Predictive Value of Preoperative Inflammatory and Nutritional Indices for Revision Surgery After Major Lower Extremity Amputation in Patients With Wagner Stage 5 Diabetic Foot.

BACKGROUND Major lower extremity amputation is a severe complication of diabetic foot disease, leading to high morbidity and repeat surgeries. Although systemic inflammatory and nutritional markers have been studied as prognostic factors in diabetic foot ulcers, their role in predicting revision surgery after major amputation is unclear. This study aims to evaluate the link between preoperative inflammatory and nutritional indices and revision surgery after major lower extremity amputation. MATERIAL AND METHODS This multicenter retrospective study reviewed medical records of patients with Wagn

Innovative Patient Positioning for Proximal Femoral Nailing in a Bilateral Above-knee Amputee: A Case Report and Technical Note.

INTRODUCTION: Proximal femur fractures in amputees present unique surgical challenges, primarily due to the inability to use standard traction table attachments. Bilateral above-knee amputations further add to this difficulty. The literature mainly describes unilateral amputees and delayed fractures in previously amputated limbs, while reports involving bilateral above-knee amputations are rare. The challenge becomes greater when the fracture and bilateral traumatic amputations occur in the same index trauma, because stump length, soft-tissue condition, fluoroscopic access and infection risk a

Perioperative Complications and Mortality Following 24,506 Below-Knee Amputations in the United States: A Large, Retrospective Cohort Study.

INTRODUCTION: Below-knee amputation (BKA) is a critical lower-extremity intervention but remains associated with substantial morbidity and mortality. Prior studies are often limited by small cohorts, single-center designs, or narrow outcome definitions, leaving contemporary national estimates of postoperative risk incompletely characterized. METHODS: The TriNetX United States Collaborative Network was used to perform a retrospective cohort study. Outcomes included orthopedic, wound/stump, medical, and psychiatric complications, as well as mortality, assessed at 30 d, 90 d, 6 mo, 1 y, and 2 y.

Advanced Eumycetoma of the Right Foot Resulting in Below-Knee Amputation in Somalia: A Case Report.

INTRODUCTION: Mycetoma is a chronic, progressively destructive neglected tropical disease that presents with localised swelling, multiple sinus tracts and a discharge containing grains, and that may extend into deep tissue and bone. Somalia lies within the recognised mycetoma belt, but the national burden is poorly documented, and few patients reach specialist care early enough for the limb to be saved. CASE PRESENTATION: A 33-year-old man from Qoryooley District, Lower Shabelle, Somalia, presented with a seven-year history of progressive swelling of the right foot. Examination showed massive

Systemic and local risk factors for major amputation following diabetic minor amputation.

OBJECTIVE: Diabetes-related foot complications could lead to amputation and death. Major amputations are known to yield higher mortality rate compared to minor amputations. In patients who already had a minor diabetic amputation, knowing the risk factors contributing to a major re-amputation is of great importance. This study aims to quantify the risk of major amputation following minor diabetic amputation and to investigate the systemic and local risk factors of the re-amputation event. METHODS: This is a retrospective comparative study among patients who had a minor amputation as a diabetic

Cut Short: Evaluation, Management, and Prevention of Pediatric Lawnmower-Related Injuries.

UNLABELLED: Lawnmower-related injuries in children have serious costs and consequences. Every year, thousands of children sustain injuries to various parts of their bodies, including burns, lacerations, and fractures secondary to lawnmowers which are also the leading cause of amputation in children under 10 years of age. This review emphasizes the importance of thorough trauma assessment, early antibiotic administration, and repeated, careful irrigation and debridement. For limbs that can be preserved, current research supports soft-tissue preservation and stabilizing bones with external fixat

Efficacy and Safety of Negative Pressure Wound Therapy in Managing Lower Limb Amputation; An Updated Systematic Review and Meta-Analysis With Individual Patients Data Meta-Analysis and GRADE Assessment.

Major lower-limb amputation often leads to wound complications that delay recovery and prosthetic use. Negative Pressure Wound Therapy (NPWT) has been suggested to improve outcomes, but evidence is inconsistent. The objective of this study is to compare NPWT with standard wound therapy in fresh amputation stumps. A systematic review and meta-analysis following PRISMA and Cochrane guidelines included randomised and controlled observational trials. Primary outcomes were wound complications, including surgical-site infections (SSI) and wound dehiscence. Secondary outcomes included re-amputation,

Successful preservation of below-knee amputation stump wound with vastus lateralis anterolateral thigh flap in a patient with peripheral artery disease.

