Mechanisms of donor-specific transfusion synergy with anti-CD154 for transplant acceptance.

Organ transplantation is a life-saving treatment for patients with end-stage organ failure but requires lifelong immunosuppression that can result in significant complications. Achieving stable and durable donor-specific tolerance, whereby immunosuppression can be stopped without precipitating graft rejection, holds the promise to circumvent these problems. While transient inhibition of the CD40/CD154 costimulatory pathway delays transplant rejection in animal models, and antibodies blocking this interaction are currently in clinical trials, the efficacy of anti-CD154 (αCD154) in mouse models

[Long-term mechanical circulatory support in childhood in Hungary].

INTRODUCTION: Mechanical circulatory support devices play a key role in stabilizing patients with end-stage heart failure and preparing them for heart transplantation. These devices significantly reduce waitlist mortality, especially in the pediatric population, where donor shortage and treatment challenges pose major difficulties. OBJECTIVE: The aim of our study was to summarize domestic experiences with long-term mechanical circulatory support in pediatric patients and compare our outcomes with international data. METHOD: We retrospectively analyzed data from 27 pediatric patients implante

Endomyocardial biopsy.

Endomyocardial biopsy (EMB) has evolved from a single-indication test for the early diagnosis and monitoring of heart transplant rejection to the gold-standard technique to reach a definite and aetiological diagnosis in different cardiac disorders such as myocarditis and cardiomyopathies. It is currently considered a fundamental tool in the diagnostic workup of unexplained acute heart failure with haemodynamic compromise. For interventional cardiologists, EMB represents a unique opportunity to bridge invasive diagnostics with personalised care. By embracing technological advancements, integrat

Optical coherence tomography for early detection of cardiac allograft vasculopathy in pediatric heart recipients.

BACKGROUND: Cardiac allograft vasculopathy (CAV) is a leading cause of graft failure following pediatric heart transplantation (HT). Early detection could improve long-term outcomes, but conventional coronary angiography often misses early-stage disease. Optical coherence tomography (OCT), a high-resolution intravascular imaging modality, may enable earlier and more sensitive CAV detection. METHODS: This retrospective study analyzed pediatric HT recipients who underwent OCT between 2012 and 2024. OCT-derived maximal intima thickness (MIT) and cross-sectional area ratios were correlated with c

Transcatheter Tricuspid Valve Edge-to-Edge Repair in a Patient Following Heart Transplantation.

BACKGROUND: Transcatheter tricuspid valve edge-to-edge repair (T-TEER) is an emerging therapy for severe tricuspid regurgitation (TR), but experience in heart transplant recipients is limited. CASE SUMMARY: A 42-year-old man, 1-year post heart transplantation, presented with severe volume overload and dyspnea. Echocardiography confirmed severe TR with a flail posterior leaflet. Given high surgical risk, he underwent T-TEER with TriClip (Abbott), reducing TR to moderate with resolution of hepatic vein systolic flow reversal. Later, recurrent hypervolemia from missed dialysis led to TR progress

Combined Multiorgan Heart and Kidney Transplants With Single Donor Allografts: Simultaneous Versus Staged?

BACKGROUND: Multiorgan heart and kidney transplants (HKTx) performed for patients with end-stage heart failure and chronic kidney disease have increased in recent years. However, no established protocols exist on whether a heart and kidney from the same donor should be transplanted in the same operation versus 1-2 days apart. METHODS: Using SRTR data 1993-2023, we compared same-donor HKTx recipients with both transplants performed on the same day ("simultaneous") to recipients with kidney transplants performed within 1 day of the heart transplant ("staged"). We examined differences in weighte

Candida auris in Heart Transplant Donor: First Isolation in a Southern Italy Heart Transplant Center.

Candida auris is an emerging multidrug-resistant fungal pathogen increasingly implicated in healthcare-associated outbreaks worldwide. Its presence in organ donors poses a significant threat to transplant recipients due to the risk of invasive infection and limited antifungal treatment options. We report the first isolation of Candida auris in a heart transplant donor at a transplant center in Southern Italy. The donor, a 45-year-old woman from Greece, was colonized with C. auris in the bronchoaspirate sample collected at the time of organ retrieval. Despite this colonization, the donor heart

Comparable outcomes of HSCT up-front and after failure of IST in pediatric aplastic anemia in recent years.

We investigated the outcomes of 179 pediatric patients with severe aplastic anemia (SAA) who received up-front hematopoietic stem cell transplantation (HSCT; n = 87) or received HSCT after failure of immunosuppressive therapy (IST; n = 92). With a median follow-up of 4.0 years (range, 0.2-10.7) after HSCT, 161 patients were alive at last follow-up. The cumulative incidence of graft failure, grade 2 to 4 acute and chronic graft-versus-host disease (GVHD) was 10%, 18%, and 8%, respectively. Five-year overall survival (OS) and GVHD-free, relapse/rejection-free survival (GRFS) were 89% and 79%, re

Sodium-Glucose Cotransporter 2 Inhibitors in Diabetic Solid Organ Transplant Recipients: A Systematic Review and Meta-Analysis of Comparative Studies.

BACKGROUND: Solid organ transplant recipients with diabetes mellitus face unique challenges in glycemic control, compounded by the metabolic effects of immunosuppressants. Although sodium-glucose cotransporter 2 (SGLT2) inhibitors are effective in diabetes, evidence for their use in transplant recipients remains limited. We aimed to systematically review the efficacy and safety of SGLT2 inhibitors in transplant recipients with diabetes. METHODS: A systematic review and meta-analysis was conducted by searching the MEDLINE, Embase, and Cochrane CENTRAL databases for studies published before Jul

Macrophage-Mimicking Biomimetic Microbubbles for Early Ultrasound Molecular Imaging of Acute Rejection in Heart Transplantation.

