In-situ collagen injection in postamputation symptomatic neuroma: A case report.

RATIONALE: Postamputation pain remains a significant clinical challenge, affecting a substantial proportion of individuals with limb loss. Among its etiologies, symptomatic neuromas-resulting from aberrant peripheral nerve regeneration-are a common source of residual limb pain. These lesions often lead to severe discomfort and a considerable decline in the quality of life. Although surgical treatments are available, they are often invasive and carry a risk of complications, especially in elderly or frail patients. Nonsurgical interventions, such as steroid injections, provide temporary relief

Targeted Muscle Reinnervation and Regenerative Peripheral Nerve Interface for Myoelectric Prosthesis Control: The State of Evidence.

Prosthetic rehabilitation after amputation poses significant challenges, often due to functional limitations, residual limb pain (RLP), and phantom limb pain (PLP). These issues not only affect physical health but also mental well-being and quality of life. In this review, we describe targeted muscle reinnervation (TMR) and regenerative peripheral nerve interface (RPNI) and explore their clinical role in the evolution of myoelectric prosthetic control as well as postamputation pain and neuroma management. Early myoelectric prostheses, which detected electrical potentials from muscles to contro

Ultrasound Imaging and Machine Learning to Detect Missing Hand Motions for Individuals Receiving Targeted Muscle Reinnervation for Nerve-Pain Prevention.

Targeted muscle reinnervation (TMR) was initially developed as a technique for bionic prosthetic control but has since become a widely adopted strategy for managing pain and preventing neuroma formation after amputation. This shift in TMR's motivation has influenced surgical approaches, in ways that may challenge conventional electromyography (EMG)-based prosthetic control. The primary goal is often to simply reinnervate nerves to accessible muscles. This contrasts the earlier, more complex TMR surgeries that optimize EMG signal detection by carefully selecting target muscles near the skin's s

TMR/RPNI Awareness and Pain Outcomes: A Nationwide Survey of Amputees.

BACKGROUND: Amputation leads to a symptomatic neuroma in 5 to 25% of amputees, causing debilitating pain. Targeted muscle reinnervation (TMR) and regenerative peripheral nerve interface (RPNI) are novel peripheral nerve interventions used to prevent/treat neuromas. Our objective was to assess whether amputees who underwent TMR or RPNI at primary amputation reported less pain and greater ability to use prosthetics than those receiving a delayed (secondary) TMR/RPNI or no TMR/RPNI. METHODS: A REDCap survey was administered to 1,377 amputees and 294 responded. Participants were recruited via soc

Stump pain management in patients with lower limb osseointegration.

The study aims to define a management protocol to outline a variety of clinical presentations associated with residuum pain after osseointegration. This is expected to assist clinicians in diagnosing and treating adverse events. In the present cohort study, a total of 406 patients with 429 (262 transfemoral and 167 transtibial) osseointegration cases were evaluated over the period spanning from November 2010 to November 2023 at Macquarie University and Norwest Private Hospital. International patients were excluded from the study due to the lack of detailed imaging and regular follow-up care. T

Targeted Muscle Reinnervation and Regenerative Peripheral Nerve Interface: The Evolving Landscape in the Treatment of Postamputation Pain and Prosthetics.

Targeted muscle reinnervation (TMR) and regenerative peripheral nerve interface (RPNI) are two novel microsurgical techniques that can improve prosthetic control and prevent and treat chronic limb pain following amputation. Both techniques use nerve transfer to reroute the neural input from a transected nerve to new muscle targets, thereby preventing neuroma formation and creating a new functional pathway between peripheral nerves and the brain. These techniques were originally developed to improve myoelectronic bioprosthetic control, but both TMR and RPNI have expanded in their indications to

Prevention of nerve growth and evoked pain with a nerve cap graft device.

Neuroma following nerve injury and/or amputation is a debilitating condition with significant impacts on quality of life. Several approaches exist to prevent or treat neuroma and/or reduce associated pain; however, these approaches are not consistently effective, facile, or widely accessible. The present study characterizes a xenogeneic nerve cap graft device (NCGD) composed of decellularized porcine nerve. The NCGD was assessed for its ability to inhibit nerve growth, neuroma formation, and pain in rodent models of sciatic neurectomy and tibial neuroma transposition. The NCGD provided a neuro

[Prosthetic Fitting Concepts after Major Amputation in the Upper Limb - an Overview of Current Possibilities].

