The cerebrospinal fluid immune microenvironment in lung cancer leptomeningeal metastasis: cellular ecosystems and therapeutic possibilities.

Leptomeningeal metastasis (LM) is a devastating form of central nervous system (CNS) dissemination in lung cancer, particularly non-small cell lung cancer (NSCLC). Current treatment frameworks emphasize CNS drug penetration, oncogenic driver evolution, and intrathecal therapy. Emerging evidence supports viewing the cerebrospinal fluid (CSF) compartment as an active tumor-immune space, but the field remains early, with limited lung cancer-specific mechanistic and single-cell datasets. Lung cancer cells in CSF appear to adapt to low protein content, limited nutrients, fluid shear, weak matrix su

HIV-1 compartmentalization in the central nervous system during cryptococcal meningitis: Analysis of viral, fungal, and host factors.

Opportunistic infections in the central nervous system (CNS), such as cryptococcal meningitis (CM) have profound consequences for people living with HIV-1 (PWH), including blood-brain barrier impairment and neuroinflammation. To investigate how CM may shape the HIV-1 populations in the CNS, we characterized viral variants in the blood and cerebrospinal fluid (CSF) compartments in PWH with active CM. Thirty-nine participants with CM had paired plasma and CSF viral load measurements prior to the start of anti-retroviral therapy (pre-ART), with overall significantly higher HIV-1 viral loads in pl

Cerebrospinal Fluid Cytokine Signatures in Real-Time PCR-Confirmed Acute Bacterial Meningitis: Diagnostic Insights from TNF-α and IL-1β.

BACKGROUND: Acute bacterial meningitis (ABM) is a rapidly progressive central nervous system infection where excessive neuroinflammation contributes significantly to morbidity and mortality. Cerebrospinal fluid (CSF) cytokines are key mediators of host immune responses; however, their association with molecular pathogen detection and host inflammatory responses remains incompletely understood OBJECTIVE: To evaluate CSF cytokine responses and their association with molecularly confirmed acute bacterial meningitis METHODS: A total of 246 CSF samples from patients with suspected meningitis, submi

Invasive Cryptococcal Meningitis: A Sequelae of Primary Intestinal Lymphangiectasia.

Primary intestinal lymphangiectasia (PIL) is a protein-losing enteropathy causing enteric loss of albumin, immunoglobulins, and lymphocytes. Untreated or inadequately monitored PIL may result in secondary immunodeficiency and opportunistic infections. We report a 30-year-old man with a childhood diagnosis of PIL who developed cryptococcal meningoencephalitis. Evaluation revealed secondary immunodeficiency with lymphopenia and hypogammaglobulinemia which was attributed to ongoing protein-losing enteropathy despite absence of gastrointestinal symptoms, supported by hypoalbuminemia and fat-solubl

Rapid CSF Molecular Diagnosis of Listeria monocytogenes Meningoencephalitis in an Elderly Immunocompetent Patient: A Case Report.

Background/Objectives: Listeria monocytogenes is an uncommon but potentially life-threatening cause of central nervous system infection, predominantly affecting older adults and immunocompromised patients. Neurolisteriosis may present with nonspecific neurological manifestations, and the absence of classical meningeal signs may delay diagnosis. We report a case of L. monocytogenes meningoencephalitis in an elderly patient without known immunosuppression, with rapid cerebrospinal fluid (CSF) molecular testing facilitating early diagnosis and appropriate antimicrobial treatment. Case presentatio

Skull Bone Marrow as a Cranial Neuroimmune Reservoir in Alzheimer's Disease: Amyloid, Osteoimmune Remodeling, and Translational Imaging Evidence.

Alzheimer's disease (AD) has traditionally been conceptualized as a disorder driven primarily by cerebral amyloid-β accumulation, tau pathology, synaptic failure, vascular dysfunction, and chronic neuroinflammation. However, emerging evidence suggests that AD pathobiology may also involve extracerebral cranial compartments positioned at the interface between the brain, meninges, cerebrospinal fluid, and skull bone marrow. Recent anatomical and experimental studies have identified direct vascular and osseous channels connecting skull bone marrow with the dura mater, allowing bidirectional excha

In Situ Cytokine Expression, Nitric Oxide and Reactive Oxygen Species Production During Primary Amoebic Meningoencephalitis Development.

Primary amoebic meningoencephalitis (PAM), caused by Naegleria fowleri, is a rapidly progressive and fatal infection, and inflammation of the olfactory bulb plays a key role in disease pathogenesis. Although inflammatory and oxidative mediators have been studied in vitro, their production during the progression of PAM remains poorly understood. This study evaluated the temporal production of TNF-α, IL-1β, IL-6, nitric oxide (NO), and reactive oxygen species (ROS) in the olfactory bulbs of BALB/c mice from 1 to 7 days after intranasal infection with N. fowleri trophozoites. RT-PCR detected incr

Neurological Manifestations in People Living with Human Immunodeficiency Virus (PLHIV) in India: A Scoping Review and the Need for a National HIV-Neurology Special Interest Group.

BACKGROUND AND OBJECTIVES: Despite a low national prevalence of 0.20%, India's absolute burden of 2.544 million people living with human immunodeficiency virus (HIV) presents clinicians with varied neurological complications. While combination antiretroviral therapy (ART) has extended survival, the neuro-HIV profile in India, characterized by the predominance of HIV-1 clade C, remains distinct from that reported in Western cohorts. This scoping review aimed to identify patterns of neurological manifestations in India across the pre-ART, ART, and "treat-all" eras and to support the establishmen

Liquid Embolic Volume Does Not Predict Outcome After Middle Meningeal Artery Embolization for Chronic Subdural Hematoma.