The preservation of a below-knee amputation (BKA) stump is essential for maintaining optimal limb function and facilitating future prosthetic use. Salvaging an infected stump is particularly challenging due to the loss of healthy soft tissue necessary to preserve adequate limb length. Patients with peripheral artery disease are often not considered candidates for complex reconstruction given concerns for poor perfusion, active smoking, and medical comorbidities. This case demonstrates a patient who underwent a BKA after failed revascularization for acute limb ischemia. Successful salvage of a

Regenerative peripheral nerve interface following forequarter amputation: a case report.

BACKGROUND: Forequarter amputation is a radical surgical procedure for malignant bone and soft tissue tumors involving the shoulder girdle, typically indicated for cases where limb salvage is not feasible due to extensive local invasion. However, post-amputation neuroma and phantom limb pain are major complications, occurring in 80%-90% of cases and severely impairing patients' quality of life. Traditional nerve management techniques often fail to prevent recurrent neuroma formation. The Regenerative Peripheral Nerve Interface (RPNI) is an emerging technique designed to guide axonal regenerati

Successful in situ replantation of fingertip amputation without vascular anastomosis: A case report.

RATIONALE: Fingertip amputations at Tamai zone I, the most distal anatomical level, remain a reconstructive challenge. Although vascular anastomosis has been emphasized in replantations, viable outcomes may still be achieved without revascularization under specific conditions. PATIENT CONCERNS: A 17-year-old female factory worker incurred a sharp oblique amputation of the distal left index finger due to a machine-related occupational injury. The amputation plane was located distal to the lunula, as confirmed by radiographic assessment, which demonstrated that less than one-third of the distal

Management of infection-prone dehisced surgical wounds with adjunctive bovine-derived collagen matrix: a case series.

OBJECTIVE: Postoperative surgical wound dehiscence remains a challenging complication, particularly in lower extremity procedures where impaired perfusion, systemic comorbidities, mechanical stress from ambulation, and exposed deep structures increase susceptibility to delayed healing and infection. Collagen-based matrices are used adjunctively to standard of care (SoC) in complex wounds; however, evidence supporting their use specifically in postoperative dehiscence is limited. This small case series describes clinical outcomes in three high-risk patients with infection-prone dehisced lower e

Contemporary Outcomes of Staged Versus Primary Major Amputation for Severe Foot Infection.

BACKGROUND: Lower extremity amputation is a necessary, life-saving intervention for patients with severe foot infection. Previous studies have suggested that a staged amputation (SA) approach to severe foot infections may be associated with higher technical success and lower mortality rates, when compared with primary amputations (PAs). In this study, we examined our single-center experience comparing outcomes between SA and PA in patients presenting with unsalvageable severe foot infection. METHODS: Institutional medical records were retrospectively reviewed for all lower extremity amputatio

Squamous Cell Carcinoma of Finger Invading Middle Phalanx: Surgical Considerations.

Squamous cell carcinoma (SCC) of the nail unit is a rare but highly treatable malignancy. A tissue biopsy is recommended to establish the diagnosis and guide treatment. Mohs surgery is the standard of care to preserve tissue and function for early SCC. The authors report a 76-year-old patient with a very advanced and neglected SCC of the nail unit that grossly extended to the middle phalanx. No prior reports of such an extensive growth have been documented. Due to extensive skin and bone involvement, we proceeded with amputation at the level of the proximal phalanx to obtain negative margins.

Functional and Aesthetic Restoration of Amputated Fingers Using Osseointegrated Dental Implants: Case Presentations.

Traumatic finger amputations significantly impair hand function and aesthetic appearance. Conventional prosthetic options are often limited when the residual stump is inadequate for secure prosthesis retention. Osseointegrated dental implants offer a promising alternative for anchoring custom-made prostheses. This case series presents three patients: Patient 1 was a 23-year-old male with traumatic disarticulation at the proximal interphalangeal joint of the right ring finger; Patient 2 was a 40-year-old male with traumatic amputation of the right index finger at the level of the middle phalanx
