Case Report: Fulminant meningococcus-meningitis under treatment with ravulizumab despite complete vaccination-a rare but severe complication.

OBJECTIVES: Complement C5 inhibitors such as ravulizumab are increasingly used for acetylcholine receptor-positive generalized myasthenia gravis (MG) but confer a relevant risk of invasive meningococcal infection. We report a rare case of fulminant meningococcal meningitis despite complete vaccination, highlighting the limits of current preventive strategies. METHODS: Case report. RESULTS: A 41-year-old woman with acetylcholine receptor-positive MG was treated with ravulizumab after two-dose series meningococcal vaccination (MenACWY and MenB). One month after therapy initiation, she presente

Climate change and vertically transmissible infections in maternal and neonatal populations: a systematic review.

BACKGROUND: Climate change influences infectious disease dynamics and disrupts maternal and neonatal health services, both of which are pathways through which the risk of vertically transmissible infections may increase. METHODS: We systematically reviewed evidence on the association between climate-related environmental exposures and infection-related outcomes relevant to vertical transmission in pregnant or intrapartum women, neonates, and infants. Screening, extraction, and risk-of-bias assessment were performed independently in duplicate. Owing to substantial heterogeneity, findings were

Pathogen and host biomarkers to aid early diagnosis and prognosis of tuberculous meningitis.

BACKGROUND: Tuberculous meningitis (TBM) is the most sinister form of extrapulmonary tuberculosis (EPTB), associated with high mortality due to delayed diagnosis and limited sensitivity of conventional and molecular tests. Current study evaluated the diagnostic utility of Lipoarabinomannan antigen (LAM) detection in CSF and urine and explored host inflammatory biomarkers for diagnosis and prognosis of TBM. METHODS: This prospective observational cohort study enrolled 80 HIV-negative adults with presumptive TBM at a tertiary care center. Patients were classified according to the uniform TBM ca

Insulin Enhances Nitroglycerin-Induced Hyperalgesia Through Insulin-Responsive Receptor Signalling Involving the Trigeminovascular System and Meningeal Mast Cells.

Migraine headache prevalence is lower in patients with diabetes, but the underlying mechanisms remains unclear. This study explored the influences of hyperglycemia, insulin and insulin receptor (IR)/insulin-like growth factor 1 receptor (IGF-1R)-related signalling on migraine-related mechanisms in in vivo and ex vivo rat models. In in vivo experiments, migraine-relevant responses were induced by systemic injection of nitroglycerin (NTG), whereas hyperglycemia was induced by streptozotocin (STZ). The effects of hyperglycemia, insulin and the dual IR/IGF-1R inhibitor BMS-754807 were investigated

Gut irradiation and microbiota status modulate the meningeal immune landscape.

Radiotherapy (RT) is an important part of curative treatment in many common abdominal and pelvic cancers. However, the gastrointestinal toxicities can affect the quality of life and outcomes for many patients. The underlying local gut toxicity and systemic effects, including effects on mental health, are underexplored and poorly understood. Given emerging evidence of immunological communication across the gut-brain axis, we investigated whether abdominal RT could induce distal immune alterations in the dural meninges and explored potential behavioural outcomes. Using a preclinical model of ima

Clinical Utility of Liquid Comprehensive Genomic Profiling in Advanced Lung Adenocarcinoma Complicated by Disseminated Intravascular Coagulation: A Case Report.

Comprehensive genomic profiling (CGP) is increasingly important for identifying actionable driver mutations and selecting targeted therapies in advanced lung adenocarcinoma, particularly when tissue re-biopsy is precluded by severe complications. A 74-year-old woman with no history of smoking presented with hemoptysis and abnormal findings on chest imaging. She was diagnosed with lung adenocarcinoma, cT2bN2M1c, Stage IVB, with bone and brain metastases. Initial molecular testing using AmoyDx® (AmoyDx Essential NGS Panel, Amoy Diagnostics, Xiamen, China) did not reveal any actionable driver mut

Escherichia coli ST131 isolates are rare in human meningitis but belong to a distinct O25b-fimH22 clade B lineage shared with avian pathogenic Escherichia coli.

Escherichia coli ST131 is a globally disseminated extraintestinal lineage best known for its resistance-driven expansion, yet it is rarely reported as a cause of neonatal meningitis. Whether specific ST131 sublineages retain neuroinvasive potential remains poorly understood. We analysed a 25-year nationwide French collection of 355 neonatal meningitis E. coli isolates recovered between 2000 and 2024. ST131 isolates were characterized by whole-genome sequencing, and ST131-O25b-fimH22 genomes were contextualized in a phylogeny including human-associated genomes and selected animal- and food-asso

Breaking barriers in leptomeningeal disease: A Society for Neuro-Oncology and The American Society of Clinical Oncology consensus review on clinical trial design and basic science integration.

