[Keratoplasty and donor age distribution].

BACKGROUND: In the past corneas from young donors were favored. There are only few reports of PK with old donor corneas. Concerning donor age we were not very stringent in our patients. Therefore many old corneas were transplanted. We present long-term results of these patients and enter again into the unanswered question: Is there a correlation between graft failure and donor age? PATIENTS AND METHODS: We retrospectively analyzed 615 PK out of 1986 to 1991. In addition to own records we requested reports from retreating ophthalmologists concerning graft clarity. Mean follow-up was 37.3 month

Initial endothelial cell density and chronic endothelial cell loss rate in corneal transplants with late endothelial failure.

OBJECTIVE: To determine when corneal transplants develop the decreased endothelial cell density that predisposes to late endothelial failure (LEF). DESIGN: Noted cohort study within a prospective case series. PARTICIPANTS: The authors compared 21 grafts that developed LEF with the remaining transplants (controls) in a longitudinal cohort study of 500 consecutive penetrating keratoplasties by 1 surgeon. Eyes regrafted during the study, fellow eyes of bilateral cases, and patients who did not want their data used for research purposes were excluded, leaving 389 grafts in 389 patients for analy

[Practice guidelines of the Spanish Society of Cardiology. Cardiac and heart-lung transplants].

Cardiac transplantation is the only therapy that is able to substantially modify the natural evolution of patients with severe heart failure, along with angiotensin converting enzyme inhibitors. Nevertheless, because of the limited number of donors, its impact is scarce compared to the magnitude of the problem. Up to the end of 1998, 48,541 orthotopic cardiac transplantations and about 2,510 heart and both lung transplantations have been registered throughout the world. In Spain 2,780 procedures have been performed in the last 15 years. The survival expectations for a transplanted patient is 7

Present status and future prospects in liver transplantation.

Results of liver transplantation (LTx) have largely improved over the last few years and a one year patient survival rate of around 90% for elective transplants can now be reached. This is mostly the consequence of better patient selection and preparation, improved surgical techniques, improved intra- and postoperative care, reduced incidence of primary non function (< or =5%) and hepatic artery thrombosis (< or =5%), the understanding that the liver induces a tolerogenic response and the development of more judicious immunosuppressive protocols. Because patient and graft survival have dramati

Unrelated donor bone marrow transplantation in Fanconi anaemia: the Leiden experience.

Fanconi anaemia (FA) is an accepted indication for treatment with allogeneic HLA-identical BMT. Most patients, however, lack a suitable HLA-identical donor. In our centre, six FA patients were transplanted with a matched unrelated donor. Due to hypersensitivity to DNA cross-linking agents, a low-dose cyclophosphamide (CY) and thoraco-abdominal irradiation (TAI) regimen is recommended for conditioning in FA. We added Ara-C upfront and anti-T cell antibodies to enhance engraftment and to prevent GVHD, in combination with T cell depletion in four out of six of the first transplants. One patient d

Lung transplantation for cystic fibrosis.

Cystic fibrosis (CF) is an inherited disease in which the fundamental physiological defect is failure of cAMP regulation of chloride transport. More than 90% of patients with CF will die of chronic, suppurative, obstructive lung disease, with the median survival in the United States currently being 29 years of age. Currently, although other therapies are being aggressively investigated, bilateral lung transplantation offers the only hope for short-term and mid-term survival in patients with CF and end-stage pulmonary disease. Since 1989, 103 bilateral sequential lung transplants (BLT) for CF h

Combined heart and kidney transplantation using a single donor: a single center's experience with nine cases.

BACKGROUND: Simultaneous double-organ transplants comprising various organ combinations have become frequent. The purpose of this article is to report on a single center's experience of simultaneous heart and kidney transplantation (HNTX) with particular emphasis on selection criteria and patient outcome. METHODS: From September 1990 to January 1997, nine patients underwent HNTX, receiving both grafts from a single donor selected on ABO blood group compatibility and a negative lymphocytotoxic crossmatch, but without regard to HLA-antigen matching. RESULTS: One patient died of acute humoral r

Novel complement inhibitors.

The complement system provides natural immunity against microbes and is an effector arm of antibody-mediated humoral immunity. It promotes the inflammatory process by activating cells and facilitates microbial destruction through opsonisation and lysis. Given this tissue damaging potential, it is not surprising that nearly half of the proteins of the complement system are regulators. The complement system can mediate undesirable cellular damage in autoantibody-mediated conditions, for example myasthenia gravis, immune-complex excess syndromes, such as systemic lupus erythaematosus, ischaemia-r

Constrictive (obliterative) bronchiolitis: diagnosis, etiology, and a critical review of the literature.

Constrictive bronchiolitis (CB) (or obliterative bronchiolitis) designates inflammation and fibrosis occurring predominantly in the walls and contiguous tissues of membranous and respiratory bronchioles, with resultant narrowing of their lumens. It differs from bronchiolitis obliterans-organizing pneumonia in its histopathology and clinical course. Most cases of CB occur in the setting of organ transplants, particularly lung and heart-lung transplants, but also in bone marrow transplants. Other bona fide cases are rare: infection, particularly viral infection, appears to be a well-documented p

The UNOS Scientific Renal Transplant Registry--ten years of kidney transplants.

1. The number of kidney transplants reported to the UNOS Scientific Renal Transplant Registry (excluding multiorgan transplants) increased from 8,831 in 1988 to 10,204 in 1996, mainly due to increased donation by living donors (1,812-3,149 during the same period). 2. Overall projected 10-year primary graft survival rates were 73% for 3,515 recipients of HLA-identical sibling grafts, 55% for 16,160 recipients of other living donor transplants or 3,940 HLA-matched cadaveric grafts and 39% for 50,900 recipients of HLA-mismatched cadaver donor kidneys. 3. One-year first cadaver graft survival rate

Preliminary risk-benefit assessment of mycophenolate mofetil in transplant rejection.

