Risk factors for revision amputation and infection after definitive closure of traumatic amputations: a retrospective cohort study at a Level I academic trauma center.

PURPOSE: The purpose of this study was to identify risk factors for revision amputation and postoperative infection after definitive closure of traumatic amputations. METHODS: We conducted a retrospective cohort study of patients presenting with traumatic lower extremity amputation or injuries requiring amputation during the initial operative intervention at a level one trauma center between January 2005 and April 2024. Patients under 18 or without an acute fracture in the amputated limb were excluded. Collected variables included mechanism of injury, time to antibiotics and debridement, and

Severe Arterial Thrombosis Revealing Underlying Nephrotic Syndrome.

A man in his 30s with hypertension presented with acute left lower limb pain and numbness for one week. Imaging revealed a thrombus at the left iliac bifurcation, following which he underwent urgent thromboembolectomy. Despite repeated interventions and anticoagulation, re-occlusion occurred the following day, ultimately requiring an above-knee amputation. Persistent hypoalbuminaemia and proteinuria prompted further evaluation, revealing nephrotic syndrome. Due to heparin resistance from severe hypoalbuminaemia, anticoagulation was switched from unfractionated heparin to argatroban. Immunosupp

Risk Factors for Neuropathic Pain in Digital Amputations.

Background/Objectives: Finger amputation is frequently followed by complications, with reported revision rates of up to 20%. One of the most disabling sequelae is the formation of painful neuromas, occurring in approximately 3-9% of cases. Several biological and mechanical risk factors have been proposed, but the potential influence of psychological traits remains poorly understood. This study aimed to investigate whether a correlation exists between patients' personality traits and the development of neuropathic pain or related symptoms. Methods: A retrospective study was conducted at a Level

Complex regional pain syndrome and amputation.

Complex regional pain syndrome (CRPS) is a debilitating poorly understood condition with unpredictable course and heterogeneous therapeutic responses. Early, coordinated multidisciplinary management, including psychological and psychiatric support, is essential. In severe, therapy-resistant CRPS (long-standing CRPS), particularly intractable infection or severe recurrent wounds, amputation may be considered as a last-resort intervention after thorough multidisciplinary assessment and failure of conservative or less invasive strategies. Amputation rarely results in pain resolution, as CRPS may

Extensive MRSA-positive sternoclavicular septic arthritis with uncommon origin: a case report.

BACKGROUND: Sternoclavicular joint (SCJ) septic arthritis is a rare entity, representing less than 1% of all bone infections. It is generally associated with hematogenous spread from sources such as skin or soft tissue infections, intravenous drug use, or central venous access devices. Methicillin-resistant Staphylococcus aureus (MRSA) is an uncommon but aggressive pathogen in this setting. CASE DESCRIPTION: We report the case of a 52-year-old male with a history of type 2 diabetes mellitus with poor glycemic control, hypertension, hypercholesterolemia, coronary artery disease, and prior lowe

Association of Post-Amputation Stump Infection with Persistent Residual Limb Pain: A Prospective Cohort Study.

BACKGROUND: Persistent residual limb pain (RLP) is a common and disabling complication following limb amputation. Although nerve injury and neuroma formation are well-established contributors, the potential association between stump infection and long-term pain outcomes has not been fully elucidated. AIM: To evaluate the association between post-amputation stump infection and persistent residual limb pain, with particular emphasis on infection severity, microbiological profile, and antimicrobial resistance. METHODS: This prospective observational cohort study included 430 adult patients who

Bidirectional Limb Salvage and Its Impact on Residual Limb Survivorship: Is a Partial Calcanectomy in Ambulatory Midfoot Amputees Worthwhile?

Partial calcanectomies and midfoot amputation are commonly used functional limb salvaging interventions; however, it is well documented that they independently result in global biomechanical disruption of diabetic foot, further placing the residual limb at risk for limb loss. We retrospectively identified and reviewed 9 ambulatory midfoot amputees (10 feet) who developed heel osteomyelitis, whom were treated with an ipsilateral partial calcanectomy to determine whether the combined anterior and posterior interventions result in additive dysfunction that impacts function or places the residual

Endovascular Revascularization of Symptomatic Superficial Femoral Artery (SFA) Occlusion After Below-Knee Amputation Despite Absent Distal Runoff.

Peripheral arterial disease is a major cause of morbidity, with revascularization traditionally aimed at limb preservation and prevention of major amputation. However, management strategies are less clearly defined once amputation has already occurred. In patients with a prior below-knee amputation, the clinical significance of restoring proximal arterial inflow in the absence of a distal limb remains uncertain. We present a 73-year-old man with a history of peripheral arterial disease who developed subacute, progressive pain in his residual limb along with a stump wound approximately one year

Amputation and reamputation for dry gangrene of both lower extremities in with chronic kidney disease patients with calciphylaxis accompanied by multidrug-resistant bacterial infections: A case report and literature analysis.