PURPOSE: Middle meningeal artery embolization (MMAE) is increasingly used for chronic subdural hematoma (CSDH), but the relationship between injected liquid-embolic volume and outcomes is unclear. We evaluated dose-outcome associations and searched for a minimum effective dose. METHODS: This retrospective multicenter cohort included 313 patients treated with liquid embolic agents at 12 MESH Registry centers from 2016 to 2024. MMAE was performed alone in 172 patients (55.0%) and adjunctively with surgical evacuation in 141 (45.0%). Dose was assessed as total injected volume per patient and nor

Antimicrobial resistance trends and lineage distribution among invasive pneumococcal disease isolates pre- and post-pneumococcal conjugate vaccine introduction, South Africa.

OBJECTIVES: Pneumococcal conjugate vaccines (PCV) have reduced antimicrobial-resistant invasive pneumococcal disease (IPD) globally. We evaluated trends in antimicrobial-resistant IPD and pneumococcal lineages before and after PCV13 introduction in South Africa. METHODS: We analysed national, laboratory-based IPD surveillance reported cases (2005-2020) and compared the incidence of resistant-IPD pre- and post-PCV introduction. Antimicrobial susceptibility testing was performed for six antibiotics, and penicillin-multidrug resistance (penicillin-MDR) was defined as penicillin nonsusceptibility

Towards a holistic epidemiology of Streptococcus agalactiae using the BakRep repository.

BACKGROUND: Streptococcus agalactiae is a versatile multi-host pathogen that can cause severe neonatal disease in humans and mastitis in dairy animals. Its ability to infect a wide range of hosts is largely driven by its high genomic plasticity and the acquisition of distinct accessory genes. The global population of S. agalactiae is characterized by multiple capsular serotypes and clonal complexes that differ in their propensity to cause invasive disease, including hypervirulent CC17 (often serotype III) associated with neonatal meningitis, whereas CC1/CC19/CC23 are more often colonizing line

Pneumococcal meningitis secondary to chronic orbital inflammation with skull base erosion: a case report.

This case report describes a 32-year-old female patient who developed acute pneumococcal meningitis following six-year chronic orbital inflammatory changes after strabismus corrective surgery. The patient experienced gradual deterioration of right-eye vision and recurrent retro-orbital headache; serial magnetic resonance imaging (MRI) from 2021 to 2023 demonstrated diffuse intraconal retrobulbar abnormalities and extraocular-muscle thickening. She subsequently presented with high fever, generalized seizures and impaired consciousness. Cerebrospinal fluid (CSF) metagenomic next-generation seque

Targeting Piezo1/2 ion channels for modulation of the lymphatic system: mechanisms, applications, and therapeutic prospects.

In recent years, the role of mechanosensation in lymphatic system development, functional homeostasis, and disease progression has received increasing attention. As key mechanosensitive receptors in lymphatic endothelial cells, Piezo1/2 ion channels respond to biomechanical stimuli such as fluid shear stress and interstitial pressure, thereby regulating calcium influx and downstream signaling pathways. These channels play pivotal roles in physiological processes, including lymphatic network formation, valve development, and the maintenance of meningeal lymphatic function. This review systemati

Spinal neurocysticercosis: synthesizing evidence on clinical presentations, anatomical patterns, diagnostic approaches, and therapeutic strategies.

Spinal neurocysticercosis (NCC) is an uncommon manifestation of NCC involving the spinal cord, meninges, nerve roots, and subarachnoid spaces. Although less common than intracranial disease, spinal involvement can cause substantial neurological morbidity because even small lesions within the confined spinal canal may produce progressive myelopathy. Disease may result from hematogenous dissemination or cerebrospinal fluid (CSF) spread of Taenia solium larvae and may present as intramedullary, intradural extramedullary, subarachnoid, or, rarely, extradural lesions. Clinical manifestations are di

Spatial transcriptomics profiling across tissue boundaries in the developing mouse brain.

Tissue boundaries preserve structural integrity and guide cell differentiation during development, despite constant stress from proliferation, cell death, and tissue deformation. To study gene regulation in these regions we developed REBOSEP, a workflow for analyzing spatial transcriptomics data to detect tissue boundaries and measure gene expression relative to boundary distance. REBOSEP uses hierarchical clustering to uncover shared regulatory patterns among genes, revealing highly diverse expression changes that can shift sharply across just a few cell layers. Using this approach, the study

Markedly Delayed Onset ICANS in the Setting of CAR T-Cell-Mediated Aseptic Meningoencephalitis: Two Cases.

OBJECTIVE: Immune effector cell-associated neurotoxicity syndrome (ICANS) typically manifests within the first week following chimeric antigen receptor (CAR) T-cell therapy and has rarely been described to develop beyond 1 month after treatment. This report describes the occurrence of markedly delayed ICANS in the setting of non-infectious meningoencephalitis. METHODS: We report two cases of markedly delayed ICANS with cerebrospinal fluid (CSF) pleocytosis comprised primarily of CAR T-cells. RESULTS: Two patients, aged 54 and 67 years, developed ICANS at 71 and 46 days, respectively, followi

New insight into Alzheimer's disease: small molecule-Aβ in the brain transport and peripheral organ clearance.

Alzheimer's disease (AD) represents one of the most prevalent neurodegenerative disorders without approved disease-modifying therapies. A major contributing factor is that, for decades, drug development has focused almost exclusively on repairing or removing lesions within the brain itself. Yet a growing body of work now points to AD as a systemic disorder, rooted in a breakdown of whole-body Aβ homeostasis. Nevertheless cerebral Aβ transport and peripheral Aβ clearance are commonly investigated as independent processes, and the coordinated interplay… remains insufficiently characterized. This
