Donor coronary artery disease and outcomes after heart transplantation: Effect modification by donor age and allocation era.

BACKGROUND: The limited availability of donor hearts has led to an increased utilization of organs from donors with pre-existing coronary artery disease (CAD). However, the impact of donor-transmitted CAD (DTCAD) on post-transplant outcomes stratified by donor age remains insufficiently characterized. METHODS: Using the United Network for Organ Sharing database, adults undergoing orthotopic heart transplantation (2000 to 2025) were analyzed. Donors were stratified by age and CAD status on pre-procurement angiography. The primary outcome was all-cause mortality. Effect modification by donor ag

Airway molecular signatures in antibody-mediated lung transplant rejection.

Severe antibody-mediated rejection (AMR) is associated with chronic lung allograft dysfunction (CLAD) and death in lung transplant recipients. However, AMR diagnostics are imprecise, and new biomarkers are needed. We assessed whether airway inflammation gene signatures could distinguish AMR cases from controls. We analyzed small airway brush RNA sequencing from 16 AMR cases and 39 controls across 2 centers. We compared gene signatures for complement activation, molecular target of rapamycin (mTOR) signaling, and natural killer cell-mediated injury. Differential gene expression and pathway anal

Longitudinal Transcriptomic Analysis Identifies Persistent Immune-Related Signature Associated With Restrictive Left Ventricular Filling Patterns After Heart Transplantation.

BACKGROUND: Diastolic dysfunction is a recognized chronic allograft complication, and a restrictive left ventricular filling pattern is associated with adverse outcomes. However, the underlying pathophysiological mechanisms remain unclear. METHODS AND RESULTS: This study included 32 heart transplant recipients with a normal filling pattern at 1-year after transplantation. Five-year echocardiography classified patients into restrictive (n=12) and non-restrictive (n=20) groups. All patients underwent right heart catheterization and right ventricular endomyocardial biopsy at 1 and 5 years. Bulk

Plasma Extracellular Vesicle Profiles After Heart Transplantation are Associated With the Type of Allograft Rejection.

BACKGROUND: Heart transplantation (HT) remains the primary treatment for end-stage heart failure, but graft rejection-including acute cellular rejection (ACR), antibody-mediated rejection (AMR), and chronic rejection such as cardiac allograft vasculopathy (CAV)-significantly impacts long-term patient outcomes. This study investigates the role of circulating plasma extracellular vesicle profiles as potential biomarkers for distinguishing between different rejection types following HT. METHODS: We enrolled 85 HT patients with post-transplant follow-up ranging from 2 to 140 months. The cohort in

PD-1 blockade does not enhance alloimmunization after allogeneic dendritic cell vaccination in cancer patients.

BACKGROUND: Blocking programmed cell death protein 1 (PD-1) has become a standard cancer immunotherapy, increasingly used in kidney, liver, or heart transplant recipients who develop skin cancer or hepatocellular carcinoma, despite the increased risk of graft failure or rejection. The mechanism of action of PD-1 blockade relies on stimulating CD8+ T cell activity, but its impact on humoral immunity in general and on alloimmunization in particular remains uncertain. OBJECTIVE: The aim of this study was to investigate the impact on anti-PD-1 treatment on alloimmunization. METHODS: The effect o

Nonmyeloablative pentostatin-cyclophosphamide preconditioning improves rates of engraftment in adults undergoing haploidentical HCT for sickle cell disease.

UNLABELLED: The morbidity and mortality of sickle cell disease (SCD) remain high. Novel nonmyeloablative haploidentical hematopoietic cell transplant (HCT) regimens are being proposed. METHODS: This report compared the effect of adding an IV pentostatin and oral cyclophosphamide (PC) preconditioning to the nonmyeloablative NIH HCT platform of alemtuzumab and total body irradiation (TBI) in adults with severe SCD. Transplant outcomes were compared to a historical HCT cohort that did not receive the PC preconditioning. RESULTS: Thirty-nine adult SCD patients were included. The median age was 3

Beyond Tacrolimus: Toward Personalized Immunosuppression in Heart Transplantation.