Acute rejection (AR) is an important cause of heart graft injury and failure. Accurate diagnosis of AR is essential to achieving early intervention and improving the prognosis of patients. Current clinical monitoring of AR requires repeated invasive endocardial biopsies, and there is an urgent need to develop a noninvasive diagnostic technique. As antigen-presenting cells, macrophages play an important role in adaptive immunity, such as the early activation of T cells in AR, and are considered to be key drivers of AR. Macrophage membrane biomimetic microbubbles (MB-Mφ) inherit a variety of fun

Outcomes of Mechanical Circulatory Support Bridge From Extracorporeal Membrane Oxygenation Before Heart Transplantation.

Patients with advanced heart failure or cardiogenic shock often require a mechanical circulatory support device (MCSD) before heart transplantation (HT). While extracorporeal membrane oxygenation (ECMO) is commonly used for emergent stabilization, transitioning to alternative MCSDs may optimize patients before transplant. Prior studies suggest worse outcomes with ECMO alone. We examined post-HT outcomes in patients bridged from ECMO to other MCSDs. We used the United Network for Organ Sharing (UNOS)/Organ Procurement & Transplantation Network (OPTN) database to identify adults (age ≥18) underg

The role of estrogen in cardiac transplantation: mechanistic insights and effects on clinical outcomes.

Sex hormones profoundly shape immune responses and influence outcomes after heart transplantation. Estrogen enhances allosensitization and is associated with a higher incidence of acute rejection in female recipients. Beyond its immunological effects, estrogen also modulates the pharmacokinetics and pharmacodynamics of calcineurin inhibitors-particularly cyclosporine A-thereby influencing immunosuppressive efficacy and early graft performance. Donor-recipient sex mismatch further modulates transplant outcomes. Female-to-male transplants in particular exhibit the poorest short- and long-term su

A Review of the Therapeutic Efficacy and Safety of Human-Induced Pluripotent Stem Cell-Derived Cardiomyocytes in Preclinical Models of Subacute and Chronic Myocardial Infarction.

For the past decade, cell-based therapies have been the focus of research to investigate their potential to treat ischemic heart disease. The translation to human clinical studies depends on the demonstration of therapeutic efficacy and safety, particularly when transplanted in the subacute and chronic post-MI phase. A number of studies were identified that reported the effect of hiPSC-CMs on cardiac outcomes when transplanted at least 7 days post-myocardial infarction. The mean sample size of the published studies was 30 (±17) animals with a mean follow-up duration of 51 (±37) days. hiPSC-CM

Are the Results of Late Heart Re-Transplantation Influenced by the Time Interval from Primary Transplantation?

Background: Heart re-transplantation represents an effective option in selected patients with graft failure. Although the results of heart re-transplantation have improved in recent years, little is known about the outcomes in patients requiring late (>1 year) re-transplantation or about the effects of prolonged exposure to immunosuppression on multiorgan function. Methods: Among all the heart re-transplantations performed, from November 1988 to April 2025, a total of 30 patients underwent late re-transplantation. Since literature data on late re-HTx are generally limited to those performed wi

Alternative Measures of Body Composition and Outcomes Following Heart Transplant.

Current guidelines for heart transplant (HT) listing support body mass index (BMI) < 35 kg/m2, though data supporting this recommendation is mixed. It is unclear if other more specific measurements of body composition are better predictors of outcomes post-HT. This retrospective study included patients who underwent HT between 2014 and 2019 and underwent an abdominal CT scan within 3 months of HT. Tissue characterization was performed using Slice-O-Matic software to derive measurements including BMI, fat mass index (FMI), visceral adipose tissue to subcutaneous adipose tissue ratio (VAT/SAT),

Bridging machine learning and clinical practice: a multicentre nomogram for 90-day graft failure risk stratification in heart transplantation.

BACKGROUND: Early graft failure within 90 postoperative days is the leading cause of mortality after heart transplantation. Existing risk scores, based on linear regression, often struggle to capture the complex, multifactorial biological interactions necessary for personalised donor-recipient matching. This study utilised explainable machine learning (ML) to identify robust predictors of 90-day graft failure and developed a clinically interpretable, ML-informed nomogram designed specifically for cross-population generalisability. METHODS: Using the UNOS registry (2008-2020; n=25 200), XGBoos

Safety and Tolerability of Tocilizumab in a Case Series of Heart Transplant Recipients With Chronic Antibody-Mediated Rejection.

BACKGROUND: Chronic antibody-mediated rejection (AMR) after heart transplantation is associated with progressive graft dysfunction and increased mortality, and effective treatment options remain limited. Interleukin-6 (IL-6) inhibition with tocilizumab has emerged as a potential therapeutic strategy, although evidence in cardiac transplantation is scarce and limited to small case series. OBJECTIVE: To describe the safety and clinical course of heart transplant recipients with chronic, treatment-refractory AMR treated with tocilizumab. First implantation of a bioprosthetic total artificial heart for a mediastinal paraganglioma.

Resection of cardiac-invasive tumors is challenging, as transplantation is not a viable option for patients with cancer. We report the first implantation of the Aeson total artificial heart (TAH) (Carmat, Vélizy-Villacoublay, France), as a bridge to transplant in a patient with a mediastinal paraganglioma. A 54-year-old man presented with restrictive heart failure related to a paraganglioma. The tumor was invading the right ventricle. No tumor reduction was achieved after chemotherapy. A surgical resection of the tumor combined with the implantation of the Aeson TAH was decided. Embolization o
