The glymphatic-lymphatic axis: known pathways, open questions, and emerging roles in neurodegeneration.

The discovery of the glymphatic system and meningeal lymphatic vessels has transformed our understanding of brain fluid homeostasis and central-peripheral communication, highlighting the critical role of cerebrospinal fluid in brain clearance. While substantial progress has been made in defining the structural components and physiological regulators of brain clearance, major uncertainties remain regarding (1) how these systems interact as an integrated physiological network, (2) how they fail in disease, and (3) whether they can be therapeutically manipulated. In this review, we first summariz

B Lymphocytes in Acute Ischemic Stroke: From Immune Tolerance to Neuroprotection and Autoimmunity.

Whereas the pathobiology of stroke is traditionally thought to be mediated mainly by innate immune cells, the recent discovery that immune tolerance to brain-specific antigens takes place in the meninges suggests that the CNS makes use of unique pathways to respond to brain-derived antigens. By virtue of their adaptive and innate-like functions, B lymphocytes can play multiple roles in stroke. Following acute ischemic stroke, B cells home to the site of injury and produce cytokines that can either promote or exacerbate inflammation. They also secrete autoantibodies against neuronal and/or vasc

Financial impact of unplanned readmission following surgical treatment of chronic subdural hematoma and its implications for interventional adjuncts.

OBJECTIVE: Chronic subdural hematoma (cSDH) is an increasingly prevalent neurosurgical condition. Unplanned readmissions following surgical evacuation represent a significant clinical and economic challenge. This study aims to identify predictors of 30-day readmission, evaluate their financial impact within the German Diagnosis-Related Group (DRG) system, and determine the economic viability of adjunctive middle meningeal artery embolization (MMAE). METHODS: A monocentric retrospective analysis was conducted on 570 adult patients surgically treated for cSDH at a tertiary neurosurgical center

Palatal myoclonus in tuberculous meningitis: a case report.

Palatal myoclonus is a clinical manifestation caused by lesions involving the Guillain-Mollaret triangle (dentato-rubro-olivary pathway) and is often associated with hypertrophic olivary degeneration. Reported causes of palatal myoclonus include infectious, inflammatory, autoimmune, demyelinating, neoplastic, vascular, and drug-related disorders. We report a case of drug-resistant tuberculous meningitis presenting with palatal myoclonus and describe the patient's clinical course and outcome.

Non-invasive brain stimulation and glymphatic function: an aquaporin-4 perspective.

The glymphatic system is a brain-wide fluid transport network in which cerebrospinal fluid enters along periarterial spaces, exchanges with interstitial fluid within the parenchyma, supports the redistribution and clearance of interstitial solutes, and drains toward meningeal and extracranial lymphatic pathways. Aquaporin-4 (AQP4), a water channel highly enriched at perivascular astrocytic endfeet, forms a polarized astroglial membrane interface that facilitates water exchange across the gliovascular compartment and critically shapes glymphatic transport. While non-invasive brain stimulation h

Atypical Lyme Neuroborreliosis Presenting With Ventriculitis and Cerebritis: A Case Report.

Lyme neuroborreliosis is a neurological manifestation of infection by Borrelia burgdorferi sensu lato and most commonly presents with lymphocytic meningitis, cranial neuropathy, or painful radiculitis. Central nervous system parenchymal involvement is uncommon, and ventriculitis is rarely reported. We describe a man in his late 70s who presented with acute confusion, dysarthria, and left-sided weakness following several days of severe lower-limb pain and progressive mobility impairment. His neurological status rapidly deteriorated, with secondary seizures requiring invasive mechanical ventilat

Targeting glymphatic-associated pathways for Alzheimer's disease: a review of non-invasive modulation strategies, mechanisms, and emerging evidence.

The glymphatic system, a cerebral waste clearance pathway identified over a decade ago, has reshaped understanding of Alzheimer's disease (AD) pathogenesis by linking impaired brain homeostasis to amyloid-β and tau accumulation. This review synthesizes the biological mechanisms governing glymphatic function, including arterial pulsation, vasomotion, cerebrospinal fluid turnover, norepinephrine oscillations, aquaporin-4 polarization, and meningeal lymphatic drainage, and evaluates emerging non-invasive modulation strategies. Unlike immunotherapies that directly target pathological proteins with

Varicella zoster virus meningitis with a dermatomal rash in immunocompetent hosts: a systematic review and novel case report.