Heart transplantation remains the definitive treatment for end-stage heart failure, and contemporary immunosuppressive regimens, typically a calcineurin inhibitor, antiproliferative agent, and corticosteroids applied with minimal preemptive individualization, despite profound heterogeneity in recipient immune biology, pharmacokinetics, comorbidity burden, and alloimmune risk. This review examines the case for personalized immunosuppression in heart transplantation across 4 domains. First, we survey the current landscape of immunosuppression, identify key gaps, and discuss emerging strategies.

Association of pre-transplant torque teno virus load with early post-transplant outcomes in lung transplant recipients: A multicenter cohort study.

BACKGROUND: Torque teno virus (TTV) load has emerged as a potential surrogate marker of net immunosuppression in solid organ transplantation. However, the clinical significance of pre-transplant TTV levels for early outcomes after lung transplantation remains unclear. METHODS: In this multicenter cohort of 334 adult first-time lung transplant recipients (2015-2022), plasma samples were collected immediately prior to transplantation. The primary endpoint was infection within 3 months. One hundred healthy individuals served as controls for TTV comparisons. Secondary outcomes were acute rejectio

Prognostic implications of one-year left ventricular systolic function after heart transplantation.

BACKGROUND: The role of donor left ventricular ejection fraction (LVEF) at heart procurement has been extensively studied. However, there remain scarce data on how 1-year post-heart transplant (HTx) LVEF affects recipient outcomes. Our study seeks to address this gap. METHODOLOGY: The OPTN database was analyzed for adult orthotopic HTx recipients between 2010-2022. Patients with missing LVEF data during their 1-year follow-up were excluded. Recipients were stratified into four groups based on their 1-year LVEF: preserved: ≥ 50%, mildly reduced: 40-49%, moderately reduced: 30-39%, and severely

Acute lung allograft dysfunction predicts lung allograft failure: Construct validity of the 2026 International Society for Heart and Lung Transplantation spirometric definition.

INTRODUCTION: Acute lung allograft dysfunction (ALAD) has been proposed as a screening criterion to identify lung transplant recipients at increased risk for graft loss. ALAD was defined formally in a 2026 International Society for Heart and Lung Transplantation (ISHLT) Delphi consensus statement but has not been empirically evaluated. We sought to test the construct validity of the ISHLT ALAD definition by testing the hypothesis that ALAD is associated with increased risk of graft failure. METHODS: We included 86,885 spirometry measurements from 2,877 participants in the 8-center Lung Transp

Outcomes of Heart Transplantation in Adults With Transposition of the Great Arteries: A Single-Center Experience.

BackgroundTransposition of the great arteries (TGA) is a congenital heart disease characterized by ventriculoarterial discordance. Corrective surgeries can provide long-term palliation, but many patients ultimately develop systemic right ventricular failure and end-stage heart failure. Heart transplantation (HT) is an established treatment in adults with congenital heart disease, but data specifically focused on adults with TGA undergoing HT remains limited, with only small single-center series reported to date. The aim of this study was to describe perioperative complexity, immunologic burden

An integrated clinical-histopathologic prediction model for cardiac allograft rejection: Translating machine learning into clinical risk frameworks.

BACKGROUND: Cardiac allograft rejection (CAR) remains the leading cause of early graft failure after heart transplantation (HT). Current diagnostics, including histologic grading of endomyocardial biopsy (EMB) and blood-based assays, lack accurate predictive power for future CAR risk. We developed a predictive model integrating routine clinical data with EMB-derived quantitative morphologic features to demonstrate the precision medicine potential of mining existing data sources in post-HT care. METHODS: In a retrospective cohort of 484 HT recipients with 1,188 EMB encounters within 6 months p

Old donors for young heart transplant recipients: Clinical implications of an emerging phenomenon.

BACKGROUND: Historically, heart transplantation (HTx) has focused on matching recipients with organs from donors of similar or younger age. However, due to the growing shortage of donors, especially in Europe, organs from older donors are being used more frequently. OBJECTIVE: We aimed to evaluate whether transplant matchings with donor older than recipient allocation (DORA) have become more frequent in recent years, if they involved different donor/recipient characteristics, and whether this age mismatch might be associated with less favorable outcomes. METHODS: The temporal evolution of th

10-Year Survival and Renal Outcomes After Simultaneous Heart-Kidney Transplantation in Heart Retransplant Recipients.

