart failure and preparing them for heart transplantation. These devices significantly reduce waitlist mortality, especially in the pediatric population, where donor shortage and treatment challenges pose major difficulties. OBJECTIVE: The aim of our study was to summarize domestic experiences with long-term mechanical circulatory support in pediatric patients and compare our outcomes with international data. METHOD: We retrospectively analyzed data from 27 pediatric patients implante

Endomyocardial biopsy.

Endomyocardial biopsy (EMB) has evolved from a single-indication test for the early diagnosis and monitoring of heart transplant rejection to the gold-standard technique to reach a definite and aetiological diagnosis in different cardiac disorders such as myocarditis and cardiomyopathies. It is currently considered a fundamental tool in the diagnostic workup of unexplained acute heart failure with haemodynamic compromise. For interventional cardiologists, EMB represents a unique opportunity to bridge invasive diagnostics with personalised care. By embracing technological advancements, integrat

Optical coherence tomography for early detection of cardiac allograft vasculopathy in pediatric heart recipients.

BACKGROUND: Cardiac allograft vasculopathy (CAV) is a leading cause of graft failure following pediatric heart transplantation (HT). Early detection could improve long-term outcomes, but conventional coronary angiography often misses early-stage disease. Optical coherence tomography (OCT), a high-resolution intravascular imaging modality, may enable earlier and more sensitive CAV detection. METHODS: This retrospective study analyzed pediatric HT recipients who underwent OCT between 2012 and 2024. OCT-derived maximal intima thickness (MIT) and cross-sectional area ratios were correlated with c

Transcatheter Tricuspid Valve Edge-to-Edge Repair in a Patient Following Heart Transplantation.

BACKGROUND: Transcatheter tricuspid valve edge-to-edge repair (T-TEER) is an emerging therapy for severe tricuspid regurgitation (TR), but experience in heart transplant recipients is limited. CASE SUMMARY: A 42-year-old man, 1-year post heart transplantation, presented with severe volume overload and dyspnea. Echocardiography confirmed severe TR with a flail posterior leaflet. Given high surgical risk, he underwent T-TEER with TriClip (Abbott), reducing TR to moderate with resolution of hepatic vein systolic flow reversal. Later, recurrent hypervolemia from missed dialysis led to TR progress