BACKGROUND: The upper extremity and particularly the hands are crucial for patients in interacting with their environment, therefore amputations or severe damage with loss of hand function significantly impact their quality of life. In cases where biological reconstruction is not feasible or does not lead to sufficient success, bionic reconstruction plays a key role in patient care. Classical myoelectric prostheses are controlled using two signals derived from surface electrodes in the area of the stump muscles. Prosthesis control, especially in high amputations, is then limited and cumbersome

[Bionic Surgery Meets Bionic Reconstruction - First In-human use of Robotic Microsurgery in Targeted Muscle Reinnervation].

Robotic microsurgery is an emerging field in reconstructive surgery, which provides benefits such as improved precision, optimal ergonomics, and reduced tremors. However, only a few robotic platforms are available for performing microsurgical procedures, and successful nerve coaptation is still a challenge. Targeted muscle reinnervation (TMR) is an innovative reconstructive procedure that rewires multiple nerves to remnant stump muscles, thereby reducing neuroma and phantom limb pain and improving the control of bionic prostheses. The precision of surgical techniques is critical in reducing ax

Active nerve management for above the knee amputation: A comparison of through the wound versus posterior approach.

Targeted muscle reinnervation (TMR) and regenerative peripheral nerve interface (RPNI) are used to prevent or treat neuromas in amputees. TMR for above-the-knee amputation (AKA) is most commonly performed through a posterior incision rather than the stump wound because recipient motor nerves are primarily located in the proximal third of the thigh. When preventative TMR is performed with concurrent AKA, a posterior approach requires intraoperative repositioning and an additional incision. The purpose of this study was to evaluate feasibility of TMR and operative times for nerve management perf

Targeted Muscle Reinnervation Compared to Standard Peripheral Nerve Management Following Amputation: A Systematic Review and Meta-Analysis.

BACKGROUND: Chronic pain remains a significant challenge for individuals following limb amputation, with incidence of painful neuromas, phantom limb pain (PLP), and residual limb pain (RLP). Targeted muscle reinnervation (TMR) is a surgical technique designed to restore motor control information lost during amputation by redirecting residual nerves to new muscle targets. This systematic review and meta-analysis aims to compare patient-reported and functional outcomes following amputation with either TMR or standard neurological treatment (SNT). The study also includes an examination of primary

The influence of nerve surgical techniques at time of amputation on the prevalence of heterotopic ossification in transtibial amputees.

BACKGROUND: Heterotopic ossification (HO) can form after amputation and may cause pain and functional impairment. We aimed to describe the prevalence of HO in a civilian population of transtibial amputees. We hypothesized that the decreased rate of symptomatic neuroma following active nerve surgery (Targeted Muscle Reinnervation (TMR) or Regenerative Peripheral Nerve Interface (RPNI)) may subsequently lead to a lower prevalence of HO compared to passive nerve surgery (i.e. traction neurectomy) performed at the time of amputation. METHODS: Adult patients undergoing transtibial amputation at a

Phantom limb pain, traumatic neuroma, or nerve sheath tumor? Illustrative case.

BACKGROUND: Phantom limb pain and traumatic neuromas are not commonly seen in neurosurgical practice. These conditions can present with similar symptoms; however, management of traumatic neuroma is often surgical, whereas phantom limb pain is treated with conservative measures. OBSERVATIONS: A 77-year-old female patient with a long-standing history of an above-the-knee amputation experienced severe pain in her right posterior buttocks area for several years' duration, attributed to phantom limb pain, which radiated down the stump of her leg and was treated with a variety of conservative measu

Histopathological Confirmation of Axonal Sprouting in Regenerative Peripheral Nerve Interface.

Symptomatic neuroma represents a debilitating complication after major limb amputation. The regenerative peripheral nerve interface (RPNI) has emerged as a reproducible and practical surgery aimed at mitigating the formation of painful neuroma. Although previous animal studies revealed axonal sprouting, elongation, and synaptogenesis of proximal nerve stump within the muscle graft in RPNI, there is a lack of reports confirming these physiological reactions at the histopathological level in human samples. This report presents a case of below-knee amputation with RPNI due to foot gangrene result

Targeted muscle reinnervation in above knee amputation: surgical technique.

In the United States, an estimated 185,000 individuals undergo amputation of their upper or lower limb. This results in residual limb pain in up to 85% of cases. Targeted muscle reinnervation (TMR) is a technique that has been shown to prevent symptomatic neuroma formation. In this video, the authors demonstrate their technique utilizing TMR at the time of above-the-knee amputation. Coaptations are made to provide motor targets for branches of the saphenous, tibial, and peroneal sensory nerves. At the featured patient's most recent follow-up visit 3 months postoperatively, she reported no stum