RATIONALE: Nephritis can further develop into chronic kidney disease (CKD). CKD with calciphylaxis is mainly manifested as necrotic ulcers of terminal tissues or even gangrene in the skin and distal limbs. Patients with CKD have a high risk of developing multidrug-resistant bacterial infections. PATIENT CONCERNS: Our patient developed dry gangrene of both feet for 1 month, accompanied by complications, such as anemia, hypoproteinemia, and pulmonary infection. The patient had a history of CKD for 30 years, calciphylaxis for 5 years, and multidrug-resistant bacterial infection for 1 month. DIA

NovoSorb® Biodegradable Temporising Matrix (BTM) for reconstruction of paediatric acute ischaemic limb injuries secondary to sepsis: A literature review and case series.

BACKGROUND: NovoSorb® Biodegradable Temporising Matrix (BTM) is a synthetic polyurethane bilayer matrix, used in trauma and burns reconstruction. However, there is little evidence of its use for paediatric limb ischaemia. We present a review of literature and three paediatric cases of sepsis-induced ischaemic limb injuries, reconstructed using BTM. METHODS: Data were collected on demographics, management and outcomes of three patients cared for by the senior author and plastic surgery service. The operative regimen and challenges faced are discussed.Three children, aged 1-8 years, were manage

Right Knee Ureaplasma Septic Arthritis in an Immunocompromised Host, Requiring Right Hip Disarticulation for Definitive Source Control.

INTRODUCTION: Ureaplasma urealyticum is a fastidious, urease-producing bacterium typically responsible for urogenital infections. In immunocompromised hosts, a full-thickness mucosal breach may be complicated by hematogenous spread, leading to septic arthritis in distant joints U. urealyticum is difficult to isolate on routine culture and requires special media or 16S rRNA polymerase chain reaction for detection. CASE REPORT: We report a 26-year-old immunocompromised female with neuromyelitis optica on long-term cyclosporine and Grave's disease, who developed chronic right knee U. urealyticum

Lactoferrin concentration in breast milk during the first postpartum year in Lima Norte, Peru.

Bile duct injuries (BDI) tend to be more complex in laparoscopic than in open cholecystectomy procedures, and frequently involve young adults with benign pathologies. The ultimate consequence may be a liver transplantation (LT), making this situation one of the most rare transplant indications. Fatal post-transplant outcome is extreme infrequently reported. Aim of this study is to report on our single-case experience and to review the literature concerning lethal outcome after LT for major BDI following cholecystectomy. A 36-year old obese caucasian woman underwent a laparoscopic cholecystecto

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Surgical technique of minor revision of a transcutaneous osseointegration prosthetic system (TOPS) with implant retention.

OBJECTIVE: Transcutaneous osseointegration prosthetic systems (TOPS) offer a stable skeletal attachment for artificial limbs post-extremity amputation, serving as an alternative to socket attachment. Press-fit osseointegration implants (OI) utilized in TOPS consistently enhance quality of life and mobility for amputees, particularly those experiencing socket-related issues. Despite notable benefits, late complications such as infection and implant loosening pose challenges unique to TOPS due to their percutaneous nature. Recent studies indicate a low risk of implant failure but highlight the p

Surgical technique of a transcutaneous osseointegration prosthesis system (TOPS) for transtibial amputees.

Transcutaneous osseointegration prosthetic systems (TOPS) are intended to provide stable skeletal attachment for artificial limbs after extremity amputation and is an alternative for socket attachment. TOPS for individuals with limb amputation using osseointegration implants (OI) has proven to consistently and significantly improve quality of life and mobility for the vast majority of amputees, previously using a socket prosthesis also experiencing socket-related problems. As with any implant, complications such as infection, aseptic loosening, or implant fracture can occur, which may necessit

Impact of delayed amputation on clinical outcomes compared to that of early amputation in patients with blunt polytrauma.

Delayed amputation after failed limb salvage can lead to negative clinical and functional outcomes due to complications, including re-amputation. This study aimed to compare clinical outcomes and identify risk factors according to the timing of amputation. A retrospective review of lower-extremity injuries managed between January 2016 and December 2022 at a level 1 trauma center was conducted. Outcomes were compared between early amputation (within 48 h) and delayed amputation (beyond 48 h) groups. The primary outcome was the risk of a more proximal amputation level according to the timing of

[Osseointegration in amputation surgery : Representative studies].

Osseointegration is an innovative procedure to improve the mobility and quality of life of amputees by connecting exoprostheses to the bone as a replacement for the conventional socket prosthesis. This article highlights two main approaches: the osseointegrated prosthesis for the rehabilitation of amputees (OPRA) system, which uses a thread-based implant and the integral leg prosthesis (ILP) and osseointegrated prosthetic limb (OPL) systems, which rely on the press-fit method. While osseointegration offers benefits such as better prosthesis fixation and fewer skin problems, there are also chal

A Novel Technique of Hip Disarticulation with a Stump with Soft Tissue of the Proximal Thigh: A Case Report.

CASE: Two women (aged 35 and 29 years) with intractable chronic knee infection of tumor prostheses opted for amputation. As only the trochanteric region could be preserved, we resected the remaining proximal femur and created a 15-cm long stump using unaffected proximal thigh soft tissue to avoid the drawbacks of ultra-high-thigh amputation and conventional hip disarticulation. Both had no contracture, could move their stump voluntarily, and ambulated using a standard transfemoral prosthesis without a pelvic belt at 4-year and 3-year follow-ups, respectively. CONCLUSION: Our procedure might b
