BACKGROUND: Brucellosis is one of the most widespread zoonotic diseases globally. Of the twelve known Brucella species that affect mammals, Brucella melitensis is the most significant cause of human infection. Others include B. abortus, B. suis, and B. canis, with transmission occurring via unpasteurized dairy, direct animal contact, or occupational exposure (veterinarians, farmers, and laboratory workers). Although hemophagocytic lymphohistiocytosis (HLH) and neurological complications are known sequelae of brucellosis, their simultaneous occurrence with multiorgan failure is rarely reported.

INTRODUCTION: Angiostrongylus cantonensis is a zoonotic metastrongyloid nematode that causes eosinophilic meningitis and ocular angiostrongyliasis in humans and other accidental hosts. In India, reported human cases are sporadic, with several associated with monitor lizard consumption, while systematic studies remain limited.
METHODS: We examined 60 opportunistically obtained rodent carcasses from six localities along the Pondicherry-Tamil Nadu border. Molecular identification and phylogenetic analysis were performed on adult nematodes recovered from the rats.
RESULTS: A. cantonensis was det

Carcinomatous meningitis, or leptomeningeal metastasis, is a severe neurological complication of solid and haematological malignancies caused by dissemination of tumour cells within the leptomeninges and cerebrospinal fluid pathways. Clinical presentation is heterogeneous, including headache, intracranial hypertension, cranial nerve palsies or radiculopathy, which frequently leads to diagnostic delay. Diagnosis relies on converging clinical, radiological, and cerebrospinal fluid findings, with gadolinium-enhanced magnetic resonance imaging of the brain and spine and cerebrospinal fluid cytolog

BACKGROUND: Cerebral small vessel disease (cSVD) encompasses white matter hyperintensities (WMH), enlarged perivascular spaces (PVS), intracerebral hemorrhage (ICH), small vessel stroke (SVS), and cognitive impairment-phenotypes that frequently co-occur yet have been studied as independent genetic endpoints. Whether a shared genetic architecture underlies these co-occurring cSVD-related phenotypes remains unclear.
METHODS: We applied Genomic SEM to GWAS summary statistics from six phenotypes (WMH, PVS, ICH, SVS, cognitive performance, systolic blood pressure), followed by multivariate associa

BACKGROUND: Post-hemorrhagic hydrocephalus (PHH) is a serious complication of intraventricular hemorrhage (IVH) in preterm infants and is associated with increased morbidity and mortality. Although neonatal risk factors are well described, the contribution of maternal and perinatal factors remains inconsistent across studies.
OBJECTIVE: To determine the incidence, clinical characteristics, and independent predictors of PHH among preterm neonates with IVH, and to evaluate early outcomes in a tertiary referral center.
METHODS: This retrospective cohort study included preterm neonates (< 34 wee

BACKGROUND: Toxoplasma gondii surface proteins play critical roles in parasite invasion, proliferation, and host immune modulation, and represent promising targets for anti-toxoplasmosis intervention. SRS67 and SRS20A are poorly characterized surface proteins of T. gondii, and their biological functions and host regulatory effects remain largely unclear.
METHODS: In the present study, we prepared recombinant SRS67 and SRS20A proteins and generated corresponding polyclonal antibodies to explore their functional characteristics and protective potentials.
RESULTS: Immunofluorescence analysis lo

BACKGROUND: Intradural extramedullary hemangiomas of the spinal canal are rare, and the venous subtype is particularly uncommon. Their imaging appearance can overlap with more frequent intradural extramedullary tumors, which increases the risk of misdiagnosis. This case report aims to describe an intradural extramedullary venous hemangioma that mimicked meningioma on MRI and to highlight key surgical considerations.