Varicella zoster virus (VZV) reactivation with central nervous system (CNS) involvement is an uncommon but important cause of lymphocytic meningitis in both immunocompromised and immunocompetent individuals. While cranial presentations, especially V1 involvement, are well documented, spinal-dermatomal VZV meningitis is rarely reported. We conducted a systematic review of PubMed, Scopus, and Embase using PRISMA guidelines to identify all cases of laboratory-confirmed VZV meningitis or encephalitis with a dermatomal rash in immunocompetent adults affecting spinal levels C1 and below. Epistemological Anatomy: A Conceptual Framework for Understanding Anatomical Knowledge.

Anatomy is often regarded as settled knowledge. Yet anatomical propositions differ enormously in their evidentiary foundations, while textbooks and atlases frequently present them with similar certainty. How, then, does an anatomical observation become anatomical knowledge? We introduce epistemological anatomy as a framework for the systematic study of how anatomical knowledge is generated, validated, transmitted, challenged, and revised. The framework treats the anatomical proposition as a unit of analysis and distinguishes among evidence, scientific confidence, and authority. It also introdu

Echovirus 19 Molecular Epidemiology in Russia and Some Central Asian Countries in 1999-2024.

Echovirus 19 (E19) is a rarely reported non-polio enterovirus (NPEV), although it has been associated with a spectrum of severe syndromes, including encephalitis, meningitis, and multisystem hemorrhagic disease in infants. Moreover, Echovirus 19 has been detected in patients with acute flaccid paralysis (AFP), suggesting a potential association between viral infection and CNS damage. Here we describe molecular epidemiology of Echovirus 19 in Russia and Central Asia from 1999 to 2024 using data on poliomyelitis surveillance. Echovirus 19 accounted for no more than 4% of annual NPEV isolates, ho

Congenital Nasal Encephalocele Presenting With Pneumococcal Meningitis and Subsequent Bilateral Sensorineural Hearing Loss: A Case Report of a Pediatric Patient.

Congenital nasal encephaloceles are rare anterior skull base defects that may predispose patients to cerebrospinal fluid (CSF) leakage and subsequent meningitis. While sensorineural hearing loss (SNHL) is a known complication of pediatric meningitis, progression from an unrecognized nasal encephalocele to meningitis and bilateral profound hearing loss is infrequently reported. We describe a seven-month-old male who presented with pneumococcal meningitis and status epilepticus requiring intensive care management. Following recovery, he developed severe-to-profound bilateral SNHL. Further evalua

An Active Cytomegalovirus Infection and Epstein-Barr Virus Reactivation Complicated by Cytomegalovirus Meningitis and Virus-Associated Hemophagocytic Lymphohistiocytosis during Methotrexate and Upadacitinib Therapy in a Patient with Rheumatoid Arthritis.

Methotrexate (MTX) and Janus kinase (JAK) inhibitors are effective treatments for rheumatoid arthritis (RA); however, they may increase the risk of opportunistic infections. We herein report a case of a patient with RA receiving MTX and upadacitinib who developed an active cytomegalovirus (CMV) infection and Epstein-Barr virus (EBV) reactivation, complicated by CMV meningitis and virus-associated hemophagocytic lymphohistiocytosis (HLH). This case highlights that clinicians should consider not only herpes zoster, but also infection or reactivation involving non-VZV herpesviruses, including CMV

Impact of initial disease severity and metabolic stress on mortality in patients with suspected external ventricular drain-associated infection.

External ventricular drainage (EVD) remains indispensable for managing intracranial pressure and cerebrospinal fluid (CSF) disorders. Despite advances in neurosurgical care and infection prevention, EVD-related infections still contribute substantially to morbidity and mortality. This study aimed to identify predictors of mortality and evaluate the clinical relevance of infection-related and metabolic parameters in patients with suspected EVD-associated infection undergoing EVD placement. We retrospectively analyzed all adult patients who underwent EVD insertion and were clinically suspected o

Associations of recorded in-hospital linezolid and fluoroquinolone exposure with mortality and documented drug-induced liver injury in non-HIV tuberculous meningitis: a propensity score-weighted real-world cohort study.

BACKGROUND: Tuberculous meningitis (TBM) remains the most severe form of tuberculosis. Linezolid- and fluoroquinolone-containing regimens are used in clinical practice, but their associations with mortality and specific toxicities in non-HIV TBM remain uncertain. METHODS: We conducted a single-center retrospective cohort study of non-HIV patients with TBM hospitalized between October 2013 and January 2024. The exposures were recorded linezolid use, fluoroquinolone-class use, and combined linezolid-fluoroquinolone use during the index hospitalization. Each indexed exposure was compared with no