BACKGROUND: Renal dysfunction is common in heart retransplant (re-HT) candidates and adversely influences outcomes. Although simultaneous heart-kidney transplantation (SHKT) is established in primary transplantation for severe renal dysfunction, its long-term benefit in the re-HT setting remains incompletely defined. OBJECTIVES: This study sought to evaluate 10-year survival and renal trajectories after SHKT compared with isolated re-HT. METHODS: Adult patients undergoing re-HT (2000-2024) were identified from the UNOS (United Network for Organ Sharing) registry and categorized as isolated r

Regulatory T cell therapy in solid organ transplantation: mechanisms, translational progress, and remaining barriers.

The long-term outcomes of solid organ transplantation are constrained by alloimmune rejection and the toxicities of lifelong immunosuppression; durable tolerance therefore remains an unmet goal. Regulatory T cells are central mediators of peripheral tolerance, capable of restraining cellular and antibody-mediated rejection while shaping the intragraft inflammatory and repair milieu, supporting a mechanistic rationale for immunosuppression minimization. This review integrates key mechanisms by which Tregs regulate transplant immunity and summarizes evidence for Treg-directed interventions from

International Society for Heart and Lung Transplantation Scientific Statement on pulmonary antibody-mediated rejection and proposed graft, antibody, and pathology (GAP) definition.

Antibody-mediated rejection (AMR) is an increasingly recognized form of rejection and cause of graft failure after lung transplantation. AMR has been the focus of extensive research over the past decade. Despite growing awareness and recent advances in our understanding of AMR, outcomes remain dismal with a 2-year survival of only 20%. The International Society for Heart and Lung Transplantation convened a multidisciplinary workgroup of experts in AMR to review the most up-to-date research and clinical experience and to update the 2016 definition. The workgroup was divided into 9 subgroups cov

Donor-Derived Cell-Free DNA at Diagnosis of Cardiac Allograft Rejection Stratifies Risk of Mortality and Graft Dysfunction.

BACKGROUND: Cardiac acute rejection (AR) is a risk factor for poor outcomes; however, there are limited risk prediction models to stratify these patients for death or prolonged left ventricular (LV) dysfunction. We assessed the prognostic utility of percent donor-derived cell-free DNA (%dd-cfDNA) measured at AR diagnosis for predicting adverse outcomes. METHODS: The prospective multicenter GRAfT study (Genomic Research Alliance for Transplantation) enrolled heart transplant recipients and collected serial plasma samples to quantitate %dd-cfDNA. AR was defined as acute cellular rejection, anti

A two-threshold donor-derived cell-free DNA algorithm improves detection of molecular rejection in heart transplantation.

BACKGROUND: Donor-derived cell-free DNA (dd-cfDNA), a noninvasive biomarker for heart allograft rejection, was initially developed based on the percent of total cfDNA. A two-threshold algorithm (2TA) was recently developed, assigning high risk of rejection if dd-cfDNA was ≥0.26% or donor-quantity score (DQS) was ≥18 copies/mL based on histologic biopsy assessment. The present study evaluated 2TA for its performance in detecting molecular rejection using the Molecular Microscope Diagnostic System (MMDx). METHODS: We assessed endomyocardial biopsies with matched dd-cfDNA assessments from the mu

Molecular Heterogeneity and Prognostic Implications of Low-Grade Acute Cellular Rejection (1R/1A) in Heart Transplantation.

INTRODUCTION: International Society for Heart & Lung Transplantation (ISHLT) grade 1R/1A cellular rejection (ACR) is generally considered clinically low risk and does not routinely prompt treatment. Whether this histological finding reflects biologically meaningful alloimmune activation and confers prognostic risk remains uncertain. We sought to define the molecular and clinical significance of low-grade ACR by integrating intragraft transcriptomic profiling with circulating biomarkers and post-biopsy outcomes. METHODS: We analyzed consecutive for-cause endomyocardial biopsies (EMBs) with pai