INTRODUCTION: Febrile infants ≤ 60 days of age are at increased risk for serious bacterial infections (SBIs), often necessitating extensive diagnostic evaluation and hospitalization. Although respiratory viral infections are common in this population and may influence the likelihood of concurrent bacterial infection, the relationship between respiratory viral panel (RVP) results and SBI risk remains an important consideration in clinical decision-making. The objective of this study was to evaluate the association between RVP results and the occurrence of SBIs among febrile infants ≤ 60 days of

Respiratory syncytial virus (RSV) affects all ages, with infants and older adults most affected by severe disease. The RSV prevention landscape includes 39 candidates in clinical development, alongside setbacks such as the discontinuation of a promising paediatric live-attenuated vaccine candidate. Several products have been approved, including a maternal vaccine, infant monoclonal antibodies, and vaccines for older adults and adults at high risk. Due to high cost, implementation of these products remains limited to higher-income settings. Real-world evidence confirms high effectiveness, with

Postoperative meningitis is a rare but serious complication in endoscopic-endonasal skull-base surgery. Although prophylactic antibiotics are routinely administered, the optimal regimen and duration remain unclear. This systematic review and meta-analysis evaluated the incidence of meningitis and intraoperative cerebrospinal fluid (CSF) leakage associated with single-agent perioperative antibiotic prophylaxis. Following PRISMA 2020 guidelines, PubMed, EMBASE, and Web of Science were searched through February 2025. Eligible studies reported postoperative meningitis or intraoperative cerebrospin

Neisseria meningitidis is a notifiable pathogen in Japan when isolated from any sterile site. We describe a rare case of primary meningococcal arthritis without meningitis or bacteremia. A 48-year-old man presented with acute septic arthritis of his knee. A synovial fluid analysis using matrix-assisted laser desorption/ionization time-of-flight mass spectrometry revealed gram-negative diplococci, identified as N. meningitidis. Further antimicrobial susceptibility testing showed the isolate was pan-susceptible, allowing de-escalation from ceftriaxone to ampicillin therapy. The patient recovered

BACKGROUND: The invasive giant African snail (Achatina fulica) poses serious agricultural and ecological threats and serves as an important intermediate host of the zoonotic parasite Angiostrongylus cantonensis, which causes human eosinophilic meningitis. While its gut bacterial microbiome has been extensively characterized, the gut mycobiome remains largely uncharacterized. In particular, variations in gut fungal communities across developmental stages remain unknown, limiting our understanding of the A. fulica gut holobiont.
METHODS: In November 2025, giant African snails were collected fro

BACKGROUND: Human herpesvirus-7 (HHV-7) is a member of the herpesvirus family that is commonly asymptomatic during infection, but it can lead to the development of exanthema subitum. Although HHV-7 primarily affects children and establishes latency in the body, reactivation of HHV-7 can occur in individuals with compromised immune systems and those using immunosuppressive medications, resulting in diverse clinical manifestations. However, the acute HHV-7 presentation in immunocompetent adults is uncommon.
CASE PRESENTATION: Here, we report a rare presentation of HHV-7 meningitis in an immunoc

The glymphatic system supports cerebrospinal fluid (CSF)-interstitial fluid (ISF) exchange and has been implicated, to different extents, in the transport or clearance of amyloid-β (Aβ), tau, lactate and other solutes. Although most studies have focused on CSF influx, astrocytic aquaporin 4 (AQP4) expression, perivascular localisation and polarisation, AQP4-mediated transmembrane water transport, and perivascular exchange, brain clearance also depends on efficient extracranial drainage of waste-containing fluid. In this narrative Review, we examine this 'outflow end', including meningeal lymph

Blood-brain barrier (BBB) preserves brain homeostasis yet restricts therapeutics; in bacterial meningitis, antibiotics' limited penetration impairs delivery to the parenchyma and cerebrospinal fluid, constraining efficacy and motivating strategies to overcome the barrier. To address this, we developed a chiral virus-mimicking mesoporous silica nanostructure (CVSN) functionalized with L-alanine for antibiotic delivery. It offers several distinct advantages: (1) nanoscale dimensions facilitating BBB transport; (2) virus-like morphology enhancing vascular accumulation and penetration, providing a