Leptomeningeal disease (LMD) is a devastating complication of cancer, and its incidence is rising. The inaugural LMD Think Tank Meeting took place in 2023 during the Society for Neuro-Oncology (SNO) and American Society of Clinical Oncology (ASCO) Central Nervous System (CNS) annual meeting, convening an international group of clinicians and scientists dedicated to understanding and treating LMD in a multidisciplinary fashion. While this meeting laid the foundation for a SNO/ASCO consensus review on the clinical management of LMD, the frequent exclusion of LMD patients from trials was identifi

Deep cervical lymphovenous reconstruction in Alzheimer's disease: three target-defined interfaces with distinct mechanistic implications.

Age-related impairment of brain solute clearance has been implicated in Alzheimer's disease (AD), yet procedures described as deep cervical lymphovenous reconstruction do not necessarily target the same lymphatic structure. This review examines glymphatic exchange, meningeal lymphatic drainage, cervical outflow, human lymphatic imaging, lymphatic surgery, early clinical studies, registered trials, and reported harms. Three interfaces can be distinguished: lymphatic flap-vein, lymph node-vein, and collecting lymphatic vessel-vein anastomosis. They differ in the reconstructed structure, pressure

Peritumoural brain edema is associated with seizure-related presentation in intracranial meningiomas.

PURPOSE: Peritumoural brain edema (PTBE) is associated with preoperative seizures in meningioma, but its association with seizure-related rather than other symptomatic presentation is less clear. We examined this relationship and associations of PTBE with tumour volume and anatomical location. METHODS: We retrospectively analysed 213 patients with surgically treated intracranial meningiomas. PTBE was classified on preoperative T2-weighted MRI by two independent neurosurgical reviewers, with disagreements resolved by consensus. The primary analysis compared seizure-related with other symptomat

Efficacy of Standard Meropenem Dosing in Healthcare-Associated Meningitis: Is It Universal?

Central nervous system (CNS) infections related to external ventricular drains (EVD) represent a frequent and serious complication of invasive CNS procedures, particularly those due to Gram-negative bacilli (GNB). Meropenem is one of the first-line treatments recommended whereas its limited CSF diffusion, with few studies with proven CNS infections. The multicenter PK/PD study evaluated the impact of EVD-related infections on meropenem distribution in cerebrospinal fluid (CSF) in 28 neuro-ICU patients with EVD, receiving meropenem via intermittent or continuous infusions (6-8 g/day), by compar

Outcomes after middle meningeal artery embolization using coils alone for chronic subdural hematoma: A systematic review and single-arm meta-analysis.

Coil-only middle meningeal artery embolization (MMAE) remains less well characterized in the literature compared to liquid or particulate embolization for chronic subdural hematoma (cSDH). This systematic review and single-arm meta-analysis summarized clinical, radiographic, procedural, and safety outcomes after coil-only MMAE for cSDH. This systematic review followed PRISMA guidelines. PubMed, Ovid MEDLINE, Web of Science Core Collection, and Scopus were searched for English-language studies evaluating coil-only MMAE for cSDH with ≥ 5 patients and ≥ 1 predefined endpoint. Risk of bias was ass

Occurrence of Sensorineural Hearing Loss in Viral Meningitis and Encephalitis.

Hearing loss in viral neuroinfections is poorly investigated. The present study aimed to clarify if hearing loss can develop in viral meningitis and encephalitis. An OAE (oto-acoustic emission) was measured in adult patients with viral meningitis and encephalitis on the day of admission and 30-60 days after discharge. Frequencies were categorized as Low (1, 1.5, 2 kHz), Mid (3, 4, 5 kHz), Mid-high (6, 7, 8 kHz), and High (9, 10 kHz). An OAE was measured in 41 patients with viral meningitis and 8 patients with encephalitis. Six patients with viral meningitis (15%) experienced a decrease of > 5

Application of procalcitonin in pediatric emergency and critical care settings.

This expert consensus on the clinical application of procalcitonin (PCT) in pediatric emergency and critical care was convened by the Taiwan Society of Pediatric Emergency Medicine (TSPEM) and formulated by a committee of 14 leading experts. In pediatric emergency and critical care medicine, distinguishing between bacterial and viral etiologies remains a significant challenge, as clinical symptoms often lack specificity. While traditional biomarkers such as white blood cell count (WBC) and Creactive protein (CRP) provide reference values, they possess limitations regarding diagnostic timelines