Mycophenolate mofetil (the morpholinoethyl ester of mycophenolic acid) inhibits de novo purine synthesis via the inhibition of inosine monophosphate dehydrogenase. Its selective lymphocyte antiproliferative effects involve both T and B cells, preventing antibody formation. Mycophenolate mofetil has immuno-suppressive effects alone, but is used most commonly in combination with other immunosuppressants. Mycophenolate mofetil, in combination with cyclosporin and corticosteroids, has been studied in large, randomised clinical trials involving nearly 1500 renal allograft transplant recipients. The

Grandparent donors in a living related renal transplant program.

Clinical renal transplantation is limited by the number of cadaver and living related donors. The use of kidneys from older donors and non-first-degree relatives, including grandparents, has increased the supply of organs for transplantation. The purpose of this study was to assess donor and recipient outcomes after living related renal transplants between grandparent donors and grandchild recipients. Fifteen living related renal transplants using grandparent donors were performed at the University of Minnesota from 1971 to 1995. All medical records from donors and recipients were retrospectiv

Significance of the donor age effect on kidney transplants.

The shortage of cadaveric donor kidneys for transplantation has forced a re-evaluation of the limits on donor age acceptability. However, as more kidneys from older donors have been transplanted, a significantly lower graft survival has been noted among their recipients. The impact of utilizing older donor kidneys and the relative importance of donor age with respect to other factors has not been clarified. A total of 43,172 cadaver donor transplants reported to the UNOS Scientific Renal Transplant Registry between 1987 and 1995 were the subjects of this study. Cox regression analysis was util

Excellent outcome of cardiac transplantation using domino donor hearts.

OBJECTIVE: Domino cardiac transplantation affords flexible and optimal organ utilization, provides hearts unaffected by brain death, allows prospective tissue matching, and subsequent transplantation with short allograft ischemic times. A retrospective review of our experience with domino cardiac transplantation has been made. METHODS: Seventy-two of 119 patients who underwent heart-lung transplantation from 1988 on served as domino cardiac donors (40 males, 32 females; mean age of 32 years; mean weight of 51 kg). The domino donor diagnoses were cystic fibrosis (n = 47), bronchiectasis (n = 9

Immunosuppressive regimens and their effects on renal allograft outcome.

The distribution and effectiveness of different immunosuppression protocols among recipients of first cadaver donor renal transplants reported to the UNOS Scientific Renal Transplant Registry whose graft survived at least 14 days after transplantation were analyzed. The results showed that between 1988-1993, 50-60% of recipients received triple therapy regimens including cyclosporine, azathioprine and prednisone (CAP). An additional 20% received CAP with antibody induction therapy (OKT3 or ALG). After 1993, there was an increase in the use of other drug combinations which include FK506 and, mo

Detection of donor-specific hyporesponsiveness following late failure of human renal allografts.

Limiting dilution assays to measure the frequency of interleukin-2-secreting peripheral blood T cells were carried out in patients, whose renal allografts had failed due to acute rejection (9 patients) and in patients whose grafts failed more than two years after transplantation without any recent evidence of acute rejection. Using a modified form of the assay we demonstrate that nearly half of 18 patients whose renal transplants had failed after more than two years have low or undetectable HTLp frequencies against donor, but not third-party DR antigens. No such difference was observed in any

Outcome of cardiac transplantation in children. Survival in a contemporary multi-institutional experience. Pediatric Heart Transplant Study.

BACKGROUND: Meaningful analysis of survival and risk factors for death in children who undergo heart transplantation is problematic because of the small number of heart transplantations performed at individual institutions. METHODS AND RESULTS: To more accurately examine survival and risk factors for death in children undergoing heart transplantation, we analyzed 191 patients between 1 and 18 years old who received transplants at 22 centers in the Pediatric Heart Transplant Study between January 1, 1993, and December 31, 1994. Cardiac diagnosis was congenital heart disease in 74 patients (39%

When perfectly HLA-matched kidneys are refused for transplant: implications for a national cooperative sharing system.

BACKGROUND: The transplant community attempts to maximize overall renal graft survival rates through nationwide sharing of perfectly-matched cadaveric kidneys. Although the number of such transplants is determined annually, the number available but not transplanted has never been assessed. There has also been no verification of the widespread claim that kidneys transplanted as paybacks for perfect matches are inferior. STUDY DESIGN: From records of the United Network for Organ Sharing, a complete accounting of six-antigen-matched kidney disposition was obtained, including a frequency distribu

The UNOS Scientific Renal Transplant Registry. United Network for Organ Sharing.

1. The number of cadaveric transplants performed each year at United States transplant centers has increased very little, from 7,200 in 1988 to 8,100 in 1993. Living-donor transplants increased during the same period from 1,656 to 2,562. 2. The recipient and donor populations have aged since UNOS began collecting data. In 1988, 39% of first-cadaver transplant recipients were over age 45 compared with 45% in 1993. During the same period, the percentage of cadaver kidneys from donors over age 45 increased from 16% to 26%. 3. Recipients over age 60 or under age 19 had 65% 3-year graft survival ra

Many of the most recent patients involved higher risks than in previous eras. 2. The type of donor (LD or CD) did not make a significant difference in patient or graft survival rates up to 4 years. However, the survival rate of LD kidney transplants was better in later follow-ups than that of CD transplants. Pediatric and older kidneys can be used safely, although a lower survival rate is achieved by these grafts. 3. The sex and the side (right or le
